This report is one of a series that began in 1994. Section One provides technical documentation.Section Two provides detailed information by race/ethnicity, sex, and age on the leadingcauses of death for Chicago residents for 2002, the most recent locally available data.

METHODS
BACKGROUND
In order to understand and appropriately interpret the data, it is important to understand(a) cause of death determination and (b) ranking procedures.
Cause of Death Determination
All deaths are required to be registered using death certificates. Death certificates areprocessed and analyzed by a standardized system that is used in most countries throughoutthe world, the International Classification of Diseases (ICD). On the death certificate, thecertifier (a physician, coroner or medical examiner) enters information about the causesand circumstances of death in a specific sequence. When received by the vital statisticsoffice, the causes are placed in standard categories, and the underlying cause of death isthen determined. The underlying cause of death is defined as:
the disease or injury which started the sequence of events leadingdirectly or indirectly to death, or the circumstances of the accident orviolence which produced a fatal injury.
Underlying cause of death is not necessarily the first medical condition listed on the deathcertificate. All of the information on the certificate is examined systematically, usingclassification rules, to determine the most appropriate underlying cause of death.
The system is periodically revised to reflectadvances in medical knowledge and publichealth priorities. The ninth revision, ICD-9,was used for deaths in the United States fortwenty years, 1979-1998. The ICD-10 is themost recent update, and is being used for USdeaths effective with data from 1999. Thereare important differences between ICD-10and it’s predecessor, ICD-9 as highlighted inthe right hand box.

Therefore, even with the same information being recorded on the death certificate, a death canbe attributed to a different underlying cause of death. Due to these differences, mortalityrates based on data classified under ICD-10 cannot be directly compared to ICD-9.
To help evaluate trends, comparability ratios are calculated by the National Center forHealth Statistics (NCHS). These ratios are obtained by coding a set of death certificatesunder both ICD-10 and ICD-9, then comparing the differences. These ratios can then beapplied to data coded under ICD-9 to make the rates appear comparable to ICD-10 codeddata. All data in this publication cannot be compared to data from 1998 and earlier that areNOT comparability adjusted.
Tabulation Lists and Ranking
Tabulation lists are part of the ICD system. These lists standardize the reporting of datawhich makes it easier to compare data from different times and places. The lists aggregatedetailed categories for cause of death (e.g cancer of the uterine cervix) into appropriategroupings, but also highlight specific causes of death that are of public health and medicalimportance.
Ranking is performed on the basis of the tabulation lists. Not all causes of death areconsidered rankable. For example, if sub-categories of a larger category are listed on atabulation list (e.g. specific forms of cancer are listed in addition to all cause cancer), thenonly the larger category is considered for ranking purposes. Another important exceptionare causes of death that are “remainder” categories for larger groupings. As an example,the grouping “Other diseases of the respiratory system” is not considered in leading causeof death ranking.

SPECIFIC METHODS FOR THIS REPORT
Data Source
All data come from the Illinois Department of Public Health electronic vital recordsdatasets.
Ranking
For infants under 1, the number of deaths from each of the rankable causes of the l30 cause“List of 130 Selected Causes of Infant Death”, as well as all cause mortality, is tabulated.For all other age groups, the number of deaths from each of the rankable causes of the “Listof 113 Selected Causes of Death”, as well as all cause mortality, is tabulated. The ten causeswith the highest frequencies for each age group are then reported. The rankable causesfrom each list and the publication abbreviations used in this edition are listed in Tables 1and 2.
Tables
The data tables in this document include information for all Chicago residents, and tablesfor subgroups defined by gender, by selected ethnicity categories (Non-Hispanic Blacks,Non-Hispanic Whites, All Hispanics, Mexicans, Puerto Ricans and Non-Hispanic Asians) andby combined gender and ethnicity categories.
The tables provide leading cause of death information for detailed age categories. For eachof nine age categories and all ages combined (which includes people whose age at deathwas unknown), the tables list the ten leading causes of death in descending order togetherwith the total number of deaths associated with each leading cause for 2002. In the caseof a tie, deaths are listed in order of their appearance in the ICD-10 (e.g. causes that appearin Chapter 1 will be listed before causes that appear in Chapter 9). The bottom row in eachtable lists the annual number of deaths due to all causes in that age category.

Other ConsiderationsThe number of deaths is dependent on the burden of a disease in a particular group and onthe population size. Therefore, while valid comparisons can be made within a group withregard to ranked causes, it is helpful when comparing ranked causes between groups, orcomparing causes in groups over time, to calculate rates (deaths/population) in additionto using the statistics provided in this report.
In addition, there are other methods for ranking causes of death, such as calculating Yearsof Productive Life Lost (YPLL). YPLL is calculated by first determining for each death thenumber of years of life lost under age 65. This quantity is then summed across all deathsdue to a given cause to determine the YPLL for that cause. Because deaths at younger agesare weighted more heavily in the YPLL calculation, causes of death that primarily affectyounger people tend to be ranked higher using this criterion as compared to the criterionof number of deaths. Each method of evaluating the relative impact of causes of death addsits’ own unique insights.
