Insurance or Contract Codes

This module is for building new insurance or contract codes, or

changing existing codes

On the portal page, click on Insurance or

Contact Search

Insurance or Contract search

Name – enter the Insurance or Contract Name. It is not required

District – enter the district you would like to search. This is required.

A drop down is available for the districts you have access too.

Type – a drop down is available for either insurance or contract. This field is not required.

Record Status – a drop down is available for either active or deleted. This field is not required.

New button – this button is grayed out. It will not be available until after you perform a search.

Once our search options are set, we are ready to click the search button.

Our search results are returned.

We can either build a new insurance/contract code or edit an existing code.

Let’s build a new code.

To build a new code, we can click the NEW button.

Top portion of the Add Insurance/Contract screen.

Type has a drop down that offers Insurance or Contract

Copay has a drop down that offers No, Flat Rate, and Percentage

Payor Source is the name that will show in the drop down on the household screen.

Bottom portion of the Add Insurance/Contract screen.

Electronic Billing? – drop down offers Yes or No

Bill Nurse Visits? – drop down offers Yes or No

Anthem? – drop down offers Yes or No

Save will save the information, Cancel will not save the information. Both will return you to the

search page.

Now let’s edit an existing code.

To edit an existing code, click the edit icon next to the code you would like to edit.

Top portion of the Edit Insurance/Contract screen.

Code, Contract Code Alpha, and Type can’t be changed.

Payor Source is how it will show on drop-downs.

Bottom portion of the Edit Insurance/Contract screen.

Record Status can be changed from Active to deleted.

The save button will save your changes, the cancel button will cancel any changes.
