“The REAL Production Formula - Transforming The Way You Achieve Success”

LECTURE – PRACTICE MANAGEMENT

“The REAL Production Formula - Transforming The Way You Achieve Success”Friday, October 11, 2019 8 a.m. to noon(4 hrs. of AGD Practice Management Credit - there will be NO CE for the lecture on Friday as it is Practice Management.)

Production is a result of multiple Key Performance Indicators (KPIs) interacting as one, symbiotic formula. Most owners and practice administrators aren’t even aware these KPIs exist let alone understand, influence, and leverage them to their advantage. In this eye-opening, jaw-dropping course, attendees will learn exactly what to do with these KPIs and how to finally achieve their production goals.

Dentists tend to focus on production as a measurement of their success. Too often they are disappointed when they set goals, bonus their staff, track their numbers, and still fall short of what they wish to produce. The reality is that production is a result of a combined effort of behaviors, influencing multiple Key Performance Indicators (KPIs) that their total sum equals production.

COURSE OBJECTIVES:• Understand Production KPIs and its

formulaic impact • Accelerate success by applying the

production formula to their own practice numbers

• Broaden knowledge of business acu-men and development of practice KPIs

• Outline specific behaviors team mem-bers will change to achieve KPI goals

President’s Message

Hello everyone! I hope you had a great summer . We are looking forward to this fall and have an

exceptional Mastertrack session planned for Oct . 11 to 13 . This session is a special one because it will be a joint session with the Nebraska Mastertrack held in Omaha . It will cover the very important topics of practice management and special needs consideration . These are useful topics and as always, it is nice to be able to connect and network with many great dentists . We hope to see you there .

Another special event that is coming up for the IAGD board is a STAR visit . That will be happening on Sept 27th . This meeting is important because a representative from AGD national joins our board members to facilitate in creating and achieving our goals . It’s a very exciting meeting where we discuss various ideas to grow our academy and give our members what they want . If there are any ideas that you would like us to discuss, please don’t hesitate to email me at robertomoreno .dds@gmail .com

or our executive director at julieberger50@gmail .com .

Finally, I wanted to share my great experience at the AGD’s annual scientific session that was held on July 18 to 20 at the Mohegan Sun in Connecticut . It was a wonderful event . I learned a lot at all of the classes that I took, from the latest techniques on placing and restoring implants to the newest uses for dental lasers . The dental pearls sessions were great because they covered many topics in a short period of time . The venue was amazing . A little bit off the beaten path but once you were there it had everything, from fabulous restaurants, a grand casino, and even a bowling alley . I’ll never forget the awesome experience I had at the Mohegan Sun interacting with fellow AGD members from all over, and I am looking forward to next year’s scientific session in Las Vegas from July 15 to 18 . I hope you join me next year!

At this year’s Convocation Ceremony, Saturday, July 20, 243 AGD members received their Fellowship, 63 received Mastership, and 21 were recognized as LLSR recipients .

“Congratulations to all recipients of the Fellowship, Mastership, and Lifelong Learning Service Recognition,” said Neil J . Gajjar, DDS, MAGD, AGD president . “These members embody AGD principles and ideals, and accept the obligation to prove themselves worthy of these designations throughout their professional lives . Thank you for taking this pledge to dedicate yourselves through education and service .”

After Convocation, AGD leaders and members were invited to attend a Convocation Celebration in honor of the new Fellows, Masters, and LLSRs . Guests enjoyed hors d’oeuvres, champagne bars and live music .

William Stoffel DDS, FAGD

Dr . William Stoffel and I talked about his Fellowship in the Academy of

General Dentistry award and I soon learned that he has pursued continuing education his entire career . Dr . Stoffel told me from the 1980’s to the present he took a series at The Pankey Institute, then later the Wisconsin Orthodontic Society, and added courses taught by Dr . Bob Gallagher, Dr . Duane Kellar, Dr . Joe Sims and more recently a two- year cosmetic dentistry offering at the University of Minnesota . He told me that he has incorporated Itero, digital record keeping, digital radiographs, and Invisalign in his practice, and have left behind models of teeth, hydrocolloid impressions, and gold . He said, “We have kept, however, the same way of counting teeth .”

Dr . Stoffel said that his father, the senior Stoffel, taught at Loyola dental college, and that his older brother also graduated as a dentist from Loyola, however because Marquette offered admission after two years of undergraduate that he chose Marquette instead, graduating in 1976 .

Dr . Stoffel did not join the AGD until 2000 and said that one of his lifetime goals was to get a FAGD, and the final step was preparation for the examination which he took at a local community college . He told me that he studied every discipline of dentistry for weeks ahead and on passing, was happy that the exam results were back right away .

Regarding the ceremony, he said that is really was a nice occasion to commemorate his continuing education efforts . His wife Jan attended the convocation and while they were out East, they visited a world class art museum and library in Providence and New Haven .

2019 Fellows & Masters Recipients

Dr . Stoffel currently practices at Midwest Dental in Dubuque with another AGD member Dr . William McBride . Dr . Stoffel said that he was an active in competitive swimming in high school and college days and joined in triathlons too . His wife Jan has worked in the administrative and marketing side of his dental practice . At the close of our interview Dr . Stoffel returned to the FAGD award ceremony and recalled the speaker there saying the FAGD distinguishes dentists, in that only 6% of all dentists receive the FAGD . Dr . Stoffel is also an ADA member and has served in the local offices .

Ashley Edwards, DDS, FAGD

Dr . Edwards received her Fellowship in the Academy of General

Dentistry at the AGD annual session in Connecticut . Like many of my colleagues who have gone to the award ceremony, one comes into the annual session with their own sense of accomplishment in their everyday work applying continuing education endeavors . It was good to hear Dr . Edwards say that she gained an appreciation for the significance of the actual award by being there . She told me about the encouraging comments of her peers who were also receiving their FAGD award, and the convocation speaker Dr . Steve Ratcliff who represented himself as one of the older generation of dentists, who was encouraged to see the younger ones advance in their dental career . Dr . Edwards said that the community of dentists, their call to leadership, their own commitment to organized dentistry providing quality continuing education helped her appreciate the FAGD award . Dr . Edwards added, “This meeting really encouraged me to continue lifelong learning .”

Dr . Edwards said that she gained a lot from Dr . Bob Margeas’ lectures and Dr . Frank Spear . She shared with me that her confidence in taking care of cases has included an avulsion and reimplantation for a recent patient . In the course of our interview we touched on treatment planning, digital photography, oral surgery, periodontal health, and consulting with specialists and general dentistry colleagues .

Dr . Edwards graduated from West Des Moines Valley High School, and received her BS in Psychology from the University of Iowa in 2006, and her DDS in 2010 from the University of Iowa College of Dentistry . Dr . Edwards also pursued a general practice residency in Kansas City and received a GPR certificate in 2011 . Dr . Edwards joined the West Des Moines Dental Center in 2014 and practices with Dr . John Meyer and Dr . Ali Meyer .

Dr . Edwards is married to Cody and they have a 2 ½ year-old son, Rylan . She has completed 12 half marathons and one marathon, and she said . “Rylan is about old enough to attend a Hawkeye football game this fall .”

Tessa Johnson, DDS, FAGD

Dr . Johnson practices dentistry at The Community Health Center of

Southeastern Iowa in Keokuk, Iowa . She has a BS in Interdependent Studies with a Certificate in Gerontology from the University of Iowa in 1998, and a DDS from the College of Dentistry at the University of Iowa in 2004 . She explained that her pursuit of at Fellowship in the Academy of General Dentistry was twofold . She was essentially curious about things, and had established a goal to achieve her FAGD by her 15th dental college reunion . She said, “I attended the Mastertrack series

curriculum over three years and proved to myself that I was current in my field .” The best teachers she mentioned to me were pediatric lectures by Dr . Michael Kanellis and removable prosthodontic lectures by Dr . Joseph Massad . (He spoke in May of 2016 .)

As a youth, Dr . Johnson told me that she was naturally concerned about her brother who suffered with bad teeth, and followed his care . She said, “This did inspire me to get involved in dentistry, and I always was interested in life sciences and biology .” In undergraduate she enjoyed gerontology classes taught by Hermine McLaren, RDH who had left the college and was then teaching gerontology . During this time, she gained experience caring for elderly patients in their care facility .

In talking with Dr . Johnson, it affirmed to me her lifetime interests, when she told me that in her clinic, she does care for both the pediatric and geriatric patients . Then, at the national meeting she attended to receive her award, she attended both pediatric and geriatric lectures .

Dr . Johnson was very perceptive about one thing at the award ceremony, and in all the interviews I have done over the years, no one has mentioned to me that inside the FAGD academic costume mortarboard has each candidate’s graduating institution colors . Iowa’s being black and gold . She explained that the inside of the Mastership award costume was violet, the color representing dentistry .

Finally, it was neat to hear that Dr . Johnson’s hobby was quilting, and I saw a quilting show and learned how varied, complex and colorful these quilts are these days . She told me that she grew up on farm in Illinois . She has one son and one grandson and extended family members in the area .

Cassidy Rider, DDS, FAGD

Hello! My name is Cassady Rider . I was born in New Mexico but spent most of my childhood

in the mountains of Utah in a small rural town . I went to the University of Utah for undergrad and graduated in 2006 with a bachelor’s of science in Sociology with an emphasis on Criminology . I never really liked the science classes so I decided to study subjects that I loved, those being about people .

I originally began college with the idea of becoming a 2nd grade teacher . I quickly realized that teaching was not for me and was best left to those that enjoyed large quantities of children . I then did some research and settled on dentistry . I had always had limited but positive experiences at the dentist as a child and thought it would fulfill my love for people and the ability to give back . You see, I grew up as a single child in a single parent household with very limited income . I realized that the best way to end the cycle of poverty was to get an education . I had decided early on that I would love to be able to give back should I be able to better myself and my family .

After being accepted at multiple schools, my husband and I decided that the Midwest was a close match to where we would want to end up . No large cities for us, we enjoy the small town communities and the comradery that is present in them . I attended Creighton University School of Dentistry for my dental degree in Omaha, NE . We both thoroughly enjoyed the Midwest and we landed in Cherokee, Iowa after graduation from dental school . It was perfect for us, population 5000 . I started at a DSO practice and after 7 years approached them to allow me to purchase the practice . I have now owned the practice for 2 years and our new, larger, more modern facility is in the final 9 months . We are excited for its’ completion and the big move downtown!

Being a rural dentist means we have to know something about every aspect of dentistry . I loved school and if I am not working towards some goal I feel somewhat lost . I love learning more about the ever changing and improving divisions of dentistry and how best I can treat my family of patients .

My husband and I have been married for 13 years and have two children . Levi is 7 and in 2nd grade, Knox is 2 and a ball of energy! We enjoy traveling together as a family and camping . I enjoy reading and quilting .

My husband and oldest son went, we spend 3 days in NYC prior to the conference as a little vacation . Levi was excited to see real sky scrapers . I am humbled to be recognized for something I would have done anyways as lifelong learning is a passion .

Samuel Koth, DDS, MAGD

Dr . Koth received his Mastership in the Academy of General Dentistry at this

year’s award ceremony in Connecticut . Speaking about this he said, “Pursuing the MAGD Mastertrack has been one of the most rewarding CE ventures I have done . The group of dentists that shared my Nebraska class were exceptional, and I made many last lasting friendships . The case presentation was nerve wracking at first, but ended up being a great component…to see how others approach cases and to see your own work on a big screen is humbling . It was satisfying to learn together, and help others . Naturally, in the process, I improved myself .”

Dr . Koth’s family came out to attend the ceremony, his wife Katy and four young children, Peter (8), Davy (6), Rockwell (4) and Rosalyn (1½) . He said that many of his closest dental college classmates were at the ceremony with him .

Dr . Koth has been in private practice in Rock Valley, IA since 2010 and is a graduate of South Dakota

State University and Creighton Dental College . He is a member of the ADA and constituent societies . He told me that Dr . Amber Meyer has joined his practice there . Dr . Koth has trained with The Pankey Institute and has taken courses with Dr . Frank Spear . He said that he is interested in sleep and airway management and how being aware of these things helps better treat his patients . He said, “Sometimes I have to keep digging for the answers .” Dr . Koth acknowledged Julie Berger’s heart-felt care for all of the doctors going through their AGD courses .

Dr . Koth said that he likes the hands-on aspects of dentistry, and that he chose this career over many other health care professions . Dr . Koth received his FAGD in Boston in 2016 . Dr . Koth said that he especially is thankful for his wife to manage the family while on his education commitments and when they get away from the office, he enjoys outdoor activities including fishing, hunting, and sailing catamarans .

Amanda Foust, DDS, FAGD

Dr . Amanda Foust earned her Fellowship in the Academy of General

Dentistry at this year’s convocation in Connecticut . She commented on her web site “My practice is dedicated to compassionate and personalized care, always (in view) on improving my patient’s overall wellbeing as an individual, not just a patient .” Dr . Foust has a BS in Psychology from the University of Iowa and her DDS in 2001 from the University of Iowa College of Dentistry . Dr . Foust has been an AGD and ADA member since then . She is currently participating in the IAGD Mastertrack program . Her practice is called Dental Impressions, and has been in Ankeny since 2006 .

Iowa Dental Board Report – August 2019

Dr . Hardinger Reports On The AADEJ

I am writing to bring some things home from my recent meeting in San Francisco at the annual

American Dental Association Editors and Journalists (AADEJ) meeting on Tuesday afternoon and all-day Wednesday September 3 and 4 . I would like to acknowledge ADA Editor’s reaching out to me as a constituent AGD member by their invitation for me to come . I do submit IAGD’s publication in some award offerings by the American College of Dentists and

the International College of Dentists, but not the ADA ones .

A good part of our presentations was about ADA media, another about editorial skills and finally a rapid-fire presentation by the executive director of the ADA who talked about current issues that we can bring to our members . Most of these things applied to general dentists as well . Overall, in a day and a half we had eleven biography’s and 10 presentations; this was a good mix of content interesting to editors .

Let me begin with Dr . Kathy O’Loughlin, the ADA Executive Director . She related to us the trends in dentistry and began with the astonishing increase from 12% to 51% in 20 years of female graduating dentists . Then she said, “Smile Direct is skirting the dentist to disintermediate, as other speakers said, to offer orthodontic services without dental supervision .”

She referenced Cologne, Italy - where their dental meeting’s vendor display floor was much larger than anything here

Page 9: A PUBLICATION OF THE IOWA ACADEMY OF GENERAL …...Approach to Dentistry” Objectives: 1) Demonstrate understanding of special care and geriatric dentistry as it relates to access,

- to tell about companies who are looking at self-diagnostic oral devices that will aim to check your brushing time, your blood sugar levels, your saliva pH and types of pathologic bacteria .

She thought our readers should know about changes to the current payment models, the 100/80/50 model developed in 1966 . That is prevention, restorative, and major dental restorative work . We have all seen the unused yearly maximums which do not carry over and the plans which sell as dental but hardly make the overhead of a dental office and have limited doctors who subscribe . Of course, of interest is the relationship dentists have with third parties and the ADA has tried to intercept the creep of third parties in charging even the debit card service fee transactions, when they can, to the dentist . She also commented on outcomes-based reimbursement and quality care . The days are waning when doctors can simply say, “It works in my hands;” rather “My work is outcome-based proven quality of care .” Finally, the transfer of patient records is a present and future concern in that there are patients

D E N T I S T R Y … I N G E N E R A L • F A L L 2 0 1 9 9

Gold Gate Bridge, San Francisco at dusk

who are asking for their records to be easily transferred . (This reminds me of Facebook’s recent commitment to make transfer available for users .)

She offered a quotable statement about ADA members, “without members there is no mission, and no money .” She said that the ADA has specific initiates to maintain the old and improve the number of new graduates and this includes reaching out and asking nonmembers to join . She drew an analogy with a Venn diagram to show their expertise in one circle and the members needs intersecting with their own circle . She stated that the ADA also “faces forward” to the public to be the trusted source, and also an expert source for professionals and other interested parties .

Perhaps the most meaningful part of the presentations for my publication was the San Diego CA, Dr . Brian K Shue, who was the last presenter on Wednesday . In his talk, he had collected tips and advice from our room of editors, which was helpful to me to maintain and improve our publication . In sharing one detail, a dentist sitting next to me asked me to take his picture when the awards were given out, and I respected his desire not to have a yellow spotlight on his forehead from overhead lighting . I briefly delayed

the presentations to get the commonly used overhead spot lights turned off to eliminate this photographic problem .

I made friends with Dr . Donald Patthoff, who for many years has contributed an ethics column to the AGD IMPACT and also Dr . John Svirsky an oral pathologist from Virginia Commonwealth University, who had been invited to speak in all but two states, and Iowa was one of them . Perhaps my connections will lead him to our state .

Our non-dental presentation over a lunch hour was by Robin Warner, who writes for AUTOWEEK and does a podcast on cars we would all love to drive . His interesting story included his recovery from a serious accident, and his oral care with that led him to be cared for by our own editor colleague, Dr . Michael Diorio .

I did a trek to the Golden Gate Bridge Monday night to walk the pedestrian side and back, and Tuesday morning a guide showed me the city . He was telling me that my evening activity to see the bridge was a good choice otherwise it is often shrouded in fog . I could see Oakland and the Bay Bridge . This guide told me that the Bay bridge carries 250 thousand cars a day one way and the Golden Gate 50 thousand . To me that spoke louder than this city’s reputation for street people, who although garner a worldwide reputation are few in comparison . My guide also explained that this harbor was not discovered by explorers for many years because it was always shrouded in fog . I learned that the mountain ranges hold the fog in place hours a day, and just across the mountains it is sunny where today’s residents commute into the city to work .

Getting Ready For E Prescriptions

My journey into complying with mandated electronic prescriptions began with asking my front desk administrator, Brittany, to contact my software company, Dentrix, and find out how to integrate the upcoming electronic prescriptions mandate into my practice . Just in case there are dentist colleagues complaining about this, I too felt that this is an excessive burden for me to implement in my practice . I reminded myself that I have known colleagues who were so bothered at new regulations that they quit practicing . Yet a vigorous spirit in me also said, “it is easier to complain than to learn this new thing!” I also recognized that there were shortcomings to my own in office printed prescriptions . It is, of course, my own office problem but the printer never could overcome the watermarked paper meant for prescriptions, the printed copy was hard to read, and I knew we could do better . Plus, there were many patients who would not carry their prescription document to the pharmacy, lose it and either never get filled or to redo our work .

The first thing I learned from Brittany was that I was finally going to put a passcode for each staff member into the dental software, which in my single doctor office had been delayed thinking it would be a nuisance . The second was that there would be a monthly fee of about 45 dollars to have access to this software .

My first tip at creating a passcode was to carefully read the requirements and plan on a new one every 90 days . I created several passcodes, one for each staff member in the office . (I used their staff member name in the time log files and changed one character for each new log in .) This simplicity has worked well for me and the staff . Next, each staff member who was at their workstation logged in the start of the day along with their start work time and their time sheet and, glad to see that the workstation did not log them out all day . My dental software company sent me a flow sheet and predicted 2 .5 hours to activate the electronic prescription service . I did not keep track of time, but it was a lot more than that for me .

Now, I have colleagues who say that their dental software does not have

electronic prescriptions, so it is possible to go directly to All Scripts or now called Veradigm (1-877-933-7274) and start there . I also found a list of vendors on the Board of Pharmacy web site . I would advise asking whether these vendors download to your dental software . Even with my vendor doing this, I learned that my own records of patients who have allergies is not uploaded to Veradigm . This, however, is a field that can be added at the web site .

Now, another point is that only the electronic prescriptions that have been generated for the patient will show up on the prescription history . In other words, if the patient got a written prescription it would not be in the history . Of course, once the mandate is implemented January 1, 2020 there will be a lot more . I don’t think this means all-inclusive medications as there are some exemptions . It is supposed to be, but not always consistent, that the prescribed medication has information about the doctor provider and their physical location in the electronic history . An important exception, for example is an injection of medication for osteoporosis .

I think it is important to distinguish the new account with All Scripts application from your dental software . With All Scripts, I was able to use my same passcode, however, my office username was not accepted . They ask for several layers of authentication . This included my NPI number, state pharmacy board number and dental board license number . I also had to scan my drivers license and passport then notarize them and email them to All Scripts .

I learned that they wanted me to use my smartphone to complete regular authentication . I was very concerned that this would have patient data on it, and that my smart phone would be subject to the HIPPA regulations . However, my mind was put to rest when it was only a two-factor identification number being asked for . Later this became a big hang-up in getting my first narcotic prescription written . But I digress . My training with the people at All Scripts helped me to get the first “practice” electronic prescriptions written, and I learned how to correct any mistakes with follow up ENTERED IN ERROR .

It helped a lot to upload my top 15 prescriptions to the All Scripts or Veridym web portal, and further to upload my top area pharmacies . I got an excellent tip from my local pharmacist, Wendy, at Hy Vee 1410, who said that I can search for a pharmacy with their phone number and leave all other fields blank, and this worked perfectly except for the Veterans Administration .

I found out later that my local VA’s in Iowa have not yet implemented electronic prescriptions, however they continue as before with mail and faxed prescriptions . Yes, they said even the water marked faxed prescriptions are honored! My advice for now is to have the veteran take their penicillin prescription on printed paper to the pharmacy and this is minimal in cost .

I would like to add that for the patients who are getting electronic prescriptions, and downloaded into the dental software, it is helping us in our medical records . I can see the spelling of these medications, and in most cases know why they are being prescribed . One of my patients has a blood disorder and electronic prescriptions was especially helpful . It is notable that many of these medications are repeated in the downloads, likely these are refills .

I have a good primary resource for my readers, and this is the Iowa Board of Pharmacy, who write a regular newsletter and a 10-minute summary podcast on their board meetings . I also found on this web site an application to be exempted from electronic prescribing . A petition must be received by October 1st this year and if granted annually thereafter . Yet, I perceive that this is not easily granted . I also read that the current law allows an emergency non-electronic prescription can be used in certain obvious situations when your Internet is down .

This is an in-progress concept, and I would like to tell you about a nurse who works at Mercy One and does electronic prescriptions for her doctor . She lightly told me, after all the work I had done to get ready myself, that the IT department at Mercy One takes care of all the things I have just written about!!

It’s hard to believe that summer is over . Where did it go? In my conversations with a lot of people,

most feel this summer went way too fast . BUT…I love fall! Fall is my favorite time of year . I love the crisp mornings, the fall colors and of course the start of football season . And this year, with one win (albeit ugly) under our belts, there is hope! Also, my granddaughter Alice is now 10 months old! She is just the happiest baby, starting to crawl and smiles when I walk into the room . My heart is full!

I’d like to take this opportunity to congratulate the following IAGD doctors who received their FAGD and MAGD this summer in Connecticut at the AGD Annual Meeting . FAGD Honorees: Drs. Cassidy Rider, Amanda Foust, Ashley Edwards, William Stoffel, Travis Harbaugh and Tessa Johnson . Congratulations also to Dr. Sam Koth for receiving his MAGD! I am so proud of all of you!

The IAGD Mastertrack continues to grow . We currently have 24 doctors in the core class! We had a great turnout for Dr . Sam Low at the Annual Meeting in March and again a record number of exhibitors . If you know

of any companies that would like to exhibit at our meeting, please let me know . BUT…you may want to mention that we have sold out the past few years . We are grateful to have the relationship we do with so many companies!

I’m excited for the fall course on October 11th – 13th! This weekend will be a combination of Practice Management/Special Needs/Special Patient Care disciplines . This is also the course that we hold jointly with the Nebraska AGD . The course will be held at the Hilton Garden Inn Omaha West . These disciplines are hard to find so if you are working on your MAGD, this is a great opportunity to obtain those hours within a close proximity . Registration information can be found on our website at: www .IowaAGD .com!

Mark your calendars for March 6-8, 2020 for the IAGD Annual Meeting featuring Drs . Karl Koerner and Andrew Drollinger! We are excited to have both of them be here to lecture . Dr . Drollinger has been lecturing with Dr . Koerner for several years now . We are still working out all of the details but Dr . Koerner will provide the lecture on Friday and part of the hands-on on Saturday . Dr . Drollinger will also be doing some of the hands-on on Saturday and then will speak on Sunday as well . We’ve had so many requests to bring back Dr . Koerner so this is very exciting for the IAGD! Registration information will be available in the next month . I encourage you to register early and take advantage of huge membership discounts . This meeting will again be held at the Hilton Garden Inn in Johnston .

Sometimes I feel like a broken record but The IAGD Mastertrack is one of the best benefits we offer! High quality continuing education, all laid

out for you in a 6-year program…right in your back yard! And you can start at any time . If you have been considering starting the Mastertrack, give me a call and let’s chat .
