Post Tensioning Jacks
Section 430-6 (B) Cored Slabs When erecting prestressed cored slabs, place the 0.6" diameter transverse post tensioning strands and tension to 43,950 lb in each span. Grease the transverse strands and place in a non-corrosive 0.6" diameter, 1/16" minimum wall thickness black polyethylene pipe meeting ASTM D2239. Do not apply grease or extend the pipe in the area of the recesses at the ends of the tensioning strands where grout is applied. After tensioning the 0.6" diameter transverse strand in a span and before placing any equipment, material or barrier rail on the span, fill the shear key, dowel holes and recesses at the ends of transverse strands with an approved non-metallic, nonshrink grout and cure for 3 days minimum and until the grout reaches a compressive strength of 3,000 psi.

After tensioning the 0.6" diameter transverse strand in a span and before placing any equipment, material or barrier rail on the span, fill the shear key, dowel holes and recesses at the ends of transverse strands with an approved non-metallic, nonshrink grout and cure for 3 days minimum and until the grout reaches a compressive strength of 3,000 psi. When using a transverse screed on a span with a skew angle less than 75° or more than 105°, orient and operate the truss or beam supporting the strike-off mechanism parallel to the skew. Position the strike-off parallel to the centerline of bridge and make the leading edge of concrete placement parallel to the skew. If approved, operate at a reduced skew angle on very wide or heavily skewed spans where the distance between screed supports exceeds 100 ft.

• Prior to bridge demolition or renovation activities, conduct a thorough inspection of the bridge or affected components for the presence of asbestos containing material (ACM).
• Use a firm prequalified by NCDOT, with a NC accredited inspector.
• Identify all ACM as required by the EPA NESHAP Code of Federal Regulations (CFR).
• Submit a report to the Engineer, containing required information. • IF ACM is found, compensation for removal and disposal will be
considered extra work handled by supplemental agreement. • An Asbestos Removal Permit must be obtained through the
Health Hazards Control Unit (HHCU) of NCDHHS if ACM is found in minimum quantities identified in the provision: 35 cubic feet, 160 square feet, or 260 linear feet.

• Even if no ACM is found, or quantities are less than those required for a permit, Demolition Notification must be submitted to the HHCU. Requires original signature and must be submitted 10 working days prior to beginning demolition activities.
• 10 working day period starts on post-marked date or date of hand delivery. NO FAXES OR EMAIL.
• Demo that does not begin as originally notified requires a separate revision form. FAX OK. o HHCU contact information is provided in the provision.
• Special Considerations for Buncombe, Forsyth, and Mecklenburg Counties. If removal permit required, both the County and State must be notified. If demo with no RACM, only the local agencies in these 3 counties must be notified. o Contact information for 3 counties is provided in the provision.

Advance Notice of New Structure Completion Form
FHWA requires the inspection of new structures within 30 days of opening the structure to traffic. The submittal of the Advance Notice of New Structure Completion form will notify Structures Management Unit (SMU) that a bridge is nearing completion and will need to be inspected. SMU prefers that the inspection be performed prior to opening the structure to traffic.

Notice of Vertical Clearance Change Form
The Structures Management Unit (SMU) is required to maintain a database of the vertical clearances on all roadways to evaluate proposed routes for oversized loads. Report any changes (increases and decreases) in overhead clearances. Reported changes include:
• Vertical clearances for new bridges and sign structures • New vertical clearances due to construction or resurfacing
under structures • Removal of existing structures.

Engineer’s Acceptance Report
When all or a portion of a project is complete and ready to be accepted in accordance with Article 105-17, the Engineer Acceptance Report shall be completed. This form can be used for: • Bridge Deck & Rail Acceptance • Partial Acceptance (All work except vegetation or any other
portion of the project) • Final Acceptance.
This form should be completed within 72 hours of completion of the work being accepted.
