I. POLICY
It is the policy of the Department of Corrections to offer specialized programs to offenders with mental illness in controlled
conditions of confinement. These programs utilize planned incentive level systems to promote pro-social behavior and
treatment progress and meeting targeted behavioral goals while ensuring the safety and security of employees and offenders.
II. PURPOSE
This Administrative Regulation (AR) describes incentive level systems for offenders with mental illness and criteria for
movement/transition for residential treatment program offenders.
III. DEFINITIONS
A. Activity Therapy: Monitored social, recreational, or other activities that provide offenders with opportunities to
develop and demonstrate positive, pro-social behavior under controlled conditions. This may include out of cell
time for participation in board games, cards, artwork, etc.
B. Behavior Management Plan (BMP): An individualized behavior modification plan designed to decrease and manage
continuous disruptive behaviors and increase or strengthen the pro-social behaviors of an offender.
C. Evidence-Based Practice: the body of research and replicable clinical knowledge that describes contemporary
correctional assessment, programming and supervision strategies that lead to improved correctional outcomes such
as the rehabilitation of offenders and increased public safety.
D. Incentive Level System: A behavior driven, progressive, incentive based system which encourages appropriate
offender behavior.
E. Multi-disciplinary Team: A committee composed of representatives from custody/control, mental health, case
management, and from other disciplines such as medical or psychiatry as needed or required.

3. Mental health staff shall provide evidence based treatment, from an approved list of treatment curriculums, which
includes individual, group, or activity therapies. Therapy booths and tether tables may be used to deliver residential
treatment programs as determined by the facility.
4. Academic, drug and alcohol, and mental health employees shall work together to design program delivery
options to meet the needs of the residential treatment program population. Offenders with special education needs
will receive appropriate treatment within the program.
5. Levels 1 through 3 will be afforded at least one monthly out of cell meeting with a multi-disciplinary team.Discussion should include but not be limited to the following topics:
a. Current incentive level;b. Current progress in the program;c. Current active participation/involvement in the program;d. Program compliance;e. Offenders efforts to obtain life skills necessary to be successful upon release, willingness to participate in the
program;
f. Current behavior while in the program;g. Prior behavioral problems that the offender has exhibited that may or may not have resulted in COPD
convictions. Talk about what is needed to overcome these problems, i.e. making better choices;
6. Mental Health employees and/or case mangers, in collaboration with custody/control employees, will submitoffenders for consideration of level changes in the multi-disciplinary meeting. This meeting will convene on a
weekly basis for all Residential Treatment Programs.
B. REFERRAL AND ASSIGNMENT
1. Mental Health staff will review offenders with mental illness who meet criteria for specialized programs based on
diagnosis and treatment needs. If offenders are referred from other facilities, the mental health clinicians shall
exchange information on the offenders most recent incentive level privileges, treatment plan and
compliance, psychiatric stability and medication compliance, current institutional adjustment and behavior,
including any COPDs, and any current behavior management plans. Offenders with mental health needs will
be reviewed for placement according to treatment needs by diagnosis:
a. Offenders with serious mental illness (primarily Axis I)b. Offenders with adjustment disorder(s) or Axis II disorder(s) that impair their ability to function in general
population.c. Male offenders with ADA or mobility issues will be referred to San Carlos for treatment placement and will not behoused at CCF.
2. Prioritization for Treatment: Offenders will be prioritized for program placement according to the following needs:a. 1stPriority: Serious mental illness:
1) Psychotic disorder (schizophrenia, bipolar disorder, major depression disorder, delusional disorder)
2) Organic disorders (Traumatic Brain Injury, dementia)
b. 2ndPriority Long term impairment as evidenced by numerous or serious:

1) Crises

4) Developmental disabilities

3. Offenders assigned to residential treatment programs shall receive an orientation to the facility as described in AR
850-07, Offender Reception and Orientation.
4. Once offenders are assigned to a residential treatment program, an individualized treatment plan shall be developed
by the mental health staff and an incentive level shall be assigned by the multi-disciplinary team.
5. The mental health clinician shall share relevant behavioral and clinical information with the multi-disciplinary team.The multi-disciplinary team shall convene within five working days of the offenders arrival to the San Carlos
Correctional Facility. Offenders arriving from the San Carlos Correctional Facility will be placed in a unit with
comparable privileges to the unit they left.
6. A Behavior Management Plan (BMP) may be developed to address specific offender behavioral problems that are
unable to be addressed under the guidelines of the residential treatment program. A BMP will govern offender
management to ensure safety and security, regardless of level and privileges warranted under the residential treatment
program. Upon removal of a BMP, offenders shall be reviewed by the multi-disciplinary team for appropriate level
placement.
7. A planned intervention, through a multi-disciplinary meeting, consisting of a minimum of a mental health staff, a
housing lieutenant, and a case manager or their designees may be held with an offender to address negative behavior,
negative actions, or conditions that negatively affect an offenders ability to succeed in the residential treatmentprogram. Offenders will be informed of expected behavior required and expected participation in groups in order to
complete the program. If after efforts have been exhausted and an offender continues to refuse to participate in the
Residential Treatment Program, the offender will be returned to the facility that referred him to the program. In some
instances, placement at San Carlos Correctional Facility may occur if an offenders mental health status significantly
deteriorates.
8. When a residential treatment program participant transfers to another special needs program or to general
population, a mental health clinician from the sending facility shall contact a mental health clinician at the receiving
facility to discuss continuity of care and transition of incentive level privileges. When an offender progresses from
one program to another, the incentive levels and privileges at each level will be comparable.
9. When an offender who is currently on COPD sanctions enters the program, the multi-disciplinary team may suspend
restrictions to further the goals of the program. If an offender that has been granted a suspension of LOP restrictionsreceives a new COPD with further LOP restrictions, the suspended LOP restrictions may be reinstated. This will be
discussed with the offender and documented in a multi-disciplinary team meeting.
10. Offenders in a residential treatment program will have access to faith services as defined in AR 800-01, Religious
Programs. Attendance at group activities will be done according to the incentive level.
11. The multi-disciplinary team will meet at least weekly to discuss residential treatment program progression and
regression movement for all levels. The decision to progress or regress offenders in the program will be a multi-
disciplinary decision. If the multi-disciplinary team cannot reach a consensus regarding a progression or regression, the
Health Services Administrator will make the determination.

C. TREATMENT NEEDS DELIVERY
1. Centennial Correctional Facility (CCF): The CCF residential treatment program manages offenders with
mental illness through a clinical and behavioral management system as well as through recommended mental
health treatment. CCFs offender management system utilizes clinical treatment units and provides incentives to
offenders who participate and demonstrate behavioral control.
a. Residential Treatment Program Assignment: Offenders are referred for assessment by mental health
clinicians for placement in the residential treatment program. Once an offender has been selected for the
program, they are placed in an appropriate day hall/unit based on diagnoses, behavior and treatment plan.
b. Level Descriptions: Within the CCF residential treatment program, there are 8 progressive management levels:
1) LEVEL 1: Is the most restrictive level of the treatment program. Offenders who have displayedinappropriate behavior may be assigned at level 1. Offenders at this level will be evaluated for
appropriate behavior. Offenders assigned to this level will continue to be offered mental health services.
Offenders will not normally enter the program on this level.
a) PRIVILEGES: Offenders within this level shall be afforded the following:
i. One (1) hour per day, five (5) days per week in the exercise yard and shower
ii. One (1) telephone session per month, not to exceed 20 minutes.
iii. One (1) non-contact visiting period per month.
iv. If the offenders account balance allows: Ten dollars ($10.00) per week of canteen, limited to
purchase of hygiene items and stamps.
v. Offenders at this level are not allowed special canteen items or catalog purchases.
vi. Offenders are allowed to posses two sets of personal thermals.
b) PROGRESSION: In order to progress from Level 1 to Level 2 within the residential treatment
program incentive level system, an offender must:
i. Complete a minimum of seven (7) consecutive days of positive behaviors, demonstrated by
meeting target behaviors, while at Level I.
ii. Positive participation in a minimum of one individual therapy session with a mental health therapist.
2) LEVEL 2: Is a restrictive level within the treatment program. Offenders who have displayed inappropriate
behavior may be assigned at Level 2. Offenders placed at this level will be evaluated for appropriate
behavior. Offenders assigned to this level will continue to be offered mental health services. Offenders will be
reviewed by the multi-disciplinary team weekly, until progression to a higher level. Offenders will not
normally enter the program on this level.
a) PRIVILEGES: Offenders within this level shall be afforded the following:
i. One (1) hour per day, five (5) days per week in the exercise yard and shower.
ii. Four (4) telephone session per month, not to exceed 20 minutes.
iii. Two (2) non-contact visiting periods per month.
iv. If the offenders account balance allows: Twenty five dollars ($25.00) per week of canteen.
v. Offenders at this level are not allowed special canteen items or catalog purchases.
vi. One 12" or 13" television set for access for program delivery and recreational interest. A TV
(loaner) may be allowed per TV agreement guidelines.

b) PROGRESSION: In order to progress from Level 2 to Level 3 within the residential treatmentprogram incentive level system , an offender must:
i. Complete a minimum of seven (7) consecutive days of positive behaviors, demonstrated by
meeting target behaviors, while at Level 2.
ii. Identify the behaviors that resulted in regression and develop a plan to modify the behavior.
iii. Positive participation in a minimum of one individual therapy session with a mental health therapist.
3) LEVEL 3: Upon entrance into the residential treatment program, offenders will normally be assigned to this
level. This is considered the transitional level. Offenders at this level will begin participation in recommended
therapy and will be evaluated by the multi-disciplinary team when the criteria for progression has been met.
a) PRIVILEGES: Offenders within this level shall be afforded the following:
i. Six (6) telephone session per month, not to exceed 20 minutes.
ii. One (1) hour per day, five (5) days per week in the exercise yard and shower.
iii. Four (4) non-contact visiting periods per month.
iv. If the offenders account balance allows: Thirty dollars ($30.00) per week of canteen.
v. Work assignments, (When there are no offenders at a Level 4 or higher, a porter may be
assigned from this level, once the offender has watched the required videos and has
successfully completed the required tests) (Hazardous Communications, Blood Bourne
Pathogens, J-Fill Blood Spill Clean-up, Inmate Porter Orientation).
b) PROGRESSION: In order to progress from Level 3 to Level 4 within the residential treatment
program incentive level system, an offender must:
i. Complete level 3 core curriculum (8 Lessons) or equivalent with active participation.ii. Offenders must demonstrate positive behavior, demonstrated by meeting target behaviors, for a
minimum of ten (10) consecutive days while at a Level 3.
iii. Demonstrate the ability to describe a cognitive behavioral change model in an interview with
mental health staff.
iii. Mental Health employees and/or case managers, in collaboration with custody/control employees,
will submit offenders for consideration of level changes in the Multi-disciplinary Team.
4) LEVEL 4: At this level, an offender has demonstrated program participation and is deemed less risk and
warrants increased privileges through the course of their treatment program.
a) PRIVILEGES: Offenders within this level shall be afforded the privileges from the previous level
with the addition of the following privileges: (visiting sessions are not in addition to the previous
level)
i. One (1) hour per day five (5) days per week in the exercise yard and shower.
ii. If the offenders account balance allows: Thirty five dollars ($35.00) per week of canteen.
iii. Offender will be assigned pod porter duties on a rotating monthly basis as an opportunity for
offenders to have more out of cell time. Offenders will not be forced to participate as a porter.
This is a non-paid position.
iv. Based on facility need, porters may be assigned by the housing supervisor and job board, once the
offender has watched the required videos and has successfully completed the required tests.

Orientation).

v. Four (4) non-contact visiting periods per month, maximum of two (2) per week. May have visits onthe same day.
vi. Offenders may walk to the exercise area and showers unrestrained and unescorted.
vii. Unrestrained day hall activities for a maximum of 2 offenders, (1 hour).
b) PROGRESSION: In order to progress from Level 4 to Level 5 within the residential treatment
program incentive level system, an offender must:
i. Complete level 4 core curriculum (8 Lessons) or equivalent with active participation.
ii. Offenders must demonstrate positive behavior and meet target behaviors for a minimum of
fourteen (14) consecutive days while at a Level 4.
iii. Mental Health employees and/or case managers, in collaboration with custody/control
employees, will submit offenders for consideration of level changes in the multi-disciplinary
team meeting.iv. Describe examples of how the cognitive model applies to their personal high risk situations in
an interview with mental health staff.
5) LEVEL 5: By this level, an offender has demonstrated program participation and is deemed less risk.
a) PRIVILEGES: Offenders within this level shall be afforded the privileges from the previous level
with the addition of the following privileges: (visiting sessions are not in addition to the previous level)
i. One (1) hour per day five (5) days per week in the exercise yard and shower.
ii. If the offenders account balance allows: Thirty five dollars ($35.00) per week of canteen.
iii. Unrestrained Day hall activities for a maximum of 4 offenders, (1 hour).
iv. Offenders may be escorted within the facility unrestrained, (maximum of 4).
v. An offender may participate in a structured unrestrained gym/recreation for four (4) offendersat a time.
vi. Four (4) unrestrained offenders may participate in group therapy in the programs area.
vii. Four (4) non-contact visiting periods per month, maximum of two (2) per week. May have
visits on the same day.
b) PROGRESSION: In order to progress from Level 5 to Level 6 within the residential treatment
program incentive level system, an offender must:
i. Complete level 5 specialty curriculum or equivalent with active participation.
ii. Offenders must demonstrate positive behavior and meet target behaviors, for a minimum of
fourteen (14) consecutive days while at a Level 5.
iii. Demonstrate an ability to identify antisocial thinking and behavior that contributed to their
restricted classification in an interview with mental health staff.iv. Mental health employees and/or case managers, in collaboration with custody/control
employees, will submit offenders for consideration of level changes in the multi-disciplinary
team meeting.
6) LEVEL 6:
a) PRIVILEGES: Offenders within this level shall be afforded the privileges from the previous level
with the addition of the following privileges: (visiting sessions are not in addition to the previous
level)

i. Unrestrained day hall activities for a maximum of 8 offenders, (1 hour).ii. If the offenders account balance allows: Thirty five dollars ($40.00) per week of canteen.
iii. An offender may participate in a structured unrestrained gym/recreation for eight (8) offenders
at a time.
iv. Eight (8) unrestrained offenders may participate in group therapy in the programs area.
v. Six (6) non-contact visiting periods per month, maximum of two (2) per week. May have visits
on the same day.
vi. An additional two (2) phone session for a total of eight (8) phone sessions per month, not to
exceed 20 minutes each.
vii. An offender may participate in unrestrained library access with no more than four (4)
offenders at a time.
b) PROGRESSION: In order to progress from Level 6 to Level 7 within the residential treatment
program incentive level system, an offender must:
1) Complete level 6 specialty curriculum or equivalent with active participation.1) Offenders must demonstrate positive behavior and meet target behaviors for a minimum of twenty-
one (21) consecutive days while at a Level 6.
2) Begin a Wellness Recovery Action Plan (WRAP) or Relapse Prevention Plan that describes risksand plans for successful progression.
3) Mental Health employees and/or case managers, in collaboration with custody/control employees,will submit offenders for consideration of level changes in the multi-disciplinary team meeting.
7) LEVEL 7:
a) PRIVILEGES: Offenders within this level shall be afforded the privileges from the previous level
with the addition of the following privileges: (visiting sessions are not in addition to the previouslevel)
i. Unrestrained day hall activities for a maximum of 8 offenders, (1 hour).
ii. If the offenders inmate account balance allows: forty five dollars ($50.00) per week of
canteen purchases.
ii. Six (6) non-contact visiting periods per month, may have 2 of the visits on the same day,
maximum of 3 per week.
iii. Offenders will be allowed to participate in offender photo opportunities when offered through
recreation. Offenders must have funds in their banking account in order to participate.
Recreation staff will provide information/guidelines to offenders that meet the criteria.
iv. An offender may participate in unrestrained library access with no more than eight (8)
offenders at a time.
v. Offenders may be allowed to eat meals at the tables up to (8) at a time. If more than (8) requestto do so, the Unit/Pod Sgt will implement a rotating meal schedule by tier.
b) PROGRESSION: In order to progress from Level 7 to Level 8 within the residential treatment
program incentive level system, an offender must:
i. Complete level 7 specialty curriculum or equivalent with active participation.
ii. Offenders must demonstrate positive behavior and meet target behaviors for a minimum oftwenty-eight (28) consecutive days while at a Level 7.
iii. Complete and present a Wellness Recovery Action Plan (WRAP) or Relapse Prevention Plan totheir therapist and/or group which is approved by the therapist.
iii. Mental health employees and/or case managers, in collaboration with custody/control

employees, will submit offenders for consideration of level changes in the multi-disciplinaryteam meeting.
8) LEVEL 8:
a) PRIVILEGES: Offenders within this level shall be afforded the privileges from the previous level
with the addition of the following privileges: (visiting sessions are not in addition to the previous
level)
i. Unrestrained day hall activities for a maximum of 16 offenders, (1 hour).
ii. If the offenders inmate account balance allows: A total of $60.00 per week of canteen
purchases.
ii. Unlimited phone sessions during all day halls and/or in cell.
iii. A total of seven (7), three (3) hour periods per week for day hall or yard activities, use ofphone and shower.
iv. Up to 16 unrestrained offenders may be out in the day hall at one time for activity therapy,
board games and meals.
v. Offenders are eligible for eight (8)contact visits a month, maximum of twoper week.
vi. Offenders will receive one recreation period in the gym once per week as scheduled in groups
of sixteen (16) offenders at a time.
vii. Offenders will be allowed to participate in offender photo opportunities when offered through
recreation. Offenders must have funds in their banking account in order to participate.
viii. Offenders are eligible to be escorted to the library one time per week in groups of up to eight
(8) offenders at time.
ix. Offenders are eligible to participate in scheduled religious programming in groups of up to
eight (8) offenders at a time in accordance with AR 800-001.
b) PROGRESSION: In order to progress from Level 8 to program completion from the residential
treatment program incentive level system, an offender must:
i. Complete level 8 specialty curriculum or equivalent with active participation.
ii. Offenders must demonstrate positive behavior and meet target behaviors while at a Level 8.iii. Complete and present a Transition Plan to their therapist and/or group which is approved by the
therapist.
iv. Mental health employees and/or case managers, in collaboration with custody/control
employees, will submit offenders for consideration of level changes in the multi-disciplinary
team meeting.
c. CRITERIA FOR MOVEMENT/TRANSITION TO PROGRESS OUT OF THE CCF RESIDENTIAL
TREATMENT PROGRAM:
1) Completion of an approved WRAP program and the ability to describe this recovery plan in an
interview setting with Mental Health staff.
2) Completion of an approved Transition Plan and describe it in an interview with mental health staff.
3) Adequately addressed antisocial thinking and behavior that contributed to their classification
assignment. (Must be able to describe the cognitive change model, give examples of how they apply the
model, how they addressed their target behaviors.)
4) Adequate level of psychiatric stability:
a) Brief Psychiatric Rating Scale (BPRS) score of 45 or below
b) Resource Consumption Scale (RCS) of 8 or below

7) Offenders successfully completing the requirements of the residential treatment program are eligible forreturn to general population or transitional programs at a designated facility.
8) The multi-disciplinary team will meet upon recommendation from the treating mental health clinician to
determine successful completion and refer for transitional program or general population.
2. San Carlos Correctional Facility: The San Carlos Correctional Facility (SCCF) is a specialized treatment
facility designed to assess and treat those offenders with acute mental illness who present a danger to
themselves or others. The Residential Treatment Program is delivered using incentive levels and
a multi-disciplinary team approach to assist offenders in reaching treatment goals.
a. Offenders housed at SCCF have been identified through mental health needs assessment and placement for
testing and treatment as indicated by psychiatric referral. All offenders arriving at SCCF will receive
diagnostic testing for program placement and remain housed per facility policy for safety and security.
b. SCCF offers a behavior driven program which allows for acute and chronic intervention for offenders to meet
target behavior/goals.
c. Level Descriptions: Within SCCF, there are level descriptions for each living unit.
1) 4-Right Assessment and Orientation Unit
a) All offenders arriving for evaluation at SCCF will be initially housed on 4 Right. Exception to the
intake process is authorized for offenders who are sent for the developmentally disabled unit-2 Left
or offenders deemed too violent, dangerous or disruptive for placement in 4-right. Offenders with
violent, dangerous or disruptive behavior can be sent directly to 2 West for intake and orientation.
Upon initial arrival to SCCF, all offenders will be placed on an Assessment/Orientation (A&O)
lock down for up to 10 calendar days, unless otherwise recommended for progression out ofAssessment/Orientation by the SCCF Staffing Team. Offenders will receive property per A&O
property allowable list. Offenders will be able to take a shower after the initial 72 hrs.
b) During A& O assignment to 4-Right, each offender will be evaluated by the staffing team to
determine their clinical treatment/program needs.
c) The staffing team will determine the appropriateness of an offenders continued placement at
SCCF and maintain a current internal move list based on the offenders behavioral stability and
treatment needs. All offenders will have a treatment plan completed. Once the offender has
completed the A&O process, they will be assigned as a Level 1 for 7 days. Level 1, the offenders
will have the following privileges:
(1) 5 hours out a week, 4-5 offenders at a time.(2) 1 phone call per week
(3) 5 showers a week
(4) Meals are inside their cell
(5) One non contact visit during scheduled visiting times
(6) Fifteen ($15.00) Dollars a week for canteen
(7) Property consistent with their custody level.
(8) Law Library and Library access through the Library Request Form.
d) Once the offender has completed Level 1 status, without any issues/concerns, they can progress to

a) Offenders housed on the 3-Right will be afforded all privileges allowed on the 4
th
floor plus thoseprivileges as defined on 3-Right Chronic Stabilization Incentive Level System.
b). Chronic Stabilization Level 1
All offenders on initial assignment to 3 Right will be assigned as a Level 1 for the initial 14 days. During
the initial 14 days while assigned to Level 1 on 3-Right, the offenders will have the following
privileges:
(1) Access to day hall, one hour per day a minimum of five days per week.
(2) Twenty Five Dollars ($25.00) per week of allowable canteen purchases.
(3) Law Library and Library access through the Library Request Form.
(4) Eight (8) offenders may be out at the same time for open day hall, and unit recreation. When
offenders are out of their cell, a minimum of two CDOC staff members will be present on the unit.
(5) Non -Contact visits during scheduled visiting times.
(6) Any deviation to policy under these circumstances would require a specific multi disciplinary
Behavior Management Plan is developed for Custody and Control employees to follow prior to
implementation.
c) Chronic Stabilization Level 2
After completing their 14 day review and receiving staffing team approval, the offender will
progress to a Level 2 and will be afforded all of the following privileges:
(1) Twenty Five Dollars ($25.00)per week of allowable canteen purchases.
(2) Law Library and Library access through the Library Request Form.
(3) Access to day hall, 1 hours per day 7 days per week.
(4) Eight (8) offenders may be out at the same time for open day hall, and unit recreation. When
offenders are out of their cell, a minimum of two CDOC staff members will be present on the unit.
( 5) Open access to unit telephones during scheduled day hall.
(6) Open access to showers during scheduled day hall.
(7) Non-Contact visits during scheduled visiting times.
(8) Loaner T.V. privileges (with unit employee approval and contract agreement).
d) The staffing team will conduct Level reviews once a week. Any DOC employee may request a
special review based on possible mental health or facility needs. A COIII or above may order a
temporary level reduction pending a full review. The following criteria will be used to progress to
Level 2.

(2) Non-compliance with Medical or Mental Health treatment plansor medication regimen.
f) A KIOSK for the hearing impaired is available for use on 3 Right.
4) 3 Left Progressive Stabilization Unit
a) All offenders on initial assignment to 3 Left will be assigned as a Level 2 for the initial 7 days. During
the initial 7 days while assigned to Level 2 on 3-Left, the offenders will have the following
privileges
b) Progressive Stabilization Level 2
(1) Twenty Five Dollars ($25.00)per week of allowable canteen purchases.

3, a rotating meal schedule will be utilized.
d) The staffing team will conduct Level reviews once a week. Any DOC employee may request a
special review based on possible mental health or facility needs. A COIII or above may order a
temporary level reduction pending a full review. The following criteria will be used to progress to
Level 3.
(1) Be program compliant e.g. maintain hygiene of self and cell and follow DOC employee
directives.
(2) Be treatment and medication compliant.
(3) Be 30 days COPD free.
(4) Be 15 days free of any clinical seclusion.
(5) Once level 3 has been achieved and maintained for 15days, offender will be eligible to advance
to Unit 1-Left Treatment Unit.
e) Level reductions will not occur without a complete multi-disciplinary review. Level reductions may be
considered for the following reasons:
(1) Program non-compliance, demonstratinguncooperative behavior with DOC employees, not

(k) Offenders will be allowed to eat in SCCF Food Service.
(l) Access to Offender Jobs, ie Food Service, Housekeeping, Unit Porters.
(3) The staffing team will conduct Level reviews once a week. Any DOC employee may request a
special review based on possible mental health or facility needs. A COIII or above may order
a temporary level reduction pending a full review. The following criteria will be used to
progress to Level 2.
(a) Be program compliant e.g. maintain hygiene of self and cell and follow DOC employee
directives.
(b) Be treatment and medication compliant.
(c) Be 30 days COPD free.
(d) Be 30 days free of any clinical seclusion.
(4) Level reductions may be considered for the following reasons. No level reductions will occur

(a) Program non-compliance, demonstrating uncooperative behavior with DOC employees,
not maintaining appropriate hygiene directives.
(b). Non-compliant with Medical or Mental Health treatment plans or medication regimen.
(5) A KIOSK for the hearing impaired is available for use on 2 Right.
6). 2 Left - Developmentally Disabled Unit
a) Offenders assigned to the Developmentally Disabled Program will receive their intake orientation and
assessments in 2 left upon their arrival.
b) 2 Left (Developmental Disabled) uses the Behavior Risk Assessment, Intervention and Incentive Level
System (BRAIILS) to control and monitor the behavior of the offenders on the unit. BRAIILS consist
of three (3) incentive levels. Offenders housed on this unit will have privileges (BRAIILS) determined
by an in-house group living team which consists of three (3) or more DOC employees assigned to 2
Left. BRAIILS provides for increasing or decreasing levels (Level A, Level B and Level C) of

c) All offenders assigned to 2 Left with hearing disabilities will have access to a TTY phone from the 2
Left control center to use any time they have day hall privileges to make phone calls.
d) Upon initial arrival to SCCF offenders will not receive any property until cleared by Mental Health
staff. Unit staff will search all property before issuing it to the offender. Offenders housed on 2 Left
will be afforded the following privileges.
(1) Level C
(a) 1.5 hours of day hall per day. When offenders are out of their cell, a minimum of two
correctional employees will be present on the unit.
(b) Fifteen dollars ($15) allowable weekly canteen.
(c) Law Library and Library access through the Library Request Form.
(d) Offenders will eat meals in their cell on the unit.
(2) Level B
(a) 1.5 hrs. of day hall per day. When offenders are out of their cell, a minimum of two CDOC
staff members will be present on the unit.

a) 2 West Level One: [4-4249]
(1) Within 2 west, there are three (3) incentive based programming levels. Level one is the lowest
level of the 3 level system.
(2) Offenders on level one shall be afforded the following privileges:
(a) Five (5) hours per week out-of-cell time as scheduled in the unit.
(b) One (1) phone call per month as scheduled in the unit.
(c) Five (5) supervised showers per week as scheduled in the unit.
(d) Three (3) in-cell meals per day.
(e) One (1) non-contact visiting period per month.
(f) Ten dollars ($10) per week of canteen limited to purchases of hygiene items and stamps. No
tokens.
(g) Property consistent with their custody level. Upon initial arrival to SCCF offenders will not
receive any property until cleared by mental health staff. Unit staff will search all property
before issuing it to the offender.
(h) Law Library and Library access through the Library Request Form.
b) 2 West Level Two[4-4249]
(1) In order to progress from status level one to status level two of the incentive level system, an
offender must:
(a) Have no conviction of the Code of Penal Discipline (COPD) for a minimum of 30 days.
(b) Demonstrate positive behavior for seven (7) consecutive days, unless otherwise specified
in a Behavioral Management Plan.
(c) Be approved by the staffing team.
(2) Offenders on status level two will be afforded the following privileges:
(a) Five (5) hours per week out-of-cell time as scheduled in the unit.
(b) Two (2) phone calls per month as scheduled in the unit.(c) Five (5) supervised showers per week as scheduled in the unit.
(d) Two (2) non-contact visiting periods per month.
(e) Fifteen ($15.00) per week of canteen limited to purchases
allowed on the unit. No tokens.

(3) 2-West Level Regression: From status level two to status level one
(a) The housing supervisor (or above) and mental health staff will have the authority to
reassign offenders to level one from level two prior to a multi-disciplinary team
meeting. Documentation for this decision will be to make an entry into the offenders
chronological report and complete an electronic informational report via PCDCIS
documenting the inappropriate behavior and/or program non-compliance.
(b) Level reduction to Level One is automatic anytime the force cell team is activated to
manage behavior In this case, the decision should bedocumented in an
informational incident report, progress notes and pass-on log, including a
description of the behavior that warranted theregression. [4-4260]The level
regression will be reviewed at the next scheduled level review by the staffing team.
c) 2 West Level Three: [4-4249]
(1) In order to progress from status level two to status level three, an offender must:
(a) Have been at the level of status level two for a minimum of sixty (60) consecutive days or as
specified in a Behavior Management Plan (BMP).
(b) Be free of Class I COPD convictions violation during the last sixty (60) days, or a Class II
COPD within the last thirty (30) days or as specified in a Behavior Management Plan
(BMP).
(c) Be approved by the staffing team.
(2) Offenders on status level three will be afforded the following privileges:
(a) Seven (7) hours per week out-of-cell time as scheduled in the unit.
(b) Four (4) phone calls per month as scheduled in the unit.
(c) Seven (7) showers per week as scheduled in the unit.
(d) Twenty dollars ($20.00) a week of canteen limited to purchases allowed on the unit.
Tokens will not be allowed.
(e) Law Library and Library access through the Library Request Form.
(f) Work assignments on the unit.
(g) Three (3) offenders may be out at a time for open day hall, unit recreation, and meals.
Three (3) additional offenders may be involved in supervised unit programs (e.g. anger
management group) in the multi-purpose room.
(h) Four (4) non-contact visits per month.
(i) Electrical appliances approved by the staffing team. May have a TV if the offender meetsbehavioral guidelines.
(j) Educational programs.
(k). Property consistent with their custody level.
(3) Level Regression: From status level three to status level two.
(a) The housing supervisor (or above) and mental health staff have the authority
to affect a move of an offender from quality Level 3 to Level 2 prior to a
multi-disciplinary team meeting. The initiating individuals are required to
make an entry into the offenders Chronological Report and complete an electronic

(e) All offenders with hearing impairments who are housed in 2 Left will have access to the
TTY machine for use on their scheduled telephone calls. The TTY machine will be in the
control unit.
8) 1 Right - Reintegration Unit: Offenders housed on Unit 1 Right will be afforded the following privileges:
a) Open access to day hall and shared access to the courtyard area with unit 1 left on a rotating basis.
b) Thirty-five ($35) dollars allowable weekly canteen
c) Library 3 hours per week with a maximum of 8 offenders per visit.
d) Recreation in the yard/gym with one- 1 our period per day.e) Educational/Vocational Program access outside the Unit.
f) Access to Offender Jobs, ie Food Service, Housekeeping, Unit Porters.
9) 1 Left - Treatment Unit
a) Offenders who progress or regress to 1 Left will be required to complete a 7 day orientation with
limited privileges. There is a 2 Level process for theTreatment Unit.
b) Level 1: Upon placement on 1 Left the offender is a Level 1 offender for the first 7 days with the
following privileges:
(1) Offenders will be allowed 2 hours of day hall per day 7 days per week.
(2) Up to 18offenders will be allowed out on 1 Left.
(3) Thirty Dollars ($30.00) allowable weekly canteen for the first week.
(4) Recreation on the unit only with no off unit privileges until Level 2 progression is achieved.
(5) Law Library and Library access through the Library Request Form

(1) Offenders will be allowed 3.5 hours of day hall per day, with a maximum of 18offenders
out at one time. Day hall will be opposite of recreation time to allow additional out of cell
time. When offenders are out of their cell, a minimum of two CDOC staff memberswill be
present on the unit.
(2) Thirty Dollars ($30.00) allowable weekly canteen.
(3) Library 2.25 hrs. per week with a maximum of 8 offenders per visit.
(4) Recreation in the yard/gym six days per week . If more than half the unit goes to Recreation, the
other staff member assigned to the unit will accompany them.
(5) Educational/Vocational Program access outside of the Unit.
d) All offenders assigned to 1 Left will eat meals in the Dining Hall together regardless of currentlevel.
e) A KIOSK for the hearing impaired is available for use in the common area of the 1stfloor.
3. Denver Womens Correctional Facility: Incentive level program is based on maladaptive behavior
identification, levels of intervention, and incentive levels for female offenders. This program is systematically
delivered to reinforce meeting appropriate target behaviors and/or discourage maladaptive behaviors.
a. Level Descriptions
1) Intake/Orientation: New offender admissions to the residential treatment program that arehoused in Living Unit 6, will initially be placed on incentive level one (1) for a minimum of
72 hours. Offenders will be evaluated for mental health needs through psychological testing,
clinical interviews for psychiatric evaluation, and behavior observation in order to determine
pod placement, diagnosis and course of treatment.
2) Level I: Is the most restrictive level of the treatment program. Offenders who have displayed
inappropriate behavior may be assigned or regressed to Level 1. Offenders at this level will be
evaluated for appropriate behavior for a minimum seven (7) day period. Offenders assigned to this
level will continue to be offered mental health services. Offenders will be reviewed subsequently by the
multi-disciplinary team weekly, until progression to a higher level. Offenders will not normally enter the
program on this level.
a) When escorted out of the cell, but within the day hall area, a Level I offender will be restrained withwrist restraints behind the back and a minimum of 2 staff hands on escort.
b) When escorted out of the cell and outside of the day hall area, a Level I offender will be
restrained in leg restraints with universal restraints and a minimum 2 staff hands on escort.
3) Level 2A: Following the Intake/Orientation period, offenders will normally be assigned to this level.
Offenders at this level will begin participation in recommended therapy and will be evaluated by the multi-
disciplinary team for acceptable behavior and program compliance with mental health treatment, meeting
target behavior goals. Offenders will be reviewed weekly by the multi-disciplinary team weekly, until
progression to a higher level.

4) Level 2B: Offenders at this level have demonstrated program participation and pro-social behaviormeeting target behavior goals. The program will be comprised of increased treatment participation that
will correlate with privileges matching level designation. Offenders will be reviewed subsequently by the
multi-disciplinary team weekly thereafter, until progression to a higher level.
5) Level 3A: Offenders at this level have demonstrated continued program participation and pro- social
behavior, meeting target behavior goals and are deemed less risk. Offenders at this level have consistently
shown proper interaction with DOC employees and other offenders, maintained suitable cell conditions
and appropriate behavior, and overall positive behavior. Offenders at this level have the opportunity for
job assignments within the unit and have privileges matching level designation.
6) Level 3B: The least restrictive level. Offenders who have maintained program and treatment compliance, pro-
social behavior, target behavior goals and are prepared to begin the transition back into general population or
the community. These offenders are stable and are deemed less risk. Offenders have met targetbehaviors/goals and have privileges matching level designation.
7) Level 4/Reintegration: Offenders identified as level four will receive privileges in accordance with their
housing unit assignment within general population.
b. Level Privileges
1) LEVEL 1:
a) 2 hours of day hall (may go to all assigned groups).
b) Canteen purchases up to $12.00 ($10.00 for hygiene and $2.00 for non-hygiene items).
c) Unassigned Pay.
d) Satellite meals, recreation, and library.e) One phone call per week.
f) No access to the soda machine or ice machine.
g) May not purchase/own items from the regulated list.
2) LEVEL 2A:
a) Day hall on day shift only (may go to all assigned groups).
b) Canteen purchases - $20.00 limit, 8 tokens (phone time and hygiene are not counted).
c) 30 cents incentive pay.
d) Satellite meals, recreation, and library.
e) Unlimited phone usage.
f) One job on the unit only (no off unit job).
g) May not purchase/own items from the regulated list.h) May not have access to soda machine.
i) May have access to a flex pen.
3) LEVEL 2B:
a) Day hall when not attending scheduled programs.

a) May have 75.00 Cosmetics and Cell permit
b) Canteen purchases $75.00 limit (phone time and hygiene not counted).
c) May have special cosmetology privileges
d) Recreation privileges off the unit with Incentive Unit.
e) Washer and dryer privileges
f) May purchase 2 catalog blankets and retain one hobby blanket for a total of 5 blankets.
g) Access to irons.
c. Temporary Suspension of Privileges: Offenders may temporarily lose a privilege if their behavior does not
meet standards for that quality level. An incident report must be completed and submitted to the Unit
Supervisor for review and approval. The completed report will be forwarded to the Shift Commander. This
behavior modification tool is separate from the Code of Penal Discipline (COPD) process.
1) Examples of inappropriate behavior may include but are not limited to:
a) Refusal to return any portion of their utensils or food tray to DOC employee(s).
b) Failure to clean the cell (per Posted Operational Rules) or maintain it in an orderly
fashion.
c) Poor personal hygiene.
d) Pounding on the cell door, shower wall or door, or exercise area door.
e) Failure to turn in the bedding and clothes for regular washing.
f) Exhibiting disrespectful actions toward DOC employee(s), visitors, or other
offenders.
g) Refusal to return the electric razor.
h) Hindering the cell inspection process.i) Temporary suspension of privileges, as approved by the housing captain and mental health
staff may include one or more of the following:
i. Television suspension up to three (3) days at a time, per incident, five (5) days second
incident.
ii. Day hall suspension up to 3 days.

d. Level progression/regression
1) The multi-disciplinary team will meet on a weekly basis to conduct staffing reviews, establish treatment
goals, evaluate BMPs, and review progressive and regressive movements within the unit.
2) Progression from one level to the next will depend upon the following consistent
behavior/criteria:
i. Must attend all recommended treatment groups and appointments and be program compliant.
ii. Must not display any maladaptive behaviors and display positive behavior, meeting target
behavioral goals.
ii. Progressive level changes will be approved by the Treatment Team at the weekly treatment
meetings.
iv. Progressive level changes will be documented in the Case Management Tracking System.v. Recommended level changes will be reviewed and approved by the Treatment Team at the
weekly staffing.
3) Regression: Any DOC employee has the authority to recommend the reassignment of offenders to a lower
level based on negative behavior. This behavior will be documented in the electronic chronological system.
The housing supervisor (CO III or CO IV) and mental health staff, will be notified of the recommendation
and approve or deny the reduction in level prior to a scheduled multi-disciplinary team review meeting.
i. An electronic incident report must be completed documenting the negative
behavior and the housing supervisor ( CO III or CO IV) and mental health staff who reviewed and
approved the action.
ii. Regressive level changes will be documented on the level review form (Attachment C) and in the CaseManagement Tracking System (CMTS). Recommended level changes will be reviewed and approved
by the treatment team at the weekly multi-disciplinary team meeting.
V. RESPONSIBILITY
A. It will be the responsibility of the Chief of Behavioral Health Services and the Assistant Director of Prisons, Clinical
Services, to enforce and maintain this administrative regulation.
B. It will be the responsibility of administrative, program, and correctional DOC employees and contract workers to be
familiar with this administrative regulation.
