Patient Self-Management in Rural Settings

• Collaborative effort in rural, northern Ontario community between Bell, UHN and Nortel Networks

• Chronic diseases are a growing challenge for our health and social care systems and affect 1 in 3 Canadians

• Bluetooth-enabled medical devices are used to transmit data to a mobile device which relays data back to a data repository where rules are applied sending alerts to a nurse practitioner or family physician as well as the patient by automated text or phone messages
