    CLARIFICATIONS

    1. A 100% Payment and Performance Bond with Dual Obligee Riders will be required by the general contractor awarded the project.

    2. A pre-bid conference will not be held.

    3. The project retainage will be 5%.

    4. It is not anticipated that rated fire boxes will be required in the tray ceilings at Living/Dining 127 and 228 due to the available clear space below the rated roof assembly. A partial list of known locations where they will be required are: at the tray ceiling in Lobby 301 (Refer to 10/A422), at the tray ceiling speakers in Corridors 134 and 235, for the residential bathroom exhaust fans/heat lamps, Spa 122 and 223, speakers in kitchen 203, and for all downlights at exterior locations with a GWB-3 ceiling. Contractor to verify all locations requiring fire boxes at penetration to rated assembly.

    5. An assistance pull-cord next to the shower is not required.

    6. The anticipated start date on the project is May 11th. When the contract is awarded, the contractor will need to apply for a building permit through the Allen County Building Department. A permit has already been issued by the Indiana State Department of Health for the construction of a long-term care facility.

    7. There are no prevailing wage requirements for this project.
