    On October 23 2002 Donald Freedland Director of Public Works Bernard Adams Grounds MaintenanceManager Rickey Reed Grounds Maintenance Manager William Jackson Equipment Operator I Basil FarringtonUnion Representative and William Gault Union Representative and I met to discuss an incident wltich took p lace atthe Public Works facility involving Melissa Woodling Equipment Operator I. We bad requested that Ms. Woodlingattend the meeting so both sides could air their views however Ms. Woodling declined the invitation and asked thatWilliam Gault represent her. Ms. Woodling bad reported to William Gault that William Jackson was standing nearthe telephone in the crew quaners and his presence made her uncomfortable.Mr. Jackson said that be was using the telephone in the crew quarters when Ms . Woodling entered the room and hecontinued using the telephone. Mr . Jackson stated that he did not look at Melissa and could not understand why hewas making her upset when be took every measure to make sure that be bad no contact with her.After a discussion about the situation it was established that Ms. Woodling was not accusing Mr . Jackson of doinganything to her however in the morning when she punched in on the time clock she preferred if Mr . Jackson wouldnot be in the area since it is a confined space.I explained to the group that Mr. Jackson was also an employee and had a right to use the facility as any otheremployee. f Mr. Jackson was not threatening or bothering Ms . Woodling then the City could not stop him frompunching the time clock or using the telephone or facility. I believed that as a team we needed to bring somebalance to the issue. I felt that there were some actions Ms. Woodling could take that would improve the situationwith Mr. Jackson. Again. we asked that Mr . Jackson use the time clock and leave the area immediately since Ms.Woodling had a roblem with him standing there.We asked Williemployee and wlocated she hasShe does not hagain Mr Jacks

However in the case when she is entering a confined area where Mr Jackson ise option to tum around and ask the assistance of Bernard Adams or Rickey Reed her supervisors.to go in any location Mr Jackson is since she feels uncomfortable with him . On the other handh s a right to use the Public Works facility without bothering Ms. Woodling.

    On Thursday, January 2, 2014, I was notified that you had just turned in approximately 15-20Dump Tickets dating back to February, 2013, that had obviously not been submitted to theRecycling and Solid Waste Department.Your failure to deliver these Dump Tickets to Recycling/Solid Waste in a timely manner resultedin an unnecessary burden on Recycling/Solid Waste staff to reconcile these tickets, especially inregard to the tickets for Fiscal Year 2012/2013, i.e. the Fiscal Year ended on September 30,2013.

    As you are/were well aware, dump tickets are to be turned in daily. Your negligence in failing toperform this minimally-demanding duty over such a protracted period of time suggests that thenon-performance of this duty was both deliberate and insubordinate.

    Accordingly, you are being issued this Written Reprimand for substandard/unsatisfactory workin violation of the following provisions of Article 21 (Employee Discipline), Section 2- n qual Opportunity mployer -

    On May 16 , 2005, you were involved in an accident that was determined to be preventable. You did notexercise the proper safety practices and skills necessary when operating City vehicle #8814, the aerialtruck. By not raising the driver's side out rigger and driving down MLK Jr Avenue, the outrigger struck themedian curb, breaking the curb, bending the outrigger and even breaking out the rear window of thevehicle causing an estimated total of $13,604 in damages to the vehicle. In addition if we lease areplacement vehicle for the four weeks it is estimated to take for repairs the City faces an additionalexpense of $3,600 in lease fees. In addition your work partner was in the bucket wh ile you were movingthe vehicle and could have been even more seriously injured.You have been involved in three accidents (December 2004, September 2004 and December 1999) withCity vehicles with trailers attached where you were backing up, jackknifed the trailer and indicated youforgot the trailers were attached. When we met to discuss this accident you indicated you forgot toraise the outrigger. Your forgetfulness is dangerous as well as expensive. This accident is a violation ofPersonnel Rules and Regulations, Article IX, Disciplinary Action, 9.03 (b) Violations of safety rules andpractices.You are an experienced operator with eight years on this vehicle and should have a better understandingof the vehicle you are operating than most In addition, you need to pay attention and e more aware ofyour surroundings as you operate City equipment. We expect that in the future, you will make betterjudgments while operating City vehicles and equipmentAt th is time because of the severity of the damage done to the vehicle, I am fecommending to the CityManager that you e suspended for five (5) days without pay.
