Wat is urgent?
Urgent = bedreigend
– voor het leven: sepsis, meningitis, malaria,…..
– voor een orgaan; al of niet met vitale gevolgen
hart: myocarditis, acute fulminante endocarditis
hersenen: meningitis/encefalitis (herpes simplex, toxoplasmose, slaapziekte)
long: overwhelming pneumonie
lever: fulminante virale hepatitis, leptospirose
nier: leptospirose, hantavirus, STECO157 (TTP-HUS )
darmen: Clostridium difficile, Campylobacter (Guillain-Barré) ,
– voor een lidmaat: necrotiserende fasciitis, purpura fulminans

• anamnese• klinisch onderzoek• urine-onderzoek ( zo geen goede anamnese mogelijk is)
• RX thorax ( lage drempel gezien de beperkte waarde van de stetoscoop)
• Culturen? - urine: zeker bij mannen, enkel bij invasieve infectie bij vrouwen
- (sputum),- bloed: enkel bij specifieke problemen
- “steriele compartimenten” (gewricht, pleura,…..)

Overwhelming bacteriële menigitis: diagnose
• LP (na oogfundus)
• geen CT scan voor LP tenzij– ernstige bewustzijnsstoornissen
– focale neurologische tekens
– papiloedeem
N.B.:
– Papiloedeem is geen contraindicatie voor LP indien geen intracraniële massa aanwezig is (zie pseudotumor cerebri !)
– Zo CT scan nodig geacht wordt geeft men (na afnemen van bloedkweken en nasofarynx kweek) antibiotica vóór de LP bij fulminante presentatie.
Bacterial meningitis: diagnosis
• Contra-indication for LP• intracranial mass
(abscess, tumor subdural or subarachnoidal effusion)
• infection at the site of LP
• coagulopathy is a relative contra-indication

Bacterial meningitis: diagnosis
Microbiological diagnosis• Gram-staining of smears of CSF (sensitivity 0.5 - 0.7)
• culture (sensitivity 0.8 - 0.9)
• antigen detection assay (sensitivity 0. - 0.7)
• PCR (met de juiste primers!)
+ blood cultures ! (sensitivity 0.35 - 0.8)
+ nasopharyngeal swab culture !

Profylaxe bij meningitis
• zeker bij meningokokken• huisgenoten• kleuterklas en kinderdagverblijven• andere “schoolgroepen enkel zo meer dan 1
geval voorkomt• internaten, miliciens,…… (nauw contact is meer dan 4u/d
gedurende minstens 5d/week)
• ziekenhuispersoneel met reanimatiecontact
* fluoroquinolones voor adolescenten en volwassenen 250-500 mg ofloxacine of levofloxacine of analogen
* rifampicine 2 x 600 mg (10 mg/kg) 2 dagen
* ceftriaxone 250 mg IM
* azithromycine 500 mg (10mg/kg)
Profylaxe bij meningitis
• H. influenzae
de huisgenoten zo er nog kinderen jonger dan 4 j zijn
* rifampicine (20mg/kg, 600mg max/d gedurende 4 dagen)
* fluoroquinolones of ceftriaxone even goed maar
geen officiële “status”
• niet nodig voor pneumokokken

Risico’s in de luchtwegeninfecties
• Angina– DD viraal-bacterieel is moeilijk
– Peritonsillair abces: vereist drainage
– Zo belangrijke dysfagie: R/ medrol 32 mg
• Epiglottitis
• PneumonieLongauscultatie leidt zelfs in ervaren handen in 25-50% der
gevallen tot verkeerde conclusies!!!

– S aureus: focaal en lymfogeen: antistafilokokken penicilline of 1° generatie cefalosporineCave: bij handinfecties risico voor tenosynovitis inschatten (vereist chirurgische drainage)
• Diabetische voet– Idem als cellulitis plus gram-negatieven en anaëroben: R/ Fluoroquinolone plus dalacin

Streptokokken necrotiserende fasciitis
On incision of the dorsum of the right hand, fat necrosis (so-called dishwater pus) was encountered (Panel A). The infection hadspread along the fascia to the elbow, with thrombosis of small blood vessels (Panel B). The palmar skin of the hand and the dorsum of the fingertips were spared (Panel C). Four months after skin grafting and free microvascular-flap coverage of the exposed muscle, the patient was back to work and was regaining hand function (Panel D).

Toxic shock syndromen
Toxine gemedieerde ziekten met diffuse rash, enorm capillair lek, massieve oedemen, hypotensie, diepe shock en al of niet koorts
• STSS: staphilococcal toxic shock syndroom“tamponziekte” door TSST-1 of door enterotoxine
• Streptococcal toxic shock syndroomin 50% der gevallen samen met necrotiserende fasciitis
• Clostridium sordellii toxic shock syndroomna medische abortus (mifepristone) NEJM 2005; 353: 2352
Behandeling: shocktherapie
antibiotica: penicilline, floxapen plus dalacin®: 4 x600-1200mg/d als remmer van de toxineproductie
immuunglobulines 2g/kg (± 5000,00€)
agressieve chirurgie zo nodig (debrideren, amputeren, hysterectomie,……)
Urineweginfecties
• Cystitis: anamnese en afwezigheid van koorts
korte (3-7 dagen) behandeling volstaat; bij voorkeur met een
nitrofuraan (7d) eerder danFQ (3d) of amoxy-clav(cave furadantine koorts)
• Urinaire klachten plus koorts = invasieve infectie (pyelonefritis of prostatitis): voldoende lang (10d) behandelen
• Prostatitis: mictieklachten plus bilaterale rugpijn en meestal koorts
geen plaats voor “urinaire antiseptica” zoals nitrofuranen,
voorkeur voor FQ, eventueel co-trimoxazole of amoxy-clav
minstens 2 en eventueel 3-4 weken behandelen

• herpes zoster - Varicella- Varicella bij volwassenen: pneumonie risico !
(hepatitis)
- Zona: enkel voor de ernstige vormen of bij duidelijk immuungecompromitteerden is therapie nodig
Encefalitis
• Herpes simplexinsidieus, lymfocytaire meningitis,
temporale localisatie (CT, KST, EEG)overweeg zeer snel empirische therapie (acyclovir 3x 10 mg/kg)
• Toxoplasmabij (gevorderde) HIV infectie, transplant patiënten
vrij typisch CT of KST beeld (contrast-enhanced ring lesion(s))
• Tb
• Slaapziekte (Oost-Afrikaanse vorm)

Tuberculose als urgentie
• tb meningitis: beperkte sensitiviteit van Ziehlkleuring en PCR op CS vocht !!
• miliaire tbleuko en thrombopenie als hint

Travel-related infectious diseases in the ED
Priorities
• Early diagnosis of diseases with high morbidity/mortalityif left untreated Alarm signs (septic, meningeal, respiratory distress? ….)
coagulation abnormalities (hemorhagic fever syndrome ), petechial rash (meningoccal disease, rickettsiosis?)
always consider malaria, particularly in case of acute fever without focus
• Early recognition of transmittable diseases with public health concern
What are travel related infectious diseases?
Cosmopolitan disorders but more common in tropicalcountries
typhoid fever, amoebiasis, brucellosis, Rickettsiosis, toxocara, trichinella, Q fever, dirofilaria,,….
“Tropical” diseasesUniversal tropical diseases (“in all” tropical countries)
malaria, arbovirusses (dengue), Schistosomiasis, leishmaniasis,…
Specific tropical diseases (too difficult: ask the opinion of a more experienced colleague)

Incubation period of infections
The shorter the interval between the stay abroad and the onset of symptoms the broader the differential
diagnosis.
Interval > 10 - 14 d:viral infections (dengue, …), rickettsiosis and bacterial enteritis less probable
Interval > 1 month:think primarily of malaria, typhoid fever, brucellosis, amoebiasis, leishmaniasis, fungal infections, hepatitis, tb

Malaria
Plasmodium falciparum: the deadly form
P vivax, P ovale: mostly not very severe, but risk of late disease or relapse
P malariae: chronic low grade form
P knowlesi: initially described in Malaysia (but cases from multiple SE-Asian countries);
morphologically like P malariae, but behaves clinically as P falciparum

Malaria: diagnosisNO PREVENTIVE MEASURE OFFERS 100% PROTECTION
Fever after a stay in the tropics => exclude malaria
Microscopic examination: thick smear and thin smearhigh sensitivity and specificity when performed by experienced lab technician
Antigen tests:high sensitivity and specificity (regardless of experience)
Malaria: diagnosis Lack of the classical intermittent fever pattern does not argue
against the diagnois and abdominal complaints including
diarrhea are comon and misleading
Do not wait for a new fever episode to order a thick smear
Repaet thick smear at least twice to exclude malaria,
particularly when other hints for malaria are present (leukopenia,
thrombocytopenia, hyponatremia, hemolysis)
Be aware of concomitant bacteremia
The risk of P falciparum diminishes as the delay between
potential exposure and symptoms increases (minimal risk if delay >
6 months)
In case of doubt, treat as P falciparum

Treatment of malaria: severe malaria or not?
Severe malaria?– Unarousable coma not attributable to any other
– Generalized convulsions
– Severe anaemia.
– Hypoglycaemia.
– Metabolic acidosis with respiratory distress.
– Fluid and electrolyte disturbances.
– Acute renal failure.
– Acute respiratory distress syndrome (ARDS).
– Circulatory collapse, shock, septicaemia (“algid malaria”).
– Abnormal bleeding.
– Jaundice
– Haemoglobinuria.
– Hyperparasitaemia.
Rickettsial infections
• Mediterranean spotted fever:– Rickettsia conori (fièvre boutonneuse, Marseille fever)
– Transmission by ticks (tâche noire!)
– Mostly clinically not so impressive, but
• Scrub typhus– Orienta tsutsugamushi
– Asia pacific region; Transmission by mites, eschar
– Cases often mild, but if left untreated, pneumonitis, meningoencephalitis, disseminated intravascular coagulation, or renal failure can be seen.
• Rocky mountain spotted fever– Rickettsia rickettsii, transmitted by dog and wood ticks : incubation time 7d
– Occurs in 48 states of the US, in Canada, Mexico, Central and South America
– Fever, myalgia, rash (evolving from macular on distal limbs to petechial and migrating to palms and soles),
respiratory failure, meningo-encephalitis,…
– History of tick bite absent in 40% of the cases
– Most deadly form (untreated 10-25% mortality ratio)
– R/ doxycyclin (chloramfenicol)

Case 2: PJ, male, 50 yrs old
tâche noire, inocculation eschar
Amoebic abscess
A parasitic disease whithout eosinophilia!!!
Frequently no preceding bowel symptoms
May develop months to years after g.i. infection
Relentlessly progressive disease
Diagnosis: clinical picture, negative culture, serology (highly sensitive and specific)
stool analysis for parasites frequently negative
Treatment: no surgery nor drainage but nitro-imidazoles(metronidazol 750 mg tid 5-10 d or tinidazol 2 g 3d)
Regression of the necrotic mass takes 3-12 months!!!

Typhoid fever
Incubation time: 3 - 60 d (mostly 1-3 weeks)
Vaccination offers limited (50-80%) protection and only for S typhi, not at allfor S paratyphi species
Clinical presentation is a nonspecific febrile illness not gastro-enteritis
febris continua is a textbook sign
WBC count normal and rarely above 10.000/µl!
Serology (Widal test) performs very poorly and should not be ordered(beware of pre travel vaccination!)
Diagnosis is based on blood and stool culture
Empiric antibiotic choice: ceftriaxon or cefotaxim because of FQ resistance
Be aware of the risk of gi bleeding and perforation (small intestine)

Yearly 50 to 100 million cases, mortality 1 to 15%.
Makes up 16% of the causes of fever in travellers
Treatment is supportive: antipyretics and fluid management if indicated

contact, …...?
Travel history: How long, which preventive measures, which conditions, which exposures, fresh water contact……?
Look for the most common and most serious diseases.
Consider non tropicals causes even after a recent stay in the tropics.
Ask the opinion of more experencied colleagues.
Katayama fever
Acute schistosomiasis
• Exposure to fresh water, typically lakes and rivers
• Incubation time: 2 weeks- 3 months
• fever, myalgia, (bronchospasm, urticarial rash) plus eosinophilia
• Diagnosis: serology (occasionally still negative!),
stool and/or urine microscopy for parasites is alwaysnegative in the acute phase!!!
• Treatment: praziquantel

Antipyretische middelen- paracetamol: 15 mg/kg om de 4-6 u tot 4 maal/dag
- aspirine: 15 mg/kg om de 4-6 u tot 4 maal/dag
- noramidopyrine (novalgine): (niet langer?) te mijden
- NSAID’s ( zie nevenwerkingen (vooral risico voor acute nierinsufficiëntie bij deshydratatie) en ook het effect op AVP en zo theoretisch
meer risico voor koortsstuipen)ibuprofen 10 mg/kg om de 6-8 u tot 3 maal/d
- (corticosteroiden)
