Statement from the Chair

The past year has been extremely exciting and productive, although for some members of the Department rather challenging, given the proposed and implemented changes. On April 1, 2016 I started my role as Department Chair with unambiguous support of the leadership of both the University of Nevada, Reno School of Medicine (UNR Med) and Renown Health to revitalize the Department, which is linked at the hip with Renown Children’s Hospital (RCH). The strategy is to grown both simultaneously so that we can optimize the care provided to children in our community, improve and expand our ability to serve as teachers and educators to medical students, PA students, and NP students, and to start developing capacity to partake in translational and clinical research. My role is notably also supported by the Nell J Redfield Foundation, who unequivocally supports the health and wellbeing of children in our community. Several things were achieved in 2016. First, the establishment of 5 sections (Pediatric Anesthesiology & Pediatric Radiology; Pediatric Critical Care & Pediatric Emergency Medicine; Pediatric Medical Subspecialties; Pediatric Surgical Subspecialties & Laboratory Medicine; and Pediatric Behavioral Health) and 3 divisions (General Pediatrics; Pediatric Hospital Medicine; and Neonatology). During 2016, 7 Chiefs were appointed, while the Chief position for Pediatric Behavioral Health remained vacant. The newly formed structure provides the Department with shared leadership. In addition it provides us the opportunity to build the Department with committed upcoming leaders. Monthly Division/Section Chief meetings have commenced and as a result we are in the process of improving the experience provided to 3rd year medical students who do their 6-week pediatric clerkship rotation with us. We are also exploring how the Division/Sections and their members can help each other in achieving their care, teaching, and research goals. Second, following a generous gift from the Pennington Foundation, we have started to recruit pediatric subspecialists to enhance healthcare services we provide locally, to expand the educational experience we provide to our learners, to develop the infrastructure needed to successfully apply for membership of the Children’s Oncology Group (COG), and to prepare ourselves to apply for a pediatric residency training program at RCH/UNR Med in the next few years.

• The COG is a clinical trials group supported by the National Cancer Institute (NCI) and the world’s

largest organization devoted exclusively to pediatric cancer research. Over 200 North American

institutions belong to the COG and RCH’s membership would provide children with cancer in our

community with standard of care delivered right at home.

• A residency program would allow us to train new pediatricians in our community, increasing the

pipeline of providers to meet our communities’ needs.

In 2016 we added three Board Certified General Pediatricians, a Board Certified Pediatric Emergency Physician, a Board Certified Pediatric Hematologist/Oncologist, a Board Certified Pediatric Neurologist, and a Board Certified Pediatric Critical Care physician to the Department. Several searches, including Adolescent Medicine, Pediatric Infectious Disease, Medical Genetics, Board Certified Pediatric Surgeon, Board Certified Pediatric Neurosurgeon; and additional Board Certified subspecialists (Pediatric Hematology/Oncology, Pediatric Pulmonology, Pediatric Neurology, and Pediatric Endocrinology) are ongoing. Third, from a research perspective, the Section of General Pediatrics has initiated a collaboration with the Department of Molecular Microbiology & Immunology to develop an immunoassay for dermatophyte infections. The focus is on tinea capitis, but it will work well with all dermatophytes. This research project complements the Department’s participation in several, pharmacy sponsored clinical trials, and an American Association of Pediatrics (AAP) quality improvement project. Finally, we have continued to work on integrating UNR Med’s general pediatric practice with Renown Medical Group’s one. This has been challenging and has required adjustments to both those employed by UNR Med as well as those employed by Renown Medical Group. The integration process is ongoing and expected to be fully complete in 2017. In summary, 2016 has been a transformational year for the Department of Pediatrics. My gratitude goes out to all who have contributed to what has been achieved, and to the community who continues to entrust us with the care of children and teenagers at vulnerable moments in their lives.

Section Pediatric Anesthesiology & Pediatric Radiology Members: Dr Morgan McCarroll (Chief), Dr Gordon Curry, Dr Silver Dwinell The Section of Pediatric Anesthesiology & Radiology provides clinical and academic services to children in our community. Members of the section also support services to adult patients served by Renown Regional Medical Center and the University of Nevada, Reno School of Medicine. All members of the Section are highly skilled in the diagnosis and treatment of complicated pediatric patients. Consultative services with our subspecialty board-certified leadership is available 24 hours a day, seven days a week. Our priority is the competent and compassionate management of patients and their families during the most difficult times of their lives, while working to maintain efficiency and advance the quality of care delivered. 2. Members of Pediatric Emergency Medicine: Dr Joey Gassen, Dr Mark Baier, Dr Robert Darzynkiewicz, Dr Troy Sullivan, Dr Scott, Shepherd, Dr Justin Sonderegger, Dr Stephen Thompson, Dr Brian Trimmer, Dr Brett Frey, Dr Wayne Hardwick The Pediatric Intensive Care Unit (PICU) at Renown Children's Hospital (RCH) is a 15-bed intensive care unit for neonates, infants, children and adolescents who become critically ill or injured. We also provide quality care for the many children in our region who have chronic care conditions and require specific ventilator or surgical management. Our PICU is staffed by board-certified and fellowship-trained pediatric intensive care physicians who provide in-house medical and surgical care 24 hours a day, 7 days a week. Our physicians communicate regularly with pediatric subspecialists and primary care physicians regarding coordination of care, discharge and follow-up. Additionally, specially trained critical care nurses provide care for patients of all illness levels. Our goal is to provide our patients with comprehensive, safe, and age appropriate patient- and family-centered care in a compassionate and nurturing environment. 3. Section General Pediatrics

All members of our Division are committed to excellent patient care, education, and advancing the knowledge of pediatric disorders. With regard to the employed providers in the Division, we provide care at four locations: 75 Pringle Way, 15 McCabe Drive, 901 East 2nd Street, and 21 Locust Street. Our Division is intimately involved in the education of UNR Med’s medical students as the core of their 6-week clerkship rotation involves extensive exposure to general pediatrics. 4. Division Pediatric Hospital Medicine Members: Dr David Petersen (Chief), Dr Drake Paul, Dr Shannon Michelson The physicians of the Division of Pediatric Hospitalist Medicine provide specialized 24/7 inpatient care to children admitted to the 33-bed Pediatric floor at RCH. They serve as a resource to local and regional Emergency Departments and primary care physicians to facilitate a smooth transition and coordinate care between the inpatient and outpatient environments. Its members are the primary inpatient teaching attendings for UNR Med’s Pediatrics 3rd year clinical clerkship. They also direct the inpatient Pediatric rotation for the UNR Med’s Family Practice Residency. 5. Division Neonatology Members: Dr April Henry (Chief), Dr Rebecca Perry The RCH Neonatal Intensive Care Unit (NICU) cares for the smallest and most vulnerable members of our community. It is a 39-bed, level 3 NICU, attended by physicians, neonatal nurse practitioners, nurses and

respiratory therapists specially trained to care for premature infants and sick newborns. Our NICU combines the latest advancements in neonatal medicine with a caring, gentle touch to provide the very best care to our tiny patients. 6. Section Pediatric Medical Subspecialties Members Pediatric Allergy & Immunology Dr Boris Lokshin, Dr Leonard Shapiro Pediatric Cardiology Dr Katrina Kip, Dr Leonard Ludwick Pediatric Endocrinology Dr Kathy Eckert (Chief) Pediatric Gastroenterology Dr Juan Gregory Pediatric Hematology/Oncology Dr Jake Zucker, Dr Max J Coppes Pediatric Neurology Dr Ivan Lopez, Dr Colin Nguyen Pediatric Pulmonology Dr Sonia Budhecha The section of Pediatric Medical Subspecialties currently consists of the following subspecialties: Pediatric Allergy & Immunology, Pediatric Cardiology, Pediatric Endocrinology, Pediatric Gastroenterology, Pediatric Hematology/Oncology, Pediatric Neurology, Pediatric Pulmonology. In the future, additional subspecialties will be added (pediatric Infectious Diseases, Pediatric Nephrology, Pediatric Rheumatology, Genetics, and Adolescent Medicine). Several of the members of the Sections are employed by RMG, others are in private practice. All are committed to the goals and objectives of the Department of Pediatrics and RCH. 7. Section Pediatric Surgical & Laboratory Medicine Members: Dr Frieda Hulka (Chief), Dr Ellen Clark, Dr Laura Knight The Section of Pediatric Surgical & Laboratory Medicine houses all surgical specialists specialized in the care and management of patients 18 years and younger. In addition it is home to those in Laboratory Medicine, Pathology, and Forensic Medicine that provide care to children and adolescents. Members of this sections are actively involved in the education of UNR Med’s medical students.

Teaching

The Department of Pediatrics is an integral part of UNR’s Reno School of Medicine. The Years 1 & 2 curricular structure is an integrated systems-based curriculum designed by our faculty and includes community-based preceptorships in both academic years. Year 3 features clerkships in Internal Medicine, Family Medicine, Pediatrics, Psychiatry, OB/GYN and Surgery, while Year 4 consists of predominantly electives, and incorporates a required rural-based experience, which introduces the students to practicing medicine in small communities with limited resources and geographic challenges (Figure 1) The curricular structure of Year 1 and Year 2 is made up of integrated blocks that offer a more consistent approach to teaching science concepts within a clinical context, limited lecture hours and emphasizes lifelong learning strategies. The blocks use a “case of the week” format to align course content with clearly defined objectives which map out the work necessary for successful student learning. Each block concludes with an assessment week for clinical skills testing and a comprehensive examination. Along

with the integrated blocks, the Practice of Medicine blocks run longitudinally across both Years 1 & 2 and include community-based preceptorships in both academic years.
