Step 1: Visit the Texas Department of Public Safety (DPS) Compassionate Use Registry Portal.
Step 2: Click the “Physician Registration” link located under the “Licensing” tab.
Step 3: Submit your name and email address.
Step 4: You should receive an email that contains a link providing access to the physician registration wizard.
Step 5: The physician registration wizard application has five pages.
5A: Read these instructions. They’re important.5B: “My Details”: Submit your full name, phone number, driver’s license number and driver’s license audit number.5C: “My Certifications:” Enter a valid Texas Medical Board License number, attach a scanned copy of your Texas Medical Board License and list the specific qualifications you possess that authorize you to be eligible to prescribe low-THC cannabis to
patients with intractable epilepsy. 5D: “Addresses”: Enter your primary clinic name and physical address, including the county, as well as the clinic’s mailing address.5E: “Signature”: Check all appropriate acknowledgement/attestation boxes and type your full legal name at the bottom of the page and click “submit.”
Step 6: DPS will send a confirmation email with a tracking number, and the department will review your application and verify your qualifications. The verification process can take up to 10 business days.
Step 7: You should receive a registration approval email that provides a hotlink to access the CURT system.
Step 8: Click the link in that email and create your username, password and security questions.
Step 9: Use CURT to register patients and treatment plans. (See other side for these instructions.)

Prescriber’s Guide to AddingPatient Treatment Plans
Reminder: Before you get started creating a treatment plan, you must first register with the Compassionate Use Registry of Texas. (Instructions on the other side.)
Step 1: Other prep needed in advance of making your treatment plan:
1A: Make sure your patient is a permanent resident of Texas.1B: Gather the last five digits of your patient’s social security number, as well as the last four of the patient’s legal guardian if necessary.
Step 2: Your treatment plan must include: the patient’s name and date of birth; their prescribed dosage; the means of administration ordered; the total amount of low-THC cannabis required to fill the patient’s prescription; and how you will monitor patient’s symptoms, tolerance and reactions.
Step 3: Acquire a secondary approval by a fellow CURT-registered physician, and note the physician’s CURT username; he or she is not required to have contact with the patient.
3A: It should be noted that a patient can see a CURT-registered physician solely for the purpose of receiving a CBD oil prescription, while retaining their existing relationship with their primary physician/neurologist.
Step 4: To fill the prescription, patient (or guardian) will contact the licensed dispensing organization by phone to confirm the order details and set up a time for pickup at the dispensary or schedule a delivery.
