3.Get to knowthe doctor’squalifications
• Check quality scores. Your healthplan monitors how well doctorscare for their patients acrossmany different measures and thencombines them into a single scorefor you. The higher the number, thebetter the quality score.
• Consider doctor ratings. Otherpatients rate their experience withthe doctor on a scale of 1 to 5.
• Look for recognitions. A doctor’scertifications, designations andachievements will be listed so youcan make an educated decision onwho will be the best fit for you.
Bring these items to your first appointment:
Your Wellmark Blue Cross and Blue Shield ID card. It hasall the information you need to receive services or get a prescription filled.
Your driver’s licenseor another valid photo ID.
The prescription medications and vitamins you’recurrently taking.
A copy of your medical records; if you don’t havethese, you can ask your previous doctor to send them.
Your health history,including chronic conditions and illnesses and previous surgeries.

Do you have a patient portal?
What’s the best way to get a prescription refilled?
How far in advance will I need to make an appointment?
