Dear Colleagues

Oil & Gas industry is hazardous in nature and has witnessed many disasters. While analyzing the incidents of last five years, it is observed that disregard of SOP, equipment failure, poor supervision and competency of personnel remain key reasons of concern.

There is a need to improve the internal system and controls – Job Safety Analysis, Work Permit System, Tool Box talks, Maintenance & Inspection, Supervision, Competency, Internal audit, Safety meetings and Management Review. Leadership need to introspect and strengthen internal system and controls for achieving highest standard of Health, Safety and Environment.

I firmly believe that leaders must cultivate and drive safety culture through an effective system of reporting unsafe acts and unsafe conditions (UAUC) and taking timely actions on them. This will help to mitigate risks by identifying hazards, putting in place suitable safeguards and assuring that safeguards are functioning.

In order to enhance awareness, OISD is regularly publishing Suraksha Chetna, Case Studies & Safety Alerts, which are available on OISD website. As per Official Language Act, from this issue, we have started publishing this magazine in both Hindi and English languages. I would urge industry to further disseminate this knowledge to the last level of operating personnel.

As a part of our efforts on knowledge enhancement, OISD conducts Workshops/ Seminars on regular basis. In the current year, OISD had conducted seven such knowledge sharing sessions and plans to conduct three more in coming month. We believe that Industry has benefitted from such workshops seeing the enthusiasm and participation in these endeavors.

During the year 2020-21 OISD has recorded around 7.5% increase in number of Safety Audits compared to previous year and historically highest audits till date. I would also like to share with industry members that incident in Oil and Gas industry this year has gone down and in fact lowest in last few years. It should be our endeavors to achieve NIL incident-free workplace, and we should always aim for it.

Friends, COVID pandemic has created challenges for the oil and gas industry in India, but it has also given us an opportunity to excel with utmost safety for a sustainable, and resilient future. We shall keep upgrading our business strategies, aligning sustainability to overall corporate requirement and embracing responsible leadership to meet energy demand of the nation with utmost safety. Second wave of COVID pandemic in India is changing our operational/ maintenance activities at installations in unprecedented ways. All sections of society – including employers and employees – should play a key role to protect themselves and each other and help prevent further spread of the disease.

To enable The Great Reset and to address the needs of all stakeholders, the oil and gas industry must define a new social contract, aligning sustainability to overall corporate strategies, and embracing responsible leadership. By doing so, this sector has the opportunity to contribute towards the energy transition.

Indian petroleum industry is the energy lifeline of the nation and its continuous safe performance at all the times is essential for the economic growth and prosperity of the country. INTRODUCTION

Title: Accidental Removal of Structure Pins Caused Fall of the Dog House.

Location: On-land Drilling Location

Loss/ Outcome: Fatal Injury.

BRIEF OF INCIDENT

Shift in charge conducted Tool Box Talk at start of the morning shift and crew started dismantling work for inter location movement of the rig. The doghouse structure with doghouse positioned on it was alongside the rig floor structure in erected position and its two diagonal supporting beams were pinned with base structure. Injured Person 1 climbed the base structure and released the pins of two diagonal beams, which were holding doghouse structure in its place. As a result, the doghouse structure got destabilized and started drifting away from rig floor and fell down. The skid on which doghouse was placed fell and rested in slanting position on base structure while the dog house tumbled sideways and rested on the ground. IP1, who was on the base structure, was caught in between the falling beam of doghouse structure and base structure. Another IP, who was on the rig floor, close to doghouse also fell with doghouse and sustained injuries. It was noticed that body of the injured person is free, he was removed from there. He along with IP2 shifted to the nearest hospital site ambulance.

Hospital authorities declared IP1 brought dead while IP2 was admitted for treatment. Later he was discharged from the hospital after treatment.

OBSERVATIONS/ SHORTCOMINGS

Ÿ Job Safety Analysis was not carried out before the commencement of the job. Crew members may be unaware about the hazard associated with the operation they were carrying out.

Ÿ Checklist was not prepared and followed prior to lowering of mast, keeping in view of type of rig associated equipment.

Ÿ Injured Person who later succumbed to his injuries though was having 10 years of experience in ONGC, however he was having only eight months experience on this rig.

Ÿ Four cranes were available at site and were deployed at Pipe Rack Area, besides the rig carrier, between rig carrier and Mud Pump, in front of Drilling & Mechanical stores. However, no crane was assigned for lowering/ holding of dog house structure.

Ÿ Sequence of operations to dismantle an erected structure was not followed. The lowering sequence should be the reverse of the raising sequence which implies that dog house should have been removed first.

Ÿ Two persons were deployed on derrick floor to remove railings etc. It appears the two persons on derrick floor had no idea of pins being removed from umbrella pole/ diagonal beams and the team below may not be aware of persons working on derrick floor.

Ÿ SIMOPS checklist/ program in accordance with OISD GDN 186 was in place but prescribed conditions agreed in checklist like were not adhered to. There were four wells on this drill site. Out of these four wells three wells were flowing including the current well.

REASONS OF FAILURE/ ROOT CAUSE

Ÿ Without removing dog house from its structure and holding of dog house structure, pins of the diagonal beams were removed which were holding dog house understructure in vertical position.

Ÿ Adequate supervision was not available for such critical/ hazardous job like dismantling of an elevated structure.

Ÿ Lack of knowledge of hazards on the part of the injured persons and other crew members who were involved in the assigned activity.

RECOMMENDATIONS

Ÿ Rig specific detailed sequence of operations (for all type of rigs) for rigging up and dismantling operation shall be prepared and strictly adhered to avoid such recurrences in future. (Clause 3.4 of OISD-GDN-218)

Ÿ Job Safety Analysis should be carried out in more focused manner so that personnel are aware of the associated hazards and accordingly specific safety instructions to be rendered to working personnel.

Ÿ All Activities should be supervised by competent person(s) as per clause 3.4(1) of OISD-GDN-218.

Ÿ Each team involved in rigging up or dismantling operation should be led by a leader who shall possess experience of carrying out that operation.

Ÿ Before removing pins of an elevated structure, it should be held by the crane to control the stability and movement.

Ÿ Every crew member involved in the job should have clear knowledge about his role and responsibility.

Ÿ SIMOPS checklist/ program in accordance with OISD GDN 186 should be followed strictly.

Ÿ Refresher training programs should be planned for drilling crew to minimize such incident.

Ÿ Lateral stability of doghouse structure needs to be examined. Adequate arrangement to arrest swing of the dog house structure in case of accidental removal of pins of diagonal beams should be done. OEM should be consulted in this regard.

Ÿ Weekly Safety Meeting should be held on all rigs and record be maintained.

Ÿ Mines Manager should carry out HSE inspection of all rigs under their jurisdiction.

Ÿ As per clause 3.4(i i) of OISD-GDN-218, equipment necessary to handle any emergency situation arising during production testing shall not be dismantled/ removed from the site until testing/ well completion is over.

INTRODUCTION

Title: Fire incident in gas terminal at one of the Indian Gas Processing Plant.

Location: Gas Terminal

Loss/Outcome: Fire and Explosion, Damage to Installation Assets

BRIEF OF INCIDENT

Formation of extensive HC vapour cloud followed by a fire & explosion took place in the gas terminal of a gas processing plant. The Gas along with certain amount of condensate being received from Offshore fields was processed at the plant. Gas condensate leaked from the condensate metering skid associated with incoming Gas line. No casualty or injury to any person were reported; But condensate facilities/pipelines and structures in the work station were badly damaged under the impact of the fire.

OBSERVATIONS / SHORTCOMINGS

Ÿ The leakage originated from the sealing bar of the 6” single chamber orifice meter provided for measuring the condensate flow. Correlation could be established for the orifice meter as the source of leakage based on pressure and flow fluctuations observed in downstream inlet line PT and FT to CFU.

Ÿ Six days before the incident, there was leakage from the same orifice meter. Repairing was done by replacement of O-ring on the same day. O-rings were not procured from OEM and there was no record of past POs or shelf life of the item. The investigation team could not establish the traceability and genuineness of the O-ring for assessing its integrity.

Ÿ The job of replacement of orifice meter O-ring was carried out without any set/written down procedure; No investigation was conducted for the incident of leak from the said orifice meter 6 days earlier.

Ÿ Fasteners used in the failed orifice meter were not of standard design; It had different fasteners on

the side, where the nuts got dislodged. On the other side, 02 fasteners are partially threaded and 02 fasteners are full threaded. The standby orifice meter had all the fasteners full threaded. Original design of the fasteners could not be established as drawings were not available.

Ÿ There was no provision for remote operation of the MOVs installed in pipeline header as well as metering skid.

Ÿ HC Gas detector and one H2S detector were available in the area of leakage. No logging/history record of the Gas detectors was available in the control room.

Ÿ Localized Emergency shutdown (ESD) concept was not available.

Ÿ Two pipe lines got dislocated and were in hanging position due to failure of supports. Preliminary observation showed there was no fire proofing provision for the steel truss supports.

REASONS OF FAILURE/ ROOT CAUSE

Ÿ Loss of primary containment (LOPC) due to use of nonstandard spares during maintenance of the orifice meter.

Ÿ The immediate cause of failure of orifice meter was O-ring failure resulting in condensate leakage through the orifice sealing area under high pressure. Improper clamping of sealing surface (Bolts/studs found loose) and/or use of non-standard O-ring might have contributed towards the cause of failure. Failure of O-rings due to gradual loss of clamping pressure was a possible scenario.

Ÿ Delayed detection of the leakage.

Ÿ Delayed response to isolate and depressurize the hydrocarbon circuits. Emergency shutdown system was not available.

Ÿ Inadequate automation of Fire & Gas detection and Emergency Shutdown system (ESD).

Ÿ Integrity of flame proof lighting and associated fittings were not ensured. Electric power, which may act as ignition source for HC vapor cloud, was

not cut off after formation of the HC vapour cloud.

CONCLUSION

Ÿ From the above analysis, it is concluded that the immediate cause of the fire was the result of formation of vapor cloud near the incident site due to leakage (and vaporisation) of gas condensate from the orifice meter and possible ignition from a damaged/ broken lamp cage.

RECOMMENDATIONS

Ÿ Only genuine and standard parts from OEMs to be used for the pressure equipment.

Ÿ SOP shall include normal operation, start up and shut down procedure with responsibility for each activity. Relevant SOPs to be reviewed, signed by competent authority and same shall be made available in the Control Rooms and to concern officials.

Ÿ Standard Maintenance Practice (SMP) to be developed as per manufacturer’s operation and maintenance manual and to follow it religiously during maintenance jobs.

Ÿ Root cause analysis to be carried out for all type of equipment failure and corrective action as recommended shall be implemented horizontally in all other similar equipment.

Ÿ Integrity of flame proof lighting and associated fittings to be ensured.

Ÿ Electric power, which may act as ignition source to be cut off after detecting leak / HC Vapor cloud.

Ÿ All the incidents including LOPC irrespective of minor or major, should be reported and root cause analysis to be done to eliminate the cause and identify the preventive /corrective actions to avoid major incidents in future. As per clause 4.12 of OISD-GDN-206, every incident and near miss incident must be recorded and thoroughly investigated.

Ÿ Automatic shutdown (ESD) facility is to be provided in case of Gas detection and Fire detection and necessary surge analysis to be carried out.

Ÿ Study on Fire fighting system adequacy including Fire / Gas detection system to be done and identified gaps to be bridged in a time bound manner.

Ÿ Requirements of fire proofing of structural members to be re-evaluated in line with OISD-STD-164 and implemented for process area, as per requirement.

Ÿ Trainings on Emergency Response plan to be conducted. Disaster drills need to be carried out periodically to enable faster and more effective response from the concerned personnel.

INTRODUCTION:

Incident Title: Motor Spirit (MS) spillage from Tank

Location: POL Terminal Date & Time: 23.08.20 at about 10:50 hrs.

Loss/Outcome: 75.02 KL MS loss (46.86 lakhs). No injury, fatality, or fire.

INCIDENT DETAIL:

Ÿ The incident happened when Double Block and Bleed Valve (DBBV) was being fitted as the second valve in the new 16" diameter delivery line of MS tank T-18, containing 1607 KL of product, and its Remote operated shut-off valve (ROSOV), fitted earlier as tank body valve, got opened inadvertently.

Ÿ Earlier during the tank outage, this new delivery nozzle was fabricated on 20.03.20. The location only installed Remote operated shut-off valve (ROSOV) then as a body valve and temporarily blanked spool piece after ROSOV with blind flange for future installation of DBBV.

Ÿ The location re-commissioned the tank on 04.07.20 due to operational exigency and continued using the old delivery line. Air connection was given to this pneumatically operated ROSOV on 10.07.20.

Ÿ One manually operated isolation (ball) valve was installed in this airline near the ROSOV and kept closed. The electrical connection required for energizing the solenoid valve was not given.

Ÿ On 23.08.20, the blind flange was removed at about 09:50 hrs. ROSOV was found closed and there were no traces of product in the spool piece.

Ÿ However, during the erection job of DBBV, at about 10:50 hrs, the ROSOV got opened

inadvertently.

Ÿ The product inside the Tank gushed out, spilling inside the dyke enclosure and into the TLF Pumphouse manifold area through the open delivery line laid axially with the spool piece of ROSOV.

Ÿ The leakage was stopped at about 11:04 hrs by de-pressurizing the air receiver unit of ROSOV.

Ÿ Within 14 minutes, 356.1 KL of MS spilled from the Tank. 281.08 KL MS was salvaged thereafter, resulting in a product loss of 75.02 KL (46.86 lakhs). There was no injury, fatality, or fire.

MAJOR INCIDENT OBSERVATIONS:

Ÿ The blind flange of the spool piece connected to ROSOV was opened to fit the DBBV despite the level of the product inside the Tank was 632.2 cm (1607 KL).

Ÿ The regular Project Engineer was not available and the job was executed by an Operations Officer on a make-shift basis.

Ÿ The DBBV erection job was being carried out without the direct supervision of the company official.

Ÿ DBBV was not fitted during the ROSOV erection job even though valves were available at the site. There was a five-month time gap between fabricating delivery nozzle on Tank & fitting ROSOV, giving airline connection to ROSOV and fixing DBBV.

Ÿ The installation and air connection jobs of ROSOV were done in the absence of an Original Equipment Manufacturer (OEM) representative.

Ÿ When the isolation (ball) valve in its air supply line

was accidentally opened, ROSOV opened leading to spillage from the Tank.

Ÿ Closed-circuit television (CCTV) footage could not identify how this manually operated isolation (ball) valve got accidentally opened.

Ÿ The job description in the work permit did not cover the specific job of fixing the DBBV in MS Tank T-18. The spillage of product from the Tank, if ROSOV gets opened due to accidental

operation of the airline valve, was not assessed during the preparation of Job Safety Analysis (JSA).

Ÿ Though the majority of the product was contained within the dyke, some product spilled outside the dyke through the delivery line section (Fig. 1), laid on the downstream of ROSOV axially in line to the spool piece. The flange joints of this pipeline were loosely fitted leading to the product leak inside the TLF Pumphouse manifold area.

Ÿ The 281.08 KL of total 356.1 KL MS (79%) was salvaged using gully suckers & hydro-jet pumps. The product was pumped back to tanks in a safe manner by blanketing spilled product with foam continuously. The salvaging operation was completed the next day on 24.08.20 at around 15:00 hrs.

INVESTIGATION OF ROSOV FAILURE:

Ÿ The schematic layout of the single solenoid valve type ROSOV is shown in Fig. 2. The figure shows the solenoid valve (having three ports) in the energized state.

Ÿ The airline is connected to port 3 (inlet) of the solenoid valve through the Air Filter Regulator (AFR). When the solenoid valve is electrically energized, ports 2 (outlet) & 3 (inlet) are connected and port 1 is blocked. Air will be flowing from port 3 to port 2 and ROSOV will open. When

the solenoid valve is electrically de-energized, port 2 & port 1 (drain) is connected and port 3 is blocked. Air will be vented out from port 2 to port 1 and ROSOV will close.

Ÿ In ROSOV of MS tank T-18, the airline connection was wrongly given to port 1 (drain) instead of port 3 (inlet). Port 1 is directly connected to the air chamber of ROSOV through port 2. The air traveled to the air chamber of the ROSOV directly, on the opening of the airline, even though the solenoid valve was electrically de-energized resulting in the instant opening of ROSOV.

ROOT CAUSE OF THE INCIDENT (REASONS FOR FAILURE):

Ÿ The DBBV erection work was executed even though the level of the product in the Tank was above the height of Tank nozzles. There is no internal guideline or Standard Operating Procedure for carrying out the dismantling or erection of the valves of a Tank currently under operation/having a product level more than its nozzle height.

Ÿ The pneumatic line connection to ROSOV was not isolated.

Ÿ The airline connection was wrongly given to port 1 (drain) instead of port 3 (inlet) of the solenoid valve (Refer Fig. 3) which resulted in the opening of

ROSOV even though the solenoid valve was in t he de -ene rg i zed s t a te . These w rong interconnections of ports of solenoid valves occurred at the Factory level which highlights the gaps in the vendors internal product Quality Management System and Third Party Inspection (TPI) carried out by the party assigned by HPCL at the Factory level.

MAJOR RECOMMENDATIONS

Ÿ SOP to be developed for carrying out any repair/ modification jobs in the storage tank or associated pipeline. The SOP shall bring out the requirement of emptying/degassing or bringing the level of the product in the tank below the nozzle level (as the case may be) based on hazard analysis.

Ÿ During execution of jobs in a storage tank having product, the ROSOV/DBBV valves shall be isolated from their energy sources (l ike pneumatic/electrical).

Ÿ To ensure ROSOV is fail-safe as per clause 7.6.2.(iii) of OISD-STD-244.

Ÿ The process of Factory Acceptance Testing (FAT) & Site Acceptance Test (SAT) of critical equipment needs to be implemented robustly.

Ÿ Critical equipment should be commissioned in the presence of an OEM representative.

Ÿ Work permits for critical/non-routine jobs shall be issued only after a thorough Job Safety Analysis (JSA).

Ÿ Critical jobs to be carried out only under the personal supervision of the designated & competent Officer.

Ÿ Necessary training to be imparted to officers executing the job.

Ÿ Proper communication about job responsibilities shall be ensured whenever there is a change in assignment.

Safety Council meeting

To ensure proper implementation of various aspects of safety in the Oil & Gas industry, Government of India has set up a ‘Safety Council’ at the Apex in January, 1986 under the administrative control of MoP&NG as a special self-regulatory industry agency for safety matters & procedures in respect of Hydrocarbon sector.

37th meeting of the Council was held on 7th August, 2020.

ISO 9001:2015 Surveillance Audit of OISD

In the year 2013, existing systems and procedures of OISD were validated through Certification of ISO 9001: 2008 by M/s DNV and OISD became the first ISO 9001 Certified Organization amongst all the OIDB grantee Organizations.

In the year 2020-21, the ISO certification of OISD has been continued by M/s DNV during Surveillance audit on 15th Dec, 2020.

OISD activity report on COVID-19 Campaign

Face mask and hand sanitisers were distributed to all the contractual workers and officers of Oil Industry Safety Directorate on regular basis. The distribution of these face mask and hand sanitisers were done as preventive action to fight the COVID-19 pandemic in office. The snapshots are given below:

Pledge taking ceremony at Oil Industry Safety Directorate

Swachhta Pakhwada - 2020 was commenced in Oil Industry Safety Directorate (OISD) by taking the Swachhta Pledge in Hindi and English by all the Officers and Staff of OISD on 1st July 2020. The pledge was taken by following the norms of social distancing and wearing face mask while taking the pledge. Executive Director (OISD) addressed the gathering by sharing his thoughtful insight regarding the importance of cleanliness in our life and motivated everyone to create self-awareness and a sense of responsibility in keeping our environment clean. He encouraged everyone to promote cleanliness and hygiene in and around the office as well as their own residences and society and encourage others as well. There was also an interactive sharing of knowledge and health hygiene related practises between the officers of OISD on various precautionary measures and lifestyle changes to fight the current pandemic scenario of COVID – 19.

Organisation of various programmes

Awareness drive about COVID-19, Swachhta, Environment awareness, Hindi Pakhwada and various other activities were organised during the year.

Dr. Naveen Raj

Dr. Naveen Raj, joined OISD as consultant in E&P Section. He has done M.Sc and Ph.D in Chemistry from University of Lucknow and Post-doctorate in Genetic Engineering. He has 35+ experience in the areas of Environment Management, Water Management,

Enhanced Oil Recovery, Water flood Management, Corrosion, Petroleum Microbiology and Safety aspects. Dr Raj is known faculty and published more than 50 papers in Journals of National repute and presented papers in International conferences.

Shri Dev Kumar Varshney

Shri Dev Kumar Varshney joined OISD as Consultant in E&P Section. He has done B.E. in Chemical Engineering from NIT, Raipur, ME in Process Equipment & Plant Design from IIT Roorkee.

He has about 36 years of industry experience working in E&P industry including working in OISD for about 8 years. He has vast experience in HSE, auditing, accident investigation and safety standard development, PHSERs etc.

Shri N J Vijayprasad

Shri. N.J.Vijay, Prasad joined OISD as consultant in E&P Section. He has done BE (Mechanical) from Andhra University, International Diploma in Safety Management System from British Safety Council and Post graduate diploma in Disaster Preparedness and

Rehabilitation from IP University, Delhi. He has about 34 years of industry experience working in E&P industry including handling HSE jobs viz. auditing, accident investigation, developing HSE related documents etc.

Shri Prasad Murari Mohan former DGM, HPCL joined OISD in EDS Section. He has done B.E. in Electrical Engineering from Delhi College of Engineering and MBA in Marketing Management from

Jamnalal Bajaj Institute of Management Studies. He has over 27 years of career in LPG Operations, Maintenance & Safety, Pipeline Project & Operations and Marketing Research.

Shri Harish Gupta

Shri Harish Gupta joined OISD on 15th July, 2020 as Additional Director (MO-POL) from HPCL on deputation. He has done B. Tech in Mechanical Engineering from Punjab Engg. College, Chandigarh. He has been working with HPCL for 29 years and has

varied experience of the Marketing & Refining setup in HPCL / Oil & Gas sector.

Shri Rajnish Vaid

Shri Rajnish Vaid joined OISD on July 14th, 2020 as Additional Director (MO-POL) from HPCL on deputation. He has done B.Tech in Mechanical Engineering from Regional Engineering College, Kurukshetra. He has been working with HPCL for the last 27 years

and has varied experience handling Operations and Sales functions in LPG department, pipeline projects and operation.

Shri Shivendra Singh Parihar

Shri Shivendra Singh Parihar joined OISD on 14th December, 2020 as Joint Director (P&E) from EIL on deputation. He has done B. Tech in Mechanical Engineering from U.P. Technical University, Uttar Pradesh. He has 14 years of experience in Engineering
