As President of the Montana Medical Association, I want to personally welcome you to the MMA Annual Membership and Board of Trustees Meeting. The meeting theme, Switch It On! Transformation of Clinical Practices with Telemedicine and Emerging Payment Models, is reflective of the forward thinking innovations now afforded to practices and driven by the technology revolution happening within the health care industry. Physicians are able to seize this opportunity and become empowered to transform and build our practices, create new access points for our patients, and improve patient care outcomes.

Prepare to engage with stimulating keynote and panel speakers who will present the latest news on technology trends and regulations. Get ready to be inspired upon learning how health care organizations across the state are utilizing Telemedicine technology to enhance patient care in our most urban and frontier areas of the state. Gain inside knowledge on strategies that can help your practice adopt and utilize these technologies to provide quality patient care in a growing, consumer oriented world focused on convenience.

Make sure to stay in town for the policy roundtable discussion on value-based payment models on Saturday morning. Experts in the field will be on hand to provide the basics about value-based payment models and explain what actions physicians must take by January 2017 to avoid penalties and be in the running to earn bonus pay.

At this 138th Annual Meeting I cannot help but wonder what Dr. Erasmus D. Leavitt and his colleagues who founded the association in 1879 would think about the progress we have made in the delivery of patient care. NDale C. Van Demark advises clients in the health industry on strategic transactions and the evolution of health care delivery models. He has extensive experience in health system affiliations and joint venture transactions. Dale also provides counseling on the development of technology in health care delivery, with a particular emphasis on telemedicine. Dale has been at the forefront of advising clients with respect to the globalization of the US health care industry.

ERIC R. ARZUBI, M.D.Dr. Arzubi is a Child and Adolescent Psychiatrist who completed his general psychiatry and child / adolescent training at the Yale University School of Medicine. He is Chair of the Billings Clinic Department of Psychiatry and is President-Elect of the Big Sky Regional Council of Child and Adolescent Psychiatry. Dr. Arzubi led the design of Billings’ first school-based health center, the launch of Project ECHO Billings Clinic, and he is working on opening Montana’s first psychiatry training program.

Monica Bourgeau is the Executive Director of the National Rural Accountable Care Consortium. In this role she provides leadership and outreach to a wide range of health care organizations preparing for value-based payment programs. She has more than 18 years experience in health care leadership and development. Monica earned her Master’s degree in Management with an emphasis in Organizational Leadership from Warner Pacific College and her Bachelor’s degree in Psychology from Colorado State University.

CYNTHIA BROWNCynthia Brown is the Vice President of Government Affairs for the American Medical Association. Prior to joining the AMA in April of 2008, Cynthia worked for the American College of Surgeons for 21 years. Cynthia recieved her Master’s degree in Legislative Affairs from George Washington University and her Bachelor’s degree in Political Science from Northwestern University.

Dr. Bush has been the Chief Medical Officer for St. Vincent Healthcare and for Holy Rosary Healthcare since 2012. Dr. Bush has over 30 years of experience in Emergency Medicine, working in Regional Referral Centers as well as Critical Access Hospitals. He has served on statewide trauma committees in both Indiana and Montana, and has served on the Montana State EMS Medical Director’s Committee since its formation in 1990. Dr. Bush received his medical degree from Indiana University School of Medicine and attended residency at Methodist Hospital in Indianapolis.

MICHAEL S. BUSH, M.D.

JAMES R. FEIST, M.D.Dr. Feist is the Secretary for the Montana Board of Medical Examiners. Dr. Feist was a practicing Pediatrician for more than 36 years in Bozeman, Montana. He also served as an Adjunct Professor and Preceptor for Students with the Montana State University WWAMI Program. Dr. Feist received his medical degree from the University of Wisconsin and completed his residency in Pediatrics at the University of Iowa Hospitals in Iowa City, Iowa.

NDr. Gallea specializes in Emergency Medicine and Family Medicine and works at St. Peter’s Hospital in Helena. He graduated from the University of Minnesota Medical School. He has been a member of the MMA since 2008 and is a graduate of the 2016 Physician Leadership Effectiveness Program. Dr. Gallea is the MMA’s Incoming President and currently serves on the Board of Trustees, Committee on Legislation, Legal, and Legislative Affairs, Prescription Drug Abuse Reduction Committee, and the MMA Foundation Board.

WILLIAM S. GALLEA, M.D.

JONATHAN S. GRIFFIN M.D.Dr. Griffin received his Medical Doctorate and Master’s of Health Administration from the University of Washington. He completed training at the Family Medicine Residency of Idaho Rural Training Program. Dr. Griffin is the Medical Director at Blue Cross Blue Shield of Montana. He also maintains a solo, private practice as Medical Director of two skilled nursing facilities in Helena. Dr. Griffin is the Secretary-Treasurer of the Montana Medical Association

and is actively involved in several community-based health care improvement efforts with special emphasis on health information systems.

Todd Lovshin serves as Vice President and Montana Regional Director of PacificSource Health Plans where he is responsible for business development and operations in the state. He has more than 15 years of experience in government relations and employee benefits. Todd has served on numerous advisory panels, including the Exchange Stakeholder Council, the Montana State Employee Benefits Group Advisory Council, and the Montana Comprehensive Health Association.

IAN MARQUANDIan Marquand is the Executive Director for the Montana Board of Medical Examiners. Ian serves as Executive Officer for the Board of Professional Engineers and Land Surveyors and the Board of Respiratory Care Practitioners. He also serves as Western Regional Representative to the Board of Directors of Administrators in Medicine. Ian was appointed by Governor Steve Bullock in 2015 as a member of the Interstate Medical Licensure Compact Commission where he was elected Chairperson.

Thelma McClosky Armstrong is Director of the Eastern Montana Telemedicine Network where she is responsible for the overall management of a 42-site interactive videoconferencing network providing medical and mental health services, continuing professional education, and administrative services. In 2009, Thelma was elected to the first class of the American Telemedicine Association College of Fellows. This high honor recognizes significant achievements in telemedicine, service to the general telemedicine community, and service to ATA.

THELMA MCCLOSKY ARMSTRONG

Representative Al Olszewski, M.D., House District 11, is an orthopedic surgeon with Flathead Orthopedics in Kalispell. He is a graduate of the University of Washington School of Medicine and completed his Orthopedic Surgery Residency in San Antonio Texas at the Wilford Hall Medical Center. Representative Olszewski served on the Agriculture, Human Services, and Taxation Committees during the 2015 Legislative Session and is currently serving on the Children, Families, Health and Human Services Interim Committee.

Jennifer Rieden is the Hospital Section Supervisor for the Health Resources Division of the Department of Public Health and Human Services. In this position she oversees the Medicaid programs for hospitals, Federally Qualified Health Centers, and Rural Health Clinics. She has a Bachelor’s of Science in Healthcare Administration from Montana State University - Billings and an Associates of Science degree from the University of Montana - Helena in Practical Nursing.

JENNIFER RIEDEN

DOUGLAS WALDO, M.D.Dr. Waldo is Chairman of the Mission: Lifeline Montana Initiative. He has been developing the Cardiovascular Services at the Heart and Vascular Institute of Benefis Medical Center in Great Falls, Montana. Dr. Waldo graduated from the University of Buffalo, SUNY School of Medicine and Biomedical Sciences. He completed his Internal Medicine and Cardiology training at Boston University Medical Center and Boston City Hospital. Dr.

Waldo completed his Intervention Training in San Antonio, Texas.

PAUL PEDERSENPaul Pedersen has over 26 years experience with Blue Cross Blue Shield Montana, beginning his career in 1990. Paul’s current responsibilities include the management and mentoring of the professional teams responsible for provider contracting, credentialing, education, and operations. Paul attended college at the Illinois Institute of Technology in Chicago, Illinois and the University of Montana.

Dr. Yates specializes in Family Medicine and works at Community Health Center in Butte. She graduated from Western University Health Sciences College of Osteopathic Medicine of the Pacific and completed her residency at St. Mark’s Health Care Foundation Program. Dr. Yates is a graduate of the 2016 Physician Leadership Effectiveness Program and serves on the MMA’s Board of Trustees.

NTim Fox was taught a strong work ethic early in life. At the young age of twelve he took his first job as a paperboy for the Hardin Herald, the local weekly publication. After graduating from Hardin High School, he attended the University of Montana where he received his degree in geology in 1981 and his law degree in 1987. Tim’s extraordinary work ethic and dedication to public service is apparent in his many accomplishments throughout his 29 year legal career and during his time as Montana’s Attorney General.

GREG GIANFORTEGreg Gianforte fell in love with Montana nearly 40 years ago when he first came to Montana on a school trip. After spending the summer hiking the backcountry, he knew this is where he wanted to build a life. It was in Bozeman, Montana where Greg and his wife Susan raised all four of their children. And it was in the same house that they raised their kids, where they started a small company called RightNow Technologies. That small company grew

to over 1,100 employees with an average Montana salary near $90,000 a year.

Larry Jent is an Eagle Scout, West Point Graduate, former Infantry Officer, Paratrooper, and Special Forces Officer (Green Beret). He served on Active Duty in Alaska and Fort Devens, Massachusetts, in the Army National Guard in both Colorado and Montana. Larry graduated from the University of Colorado Law School and has practiced law in Montana for 32 years. He was first elected to the Montana House of Representatives in 2000, where he served three terms. He was then elected to the Montana Senate in 2006 and re-elected in 2010.

Jesse was born and raised in Anaconda and attended college and law school at the University of Montana in Missoula. With his parents as an example, Jesse has devoted himself to doing whatever it takes to fight for Montana families. After working for a law firm, Jesse has dedicated his career to Montana public service. Jesse has experience ranging from representing his hometown in the State Legislature for over a decade, to serving as an

Assistant Attorney General and Special Assistant United States Attorney, to being the top lawyer in the State Auditor’s office.

SENATOR MATTHEW ROSENDALEMatthew Rosendale came to Eastern Montana in the early 1990s in search of a ranch to raise his family where he purchased a ranch north of Glendive. In addition to serving the people of Senate District 18 in the Montana Senate, Senator Rosendale is currently the Senate Majority Leader in the Legislature. Senator Rosendale formerly worked with his family to build a local real estate brokerage. He expanded it to include a

development company and over time, he grew the small business into a multi-million dollar full-service firm with four offices and 65 sales associates.

I went on an unexpected downward ride with my husband last month when our house deck collapsed. Fortunately, we were not injured as we were able to jump up and into our house. The signs of needed repair were there – we were actually inspecting problem areas at the time of the collapse with the intent to make a plan to rebuild over the summer. Our plans quickly changed to one of action with the loss of this convenient access point to our backyard.

Like us on our wobbly deck, Montana physicians have been trying to stand on shifting sands. While we celebrated the repeal of the sustainable growth rate (SGR) payment formula with the passage of the Medicare Access and CHIP Reauthorization Act (MACRA) of 2015, this same piece of legislation called for a shift toward a physician payment model based on the quality of care provided rather than quantity of patients seen. To implement MACRA, the Centers for Medicare and Medicaid Services has laid out a proposed framework called the Quality Payment Program which is scheduled to go into effect January 1, 2017. We appreciated the comments received on this proposal.

Like our deck, the signs of needed repair were there with ever increasing health care costs topping the list. Congress passed MACRA – now physicians and practices need to make sense of the impact and become ready to participate in one of two tracks, the Merit-based Incentive Payment System (MIPS) and advanced Alternative Payment Models (APMs). This disruptive change to value-based reimbursement is geared to transform the delivery of health care, and it is not limited to Medicare physicians. Physicians are sure to see Medicaid and other private insurers move in this direction.

You can gain better footing by joining your colleagues at the MMA Annual Meeting in Helena when a special policy forum, held on September 10, will provide information on this major transformation in health care delivery and share where physicians can find expert assistance. Plan to attend the educational sessions on September 9 to learn how telemedicine can play a role in your practice and expand patient access to care. Register for the meeting at mmaoffice.org.

The rebuild of our deck has begun. While the disruption was not welcome, we look forward to enjoying relaxing nights on a larger deck that is built for low yearly maintenance and enhanced lighting. The rebuild of the physician payment model is also underway. We will continue to work toward an effective health care delivery model that enhances patient care and supports physicians in their practice. We will advocate for changes to streamline reporting and reduce administrative burdens so that physicians can focus on caring for patients and have more time to spend on your own decks enjoying Montana’s splendor. •
