MED-1 Mobile Healthcare SystemThe MED-1 Mobile Healthcare System incorporates 3 slide-out pod (extensionareas) that expand the trailer workspace to approximately 92m2 (1000 ft2).One pod serves as a 2-bed operating room or shock-resuscitation area14m2 (150 ft2). The other 2 pods comprise a critical care unit that includes4 intensive care beds, 7 general emergency beds and pediatric supportcapability. For expanded capabilities, a medical shelter system may beattached to the roof of the trailer to completely envelop the facility – up to465m2 (5000 ft2) usable space. (See following page for configurationexamples). This enclosed and illuminated care area is capable of incorporatingup to 200 beds or can be configured to provide additional ambulatory medicalcare, patient counseling, other social service activities, and staff quarters.
Up to two simultaneous surgical interventions may be performed within theoperating theatre. There are multi-position operating room tables, 2 wall-mounted anesthesia machines/vaporizers, operating room lights,hemodynamic and cardiac monitors (including the anesthetic gas modules),and storage for ancillary surgical supplies are included in this area. All powerto this pod is routed through an isolation transformer for immediate interruption of electricity should an excessivesurge occur. The surgical area can be isolated from the rest of the facility during a procedure using the transparentaccordion-style wall.
The next four beds are designed for critical or intensive patient care. The hemodynamic and cardiac monitors allinclude non-invasive blood pressure monitoring, pulse oximetry, end-tidal capnometry, 3-lead and 12-leadelectrocardiography, continuous cardiac ST-segment trend analysis, and the capabilities of providing invasive arterialand central venous pressure monitoring. One of these four beds may be dedicated for pediatric critical care.
Oxygen is available at each patient care bed from an oxygen systemmounted under the trailer. Similarly, a vacuum system is adjacentto the oxygen ports. Each bed area incorporates two oxygen andtwo vacuum suction ports for redundant (back up) capabilities.Fluorescent lighting, heating, air conditioning, and airexchange/filtering are also incorporated throughout the facility.
Your pharmaceutical inventory, which includes prescription andnon-prescription medications, is stocked in a rolling cart that ispositioned near the critical care area. Diagnostic resources in theform of radiological and laboratory resources are also included.The portable digital x-ray unit is compact enough to be moved to anypatient bed. Digital technology allows for rapid image acquisitionand projection onto computer screens located in the operative and general care areas. The x-ray unit uses a minimalradiological source and the digital processing does not require chemicals. Travel on highways is unrestricted due tothe absence of these hazardous materials. Diagnostic laboratory devices use point of care technology to ensure easeof use, instruction retention, and minimize time required for operation and retrieval of test results. Lab tests includeelectrolytes and renal function tests, coagulation studies, lactate and base excess measurements, cardiac enzymesand hemoglobin levels, toxicological screening, urinalysis, pregnancy testing, and complete blood counts.
A complete and redundant (back up) communications infrastructure is also incorporated. An optional on-boardsatellite system provides instant broadband internet capability, cable television, and telemedicine applications.Satellite telephones are used over cellular technology in disaster areas where cell towers may be incapacitated.Further, the computer systems include individual servers for biomedical monitoring and digital radiology. Anadditional server supports the wireless infrastructure for a patient tracking system and paperless medical recorddocumentation and charting.

Critical care expansion is possible by expanding the number of hemodynamic and cardiac monitors or by usingadditional portable cardiac monitors/defibrillators, portable ventilators, and electronic intravenous pumpspreprogrammed for all formulary medication drip rates.
The infrastructure described above supports a full and comprehensive range of medical management capabilities,including the following:
1. Operative procedures required for emergent hemorrhage control (exploration, resection, and repair)2. Comprehensive and difficult airway control and support3. Conscious sedation for limited procedures (fracture reduction or immobilization, dental procedures,
complicated wound repair, pediatric procedures, abscess drainage)4. Cardiovascular and pulmonary care (acute myocardial infarction, congestive heart failure, chest pain
evaluation, asthma or chronic obstructive pulmonary disease exacerbation, anaphylaxis)5. Minor surgical procedures (suture repair, burn debridement, foreign body removal, abscess drainage)6. Dental, eye, ear, nose, and throat care (dental abscesses drainage, temporary coverings for tooth avulsion,
corneal abrasions or foreign body to the eye, epistaxis nasal packs for uncontrolled nose bleeds, foreign bodyin the throat)
7. General medical care for multiple conditions (hyperthermia and hypothermia, dehydration, gastroenteritis,vomiting and diarrhea, seizures, back or musculoskeletal pain, allergic reactions, gynecologic or urologicemergencies, diabetic emergencies, headache)
Multiple Configurations
The unique design and construction of MED-1 allows for the flexibility to create multiple configurations. This will affordopportunities for task-specific missions depending on the current need. It can be augmented to provide ambulatorymedical care, outpatient surgical procedures, and dental care in alternative or rural settings. Examples includethe following:
1. Trauma facility with surgical capability.2. Medical resource with primary, emergency, and critical care services.3. Mobile dental or ambulatory clinic4. Cost-effective temporary medical facility to sustain patient care services should a primary medical facility’s

The Main 16.2m (53 ft) Support Trailer
The Main 16.2m (53 ft) Support Tractor-trailer is used to transport and store the unassembled mobile hospitalstructure, all of the medical supplies and related items, and a full diagnostic laboratory and pharmacy. During thedeployment, it also serves as a command center and sleeping quarters for staff. It is self -contained and includes a100 KW generator and is served by an onboard HVAC system.
The trailer is divided into three separate compartments. An internal staircaseprovides access to the upper level. A lift gate can bring down equipment stored in the upper level.
1. The front of the trailer serves as a small command center that is complete with communication devices asoutlined on the specification attachment, a shower and toilet facility for staff and is equipped witha large screen television for monitoring current weather or disaster conditions.
2. The main lower section of the trailer is used to store and transport the exam beds and operating room bedsand other primary patient care facility equipment and also serves as the main backup storage area for the basicmedical supplies used in the deployment.
3. The upper section of the main trailer compartment is used to store and transport the main hospital assembly.It also serves as a master supply room for the medical supplies stored in the lower compartment. As suppliesare depleted in the lower section, support staff access the upper room supply to replenish those basic supplies.
Equipment and Supplies
MED-1 can provide recommended lists of medications and medical supplies. All expendable medical supplies arestored in an orderly fashion on the support unit.Categories include:

• Orthopedic/fracture management
• Personal protection/infectious disease
• Critical care
• Operating room

Surgical (wound and burn care) and orthopedic supplies are stocked on one rolling cart with an additional back-up cartavailable. All medications are likewise stored on one rolling cart.
Medications are categorized as to their use including:
• ACLS
• Hemostatic
• Asthma
• Allergy
• Gastrointestinal
• Anesthetic
• Altered mental status
• Antibiotics (IV and PO)
• IV fluid
• Over-the-counter meds
• Pediatric preparations
• Narcotics
Two back-up carts are available for fast exchange with the main supply carts to ensure no delay in supply ofmedications and supplies in the Main Hospital Trailer. A fully stocked Broslow bag and tape can be purchased forpediatric emergencies.
Once the Main Hospital Trailer is in position and automatically leveled, all pieces of rolling equipment and carts aremoved into the facility and placed in their appropriate, predestinated location. Expendable medical supplies, includinggloves, bandages and dressings, and other examination and treatment material are located within the five drawerslocated in each treatment bed. Each bed is arranged identically to maintain consistency.
The support unit provides for a user-friendly storage facility. The bottom floor serves to house all items in the mobilehospital and is arranged by category. Medical staff can quickly replace items that were used during a shift or stock theunit prior to a shift beginning. Additional bulk medical supplies may be stored on the second floor. Support staff canreplace the first floor, rapidly accessible items with the back-up bulk supplies. Medications may be replaced in total bysimply replacing the used cart with the fresh one or by restocking items from the unused cart.
Transport and Delivery
The MED-1 Mobile Hospital unit can be loaded and transported on a US C5A or a Russian Antonov 225.
