City of Hollywood, Florida -Tree Removal and Tree Replacement Guidelines:(The Following Shall Apply Pursuant To "City of Holywood LandscapeManual, Section 4. - "Tree Preservation, Tree Removal and TreeRelocation Permits"; and the "City of Hollywood Land DevelopmentRegulations, Article 9, Section 9.9 "Existing Plant Materials"; and AreReflected Upon The Tree Disposition/ Mitigation Plans (Sheet LD-1)and the "Landscape Planting Plans" (Sheets LP-1 thru LP-2)Accordingly. The Owner and/ or Contractor Shall Follow The TreeReplacement and Protection Guidelines, Within These Notes, As WellAs The Aforementioned Codes.)

General Conditions/ Pre-Clearing Inspections: A Pre-ClearingInspection Is Required Prior To The Initial Removal or Alteration of AnyVegetation In Order To Assure Proper Compliance With The PermitConditions. A Post-Clearing Inspection is Required Prior to Issuance ofThe Final Certificate of Occupancy. All Native Vegetation Outside ofThe Development Area Shall Be Maintained In Place As Outlined OnThe Tree Disposition Site Plans.

Tree Protection Note: Contractors Shall Use Caution DuringConstruction To Avoid Damaging Any Existing Trees Listed As ToRemain. (1) Barriers Shall Enclose Area Inside The Outer Edge ofDripline; (2) Tree Protection Barrier Shall Be Conspicuous and HighEnough To Be Easily Seen By Any Equipment Operators; (3) All TreeProtection Barrier Shall Be Constructed of Sturdy Scrap Wood (Four byFour Stock), or Other Sturdy Material (No Flagging or Ribbon TypeFencing Shall Be Allowed); (4) All Tree Protection Barriers To BeCompletely Removed From Site At The End of Construction Period(Immediately Prior To Issuance of 'Certificate of Occupancy By The Cityof Hollywood Building Department) Unless Otherwise Stipulated.

Methodology of Tree Removals: No Heavy Equipment or MachineryShall Be Used, Nor Shall Any Construction Activities or Grade ChangesOccur Within The Radius of The Drip Line of Native Trees Which ShallBe Preserved In Place, Unless Specifically Approved In Writing By TheCity of Hollywood and/ or the Landscape Architect. Use of AnyChemical Treatments Shall Have Prior Approval By The City ofHollywood and the Landscape Architect Before Application. AllProhibited Species and/ or Invasive Species Shall Be Removed asIndicated Within The Limits of Contract Area or Property To BeDeveloped Accordingly, as per The City of Hollywood's LandDevelopment Code.

Tree Locations/ Tree Removals and Tree ReplacementCalculations: Refer To 'Sheet LD-1' For "Tree Location/ TreeDisposition Plans"; Trees That Are To Be Removed Must Be MitigatedFor According To The Tree Credits Guidelines (Section 9.9.B of TheCity of Hollywood Land Development Code).

Maintenance and Monitoring Requirements: The Permit Holder ShallBe Responsible For Maintaining The Health of Any Replacement and/or Relocated Trees For A Min. of One Year From Planting. ThroughoutThe Monitoring Period, The Permit Holder Shall Maintain a (100%)Survivorship Rate For All Installed Replacement Trees. If During TheMonitoring Periods, The Survival Rate of Any of The Plants DropsBelow The Required Level, The Permit Holder Shall Have (15) Days ToPlant A Sufficient Number of Additional Trees So As To Maintain TheRequired Survival Rate.

Tree Replacement Bond Requirement: Bonds May Be Required ByThe City of Hollywood For Tree Removal and/ or Tree ReplacementPermits. Calculations For The Amount of Bonds For Non-Mature TreesShall Be Computed Based Upon The Equivalent Canopy ReplacementCriteria Applied To Each Tree To Be Relocated or Replaced and UponThe Cost of Installation and Maintenance. Determination of the BondAmount For Mature Trees Shall Be Computed Based Upon The MostCurrent Version of the Guide For Plant Appraisal, Published By TheInternational Society of Arboriculture. The Fair Market Value of the Costof Trees That Would Be Required To Compensate For The Canopy ToBe Relocated or Replaced Shall Be Posted.
