The 2017 Health Risk Assessment (HRA) process is almost upon us. For employees who
have a spouse who is covered or will be covered by Marquette’s medical plan in 2017, the
employee and the spouse must complete both parts of the HRA process in order for
the employee to receive the 10-percent premium discount for 2017. If only one of
the two complete the required steps, the employee will not be eligible for the 10-percent
premium discount.
Marquette University will not receive individual HRA results. In maintaining HIPAA compliance, we will only receive a group summary report indicating “yes” or “no” for HRA participation.
This is a two step process; both steps are outlined below.
STEP ONE: Complete the Biometric Screening
What is a Biometric Screening?
It is a simple finger stick blood test that will analyze your cholesterol, HDL and glucose.
Blood pressure, pulse, height, weight, and body mass index will also be recorded. The
results are available in about 15 minutes. You and your spouse have four options available
this year to compete this step.
Option ONE: Biometric screening with your primary care provider utilizing the
Medical Provider Form below. Biometric screenings for employees and spouses seeing
their medical provider must fall between Friday, July 1, 2016 and Friday, November 18,
2016. They may be subject to health plan co-pays. This form must be completed by your
medical provider and can be faxed directly to Justin at Aurora Healthcare at
(414) 525-2570. The form can be faxed in by either you or the medical provider. You will
need a copy of this form to complete the online questionnaire.

Option TWO: Complete your Biometric screening on-campus; free of charge. Dates for
on-campus screenings, including several Saturday options, will be announced in early Fall,
with appointments beginning in late October and running through mid-November.
Option THREE: Complete a comparable biometric screening at a spouse's employer -
sponsored HRA screening and fax results directly to Justin at Aurora Healthcare at
(414) 525-2570. Biometric screenings must fall between July 1, 2016 and November 18, 2016.
Option FOUR: The Biometric screenings can be completed at limited Aurora Clinics in various outlying
suburbs on numerous Saturdays during the annual enrollment period. Dates and locations will be announced in early
Fall, with appointments beginning in late October and running through mid-November.
STEP TWO: Complete an online questionnaire with the results from your biometric screening using one of the
options outlined above. This questionnaire will be available at the start of the annual enrollment period. Please be sure to
keep a copy of this once completed. You and your spouse will have to complete this and will have two different sign ins.

Flexible Spending Accounts (FSA) make it easy for you to set aside a portion of your salary on a pretax basis and use it to pay eligible health and/or day care expenses.
Marquette has two FSA account options:
Health— Health Care for eligible out-of-pocket medical, prescription drug, vision and
dental expenses.
Dependent Child Care—Can be used for certified day care expenses incurred for the care of your child (ren).
You may elect $2,550 per calendar year per employee for Health Care Expenses and $5,000 per calendar year per household for Dependent Child Care Expenses. The minimum contribution is $20 per month for both programs.

Marquette’s medical provider network, United Healthcare Choice Plus (UHCP), gives participating employees access to a national network of doctors, urgent care facilities, and emergency rooms around the country. Prior to your departure from Milwaukee, call UMR (800-826-9781) or visit UMR.com to print off a list of participating providers where you are traveling. If you go to one of the providers in the UHCP network, your expenses will be covered in-network, as if you were in the local, Milwaukee area.

GETTING TO KNOW YOUR BENEFIT OPTIONS—FSA
The 2016 influenza vaccine (injectable) will be available soon. Only a single dose of this year’s vaccine is needed to protect you against the flu. All individuals, including healthy college students, should be vaccinated for influenza this season. Even if you had the flu last year or received a vaccination last year, you are still at risk of getting the flu this year. Members of the campus community are urged to take measures to avoid the flu and minimize spread of infection. If billed as routine, the flu vaccination is covered at 100% and may be subject to an office visit co-pay.

recreation and exercise.
The Marquette Sports Rehabilitation Clinic is a full-service rehabilitation sports medicine clinic that offers the expertise of licensed physical therapists and licensed athletic trainers. Conveniently located in Schroeder Complex within the College of Health Sciences’ nationally ranked Department of Physical Therapy.
The clinic offers Marquette students and employees the following:
Full physical therapy and athletic training services (physical rehabilitation, strength training, athletic injury prevention and treatment, and wellness)
Bone density scans
X-ray
Free injury evaluations
Custom Orthotics
Physical Therapy, all deductibles and coinsurance are waived for employees and covered dependents with the EPO and PPO plans. The HDHP is not eligible for waiver of deductible.
The Marquette Sports Rehabilitation Clinic is an in-network provider for all Marquette University health plans. If you are enrolled in Marquette’s health plan, they will directly bill your insurance for you.
In addition to the rehabilitation clinic, you’ll have access to state of the art equipment in their body composition, strength and conditioning, exercise physiology, and motion analysis laboratories. The clinic also features handicap accessible bathrooms with showers.
The center offers convenient and flexible hours which makes it easy to fit around busy schedules.
In the Spring 2016 Benefits newsletter, a reference was made to insurance
coverage under Marquette’s medical plans for acupuncture and massage
therapy.
To clarify those benefits:
If you are on the health plan provided by Marquette University, the EPO and
PPO plans provide first dollar coverage for acupuncture up to $1,000.00 per
covered member, per year (no co-pay). Please note: Employees are
responsible for the cost of the initial evaluation, but any treatments afterwards
are covered up to $1,000.00. Benefits are paid under the HDHP after the annual
deductible has been met.
Massage Therapy is covered by the plan only if it is ordered by a
medical provider and prescribed as part of the Covered Person’s treatment
plan. Treatments are subject to deductible and coinsurance.
