• Annual Permission SlipAnnual Permission Slip– This includes permission to attend and participate in troop

and council sponsored activities, located within 1 hours driving time and which do not exceed 6 hours and are notdriving time, and which do not exceed 6 hours, and are not considered high risk, permission to participate in product sales and permission for Emergency medical treatment and health history any special accommodations for yourand health history, any special accommodations for your child should also be listed on this form as well as an emergency contact.
