Before you beginmyProvidenceisyoursecuregatewaytothetools andresourcesyou’llneedasaPEBBmember participatingintheHealthEngagementModel. Beforeyoubegin,registerfor,oraccessyourmyProvidenceaccountatwww.myProvidence.org.
Your path to success Congratulations and welcome! You’ve started the journey to greater knowledge and better health by signing up to participate in PEBB’s Health Engagement Model. As your plan administrator, Providence Health Plan will support you on your track to success. Use this guide to help navigate your journey to a healthier life. It includes step-by-step instructions to help you achieve success in PEBB’s Health Engagement Model.
To access your myProvidence account or sign up for onevisit www.myProvidence.org. You’ll see a screen that looks like this:
If you already have a myProvidence account:
1. Enter your user name
2. Enter your password
3. Select the Log in now button
myProvidencehasbeenre-vitalizedwithnewfeatures,newnavigationandanewlookandfeel.IfthisisyourfirsttimeusingthenewversionofmyProvidence,youwillbepromptedtosetanewpassword,setapasswordhintandselectasecurityquestion.
If you already have a myProvidence account but don’t remember your user name and password:

2. Selecttheorange“Getstarted”button.

If you don’t have a myProvidence account: Tocreateanaccount,besuretohaveyourProvidenceHealthPlanmemberIDcard.You’llneedyourgroupandmemberIDnumbers,whicharelocatedonyourcard.
1. Under“First time here, or need help logging in?”enteryournameandbirthdateandselecttheorange“GetStarted”button.You’llbedirectedtothemyProvidenceAccountInformationpage.
2. Completethefields,includinggeneralinformationandsecurityquestions.Theredasteriskindicatesthatthefieldisrequired.
3. Reviewandagreetotheuseragreement.
Please note: When you register, or access
your myProvidence account,
you may see a message
at the top of your screen
requesting that you enter an
activation code. This code is
required for some site users
as an added-level of security.
Entering this unique and
secure code gives you access to
additional information through
myProvidence, including your
claims data. If you do not have
an activation code and are being
prompted for one, you can
request that one be sent to you.
Take a health assessment … and set your sights on success. Selecttheorange“StartAssessment”buttononthemyProvidencelandingpage(seenumber1). Completingthequestionnairetakesapproximately15minutes.It’ssecureandconfidential. Youdecidewho,ifanyone,seesyourindividualresults.Afteryoucompleteyourassessment, you’llreceiveimmediatefeedbackindicatingpotentialhealthrisksandsuggestionstominimizeyourrisks.Usetheseresultstosetgoalsthatareimportanttoyourhealth.You must complete the health assessment by March 15, 2012 to avoid a deduction from your pay.
Get engaged in the Health Engagement Model with myProvidenceTaketwostepstoexperiencesuccessintheHealthEngagementModel. Logintowww.myProvidence.orgtostartyourhealthjourney.
Your individual results are private unless you decide to share them.
ProvidenceHealthPlanhaspartneredwiththeUniversityofMichigantoadministertheconfidential,onlinehealthassessment.Individualresultsareforyouandyoualone,unlessyoudecidetosharethem.QuestionNo.62ontheassessmentasksifyouwouldliketoshareyourinformationwithProvidenceHealthPlan.Ifyouansweryes,aProvidencecaremanageroroneofProvidence’shealthcoachingpartnersmaycontactyoutoparticipateinaProvidencecaremanagementorhealthcoachingprogramatnoadditionalcosttoyou.Individualresponsesmayalsobesharedwithyourprovider.Ifyouanswerno,neitherProvidenceHealthPlannoryourproviderwillreceivehealthinformationthatidentifiesyou.PEBBwillneverseeidentifiedindividualresponses.TheonlyinformationthatPEBBwillseeiscollectivereportinginformationthatprovidesasnapshotoftheoverallhealthofPEBB’stotalpopulationandalistofmemberswhodonotcompletetheassessmentorhealthconversations(e-lessons).
Complete two health conversations … learn and go. Onceyoureceiveyourhealthassessmentreport,thenextstepistocompletetwohealthconversations(e-lessons), whichyou’llfindlinkstounder“Self-Help”ontheleftsideofyourmyProvidencescreen.Participatingintheconversationswillhelpyoulearnmoreaboutyourhealthrisksandhowtoreducethembymakingsimple,everydaychanges.You must complete two health conversations by July 15, 2012.
