Why use the -can?Can you identify individual support needs correctly and follow up on changing needs? Do people in your service get incorrectly grouped? Do staff value and promote empowerment? Are records current, consistent and secure, regardless of staff turnover?

The I-CAN can support your service and solve these problems. Easy to use on or offline, training is provided in both administration and the philosophy of support and empowerment.

• Provide a comprehensive intake assessment • Monitor support needs, outcomes and skill levels • Inform service delivery decision making and

predict possible changing needs• Link together and streamline existing assessment

and service delivery processes • Provide an adjunct to psychological testing • Profile support and provide a basis for training or

educative programs • Improve communication with and between health,

mental health and disability professionals• Assist with funding applications

How it worksThe trained I-CAN facilitator organizes an I-CAN meeting, usually once a year, and invites the family, support staff, consulting health professionals and other stakeholders to attend or provide input through a telephone interview.

At the meeting the facilitator conducts a semi-structured interview based upon the WHO ICF framework. Abilities as well as support needs and any relevant follow-up are recorded and rated.

If the interview is completed online, an individual support needs report is automatically generated detailing all the supports needed by this person and how to empower the person to fully participate in all activities of life.

Once data have been entered, managers and administration can login to perform activities such as group reporting, cost analysis, export of MDS data and more.
