PAYMENT FOR ALL CLASSES ARE DUE AT TIME OF REGISTRATION.

Payment by check or money order preferred. If paying by cash, only exact amounts are accepted.

A collection fee will be charged for each check returned by the bank.

MAKE CHECK OR MONEY ORDER PAYABLE TO: City of Los Angeles

REFUND POLICY “Full refunds are only issued when the Recreation Center cancels the activity. A 15% cancellation fee is assessed for all refunds.

Changes or transfers per class, sports league or day camp registration; may be assess additional fees. After the first day

of a session the Recreation Center issues a partial refund to patrons withdrawing from the activity”.

Guitar $50 for 8 weeks Instructor: Staff Beginning Ages 8-10 4:00 pm– 5:00 pm Beginning/Intermediate Ages 10-Adult 5:00 pm– 6:00 pm Students will learn basic chords, strumming, picking & tuning techniques relating to rock and roll styles of music. Students must provide their own guitar.

PAYMENT SCHEDULE

There is an annual registration fee of $25.00. Each ses-sion must be paid in full two weeks prior to the be-ginning of that session. Refund policy: A 15% fee will be assessed by the recreation center for any pa-tron granted a refund, change, or transfer per class registration. No full refunds will be issued, unless a class is cancelled by the center.

TO REGISTER, PARENTS and/or GUARDIANS MUST…

•Come in the Park office and complete a

registration packet.

•Provide a copy of your child’s birth certificate

and immunization records. (Copies can be

made at the center)

• Payment is due at time of registration.

We believe that each child should be treated as an individual and given the opportunity to grow at their own pace. As educators, it is our intent to enhance each child’s ability to create and ex-plore through arts, science, sports, cooking, community awareness, multiculturalism, special guests, and community field trips. We hope to build each child’s social skills, sense of security, motivation, responsibility, self confidence thru recreation. Child must be potty trained to partici-pate.

Reseda Quack Pack provides a program designed to offer a diverse recreation experience in a positive at-mosphere. Youth will learn and have fun! We believe that each child should be treated as an individual and

given the opportunity to grow at their own pace.

Transportation is not provided.

TO REGISTER, PARENTS and/or GUARDIANS MUST… Come in to the park office and complete a registration form ; payment is due at time of registration.
