2 YOUTH FIRST RESPONDER PRE-COURSE WORKBOOKINTRODUCTION

About the pre-course workbook

The pre-course workbook must be completed and all areas signed off prior to the cadet attending the first component of the youth First Responder course. It is the cadets responsibility to ensure that this happens. If the workbook is not completed, the cadet cannot attend the First Responder course.

The pre-course workbook covers topics that are included in the first two days of the First Responder course. These topics will be revised throughout the course – but will not be retaught completely.

There are a number of practical tasks that are required to be completed, alongside a number of worksheets where the cadet will need to research the answers. When doing research, the cadet may use any means – internet, people, books etc. Where required, all signatures must be obtained.

This book can be completed by all gold level cadets, from the age of 13 years.

3. You are called to a 31 year old female who has a history of severe allergic reactions to bee stings. She has been stung by a bee. When you arrive on scene she is lying slumped on the ground, being supported by her partner. She is not unconscious, but is clearly exhausted. She appears very flushed and you can see hives all over her body. She is making high pitched noises each time she inhales. Her partner tells you that she was stung by a bee about four minutes ago and that she deteriorated rapidly. She was not able to use her adrenaline injector but she has it with her. He is afraid to use it. Upon examination you find her to be hot and dry to touch, with a rapid weak pulse and rapid laboured respirations. She has stridor and when you listen to her chest you hear an expiratory wheeze. Her vital signs are as follows:

– Pulse: 130 – Respirations: 26– Blood pressure: 90/60– GCS: 15

Describe your actions. You are 20 minutes from hospital and backup is 10 minutes away. You can use this scenario to set up a scenario with your local St John group.

4. Find a video or audio clip of stridor on YouTube. What does it sound like and what causes Stridor?

3. Why may an elderly person need more assessment if there has been a brief loss of consciousness than a younger person?

4. Write a scenario that involves a patient who has suffered a brief loss of consciousness where the patient has not just ‘fainted’. Once you have done this, discuss with a group of St John cadets and leaders a possible list of causes. At the end of the discussion, tell the group what cause of unconsciousness the patient has.

8. What are the pertinent aspects of respiratory assessment?

9. What signs and symptoms would lead you to believe that a patient has severe respiratory distress?

10. Work in groups of three. One person will be the patient, one will be the assessor (note this must be a leader or ambulance person) and one of you will be the First Aiders. The First Aiders should assess the patient, concentrating on respiratory assessment.

Work through each of the three scenarios below:

a. A patient is found leaning forwards on a chair. When you speak to them they answer you but are only able to speak a few words per breath.

b. The patient is sitting outside against a house. There has been a house fire.

c. The patient is standing in the doorway waiting for you. He looks pale. He tells you in broken sentences, as he gasps for air, that he has been feeling unwell for a week. He is unsure why.
