Functional Medicine Tenets

Each person is unique.

Everything in the body/system is interconnected.

The body's natural, default state is one of wellness.

Dis-ease precedes disease and is evidence of imbalance, blockage, and/or impairment in natural functioning.

The root cause and the complaint may be distant from one another (in the body, in time, in the biochemistry).

The collection of root causes of dis-ease in the body is unique for each person. Effective medicine is customized for each person.

Wellness includes the entire patient experience(physical, mental, emotional, spiritual).

The patient's active participation and belief in their healing are central to their ability to get (and remain) well.

All modalities that bring greater balance, ease, and flow to the patient are a key part of their unique healing journey and ongoing “health care”.

Most Common True Root Causes of Dis-ease

❖ Dis-ease Begins in the Gut

❖ Insulin Resistance

❖ Estrogen Dominance

❖ Toxicity

❖ Stress

❖ A failure to Maximize, Minimize, and Prioritize the right elements for a unique individual.

Etiology of Dis-ease in the Gut

Deficiency-Driven Dysfunction

–e.g. anemia, neuropathy, depression, headache

Gastrointestinal Dis-ease

–e.g. IBS, constipation, belching, bloating

GI-origin Inflammation

–e.g. osteoarthritis, asthma, migraine, chronic fatigue, fibromyalgia, eczema, intestinal permeability

Autoimmune Dis-ease

–Hashimoto's thyroiditis, lupus, multiple sclerosis, Crohn's disease, rheumatoid arthritis

Keepin’ It Simple: Insulin Resistance

Eat Refined (or an excess of)

Carbohydrate Foods

Make lots of insulin

Cells resist the effects of insulin

Excess sugar gets stored as body fat

Feel Hungry, Weak, and Tired

Vicious Cycle of

Insulin Resistance

❖ A Powerful example of where our focus needs to be to give the Greatest Value to our patients and clients: Educate, Educate, Educate!

Hormones 101

Hormones are molecules of communication. Messenger chemicals.– Endocrine (at a distance)

– Exocrine (internal to the cell)

– Paracrine (from one cell to another)

Allow the body to interact with and respond to its environment.

Interact with each other to give the body maximum flexibility in responding to its environment, as it changes.

Levels are shifting all the time. Thus, remember that any test data is just a moment in time.

Estrogen is typically measured in 1,000,000,000,000th of a gram.

Must balance each other for optimal functioning

Three Common Hormone Imbalances

1. High insulin-producing diet (e.g. refined carbohydrates, sugars)

increases testosterone in women and increases estrogen in men

• Acne, infertility and anovulation (PCOS), excess body hair growth in

women (e.g. above lip), excess body fat (especially mid-torso, “apple

shape”), head hair loss in women, T2 Diabetes, gynecomastia in men.

2. High estrogen exposure or absorption overwhelms progesterone

(which may also be low). Often a result of “xenoestrogens”

overload. Creating “estrogen dominance”.

• Anxiety, higher body fat (especially in the lower belly, “the pooch”),

emotional sensitivity, bad PMS, long and heavy menstrual periods,

bloating/cramping, carbohydrate cravings.

E & P: It’s All About Balance

Table details from Dr. John Lee, “Hormone Balance Made Simple”, Hatchette Book Group, 2006, p. 78-79* Cowan, et al., American Journal of Epidemiology, 1981. http://aje.oxfordjournals.org/content/114/2/209.short

Estrogen Effects Progesterone Effects

Proliferates endometrium Maintains endometrium

Stimulates breast cells Protects against breast fibroids

Increases body fat Helps use fat for energy

Salt and fluid retention Natural diuretic (clinical tip!)

Higher risk of blood clots Normalizes blood clotting

Restrains bone cell loss Stimulates new bone cell growth

Higher breast cancer risk Reduces breast cancer risk.*

Anxiety, headaches Natural antidepressant; calms anxiety

Reduced cellular oxygen Restores cellular oxygen

Creates progesterone receptors Normalizes tissue sensitivity to

estrogen

Slows testosterone clearance Speeds testosterone clearance

(clinical tip!)

Ideally: “Test; don’t Guess” (e.g. DUTCH)

Estrogen Dominance Interventions

1. Choose organic foods. Use EWG’s Clean 15/Dirty Dozen tools.

2. Eliminate all conventionally-raised animal foods and reduce or eliminate dairy foods, especially during luteal phase.

3. Clean up personal hygiene products. Use EWG’s Skin Deep database/guide.

4. Ensure magnesium is optimal (RBC magnesium; ideally upper third RR).

5. Reduce or eliminate caffeine (e.g. green tea vs. coffee).

6. Address insulin resistance (reduce dietary sweeteners/refined carbohydrates).

7. Address any constipation (e.g. low magnesium, low thyroid, low hydration).

8. Support detoxification with supplemental activated B vitamins

9. Increase progesterone with chastetree berry and tangible stress reduction(which may include reducing exercise intensity/frequency).

10. Increase estrogen detoxification with cruciferous vegetables (or supplement with I3C or DIM).

And EPA and iodine and ground flaxseed and ….. ☺
