Your Travel Insurance Policy Summary and Policy Wording Policy Number: 011012 Premier Policy Number: 011012 Advance Policy Number: 011012 Advance (Graduate) • Call the Medical Assessment team to tell them about any pre-existing serious, chronic or recurring illness, injury or disease – see the Medical Declaration for details. Failure to disclose medical conditions before booking trips will result in no cover being provided for claims arising from the undisclosed condition. You must also disclose any serious, chronic or recurring illness, injury or disease suffered by anyone upon whose health your travel plans depend, such as a close relative or travelling companion • Cover is for you, your domestic partner (providing you are both under 70 years of age) and your children under 18 years of age (or under 23 if in full time education) • All insured persons must be UK residents • For holidays in the UK, Channel Islands and Isle of Man, you need at least 2 consecutive nights pre-booked holiday accommodation for the trip to be covered • Trips should be no longer than 31 days and must start and end in the UK Please note there is no cover at all for any trip that is longer that 31 days and we will not insure any part of a trip that is longer than 31 days • No need to register before you travel Please take time to read this booklet as it contains important information about your Worldwide Travel Insurance. • Protectionforeachaccountholderandhis/herdomesticpartnerontripsanywhereintheworld.Childrenunder18yearsofage(under23yearsofageifstillinfulltimeeducationandlivingathomeoutsideoftermtime)arecoveredprovidingtheyaretravellingwiththeaccountholderorhis/herdomesticpartner,orwhilsttravellingabroadontheirowntovisitandstaywithcloserelativeswholiveabroad,forthedurationofthetrip.

• ThemaximumtimeyoucanspendoutsidetheUK,ChannelIslandsandIsleofManinanycalendaryearis183days.

• Eachaccountholderandhis/herdomesticpartnermustbeunder70yearsofageatthestartdate ofanytrip.

• HolidaysintheUK,ChannelIslandsandIsleofManwillonlybecoverediftheyincludetwoormoreconsecutivenightsstayinpre-bookedholidayaccommodation.

Pre-existing medical conditions – important declarations you must make

Pre-existing medical conditions are not covered at any time under this policy unless they have been disclosed to and accepted in writing by the Medical Assessment team.

It is important that you read and fully understand the Medical Declaration on page 20 of this booklet.

Atthetimeofopeningyourqualifyingbankaccountorbookinganytripyoumustensurethatyou calltheMedicalAssessmentteamifanyinsuredperson:1.hasreceivedadvice,medicationortreatmentforanyserious,chronicorrecurringillness,injuryordiseaseinthelast12months.

2.isunderinvestigationorawaitingresultsforanydiagnosedorundiagnosedmedicalcondition.3.isonawaitinglistfor,orawareoftheneedfor,inpatienttreatmentforanydiagnosedorundiagnosedmedicalcondition.

4.hasreceivedaterminalprognosis.5.istravellingagainsttheadviceofadoctororwouldbetravellingagainstmedicaladviceiftheyhadsoughtsuchadvice.

YoumustalsocalltheMedicalAssessmentteamifanyinsuredpersonknowsof:1.acloserelativeorclosebusinessassociatewhoisnottravellingwithyouand/or2.atravellingcompanionorpersonyouplantostaywith,whohasaseriousillness,injuryordiseasewhichcouldaffecttheirdecisiontotakeorcontinueatrip.BetweenbookingatripandthedeparturedateorpriortopayinganyfinalbalanceforthetripyoumusttelltheMedicalAssessmentteamifanyinsuredperson,oranyoneuponwhosegoodhealththetripdepends,hasbeenreferredtoaConsultant/Specialistoradmittedtoahospital.

IfthehospitaladmissionorreferraltoaConsultant/Specialistisinrelationtoanewmedicalconditionoraconditionthathaspreviouslybeenacceptedbyus,Cancellationcoverwillapply.Iftheinsuredpersonstillwishestotravel,theMedicalAssessmentteamwilladviseifcoverwillapplywhilstonthetrip.As the Travel Insurance cover will continue for as long as the account holder has a qualifying bank account, and he/she remains eligible for the cover, you must tell the Medical Assessment team about any change in the status or control of any condition previously declared or if you or anyone upon whose good health the trip depends develops another medical condition.
