outcomes, although population outcomes may also improve. Not only do patients benefit, but providers and health care systems benefit as well, through:

It is essential that ESRD Programs respond to all of the needs of their patients- physical, emotional and psychological.Ensuring their patients are engaged and that their care is patient-centered will assist in this. Barriers exist, but these barriers can be overcome through commitment and planning.
