• MEDICAL:Youwillcontinuetohaveachoiceof
comprehensivehealthcarebenefits—throughthe
ChoicePlusExclusiveProviderOption(EPO),the
ChoicePreferredProviderOption(PPO),andthe
MedicalSavingsPlan(MSP).
—NEWMSPPROVISIONSFOR2016:Out-of-pocket
maximumsforMSPfamilycoveragewillbesubstantially
lower(reducedfrom$10,000to$6,850),soyoucan
startreceiving100%coveragesooner.Inaddition,
youcancontributemoretothevoluntaryHealth
SavingsAccount.
—Ifyouneedtoenrollnewdependentsformedical
coverage,youwillbeaskedtocertifytheireligibility.
—Likemostemployers,Freescalehasbeenaffected
byrisinghealthcarecostsandwillpaymorefor
medicalandpharmacycoveragein2016.Asaresult,
mostcontributionswillincreasebetween3%and
8%,dependingonyourplanoptionandnumberof

• DENTAL: Yournewdentalplanadministratorwillbe MetLife.Whilecontributionsandnetworkcoverage
aresimilartoyourcurrentplan,theremaybesome
differencesinnetworkdentalprovidersandout-of-

• VISION: Benefitswillcontinuetobeofferedthrough VisionServicePlan(VSP).Yourcoveragehasbeen
expanded—withverylittlechangeincontributions.
• LIFE INSURANCE: YourBasicLife,Supplemental Life,andAccidentalDeath&Dismemberment
benefitswillcontinue,andsomeofyouwillsee
decreasesinSupplementalLiferates.Youwillhave
newspouseandchildlifeinsuranceoptionsavailable
toyou—withincreasedcoveragelevelsand,inmany
cases,reducedrates.
• DISABILITY: YourShort-TermDisability,Short-Term Disabilitybuy-up,andLong-TermDisabilitycoverage

TAKE ACTION AND ENROLL OCTOBER 19 - 30! If you do not make new elections by October 30, 2015, you will be automatically re-enrolled in your current benefits — including your 2015 contributions to Health and Dependent Care Flexible Spending Accounts — for all of 2016.
This means, too, that if you are currently enrolled in the Medical Savings Plan, your 2016 Health Savings Account and Limited Use FSA contributions will default to $0. You must make new elections each year to continue to contribute to these accounts.
