for Reducing Risk & Living Well:
Shaping the Journey
• Originally developed by the Alzheimer Society
of BC, then adapted for Nova Scotia
• Designed for persons in the early stage of
dementia and their care partners
• Encompasses physical, mental and social
wellbeing
• It covers topics such as the overview of
dementia, hearing the diagnosis, maintaining
your health, life after diagnosis, future planning
and maintaining your spirit
• Provides a Safe, Supportive, and
Confidential Environment
• Participants are given a
workbook to help with everyday
issues and future planning – take
home reference
• Provides an opportunity for
persons with dementia to support
each other. Care partners can
support other care partners

participants’ walkers
