licdescMajor Requirements for Certification: 1) Passage of AICPA Uniform CPA examination; 150 hours from an accredited school with a baccalaureate or higher degree that includes at least 36 hours of accounting with 30 or more being upper level (300 series) credit hours and 30 hours of related courses. Transcripts required to account for all 150 semester hours of credit; 3) One year of accounting experience. Cannot use self-employment as part of the qualifying accounting experience. Reciprocity is available with verification of passage of CPA exam. (Call or check Web site of Accountancy Board for specific details). Certificate Renewal: Every 2 years with proof of continuing professional education -- 80 hours for CPAs in public accounting firms; 60 hours for CPAs working for businesses, government, schools, non-profit organizations. Fees: $100 for CPA Application; $300 for CPA CertificateMajor Requirements for Registration:1) Evidence of successful completion of the current national professional examination or waiver of the examination pursuant to A.R.S. 32-126 and R4-30-203 in the category, and branch if applicable, for which registration is sought. Applicants shall arrange to have their examination results sent directly to the Board from the applicable testing agency holding the examination results; 2) Evidence of successful completion of the National Council of Architectural Registration Boards (NCARB) Internship Development Program (IDP) training requirement or diverse work experience substantially equal to that program. Total minimum experience shall not be less than 4,680 hours; 2) Evidence of successful completion of the National Council of Architectural Registration Boards (NCARB) Internship Development Program (IDP) training requirement or diverse work experience substantially equal to that program. Total minimum experience shall not be less than 4,680 hours; 3) Certified transcripts sent directly to the Board from the registrar of each college, university, or educational institution the applicant attended, unless previously provided to the Board; 4) State or jurisdiction in which the applicant holds any other professional or occupational registration, certification, or license; type of registration, certification or license number; year granted; how registration, certification, or license was granted (by examination, education, experience, or reciprocity); and the Major Requirements for Registration: 1. Evidence of successful completion of the current national professional examination or waiver of the examination pursuant to Arizona Revised Statutes 32-126 and R4-30-203 if applicable. Applicant shall arrange to have examination results sent directly to the Board from the applicable testing agency holding the examination results; 2) Evidence of successful completion of the National Council of Architectural Registration Boards (NCARB) Internship Development Program (IDP) training requirement or diverse work experience substantially equal to that program. Total minimum experience shall not be less than 4,680 hours; 3) Certified transcripts sent directly to the Board from the registrar of each college, university, or educational institution the applicant attended, unless previously provided to the Board; 4) State or jurisdiction in which the applicant holds any other professional or occupational registration, certification, or license; type of registration, certification or license number; year granted; how registration, certification, or license was granted (by examination, education, experience, or reciprocity); and the number of examination hours taken by the applicant; 5) Evidence of successful completion, or waiver by the Board, of the applicable in-training examination; or successful completion of in-training examination in another jurisdiction. An applicant seeking professional registration as an architect or landscape architect may take the in-Major Requirements for Registration: 1) Evidence of successful completion of the current national professional examination or waiver of the examination pursuant to A.R.S. 32-126 and R4-30-203 in the category, and branch if applicable, for which registration is sought. Applicants shall arrange to have their examination results sent directly to the Board from the applicable testing agency holding the examination results; 2) Evidence of successful completion of 80 hours of classroom training or an equivalent course conducted by an educational facility that is licensed by the applicable post-secondary education regulatory agency in the home state of the facility, or accredited by the Accrediting Commission of the Distance Education and Training Council, or by an accrediting agency approved by the United States Department of Education 3) Certified transcripts sent directly to the Board from the registrar of each college, university, or educational institution the applicant attended, unless previously provided to the Board 4) State or jurisdiction in which the applicant holds any other professional or occupational registration, certification, or license; type of registration, certification or license number; year granted; how registration, certification, or license was granted (by examination, education, experience, or reciprocity); and the number of examination hours taken by the applicant; 5) Evidence of successful completion, or waiver by the Board, of the applicable in-training Major Requirements for Certification:

1) Provide written documentation to Industrial Commission that applicant holds a certificate of competency as a inspector of boilers or lined hot water storage heaters for a state that has a standard of examination substantially equal to that of Arizona or the applicant holds a commission as an inspector of boilers and pressure vessels issued by the National Board of Boiler and Pressure Vessel Inspectors, and applicant provides proof of employment as a full- time inspector for a company operating or insuring boilers or lined hot water storage heaters in Arizona and whose duties as an inspector include making inspections of boilers or lined hot water storage heaters to be used or insured by the company and not for resale;2) If applicant doesn't meet criteria in No. 1, applicant must complete an application to take the National Board of Boiler and Pressure Vessel Inspectors Examination on a form approved by the Industrial Commission at least 45 days before the date of the examination, showing education and work experience. If applicants meets the education and work experience criteria, then applicant must pass written examination (minimum score of 70%) of the National Board of Boiler and Pressure Vessel Inspectors and an oral interview by the Industrial Commission.Certification Fee: NoneContinuing Education: 8 hoursMajor Requirements for License:

1) Applicant must be at least 18 years old. No boxer over 32 years of age shall be granted a license except by special action of the Commission considering an applicant's demonstrated competence, status as a boxer and physical condition;2) Applicant must be examined by a Commission physician and satisfy the Commission that he has the ability to compete;3) Provide proof of true identity, and other material information requested on the license application and otherwise required by the Commission4) Expenses necessarily incurred by the Commission in the investigation of an applicant will be charged back to the applicant unless suitable provision therefore has been made in the Commission's budget;Reciprocity: NoLicense Fee: Initial - $10; Renewal (Annually by Dec. 31) - $10Continuing Education: None

Major Requirements for License (for following occupations):

Boxing Judge, Referee, Physician, Timekeeper, Announcer, Second Manager, Match Maker, Trainer; and Promoter

1) Provide proof of true identity, and other material information requested on the license application and otherwise required by the Commission2) Expenses necessarily incurred by the Commission in the investigation of an applicant will be charged back to the applicant unless suitable provision therefore has been made in the Commission's budget;

License -- All licenses are for a calendar year and expire on Dec. 31; Fees are the same for initial and renewal -- Manager, $50; Promoter, $200; Matchmaker, $100; Trainer, $10; Second, $10; Judge, $25, Referee, $25; Timekeeper, $10; Announcer, $25; Ringside Physician, $25.Continuing Education: NoneMajor Requirements for License:

License by Exam --

1) Graduation and transcript from an accredited chiropractic college or that meets the standards of education for accreditation contained in The Council on Chiropractic Education Standardsfor Doctor of Chiropractic Programs and Institutions.

2) Certificate of attainment or Diplomat on NBCE Parts I and II and a score of 375 or better on NBCE Parts III and IV;

3) Passage (score of 75% or higher) on the Arizona Jurisprudence Examination;

4) List of all other states or jurisdictions in which the applicant is or has been licensed or certified to practice chiropractic or any other health care profession with a verification of good standing for each current license or certification submitted directly by the licensing agency of the other states or jurisdictions;

5) Name and professional designation of the owner or owners of the clinic or office at which the applicant will be employed;

6) Application and Fingerprint Fee - $280

License by Reciprocity --

Arizona has reciprocity with four states: New York, Colorado, Missouri and Louisiana. To be eligible forreciprocity:1) passed the licensing exam in the reciprocity state and practiced continuously at least three (3) out of the last five (5) years; 2) passed NBCE exam Parts I - III or NBCE Parts I - II and SPEC, regardless of when graduated or howlong one has practiced; score of 375 or better on NBCE Part III or SPEC3) Pass the Arizona jurisprudence exam (see above).4) Pay application and certification fees.

License Fees: Initial License -- $100; Renewal (Annually on or before Jan. 1) -- $100

Continuing Education: Complete 12 credits annually. One credit equals 60 minutes of education.

Major Requirements for following licenses:

Aesthetics (8,258 licenses), Nail Technology (24,307 licenses), and Cosmetology (59,491 licenses) License:

License by Examination --

1) Proof of completing 10th grade equivalency;

2) Proof of graduation from a Board-approved school or, if applicant attended more than 1 school, copy of transfer application or certification of hours from each school including starting and ending dates..

3) Passage of examination; Exam fees -- Written exam: $85; Practical exam -- $60

Fees:

License -- Initial, $40; Renewal (annually on or before birthday of next year) -- $30

Major Requirements for License by Reciprocity --

1) Certification of hours and proof of graduation or licensure in another state or a foreigncountry with initial and final dates of licensure; Certification Fee -- $30;

2) If previously licensed by the Board, type of license, license number, license expiration date, andthe name used on the license; 3) Statement of whether the applicant has ever had an aesthetics, cosmetology, or nail technology license suspended or revoked in any state or foreign country.

License Fees: Reciprocity License - $110; Renewal - See above.

Continuing Education: None

Major Requirements for Dental Assistant Radiography Certification :

By Credential --

1) Current certificate or other form of approval for taking dental radiographs, issued by a professional licensing agency in another jurisdiction of the United States that required successful completion of written and clinical dental radiography examinations or a single dental radiography examination with written and clinical components. States currently recognized by Arizona: California, Montana, New Mexico (If you took the NM Exam), Oregon and South Dakota. For states not listed, call the Licensure Manager for the Arizona Dental Board.2) Letter of endorsement that verifies compliance with R4-11-204.

Continuing Education: None required

Renewal: None required

Fees -- None

Major Requirements for Coronal Polishing Certificate:

5) Verification of certification/licensure/registration of each state in which the applicant holds or has held certification, licensure, or registration as a physician assistant;

6) List of all current or past employment with medical agencies or supervising physicians within the five years preceding the date of application or since graduation from a physician assistant program, if less than five years, including the agency or physician name, address, and date of employment;

1) Whether applicant holds a Certificate of Professional Qualification in Psychology, a National Register of Health Service Providers in Psychology credential, or is a diplomat of the American Board of Professional Psychology;2) Name of each jurisdiction in which the applicant is currently or has been licensed as a psychologist;3) History of employment in the field of psychology including the beginning and ending dates of employment, number of hours worked per week, name and address of employer, name and address of supervisor, and type of employment;4) Name, position, and address of at least two references who: a) are psychologists licensed or certified to practice psychology in a United States or Canadian jurisdiction and who are not members of the Arizona Board of Psychologist Examiners; b) are familiar with the applicant's work experience in the field of psychology or in a postdoctoral program within the three years immediately before the date of application. If more than three years have elapsed since the applicant last engaged in professional activities in the field of psychology or in a postdoctoral program, the references may be from the most recent three-year period in which the applicant engaged in professional activities in the field of psychology or in a postdoctoral program; and c) Recommend applicant for licensure;5) Name and address of each university or college from which the applicant graduated, date of attendance, date of graduation, degree received, name of department, and major subject area;6) official transcript from each university or college from which the applicant has received a graduate degree that contains the date the degree was received;7) official document from the degree-granting institution indicating that the applicant has completed a residency that satisfies the requirements of A.R.S. 32-2071(H);8) an affidavit from the applicant's supervisor, if available, or a psychologist knowledgeable of the applicant's internship training program, verifying that the applicant's internship training program meets state's requirements; an affidavit from the applicant's postdoctoral supervisor, if available, or a psychologist knowledgeable of the applicant's postdoctoral experience verifying that the applicant's postdoctoral experience meets the state's requirements;9) official notification of applicant's score on the national examination is provided to the Board.10) Application Fee - $350

The 15-hour qualifying USPAP course taken through distance education or taken more than two years preceding the date of the application is not accepted. Also required, associate degree, or higher, OR college transcript of successfully passing a total of at least 21 semester credit hours of collegiate subject matter courses in (1) English composition; (2) principles of economics, micro or macro; (3) finance; (4) algebra, geometry or higher mathematics; (5) statistics; (6) introduction tocomputers-word processing/spreadsheets; and (7) business or real estate law.

Certified General -- 300 qualifying course hours as specified by Appraisers Qualification Board 2008 Core Curriculum. Qualifying education courses taken through distance education may not exceed 75% of the required 300 hours. The 15-hour qualifying USPAP course taken through distance education or taken more than two years preceding the date of the application is not accepted. All courses must be approved to meet the 2008 Core Curriculum. Also required is a bachelors degree, or higher, OR college transcriptof successfully passing a total of at least 30 semester credit hours of collegiate subject matter coursesin (1) English composition; (2) micro economics (4) macro economics; (4) finance; (5) algebra,geometry or higher mathematics; (6) statistics; (7) introduction to computers-wordprocessing/spreadsheets; (8) business or real estate law; and (9) two elective courses in accounting,geography, ag-economics, business management, or real estate.

2) Minimum Experience:

Licensed Residential -- 2,000 hours in not less than 18 months;Certified Residential -- 2500 hours in not less than 24 months. Not less than two completed reports must be multi-family. Experience must have been acquired within 10 years immediatelypreceding the filing of the application for certification;Certified General -- 3000 hours in not less than 30 months. At least 1500 hours must reflect nonresidential appraisals. Experience must have been acquired within 10 years immediately precedingthe filing of the application for certification.

Real Estate Broker and Associate Broker (Licenses - 14,794 ; Active Licenses - 12,820)Major Requirements for License:1) 18 years of age2) At least three years' actual experience as a licensed real estate salesperson or real estate broker during the five years immediately proceeding the time of application. 3) If for an original cemetery broker's license, either a current real estate broker's license, or if the applicant does not have a current real estate broker's license, at least three years' actual experience as a cemetery salesperson or broker or as a licensed real estate salesperson or broker during the five years immediately preceding the time of application.4) If for an original membership camping broker's license, either a current real estate broker's license, or if the applicant does not have a current real estate broker's license, at least three years' actual experience as a licensed membership camping salesperson or broker or as a licensed real estate salesperson or broker during the five years immediately preceding the time of application.5) If for any type of broker's license, not had a license denied within one year immediately preceding application in this state or a similar statute in any other state.6) Complete the Certification Questionnaire, plus each broker who employed applicant in the previous 5-year period must complete a Broker Candidate Experience Verification form.7) Complete 90 hours of pre-licensing education and pass the examination at a real estate school, college or university. Salesperson (Licenses - 67,919 ; Active Licenses - 42,554)Major Requirements for License1) At least 18 years of age2) Complete 90 hours of pre-licensing education and pass the certification examination at a real estate school, college or university. (License applicants who do not reside in this state are required to complete a minimum of a 27-hour course that is specific to this state's real estate statutes, rules, practices and procedures and that is prescribed and approved by the Commissioner and are required to pass the real estate school examination before taking this state's examination. Applicants with previous real estate education, can also apply for and be granted a waiver of the prelicensure courses. Not be on parole, on community supervision, on work furlough, on home arrest, on release on any other basis or named in an outstanding arrest warrant; not be either of the following: a) adjudicated mentally incompetent; b) found to constitute a danger to self or others; not have a disability as defined in section ARS 41-1461, unless that person is a qualified individual with a disability as defined in section ARS 41-1461; and not have been convicted of acting or attempting to act as an associate security guard or armed security guard without a license if a license was required.4) Successfully complete all background screening and training requirements.5) Completes eight hours of pre-assignment training before applying for and being granted a security guard registration certificate;6) Complete a training verification form with a signature from an instructor and a qualifying party affirming that the security guard met the training requirements;7) For armed security guard, in addition to Nos. 5 and 6, complete 16 hours of firearms-safety instruction8) For armed security guard, complete a firearms-safety training verification form with a signature from an instructor and a qualifying party affirming that the security guard met the training requirements.

Independent Substance Abuse Counselor (1,750 licenses) --1) Master's or higher degree from a regionally accredited college or university in a behavioral health service with a minimum of 24 semester credit hours of counseling related coursework as determined by the substance abuse credentialing committee. Coursework must include a significant clinical component, such as statistics coursework. Graduate coursework completed outside of the master or higher degree and submitted to meet curriculum requirements shall constitute no more than three semester credit hours;2) Pass Level II or higher examinations offered by the National Association of Alcoholism and Drug Abuse Counselors.3) Complete a minimum of 3,200 hours of supervised work experience in substance abuse counseling in no less than 24 months. The supervised work experience is limited to the use of psychotherapy for the purpose of assessment, diagnosis and treatment of individuals, couples, families and groups as they relate to substance abuse and chemical dependency issues. The 3,200 hours of supervised work experience shall include a minimum of 1,600 hours of direct client contact.4) During the supervised work experience required in R4-6-705, an applicant for associate substance abuse counselor and independent substance abuse counselor licensure shall receive a minimum of 100 hours of clinical supervision in no less than 24 months. 5) During supervised work experience applicant shall demonstrate satisfactory performance in the following areas as they relate to substance abuse counseling: intake, diagnostics, assessment, triage, crisis intervention, treatment planning, family, group and individual psychotherapy, outreach, and consultation with other professionals by having the applicant's clinical supervisor submit a performance evaluation on forms available from the Agency.

Associate Substance Abuse Counselor (118 licenses) --

1) Bachelor degree from a regionally accredited college or university in a behavioral health service with a minimum of 30 semester credit hours of counseling related coursework as determined by the substance abuse credentialing committee; or a master's or higher degree from a regionally accredited college or university in a behavioral health service with a minimum of 24 semester credit hours of counseling related coursework as determined by the substance abuse credentialing committee. The Board shall not accept coursework that does not include a significant clinical component, such as statistics coursework, as meeting the coursework requirements in this Section.2) Pass Level II or higher examinations offered by the National Association of Alcoholism and Drug Abuse Counselors.3) Complete a minimum of 3,200 hours of supervised work experience in substance abuse counseling in no less than 24 months. Major Requirements for Certification:

General Requirements --

1) The evaluation to determine qualification for certification shall not begin until an institutional recommendation or application for certification and official transcripts, and the appropriate fees have been received by the Department. Course descriptions, verification of employment, and other documents may also be required for the evaluation.2) The effective date of a new certificate shall be the date the evaluation is completed by the Department. The effective date of a renewed certificate shall be the date the evaluation for renewal is completed by the Department;3) All one-year certificates shall expire one year from the date of issuance. All certificates issued for more than one year shall expire on the holder's birth date in the year of expiration.4) If an applicant has not met all the requirements for the certificate or endorsement at the time of evaluation, the applicant shall have a maximum of two years to complete those requirements and request re-evaluation. One re-evaluation shall be provided at no additional fee within two years of the original evaluation.5) Only those degrees awarded by an accredited institution shall be considered to satisfy the requirements for certification.6) Professional preparation programs, courses, practical, and examinations required for certification shall be taken at an accredited institution or a Board-approved teacher preparation program.7) Only those courses in which the applicant received a passing grade or credit shall be considered to satisfy the requirements for certification.8) Teachers in grades 7 through 12 whose primary assignment is in an academic subject shall demonstrate proficiency by passing the appropriate subject area portion of the Arizona Teacher Proficiency Assessment. The subject areas of demonstrated proficiency shall be specified on the certificate. If a proficiency assessment is not offered in a subject area, an approved area shall consist of a minimum of 24 semester hours of courses in the subject;9) Teachers of homebound students shall hold the same certificate that is required of a classroom teacher.10) Effective January 1, 2008, all teachers and persons who are required to be fingerprinted to work in the classroom are required to have an Identity Verified Prints (IVP) Fingerprint Clearance Card. A valid non-IVP Fingerprint Clearance Card issued prior to January 1, 2008, will be accepted by Certification. Non-IVP Fingerprint Clearance Cards issued after January 1, 2008 will not be accepted.

Birth-Kindergarten Certification --1) Must hold an Early Childhood certificate or endorsement by July 1, 2012.2) Bachelor's degree3)

Elementary Teaching Certification --Provisional Certification (10,547 certificates):1) Bachelor's degree;2) One of the following: a) completion of a teacher preparation program in elementary education from an accredited institution or a Board-approved teacher preparation program; or 45 semester hours of education courses which teach the knowledge and skills of Arizona professional teaching standards, including at least eight semester hours of practicum in grades K-8. By Examination --1) U.S. citizen 21 years of age and high school graduate or successfully completed GED and hold a valid Arizona driver's license (Class 2 or higher);2) Seek appointment to a law enforcement agency;3) Undergo a background investigation, a medical examination and a polygraph test. AZ POST audits all agency employment packages to ensure that the qualifications for certification are met;4) Enroll and successfully complete all requirements of basic peace officer course (police academy with minimum of 585 hours of prescribed mandated training).5) If applicant had prior certification and service as a peace officer in Arizona and the date of the last separation of service is less than three years from the new date of appointment, applicant can be recertified as a peace officer without being required to attend and successfully complete an academy. The appointee would, however, be required to submit to a background process that would include a physical examination and polygraph examination.

By Waiver --1) If applicant served honorably as a peace officer in another state or with a federal agency and meets all of the AZ POST requirements for eligibility, applicant may utilize the AZ POST Waiver Process.
