Acid reflux, traditionally thought of as a disease presenting as heartburn, is finally being recogniz-ed as a condition that can show up not only with stomach issues, but with throat problems too.
Acid reflux occurs when stomach acid goes the wrong way, that is, backwards up into the esophagus and throat (the esophagus is the tube that connects the throat to the stomach). There are seven things you should be aware of when it comes to this malady.
1. Acid reflux everywhere60 million Americans have acid reflux. What was once thought of as a disease of overweight, middle-aged, white men now cuts across all body types, generations, races and genders.
2. It’s not just heartburnThere are two types of acid reflux disease, “Heartburn Reflux” where the main complaint is heartburn, and “Throatburn Reflux” where the main complaints are “ccchuching” symptoms (that guttural throat sound which we all know what it sounds like, but try spelling it), such as chronic cough, frequent throat clearing, hoarseness, sore throat, and/or a lump-like sensation in your throat. With Throatburn Reflux, you have acid reflux without having that heartburn feeling.
3. Who cares about a little heartburn?Untreated, or insufficiently treated, acid reflux disease can lead to severe inflammation in the esophagus, stomach, lungs, vocal cords and throat which can cause ulcers, pneumonia, vocal cord polyps and breathing problems.
4. What’s the worst that can happen?In some cases, untreated acid reflux can even progress to esophageal cancer, the fastest growing cancer in America and Europe since the mid 1970’s.
5. The acid reflux “dirty dozen”There are 12 commonly consumed “foods” that cause acid reflux, either indirectly by having effects on the stomach, or directly because they are so acidic. The half dozen that are not very acidic, but that allow acid to fly up from the stomach are: Caffeine, chocolate, alcohol, mint, onion,
and garlic (yes, your 6 favorite food groups). The half dozen that are very acidic, and can directly inflame the throat, and everything around it, are: Sugary soda, bottled ice tea, white wine, citrus, tomato and vinegar.
6. Creative in the kitchen: Acidic food can be neutralizedHealthy, acidic foods such as berries can have their acidity neutralized by buffering them with more alkaline (less acidic) foods. For example, strawberries, blueberries and raspberries can become safe for people with acid reflux by adding almond milk to the berries in a smoothie. Almost every smoothie store offers an almond milk option.
7. Don't touch me there: The latest, and safest, in diagnosisThere is a new method to diagnose acid reflux which doesn’t require any type of sedation, thereby avoiding it’s potential complications. The alternative is called TransNasal Esophagoscopy (TNE), where an ultrathin camera, the size and softness of cooked spaghetti, is used to examine the esophagus with the patient wide awake, in a doctor’s office. As no sedation is necessary, TNE is safer, and less expensive, than the traditional exam of the esophagus. So the next time you are discussing sedation during endoscopy with your doctor, ask: Are there alternatives?

Clinical research is a branch of healthcare science that determines the safety and effectiveness of medications, devices, diagnostic products and treatment regimens intended for human use.
These may be used for prevention, treatment, diagnosis or for relieving symptoms of a disease. Clinical Research is different from clinical practice; in clinical practice one uses established treatments while in clinical research evidence is collected to establish a treatment. Many clinicians at ENT and Allergy Associates, LLP participate in clinical research. Currently, there are approximately 20 clinical trials underway by ENT and Allergy physicians. Here is a sample of what we are doing:
Allergy and Asthma TrialsPatients who experience food, seasonal and/or perennial allergy and asthma symptoms can contact us to discuss inclusion criteria for one of our allergy trials. We are recruiting for both pediatric and adult trials. Some of the
current allergy trials include cow’s milk allergy, new cat peptide immunotherapy and a new inhaler for treatment of chronic persistent asthma.
Ear TubesWe have a topical antibiotic clinical trial enrolling candi-dates needing ear tubes. This study involves administration of a FDA approved topical antibiotic post surgically.
TinnitusA phase 3 clinical trial for a new topical medication to treat acute-onset tinnitus is ongoing. The tinnitus can be the result of any acute traumatic event such as head trauma, noise exposure, barotrauma, or middle ear infection.
Eustachian Tube Dysfunction.Patient recruitment is underway for a randomized clinical study of safety and efficacy of balloon dilation procedure for adults with chronic Eustachian Tube Dysfunction.

Alpha-Gal/Mammalian Meat AllergySince the discovery of a new meat allergy was identified in regions of the country endemic with ticks, many of the allergists have been busy testing and treating patients for this condition. One of our allergists, Dr. Erin McGintee, is working with researchers at The University of Virginia School of Medicine gathering data for a large multi-center case series report to increase awareness of atypical signs and symptoms of tick borne illnesses. Patients with suspected alpha-gal allergy should contact our Clinical Research team for referral to one of our allergy specialists.
Obstructive Sleep ApneaDr. Ofer Jacobowitz is investigating a new device for patients with Obstructive Sleep Apnea. This phase 2 study is collecting data to determine the safety and efficacy of a surgically implanted hypoglossal nerve stimulator. This device is designed to reduce or eliminate sleep apnea episodes. Qualified individuals have a diagnosis of Obstructive Sleep Apnea and have not received lasting benefit from CPAP or other treatments.
SinusitisENTA is currently seeking participants for a clinical re-search study evaluating the safety and effectiveness of the investigational dilation system for frontal and sphenoid sinus treatment.
Laryngeal PainResearch into throat/neck pain with phonation, known as odynophonia, is lacking. We are conducting a retro-spective study to determine whether correlations exists between laryngeal pathology and odynophonia, sidedness of the pain with the particular laryngeal pathology and whether improvement in laryngeal pathology results in improvement in pain.
Pipeline studies include propel use in office, cocaine as an anesthetic for office procedures, a different and new sinus dilation device and a thyroid FNA genomic signature study.

As allergy season has been get-ting progressively worse each year, the number of people afflicted by seasonal allergies,
or allergic rhinitis and conjunctivitis, seems to be growing. Furthermore, those patients who have had minor symptoms in past years are presenting with more severe symptoms now. With up to one quarter of the population being affect-ed by allergies in developed countries, every primary care provider is sure to see patients with these complaints.
The mainstays of treatment for seasonal allergies continue to be allergen avoid-ance and pharmacotherapy; however, complete allergen avoidance is virtually impossible when the allergen is pollen, which is present every time a patient steps outside or opens their front door. This is especially difficult for patients who work outdoors and for those who work in buildings without air condition-ing. Pharmacotherapy most commonly includes oral antihistamines and leu-kotriene inhibitors, as well as intranasal corticosteroids and intranasal antihis-tamines. While these medications work very well for many people, there are treatment failures due to poor compli-ance, improper technique when using nasal sprays, and unresponsiveness to medications even when used properly. For patients who fail medical manage-ment, who require medications for many months of the year, or who do not wish to be taking medications, immunothera-py is another option to consider.
Immunotherapy, most commonly known as allergy shots, changes the course of the disease and can induce long-term remission for allergy sufferers. While both allergen avoidance and pharmacotherapy can improve the symptoms that patients experience with allergic rhinitis and conjunctivitis, once the patients are exposed to their allergens again, the symptoms will recur.
Immunotherapy, most commonly known as allergy shots, changes the course of the disease and can induce long-term remission for allergy sufferers. For patients who had difficulty this spring with their allergies, now is the time to refer them to a board certified allergist so they can consider starting immunotherapy and reap the benefits by next spring season!

In recent years, we have observed an increased pre-valence of food allergy. Based on a recent review of various published studies, food allergy is estimated to affect between 1% and 10% of the population,
reflecting a growing public awareness and concern for food allergy. Although there are several ideas proposed to account for the increased prevalence of food allergy, such as hygiene hypothesis (increased hygiene leading to a shift in immune system towards allergic pathway), vitamin D (vitamin D deficiency being associated with increased food sensitization), dietary fat (reduced consumption of omega-3 fatty acids leading to increased food sensitization), reduced consumption of antioxidants found in fresh fruits and vegetables (diet low in antioxidant and high in processed foods leading to higher susceptibility to allergy), obesity (leading to inflammatory state), and food processing (various food preparation methods affecting allergenicity of foods), none has been conclusive in elucidating observed
pattern of increased food allergy prevalence. Some of the more common food allergens responsible for a relatively high proportion of food allergy are cow’s milk, hen’s egg, soy, wheat, peanut, tree nuts, fish, and shellfish. As these foods are all very commonly found and contained in our diet, there exists a significant challenge for food allergic individuals as they try to maintain an appropriate diet free of relevant food allergens.
Another challenge that is frequently encountered in dealing with food allergy is the issue of food sensitivity. Food sensitivity merely indicates the presence of IgE antibodies (“allergic antibody”) to a food, often in the absence of adverse symptoms or “reactions.” When patients with food sensitivity demonstrate an adverse response upon ingestion of a food to which they are sensitized, they can be more accurately categorized as having food allergy. Given an increased awareness of food allergy in recent years, patients

undergo food allergy testing more commonly now than ever before for various medical reasons, and it is not uncommon that food sensitivity is discovered in asymptomatic patients. An important question then arises: should they start avoiding certain foods based on positive test results alone? What are the risks involved in continuing to ingest those foods in question? Should they carry epinephrine? What should they do when they dine out or travel internationally? For children, how should this be addressed in their school? The list goes on and on.
There are many intricacies involved in the accurate diagnosis and management of food allergy, and it is also important to distinguish food allergy from food sensitivity. Diagnosis of food allergy affects patients’ lives in a myriad of ways: from altering their dietary habits, to being prepared to appropriately deal with allergic reactions, to educating family members, to name a few. In children especially, food allergy can also have negative psychosocial effects, including bullying. On the other hand, a subset of patients with food sensitivity can still tolerate the food in question; however, this determination should be made only after a proper evaluation and consultation with an expert in food allergy. This is an important decision to make, however, as it can have a tremendous implication on a patient’s quality of life, including avoiding unnecessary dietary elimination. With growing public awareness for food allergy, it is likely that we will discover more patients who will be left with positive test results for foods. The answer is not always avoidance, especially if it represents a mere sensitization without clinically significant food allergy. Board-certified allergists can help these patients find the right answer so that they can receive proper treatment plans tailored to their specific clinical scenario. As we further our knowledge about food allergy with ongoing, active research studies, now is the time to face the challenges of food allergy with more confidence!

Year after year, new physicians join ENT and Allergy Associates and the Practice continues to grow. • At the end of 2015, we are relocating and con-solidating our Englewood and Hackensack offices into one brand new, state-of-the-art clinical office in Hackensack at the Continental Plaza, 433 Hackensack Avenue.
• In January 2016, we are opening a brand new Suffolk County office in Hauppauge, New York. The 16,000 sf, 28-exam room State-of-the-Art medical suite is located on the 2nd floor of the former Met Life Building, located at 2929 Expressway Drive. The Bay Shore and Smithtown physicians will work out of this new office location.

Over 35 million Americans suffer from chronic sinus disease today. Chronic sinusitis is among the most common diagnoses for those who visit their physician complaining of coughing, sinus
pressure, congestion, postnasal drainage and headaches. Often, patients misdiagnose themselves as simply suffering from seasonal allergies. While medications such as decongestants, antibiotics, antihistamines and steroids can provide relief for chronic sinusitis, many times they do not solve the problem entirely and surgery is required.
John Madera is a 32-year-old businessman and CrossFit athlete who works out six days a week. He suffered with chronic sinus headaches, and “always felt tired, and sluggish”. His quality of life was significantly altered by his chronic sinus condition.

In-office sinus relief will be more accessible for sinus sufferers as a result of new minimally invasive technologies. Thirty-seven million Americans suffer with chronic sinusitis and new in-office technologies maybe the panacea everybody has been looking for. The results of national research studies
demonstrated balloon catheters could safely widen the natural sinus opening in the office under a local anesthetic1,2, avoiding a trip to the operating room and general anesthesia. Endoscopic sinus surgery is still considered the gold standard to treat sinus disease refractory to medical therapy. Despite this, balloon sinus dilation has become more popular as doctors acquire these skills and significant subsets of patients opt for this minimally invasive procedure. The down time is less than 24 hours, there is virtually no bleeding and significantly less discomfort. Actually, the results are almost immediate. Patient selection is key and an experienced sinus surgeon who can perform in-office balloon sinus dilation is paramount. The contraindication to balloon dilation of sinuses is extensive nasal/sinus polyps because polyps need to be removed. However, there is good news for these patients since new technologies have brought disposable polyp shavers to the office and sinus implants which shrink polyps.

Dr. B. Todd Schaeffer has been an endoscopic sinus surgeon for over twenty years. His practice is focused on the diagnosis, treatment and research of nasal and sinus problems. He is an experienced surgeon in minimally invasive sinus office procedures and skull base surgery.
In-office polyp removal followed by a sinus implant will help open sinuses, maintain3 their patency and shrink residual polyps. The polyps will be removed with a small disposable shaver. We will be enrolling recurrent polyp patients this summer in a study to reduce polyps in the office and again avoiding general anesthesia and surgery. The polyps will be shaved and a steroid emitting implant will be placed. The patients who qualify have recurrent polyps after a previous sinus operation.
The patients who qualify for in-office sinus procedure have failed medical treatment. Typically, they have been on at least two to three antibiotics over a three month period. Most patients have suffered longer. A CT scan of the sinuses will demonstrate sinus infection or narrowed outflow tracts either causing sinusitis or predisposing them to recurrent infections. Another study starting this summer we will be using a novel sinus-dilating device (SinuSys Corporation) instead of a balloon. This expands over one hour by hydrostatic osmotic pressure. This may have the advantage of being less traumatic and easier to position than a balloon.
Avoiding general anesthesia and surgery The advantages of in-office-procedures are: minimal bleeding, quicker recovery (usually one day), avoiding general anesthesia and less post-operative swelling and pain. Patients who are on blood thinners (aspirin, Plavix, Coumadin, Eloquis, Xarelto and Lovenox), may be able to discontinue these medications for a shorter period of time.
Patients with co-morbid conditions such as asthma, diabetes, COPD, obesity and obstructive sleep apnea may not be ideal candidates for general anesthesia. These in-office procedures are ideal since the risk of general anesthesia is eliminated since the patient is awake under a local anesthetic.
Patients with tight work schedules who are unable to miss a week of work also opt for this less invasive but just as effective procedure.
