• 2008-9: 4 pilot states work on integration
• Others begin talking about integration, mostly coordination
• Potential chronic disease “block grant” funding
• 2011-12: All states and territories receive coordinated chronic disease funding
Interest in Integration
• To meet chronic disease needs without specific programmatic funding
• Sustainability of most important initiatives • Increased focus on physical activity and
nutrition cut across all disease prevention and management
• Focus on policy, environment and systems change
• Fiscal constraints require efficiency and effectiveness
Integration is…
• A mindset • Organizational change • A whole that is greater than the sum of the
parts • A means to an end, not the end in itself• Can’t “sort of” integrate
A deliberate, systematic process to redesign what work we do and how we do
our work.
