The Fellowship of Postgraduate Medicine wasfounded by a group of London consultants after theFirst World War to assist postgraduates, particularlythose coming to London from the Commonwealth,to pursue their studies. The Fellowship first publishedthe Postgraduate Medical Journal to give informa-

In a very rapidly advancing field this comprehensive, yetessentially practical review of the current management ofrenal failure, will be of great value to the nephrologist andto the general physician. There is inevitably some repetitiondue to overlap between the articles, but there is very littleconflict of opinion in terms of practical management.The opening article by 0. Wrong covers very completely
the diagnosis and treatment of acute uraemia, and dealshelpfully with common but knotty problems such as thediagnosis of salt depletion, and the misuse of alkalis inuraemic acidosis. Recent developments in investigationreceive timely, critical appraisal in the contributions byP. Clark on 'Surgical Aspects of Chronic Renal Failure', andby 1. Kelsey Fry and W. R. Cattell on 'Radiology in theDiagnosis of Renal Failure'. The latter includes a useful dis-cussion on the surgical and medical treatment of renal stones.T. M. Barratt, in 'Renal Failure in the First Year of Life'emphasizes important problems in diagnosis and treatment,peculiar to infants, and his contribution, together with thatof R. Shackman and A. E. Kulatilake include helpful adviceon the technique of peritoneal dialysis in children. Thespectrum of renal diseases in the tropics, and the differentapproach necessitated by socio-economic and geographicalfeatures is dealt with by M. S. R. Hutt and A. J. Wing, andmakes reading not only fascinating, but of practical im-portance in days of rapid travel. One of the highlights is theexcellent article by J. J. Brown et al. on the mechanisms ofhypertension in renal failure, which manages to make acomplicated subject, if not simple, at least understandable;they suggest, with much experimental backing, cogentreasons why hypertension in some patients on chronicdialysis is controllable, and in others uncontrollable. Themechanisms of anaemia in renal failure, with particular
emphasis on the role of erythropoietin, and on micro-angio-pathic haemolytic anaemia, are authoritatively reviewed byD. G. Penington and Priscilla Kincaid-Smith. F. O'Gradyprovides a thoughtful, scientific discussion of factors affectingthe effective concentration of antibiotics, followed by somepractical guide-lines for assessment of dosage of drugseliminated by the kidneys in patients with renal failure.J. Siddiqui and D. N. S. Kerr in their article on complicationsof renal failure include a good review of renal bone disease,substantiated by many references. In addition some of thelittle recognized complications, such as myopathy, receivetimely emphasis. The controversial question of the optimalprotein intake in renal failure is discussed by J. D. Blaineyand M. J. Chamberlain in 'Dietary Treatment of ChronicRenal Failure'. It is perhaps a pity that such scant referenceis made to the dietary management of the diabetic with renalfailure. The concluding articles on dialysis in hospital and athome by J. H. Jones, J. R. Curtis, and B. H. B. Robinson, andon transplantation by J. Swihney bring one up-to-date oncurrent techniques, complications, facilities, costs andmortality.

This is the fourth edition in the series which began in 1954.It comprises fifteen chapters, each of which is devoted to asingle aspect of paediatrics and might almost be regarded asa short monograph in its own right. The references cited arenumerous and have in this edition reached the prodigioustotal of 2500. A wide range of topics are covered and thefoetus and neonate are prominent among them, with sectionson neonatal respiration, foetal growth, asphyxia neonatorum,the respiratory distress syndrome, haemolytic disease and aninteresting section on the care of infants in incubators. In addition to the title ofthe paper there should be a 'running title' (for pageheadings) of not more than 45 letters (includingspaces). The paper should bear the name of theauthor(s) with their degrees and descriptions and ofthe laboratory or research institute where the workhas been carried out. The full postal address of theprincipal author should be given as a footnote. (Theproofs will be sent to this author and address unlessotherwise indicated.)
Arrangement. Papers should normally be divided into:(a) Summary, brief, self-contained andembodying themain conclusions; (b) Introduction; (c) Materialsandmethods; (d) Results, as concise as possible (bothtables and figures illustrating the same data will rarelybe permitted); (e) Discussion and conclusion; (f)Acknowledgments; (g) References.
References. Only papers closely related to theauthor's work should be included, exhaustive listsshould be avoided. References should be made bygiving the author's surname, with the year of publi-cation in parentheses. When reference is made to awork by three authors all names should be givenwhen cited for the first time, and thereafter only thefirst name, adding et al., e.g. Smith et al. (1958).The 'et al.' form should always be used for works byfour or more authors. If several papers by the sameauthor and from the same year are cited, a, b, c, etc.,should be put after the year of publication, e.g.Smith et al. (1958a). All references should be broughttogether at the end of the paper in alphabetical order.References to articles and papers should mention(a) name(s) of the author(s); (b) year of publicationin parentheses; (c) title of paper: (d) title of journalin full, underlined; (e) volume number; number offirst page of article. References to books and mono-graphs should include (a) name(s) and initials ofauthor(s) or editor(s); year of publication in paren-theses; (b) title, underlined; (c) edition; (d) page re-ferred to; (e) publisher; (f) place.
Standard usage. The Concise Oxford English Diction-ary is used as a reference for all spelling and hyphen-ation. Verbs which contain the suffix ize (ise) andtheir derivatives should be spelt with the z. Statisticsand measurements should always be given in figures,i.e. 10 min, 20 hr, 5 ml, except where the numberbegins the sentence. When the number does not
refer to a unit of measurement, it is spelt out exceptwhere the number is greater than one hundred.
Abbreviations. Abbreviations for some of thecommoner units are given below. The abbreviationfor the plural of a unit is the same as that for thesingular unless confusion is likely to arise.

Example: mg/100 ml, for biochemical values; mEq/lFigures. In the text these should be given Arabicnumbers, e.g. Fig. 3. They should be marked on thebacks with the name(s) of the author(s) and the titleof the paper. Where there is any possible doubt as tothe orientation of a figure the top should be markedwith an arrow. Each figure must bear a referencecorresponding to a similar number in the text.Photographs and photomicrographs should be un-mounted glossy prints and should not be retouched.Line diagrams should be on separate sheets; theyshould be drawn with black Indian ink on whitepaper and should be about four times the area of thefinal reproduction. Lines and lettering should be ofsufficient thickness and size to stand reduction toone-half or one-third. Letters and numbers must bewritten lightly in pencil. Whenever possible, theoriginals of line diagrams, prepared as describedabove, should be submitted and not photographs.The legends of all the figures should be typed to-gether on a single sheet of paper headed 'Legendsto Figures'.Tables. There should be as few tables as possible andthese should include only essential data; the datashould not be crowded together. The main headingshould be in capitals with an Arabic number, e.g.TABLE 2. Each table must have a caption in smallletters. Vertical lines should not be used.Page proofs will be submitted to the contributors forminor corrections and should be returned to theEditor within 3 days. Major alterations to the textcannot be accepted.
Offprints. These may be purchased if ordered on theform which will be sent to the senior author with theproofs.
