Executive Summary
Kroll Factual Data, Inc. (“Kroll Factual Data”) is pleased to provide The City of Fort Collins with its proposal for identity verification services to assist The City of Fort Collins with maintaining its identity theft prevention program under the Red Flag Rules of the FACT Act (Federal Register 16 CFR 681).
Kroll Factual Data views this request for proposal as a way to identify the qualities of its organization that reflect the type of partner most capable of helping The City of Fort Collins drive compliance and ensure efficiency.

Services Agreement Since 2008, Kroll Factual Data has provided various pre-employment background screening and identity verification services to The City of Fort Collins pursuant to a Services Agreement entered into between the parties on September 9, 2008. In review of the request for proposal # 7475 and the request for services, Kroll Factual Data, believes it would be in each party’s interests to leverage the existing Services Agreement and any current renewal Amendment for the provision of any future work awarded.

Key Personnel
Rod Bazzani, President Rod Bazzani is the President of Kroll Factual Data, a leading provider of independent verification services to mortgage lenders, banks, credit unions, property management firms and other businesses. Prior to coming to Kroll Factual Data, Mr. Bazzani served as the Group Vice President of Marketing Services at TransUnion. He has 33 years of experience in the consumer credit data business with numerous management and executive roles across multiple industry verticals and geographic regions. Mr. Bazzani is also a member of the Kroll Executive Committee, working with business leadership from the Kroll businesses to provide clients with more comprehensive risk mitigation solutions. Mr. Bazzani is a graduate of Northern Illinois University with a Bachelors of Science in Finance. Russell E. Donnan, Chief Information Officer Russ Donnan is the Chief Information Officer of Kroll Factual Data, responsible for technical direction and management of the software development, enterprise applications, and support services departments of the business. Mr. Donnan was previously the Senior Vice President - Chief Information Officer and Corporate Secretary for Factual Data Corp, which was sold to Kroll in August 2003. Before joining Factual Data Corp. in 1993, Mr. Donnan held various roles at Apple Computer, Convex Computer and Key Computer (now Amdahl Corporation). Mr. Donnan is a graduate of the Ohio State University with a Bachelors of Science in Electrical Engineering. Jennifer A. Byers, Senior Vice President – Branch Operations Jennifer Byers is the Senior Vice President of Operations for Kroll Factual Data, and is responsible for the day-to-day management of operations which includes: Mortgage Customer Service, Independent Verification Services, QuickScore, Technical Support, Account Services, Loan Processing and Loan Assessment Review. Ms. Byers has held various positions at Kroll Factual Data from 1986 to 2007, including vice president of support services. From 2007 to rejoining Kroll Factual Data in 2012, she worked for Kroll in London, most recently as global director of business solutions. Ms. Byers is currently attending Open University United Kingdom in pursuit of a bachelor’s degree in business management as well as an MBA in Leadership and Management. Robert N. Arrington, Senior Vice President – Sales Bob Arrington is the Senior Vice President of Sales for Kroll Factual Data, responsible for sales management of the Western Regional and Strategic Accounts Sales Divisions as well as the marketing department. His 28 years in the credit industry includes positions at CSC Credit Services (an affiliate of Equifax) and multiple mortgage associations throughout the Western United States. Mr. Arrington is a graduate of the University of San Francisco with a Bachelors of Science in Business Administration and the University of the Incarnate Word (San Antonio) with a Masters of Business Administration. Dan Suleski, Senior Vice President – Sales Dan Suleski is the Senior Vice President of Sales for Kroll Factual Data, and is responsible for all aspects of Kroll Factual Data Sales east of the Mississippi River. Mr. Suleski joined Kroll Factual Data in 1999, and has 25 years of experience in the Mortgage Settlement Services and Risk Assessment & Mitigation industry. Mr. Suleski is a past member of the Board of Directors and President-elect of the NCRA and is currently a member of the MMBA’s Corporate Advisory Board of Directors. Mr. Suleski is a graduate of Boston College in Boston, Massachusetts with Bachelors of Arts in Economics.

Ron L. Carlson, Senior Vice President – Technical Solutions Ron Carlson is the Senior Vice President of Technical Solutions for Kroll Factual Data, and is responsible for the technical direction and management of all software development for the business. Since joining the company in July 1992, Mr. Carlson has held multiple positions in the company in the technical, operations and new product development departments. Mr. Carlson is a graduate of Colorado State University in Fort Collins, Colorado with a Bachelors of Science in Psychology. Michael J. Mauseth, Senior Vice President – Technical Solutions Mike Mauseth is Kroll Factual Data’s senior vice president, product and channel development. Mr. Mauseth is responsible for overseeing all aspects of product development, channels and alliances, risk management, marketing, and analytics. He also participates in negotiating complex client and partner agreements. Prior to joining Kroll Factual Data, Mr. Mauseth served as president of TransUnion Rental Screening Solutions, overseeing the division’s overall strategy, product development, sales team and account management. While at TransUnion, Mr. Mauseth was instrumental in the creation and development of TransUnion SmartMove, a unique, patent-pending consumer-initiated screening service. Mr. Mauseth has a Juris Doctor and Master’s in International Studies (Economics) from the University of Denver and a Bachelor of Sciences in Political Science from Minnesota State University. Linda Trout, Controller / Vice President – Finance Linda Trout is the Vice President of Finance and Kroll Factual Data’s Controller. She is responsible for day to day management of the underwriting and collections, accounting and finance departments as well as the general office management. Before joining the company in 1998, Ms. Trout held numerous managerial and staff positions in the accounting and finance verticals. She is a graduate of the University of Wyoming in Laramie, Wyoming with a Bachelors of Science in Accounting. Carol Steinbock, Vice President – Human Resources Carol Steinbock is the Vice President of Human Resources for Kroll Factual Data, and is responsible for management and direction of the human resources function for the business. Ms. Steinbock joined Factual Data Corp in 1999 as a Human Resource Specialist, and serves as a member of numerous community organizations including the Northern Colorado Human Resources Association, the Larimer County Sherriff Department’s Victims Response Unit, the Loveland Chamber of Commerce, and the Larimer County Workforce Development. Ms. Steinbock is a Human Resources Professional (PHR) and a member of the Society for Human Resource Management (SHRM).

Services and Pricing As the approved screening vendor for The City of Fort Collins Utilities, Kroll Factual Data maintains a direct integration with the utility Customer Information System. Customer representatives use the direct FactualIDTM - Identity Report integration to establish utilities services for new and/or returning accounts with The City of Fort Collins. Kroll Factual Data worked directly with The City of Fort Collins to build its services into The City of Fort Collins pre-established work-flow. Through the direct integration, The City of Fort Collins has access to all identity verification reports. Kroll Factual Data provides a customized solution that can be tailored to the current screening criteria to The City of Fort Collins. Included below (and a sample report in the Appendix) is more information about the identity verification services that Kroll Factual Data is proposing to The City of Fort Collins. FactualIDTM – Identity Verification Report Kroll Factual Data’s FactualIDTM – Identity Report comprehensively identifies applicant misrepresentation and protects against even the most difficult-to-detect fraud schemes. Comprehensive Analysis FactualIDTM provides a comprehensive score and categorical risk factors based on a unique and independent evaluation of the applicant(s) that identifies inconsistencies and reduces the potential risk. Tracking, Trending and Reporting Management Solution FactualIDTM offers a streamlined approach to identifying high risk applicants allowing you to effectively manage your risk exposure. Customizable Risk Assessment Engine Kroll Factual Data’s customizable risk assessment engine allows clients to not only weigh the severity of every fraud alert according business practices, but also customize the alerts and action steps to meet business needs. How FactualIDTM Works With three basic pieces of information - the borrower's name, Social Security Number and the current address, Kroll Factual Data’s supply a report that:
• Comprehensively assesses the identity risk. • Researches possible residency misrepresentation. • Searches the OFAC, Palestinian Legislative Council, and other exclusionary lists. • Results are translated into an easy-to-understand numerical risk score.

Systems and Procedures
A. Systems and Security
Kroll Factual Data uses state-of-the-art technologies to provide customers with secure and reliable data. Using digital certificates for encryption and Microsoft IIS web servers, Kroll Factual Data hosts the production operations from its company-owned, 3,000-square-foot data center in Loveland, Colorado.
Kroll Factual Data is focused on maintaining data security for customers and vendors. Kroll Factual Data participates annually in the Verizon Business/CyberTrust SiteSecure program. In addition to this assessment, Kroll Factual Data participates in multiple third-party audits and assessments of its controls and environments, including SSAE-16 (formerly SAS70) SOC1 certification. All customers must use two-factor authentication to access Personally Identifiable Information (PII). All data is encrypted at rest (on storage devices) and while in motion (transit). Logical and physical access to data stores is restricted, requiring multiple levels of approval for access.
B. Quality Assurance
Kroll Factual Data’s quality assurance systems are built into every facet of our production processes to monitor quality, response, reliability, customer service and follow-up. Kroll Factual Data’s quality assurance systems produce management reports in real-time as well as daily, weekly and end-of-month by client, products, and partners and by geographic region for all transactions.
To ensure the highest levels of customer service we maintain strict hiring standards and conduct extensive training for all employees. Additionally, Kroll Factual Data surveys customers on a quarterly basis to ascertain the customers’ level of satisfaction and to collect and react to customers’ feedback. The survey is sent to one-third of the customer database each quarter so that each group is surveyed twice every nine months. This frequency of customer survey allows Kroll Factual Data to compile feedback, take action on the feedback and re-measure in a short time span to gauge customer satisfaction and Kroll Factual Data’s effectiveness in meeting the needs of its customers.
C. Compliance
Every Kroll Factual Data employee, at every level of our organization, is certified under the Fair Credit Reporting Act through an education and testing program implemented by the National Credit Reporting Association, Inc. Through education programs and ongoing training, our customer support personnel continue to demonstrate the expertise necessary to exceed the expectations of our valued clients.
Kroll Factual Data is in good standing in the state of incorporation (Colorado) as well as all other states. Moreover, Kroll Factual Data has all licenses and permits necessary or required to provide the products and services outlined in this request for proposal. From time to time, Kroll Factual Data is made party to routine civil litigation matters (e.g., employment matters, etc.) in the ordinary conduct of its business. Any such past or current ordinary litigation matters would not impair Kroll Factual Data’s performance if services are awarded under this request for proposal.

Kroll Factual Data sits on the Board of Directors for the National Credit Reporting Association, Inc. (NCRA), and its Compliance Officer works very closely with the Consumer Data Industry Association (CDIA) and to stay on top of and monitor any new local, state or federal laws.
D. Business Continuity/Disaster Recovery
Kroll Factual Data maintains a warm disaster recovery site in Aurora, Colorado. The site operates hot site infrastructure (major network connections and database systems) and the supporting systems are in a standby mode. Third-party network connectivity at this site is tested as continuously as part of a QA testing system, ensuring operational status at all times. In addition, our primary data center site has multiple resiliency systems, including a N+1 power generation system. This system is tested weekly, and a full data center load is run on this system once a month. Please find attached Kroll Factual Data’s Business Continuity Disaster Recovery Plan Summary in the Appendix.

Kroll Factual Data is primarily structured as a technology based corporation and is dependent
upon computer-supported information processing and telecommunications. The increasing
dependency on computers and telecommunications for operational support poses the risk that a
lengthy loss of these capabilities could seriously affect the overall performance of the company.
A Business Impact Analysis (BIA) identified several systems as Critical to business operation. It
also categorized a majority of company functions as Essential to business operations. The BIA
will be conducted on a regular basis to ensure that changes to our processing and environment
are reflected in recovery planning.
Kroll Factual Data recognizes the low probability of severe damage to data processing,
telecommunications or support services capabilities that support the company. Nevertheless,
because of the potential impact to KFD and our customers, a plan for reducing the risk of
damage from a disaster however unlikely is vital.
The company's Disaster Recovery Plan is designed to ensure the restoration of Critical business
operations within 12 hours, and all Essential business operations within 48 hours of the outage.
The Disaster Recovery Plan identifies the Critical functions of KFD and the necessary required
resources. It also provides guidelines for ensuring that needed personnel and resources are
available for both disaster preparation and response and that the proper steps will be carried out
to permit the timely restoration of services.

Assumptions
This Disaster Recovery Plan is predicated on the validity of the following assumptions:
- The situation that causes the disaster is localized to the data processing facility of Operations
and Systems in the 5100 Data Center; the building or space housing the functional area; or to the
communication systems and networks that support the functional area.
It should be noted that the Plan will still be functional and effective even in an area-wide
disaster. Even though the basic priorities for restoration of essential services to the community
will normally take precedence over the recovery of an individual organization, the company's
Disaster Recovery Plan can still provide for a more expeditious restoration of our resources for
supporting key functions.
- The Plan is based on the availability of the hot site or the back-up resources. The accessibility
of these, or equivalent back-up resources, is a critical requirement.
- The Plan is a document that reflects the changing environment and requirements of KFD.
Therefore, the Plan requires the continued allocation of resources to maintain it and to keep it in
a constant state of readiness.
Maintenance
Ensuring that the Plan reflects ongoing changes to resources is crucial. This task includes
updating the Plan and revising this document to reflect updates; testing the updated Plan; and
training personnel.
Quarterly, the Disaster Recovery Plan undergoes a more formal review to confirm the
incorporation of all changes since the prior quarter. Annually, the Disaster Recovery Plan will
undergo a complete review, which could result in major revisions to this document, as well as the
detailed plan. These revisions will be distributed to all authorized personnel, who exchange their
old plans for the newly revised plans. At that time an annual status report on Disaster Recovery
planning will be reviewed and approved by the Chief Information Officer for KFD.
Testing
Testing the Disaster Recovery Plan is an essential element of preparedness. Regular testing will
occur at a minimum of 4 times a year. A comprehensive exercise of our DR capabilities and
support by our designated recovery facilities will be performed on an annual basis
