IN THE SPOTLIGHT

Around 10 per cent of all 999 calls to the Welsh Ambulance Service are for people who have had a fall. The Service has produced a video about what to do if you have fallen at home. The video, available to watch on YouTube, highlights the importance of using your personal alarm and shows how to get up from a fall.

To learn more about simple exercises you can do at home to maintain muscle strength and reduce your chance of falling, the Chartered Society of Physiotherapy has produced a video on YouTube called ‘Falls prevention exercises’.

Edward O’Brian End of Life Lead, Macmillan paramedic, Clinical Support TeamMy ambulance service career started in 2002 beginning with the Non-Emergency Patient Transport Service (NEPTS) in Cardiff before then moving to London Ambulance Service a few months later to train as an Emergency Medical Technician. In 2009 I moved back to Wales and back to the Welsh Ambulance Service, I qualified as a Paramedic in 2014.

In 2016 I joined the Clinical Support Team. The team focus on several areas of work such as identifying and managing frequent callers. A frequent caller will often have an unmet clinical and/or social need and become reliant on unscheduled care for their support. By working in partnership with the patient, Local Health Boards and other health, social care and voluntary agencies, we aim to support them in accessing the help they need to avoid any future reliance on the unscheduled care system. This initiative has proven to be very successful in improving patient care and helping patients to maintain their independence, reduce the number of 999 calls to the Welsh Ambulance Services NHS Trust (WAST) and

admissions to Emergency Departments. The team also work with frequent service users from nursing and residential homes. A big focus recently by the team has been around residents that have had a fall. A lot of training has been undertaken with care staff in the assessment of a resident after their fall to empower them to be able to better support their residents, and reduce the need to access 999.

In addition to the work with frequent callers I’m also the End of Life Lead for the Trust. This is a role I feel very passionate about, and a job I feel privileged to be doing. As a Trust we frequently respond to end of life care patients, so my role is about assisting WAST clinicians to have the additional support, training and referral options needed to allow them to treat their patient’s symptoms in the community and prevent unnecessary and unwanted hospital admissions. I’m also working with NEPTS colleagues to assist in the setup of the End of Life Care Rapid Transport Service. This is a priority service that the Trust has introduced to ensure we

transport, wherever possible, End of Life Care patients that are dying to their preferred place of death in a timely manner. It’s about us working with palliative care teams to provide a service that contributes to a dignified and comfortable death, the service that NEPTS control staff and crews are providing is incredible. They are making a real difference to patients and their relatives at such a difficult time.

DefibuaryWhen someone goes into cardiac arrest, their heart goes into a life threatening rhythm and stops pumping blood around the body and to their brain. Without early intervention of Cardiopulmonary Resuscitation (also known as CPR) and defibrillation, a patient’s chance of surviving decreases by 10 per cent every minute.

To raise public awareness about defibrillators, throughout February we held our annual Twitter #Defibuary campaign. As well as educating the public about what they are, how they are used and where you can find them, we also encouraged staff, volunteers and the public to tweet us a selfie with a defib giving us their location details. This information is then shared on the defib location database available on the NHS Direct Wales website and with our 999 colleagues.

We would like to say a massive thank you to everyone who got involved submitting 187 selfies. 39 of them were unregistered locations which we have now added onto the system. We would also like to thank our partners for their kind generosity for offering fanastic prizes for the best selfies.

DEMENTIA GUIDEAs a Dementia Friendly Community, one of our commitments in the Welsh Ambulance Service Dementia Plan was to develop useful information for staff & volunteers, to help them better understand what it means to live with dementia, and provide helpful advice about improving communication with dementia patients and their carers. The guide was consulted on with staff and people living with dementia and was launched during Dementia Action Week. “My wife aged 76 fell on her hip (x-ray proved it was broken). She was attended to by your ambulance paramedics. We write to commend your attention to the two medics, who performed in a superb professional manner, most cheerful and helpful.”

”Called 999 yesterday because my 72 year old mother was having trouble breathing. She has COPD, a chest infection, pleurisy. Seen a GP twice in a week. Prescribed steroids and antibiotics but no improvement. First responder was wonderful. Lots of care and reassurance. Fantastic service all round including the call centre who kept in touch and provided help advice and reassurance. Amazing. Thank you.”

“On March 8th I fell in the snow in town and was admitted to Hospital and the next day I had an operation to repair a broken leg. I just felt I had to write and let you know how well I was treated. The Paramedics called later to see how I was and visited me on the Sunday.”

Will’s StoryWill, an Emergency Medical Technician (EMT) who has been learning British Sign Language (BSL) explains how he recently put his learning into practice.

“I was asked to attend a 55 year old female patient who had fallen and was deaf. On arrival, I found the patient sat in a shop having received first aid from the staff. It was apparent the shop staff were unable to communicate effectively with the patient.

The patient was very distressed and in a lot of pain. I introduced myself allowing her to lip read whilst confirming what I was saying using BSL. At first she didn’t realise and communicated with me using just her voice. After taking her into the ambulance and making conversation about her home life, social history and medications, she realised I was using BSL and that I was a new learner. Her face beamed, from this point on she also began to use BSL to confirm what she was saying. It was obvious that by using BSL, it had allowed us to create a bond which had a therapeutic effect on her perception of pain and distress over what had happened.

From this experience I feel it is of huge benefit both to those that are deaf, and also ourselves as BSL learners to aid us in creating these bonds and breaking down the barriers that exist simply by lack of understanding.

I am immensely proud of the lengths we as BSL learners are going to, and am very grateful to the Trust for giving us this opportunity. I hope my first experience will encourage others to follow suit.”

BREAKING DOWN BARRIERSWe know from visiting deaf and hard of hearing groups, the communication barriers patients can experience when accessing services. As a Service, we are continually looking at ways to improve this. Following the launch of the pre hospital communication app in 2016, we have now secured funding for 110 members of staff to learn online Level 1 British Sign Language. As well as giving staff the opportunity to learn a new language, we hope that knowing a few signs will help people feel at ease and break down communication barriers.

Visiting local communitiesWe continued to visit different communities across Wales at open days, health fairs and partnership events. We listened to your experiences, answered your questions and shared information about campaigns and improvements happening across the service including how we respond to calls, appropriate use of 999, our Promises to Children & Young People, Careers within the Welsh Ambulance Service, Dementia awareness sessions and our Promises to Older People.

Some of the people and events we visited included Beavers (Scout Association), Cwmdare older people’s group, Chinese elderly club, Swansea Pride, Ebbw Vale Emergency Services day, Non Emergency Patient Transport Service staff attending their Dementia awareness session and Future Powys Careers festival.

Online Symptom CheckersThe summer can sometimes bring unwanted common health problems such as allergies, hay fever, stings and sunburn. If you need advice and are not sure what to do, visit our online symptom checkers available on the NHS Direct Wales website www.nhsdirect.wales.nhs.uk. With over 25 to choose from you can also get advice on other symptoms including dental pain, nosebleed, lower back pain, diarrhoea, cough and contraception.

MenACWY Vaccine What is it and who should have it?The MenACWY vaccine protects against four strains of meningococcal disease. An infectious disease that can be fatal as it causes Meningitis and Septicaemia and is spread through close persistent contact (living in the same house, coughing, sneezing and kissing). The vaccine is routinely offered to young people at around 13 years of age. The vaccine is also available to those born after 1st September 1996, and university freshers under 25. Those who have not yet received the vaccine should contact their GP surgery or school nurse as soon as possible. The vaccine does not protect against all causes of meningitis and septicaemia so it is important to know the signs and symptoms. For further information, visit the ‘Children and teens’ tab under the ‘Vaccinations’ section on the NHS Direct Wales website.

CANLLAWIAU DEMENTIAFel Cymuned Cyfeillgar i Ddementia, un o’n hymrwymiadau yng Nghynllun Dementia Gwasanaethau Ambiwlans Cymru oedd datblygu gwybodaeth ddefnyddiol i staff a gwirfoddolwyr. Mae hyn i’w helpu i ddeall yn well beth mae’n ei olygu i fyw gyda dementia a rhoi cyngor defnyddiol am wella cyfathrebu gyda chleifion dementia a’u gofalwyr. Roedd staff a phobl sy’n byw gyda dementia’n ymgynghori ar y canllawiau a lansiwyd ef yn ystod Wythnos Gweithredu Dementia. ...dyma’r hyn rydych chi wedi’i ddweud wrthym niMae’r galw am ein gwasanaethau’n parhau i gynyddu - yn y flwyddyn ddiwethaf derbyniwyd 483,109 o alwadau i’n gwasanaeth ambiwlans brys 999. Rydyn ni’n parhau i weithio ar wella ein gwasanaethau ac yn gwerthfawrogi’r adborth a dderbyniwn gan ddefnyddwyr gwasanaethau; defnyddir yr adborth i ddysgu a gwella. Dyma rai o’r sylwadau a dderbyniwyd.

BETH YW EICH STORI CHI? RYDYN NI’N GWRANDO

Syrthiodd fy ngwraig 76 oed ar ei chlun (profodd y pelydr-x ei bod wedi’i thorri). Roedd eich parafeddygon yn ei thrin. Ysgrifennwn i ganmol y sylw gan y ddau swyddog meddygol, oedd yn ymddwyn mewn modd hynod broffesiynol, yn siriol ac yn garedig”.

Ffoniais 999 ddoe oherwydd roedd fy mam 72 oed yn cael trafferth anadlu. Mae’n dioddef gan COPD, haint ar y frest a llid yr ysgyfaint. Gwelodd Feddyg Teulu ddwy waith mewn wythnos. Cafodd steroids a gwrthfiotigau ond dim gwelliant. Roedd y ymatebwr cyntaf yn wych. Llawer o ofal a sicrwydd. Gwasanaeth gwych i gyd gan gynnwys y ganolfan alw oedd yn cadw mewn cysylltiad ac yn rhoi cymorth, cyngor a sicrwydd. Anhygoel. Diolch yn fawr.

Ar 8 Mawrth, syrthiais yn yr eira yn y dref a gorfu i mi fynd i mewn i’r Ysbyty. Drannoeth, cefais lawdriniaeth i drwsio fy nghoes oedd wedi torri. Teimlwn fod yn rhaid i mi ysgrifennu atoch i ddweud pa mor dda roedd y driniaeth a gefais. Galwodd y Parafeddygon yn hwyrach i weld sut oeddwn ac roedden nhw’n ymweld â mi ar y Sul.

Stori WillMae Will, Technegydd Meddygol Brys (EMT) a fu’n dysgu Iaith Arwyddion Prydain (BSL) yn egluro sut roedd yn rhoi ei ddysgu ar waith yn ddiweddar.

“Gofynnwyd I mi fynd at wraig fyddar 55 oed oedd wedi syrthio. Pan gyrhaeddais, gwelais fod y claf yn eistedd mewn siop ar ôl cael cymorth cyntaf gan y staff. Roedd yn amlwg nad oedd staff y siop yn gallu cyfathrebu’n effeithlon gyda’r claf.

Roedd y claf yn drallodus iawn ac yn boenus iawn. Cyflwynais fy hun gan ganiatáu iddi ddarllen gwefusau wrth gadarnhau’r hyn roeddwn yn ei ddweud drwy BSL. Ar y dechrau, nid oedd yn sylweddoli ac roedd yn cyfathrebu gyda mi drwy ddefnyddio llais yn unig. Ar ôl ei rhoi yn yr ambiwlans, a sgwrsio â hi am ei bywyd gartref, ei hanes cymdeithasol a’i meddyginiaethau, sylweddolodd fy mod yn defnyddio BSL a fy mod yn ddysgwr newydd. Roedd ei hwyneb yn goleuo ac o hyn ymlaen, dechreuodd hithau ddefnyddio BSL i gadarnhau’r hyn roedd yn ei ddweud. Roedd yn amlwg wrth ddefnyddio BSL ei fod wedi caniatáu i ni greu cyswllt oedd wedi cael effaith therapiwtig ar ei chanfyddiad o boen a’i thrallod am yr hyn oedd wedi digwydd.

O’r profiad hwn, teimlaf ei fod o fudd anferthol i’r rhai sy’n fyddar a hefyd i ni fel dygwyr BSL i’n helpu i greu’r cyswllt hwn a chwalu rhwystrau sy’n bodoli oherwydd diffyg deall. Rydw i’n hynod falch o’r hyn rydyn ni fel dysgwyr BSL wedi’i gyflawni ac yn hynod ddiolchgar i’r Ymddiriedolaeth am roi’r cyfle hwn i ni. Gobeithio y bydd fy mhrofiad cyntaf i yn annog eraill i wneud yr un peth”.

CHWALU RHWYSTRAUGwyddom, wrth ymweld â grwpiau byddar a thrwm eu clyw, am y rhwystrau cyfathrebu y gall cleifion eu profi wrth gysylltu â gwasanaethau. Fel Gwasanaeth, rydyn ni bob amser yn chwilio am ddulliau i wella hyn. Yn dilyn lansiad yr ap cyfathrebu cyn ysbyty yn 2016, rydyn ni’n awr wedi sicrhau cyllid ar gyfer 110 aelod o’r staff i ddysgu Iaith Arwyddion Prydain Lefel 1 ar-lein. Ynghyd â rhoi cyfle i staff ddysgu iaith newydd, gobeithiwn y bydd gwybod ychydig o arwyddion yn helpu pobl i deimlo’n gyfforddus a chwalu rhwystrau cyfathrebu.

Ymweld â chymunedau lleolRoedden ni’n parhau i ymweld â gwahanol gymunedau ar draws Cymru mewn diwrnodau agored, ffeiriau iechyd a digwyddiadau partneriaeth. Roedden ni’n gwrando ar eich profiadau, yn ateb eich cwestiynau ac yn rhannu gwybodaeth am ymgyrchoedd a gwelliannau sy’n digwydd ar draws y gwasanaethau gan gynnwys: sut rydyn ni’n ymateb i alwadau, y defnydd priodol o 999; ein Haddewidion i Blant a Phobl Ifanc, Gyrfau o fewn Gwasanaethau Ambiwlans Cymru, sesiynau ymwybyddiaeth Dementia a’n Haddewidion i Bobl Hyn.

Rhai o’r bobl a’r digwyddiadau roedden ni’n ymweld â nhw oedd y Beavers (Cymdeithas Sgowtiaid), Gr wp Henoed Cwmdâr, Clwb Henoed y Tsieineaid, Pride Abertawe, diwrnod Gwasanaethau Brys Glyn Ebwy, staff Gwasanaeth Cludo Cleifion nad ydynt yn Rhai Brys oedd yn mynychu eu sesiwn ymwybyddiaeth Dementia a gwyl Gyrfau Dyfodol Powys.

Gyda dros 25 i ddewis ohonyn nhw gallwch gael cyngor am symptomau eraill gan gynnwys y ddannodd, gwaedlin o’r trwyn, poen cefn isel, dolur rhydd, peswch ac atal cenhedlu.

Brechlyn MenACWY Beth yw hwn a phwy ddylai ei gael?Mae’r brechlyn MenACWY yn diogelu rhag pedwar math o glefyd meningococcal. Gall y clefyd heintus hwn fod yn farwol oherwydd gall achosi Llid yr Ymennydd a Gwenwyniad Gwaed ac mae’n lledaenu drwy gyswllt cyson agos (byw yn yr un t y, peswch, tisian a chusanu). Mae’r brechlyn yn cael ei gynnig yn arferol i bobl ifanc tua 12 oed. Mae’r brechlyn ar gael hefyd i’r rhai sydd wedi’u geni ar ôl 1 Medi 1996 a myfyrwyr blwyddyn gyntaf mewn prifysgol o dan 25 oed. Dylai unrhyw un nad yw eto wedi cael y brechlyn, gysylltu â’i feddygfa neu nyrs ysgol gynted ag sy’n bosibl. Nid yw’r brechlyn yn amddiffyn rhag pob math o lid yr ymennydd a gwenwyniad gwaed felly mae’n bwysig gwybod beth yw’r arwyddion a’r symptomau.
