IMA. Our last CME Programme was held on 19.06.2011. Dr.
Arvindh Damodaran gave us an interesting update on Medical
Management of Jaundice. The other three young surgeons
Dr.Nambi, Dr. Diwaker and Dr. Suresh - the faces of the future,
gave us an insight into the nature of tough work they do in Head
and Neck surgery and Oncoplastic reconstruction.
Apart from that, in our regular activities on behalf ofour Association, we met Mr. Ananadkumar IAS, our dynamic

Bidhan Chandra Roy, M.R.C.P., F.R.C.S. (Bengali: 1 July
1882-1 July 1962) was the second Chief Minister of West Bengal in
India. He remained in his post for 14 years as a Indian National
Congress candidate, from 1948 until his deathin 1962. He was a
highly respected physician and a renowned freedom fighter.
Bidhan Roy is often considered as the great architect of West
Bengal, who had founded two eminent cities Kalyani and
Bidhannagar. He was an alumnus of the Medical College Calcutta of the University ofCalcutta. He is one of the few people who completed both F.R.C.S. and M.R.C.P.
simultaneously within only two years and three months. In India, the National Doctor
s Day is celebrated on the date of his birth (and death) July 1 every year. Dr. Bidhan
Chandra Roy constituted a trust for his properties at Patna for social service and made
eminent nationalist Ganga Sharan Singh (Sinha) the trustee. He won the Bharat Ratna
in 4 February 1961, India's highest civilian honour. He was also a member of the
Brahmo Samaj.

stDOCTORS DAY" is celebrated on July 1 in commeneration of the Birth
and Death anniversary of Dr. B.C. Roy the freedom fighter, the first President of
the MCI and a noted Physician par excellence.
IMA TNSB conducts Annual Doctors Day celebration. During the celebrationas per the decision of the President, senior Doctors will be honored in the
function organized by the State office in association with a local Branch.
The awards are invited by the State Office officially in the TIMA and the
Local branch shall nominate such deserving persons.
The awards team are the three Presidents and the two Secretaries shall
decide the recipients.
Local Branches are requested to celebrate Doctors Day in their branches.
Doctors who need to be recognized for their services may be awarded and itis ideal to have a family meeting on the occasion.
Doctors Day celebration usually goes on for a month and it is the ideal time
to invite state and National Office bearers for Branch meetings.
Public personalities who have done service to the society on health aspect
and for the medical community also can be honoured.

Liver disordersSome of the common disorders affecting the liver are
Hepatitis A a common infection spreads through contaminated food orwater, causing liver inflammation and jaundice. This is usually self limiting,and can last for up to 4 to 8 weeks.
Hepatitis B and C which are spread through contaminated blood or bodyfluids, unprotected sex with an infected person, contaminated needles,razors and toothbrush shared with an infected person, or even tattooing orbody piercing if the needle used is contaminated. These infections can bepresent silently for many years, and can cause scarring and damage to theliver.
Cirrhosis is scarring of the liver, usually permanent, from many causes, andcan lead to liver failure. Common causes in India are chronic Hepatitis B andC infections and alcohol abuse.
Tumours benign and malignant (liver cancer). Metabolic disorders usually affect the newborn or children and young
adults. Fatty liver disease a very common condition that is present especially in
those who are obese or overweight, have diabetes or high levels oftriglycerides (fat), or in those who abuse alcohol. In most cases this conditionmay cause no damage to the liver, but in some can progress to liverinflammation and scarring. It is increasing in incidence with changinglifestyles and dietary habits.
Symptoms of liver diseaseLiver disease is mostly silent and is discovered many times by chance or only
when there is advanced damage to the liver. Some of the symptoms are as follows: Jaundice yellow discoloration of the eyes and skin. It is important
to note that jaundice is only a symptom and not a disease by itself, and
can occur in many types of liver diseases
Dark coloration of urine Pale stools, sometimes dark or bloody Fatigue, tiredness, nausea, loss of appetite Abdominal and leg swelling Confusion, forgetfulness, difficulty in sleeping Easy bruising and bleeding from small cuts and scrapes, or bleeding
from the gums

Alcohol and the liverThere are three kinds of liver diseases associated with alcohol
fatty liverwith liver enlargement, which will improve if you stop drinking alcoholic hepatitis, which is associated with fever and jaundice and if severe
can lead to life-threatening complications cirrhosis, a serious condition which can lead to liver failure and associated
complications. Between 10 to 20% of people who drink heavily will developcirrhosis over a period of ten years or so
Liver transplantationThis is now the procedure of choice for patients with cirrhosis and liver
failure. The diseased liver is removed and a whole liver from a cadaver donor (braindead) or a portion of liver from a living related donor is then put in its place. Thisprocedure is life saving for those with end stage liver disease who will otherwise diefrom liver failure. It can also be done for select patients with liver cancer and otherliver disorders. Many hospitals in India now perform liver transplant successfully,and bring hope to thousands of patients dying every year from liver failure.Some facts and myths about the liver
v Liver disease does not cause 'indigestion' (abdominal discomfort, gas &bloating, loose stools etc), which are usually due to a gastro-intestinalproblems
v All patients with liver disease or cirrhosis are not alcoholics; there are manyother causes for cirrhosis of the liver
'v Jaundice' has many causes; hence it is important to investigate this
thoroughly before planning appropriate treatmentv There are many herbal remedies used to treat liver diseases; whilst these
can be useful, it is important to know that some can actually harm the liver; itis important therefore to thoroughly investigate the cause of the liverdisease before undergoing any treatment
. Liver tonics do not guarantee any protection from developing liverdiseases or its complications; only a proper lifestyle, screening and early
treatment are of recommended benefit. Liver disease is silent in the majority and can only be detected by properscreening
. The liver is the only organ in the body that can grow itself, hence a person candonate part of his liver for a transplant and his or her liver will grow back toits full size in a few months
Ways to keep your liver healthy Eat well and stay healthy! A good, balanced and nutritious diet with plenty of

fresh fruits and vegetables, whole grain rice, breadand cereals, and less fried and fatty foods andreduced intake of carbs is healthy for the liver
Watch your weight! Try to maintain a near ideal bodyweight as obesity can damage the liver.
Exercise regularly! Regular exercise (at least threetimes a week) by itself reduces fat build up in theliver
Drink healthy! Limit the amount of alcohol you drinkand have alcohol free days every week; avoid heavy binge drinking.
Avoid unnecessary medications! remember that most drugs pass throughthe liver, and some are harmful
Limit toxin exposure! There is evidence that exposure to toxins fromaerosols, harsh cleaners, and insecticide sprays, household and garden
chemicals, can damage the liver. Use theseproducts in well ventilated areas, and usegloves or masks as necessary
Avoid self medication! High doses of over thecounter medication like paracetomol, painkillers and some herbal products can harm theliver; excessive use of mineral or vitaminsupplements should also be avoided
Get tested for liver disease! Early screening can prevent the many
complications of advanced liver disease, and greatly reduce the costsinvolved in managing such problems
Screen and protectyourself from Hepatitis Band C; this is especially important for thosewho have had previous attacks of jaundice,previous blood transfusion or surgery, orhealth care workers.
Remember! High risk practices like IV druguse and sharing of needles, having
unprotected sex with multiple partners canlead to infection with Hepatitis B or C; insiston sterile instruments whilst shaving your head, or tattooing, ear or bodypiercing.
Protect yourself and your family! Vaccines are now available againstHepatitis A and B.
Have a healthy lifestyle! this can go a long way in keeping you and your liverhealthy for a very long time!

Fatty Liver
There has been an alarming rise in the number of fatty livers being diagnosed. The rising
incidence of diabetes, central obesity and elevated cholesterol (particularly triglycerides) isto be blamed. Reliable population based surveys put the figure at 9 19% overall and adultage groups respectively. This is alarming because these figures resemble those fromdeveloped western countries where cirrhosis due to fatty liver disease is the second mostcommon reason for needing a liver transplant.
What is fatty liver?
Fatty liver is just what its name suggests: the build-up of excess fat in the liver cells.Normally your liver contains some fat, but if fat accounts for more than 10% of your liver'sweight, then you have fatty liver and you may develop more serious complications.
Fatty liver may cause no harm, but sometimes the excess fat leads to inflammationof the liver. This condition, called steatohepatitis, does cause liver damage. Sometimes,inflammation from a fatty liver is linked to alcohol abuse; this isknown as alcoholic steatohepatitis. Otherwise the condition is callednonalcoholic steatohepatitis, or NASH.
An inflamed liver may become scarred and hardened overtime. This condition, called cirrhosis, is serious and often leads to liverfailure requiring liver transplant.
NASH is one of the top three leading causes of cirrhosis world-wide.
What are the symptoms of fatty liver?
A fatty liver produces no symptoms on its own, preventive healthchecks or medical tests done for other conditions reveal fatty liver.NASH can damage your liver silently for years or even decades. If the disease gets worse, youmay experience fatigue, weight loss, abdominal discomfort, weakness and confusion.
What causes fatty liver?
Eating excess calories causes fat to build up in the liver. People with obesity, diabetes, hightriglycerides, alcohol abuse, rapid weight loss and malnutrition may also lead to fatty liver.However, some people develop fatty liver even if they have none of these conditions soeveryone should know about it.
How is fatty liver diagnosed?
Your doctor may see something unusual in your blood test or notice that your liver is slightlyenlarged during a routine checkup. These could be signs of a fatty liver. To make sure youdon't have another liver disease, your doctor may ask for more blood tests, an ultrasound, aCT scan may be sometimes needed. If other diseases are ruled out, you may be diagnosedwith NASH. Although rarely needed, a liver biopsy is the only way to diagnose NASH for sure.
What new treatments for fatty liver are being studied?
Fatty liver is currently the focus of intense research to provide us with better tools fortreatment in the future. Scientists are studying whether various medications can help reduce

the inflammation on your liver, including new diabetes medications that may help you evenif you don't have diabetes.
Fatty Liver can progress to Cirrhosis of liver.
How is fatty liver treated?
You can take some steps that may help prevent or reverse some of the damage. If you havefatty liver, particularly if you have NASH, you should:
Lose weight safely! That usually means losing no more than one or two pounds aweek.
Lower your triglycerides through diet, medication or both
Avoid alcohol
Control your diabetes, if you have it
Eat a balanced, healthy diet
Increase your physical activity
Get regular checkups from a doctor who specializes in liver care
If I've been diagnosed with fatty liver, what questions should I
ask my doctor? What is the likely cause of my fatty liver?
Do I have NASH? If not, how likely am I to developNASH?
Do I have cirrhosis? If not, how likely am I to developcirrhosis?
Do I need to lose weight? How can I do so safely?
Should I be taking any medication to control my triglyceride levels?
What medications or other substances should I avoid to protect my liver?
Who is at risk for fatty liver?Most (but not all) fatty liver patients are middle-aged and overweight. The risk factors mostcommonly linked to fatty liver disease are:
Overweight (body mass index of 25-30)
Obesity (body mass index above 30)
Diabetes
Elevated triglyceride levels
Abdominal surgery: Operations to remove large sections of the small intestine,treat obesity (stomach or small intestinal bypass) often lead to rapid weight loss.This may increase your risk of fatty liver disease.
Medications: particularly oral steroids eg. Prednisone, synthetic estrogens formenopause, certain medications for heart arrhythmias eg. amiodarone, tamoxifen(for breast cancer) and methotrexate (a medication for rheumatoid arthritis).
Certain other conditions: like Wilson's disease, Glycogen storage diseases,Abetalipoproteinemia,Galactosemia etc.
What is the best way to prevent fatty liver?
The best way to reduce your risk of developing fatty liver is to maintain a healthy weight andnormal triglyceride levels. You should also avoid excess alcohol and other substances thatcould harm your liver.

Once Buddha was travelling with a few of his followers. While they were passing alake, Buddha told one of his disciples, "I am thirsty. Do get me some water from the lake."The disciple walked up to the lake. At that moment, a bullock cart started crossing throughthe lake. As a result, the water became very muddy and turbid. The disciple thought, "Howcan I give this muddy water to Buddha to drink?" So he came back and told Buddha, "Thewater in there is very muddy. I don't think it is fit to drink." After about half an hour, againBuddha asked the same disciple to go back to the lake. The disciple went back, and found
that the water was still muddy. He returned and informed Buddha about the same. Aftersometime, again Buddha asked the same disciple to go back.This time, the disciple found the mud had settled down, and the water was clean
and clear. So he collected some water in a pot and brought it to Buddha. Buddha looked atthe water, and then he looked up at the disciple and said," See what you did to make thewater clean. You let it be, and the mud settled down on its own -- and you have clear water.Your mind is like that too ! When it is disturbed, just let it be. Give it a little time. It willsettle down on its own. You don't have to put in any effort to calm it down. It will happen. Itis effortless." Having 'Peace of Mind' is not a strenuous job; it is an effortless process!

In June 2011, 06 Blood Donation Camps were held. Total of 783Units of Blood were Collected.

27 Free eye camps , 2744 patients screened, 630 Free surgeries performed , 27 pairs of
eyes collected, 3 Keratoplasty done.
4 MONIKA DIABETES CENTER
June 2 Free Diabetes Detection a Awareness Programme was conducted. Totally 350patients were screened.
4 SENTHIL MULTI SPECIALITY HOSPITAL
Free Neuro Medical camp was conducted June 6th. 90 Patients were screened.
4 MAARUTHI MEDICAL CENTRE AND HOSPITALSErode Diabetes Foundation functioning in Maaruthi Medical Centre andHospitals, conducted free Diabetes Detection and Awareness Programme on26.06.2011, on Diabetes and Prevention of foot Complication. Free Blood glucoseestimation done for around 240 patients and for other potential diabetics.
