The Academic Mental Health Program was one of the first to be established in the newly formed UNSW

Clinical School in the Southwest Sydney Area Health Service. The Program comprises a multidisciplinary team

working across three collaborating units [The Psychiatry Research and Teaching Unit (PRTU), Academic

Infant, Child and Adolescent Unit, and the Schizophrenia Research Unit (SRU)].

The program has demonstrated a strong and sustained commitment to high quality translational research, to

teaching, training and supervision at all levels, to service and clinical work, and to the associated roles of

leadership, mentorship, consultancy and provision of expert advice (in formulation of policy, guidelines, etc.).

In the past five years (2010-), the program has published 323 publications, many in the highest ranking world

medical, psychiatric, neuro-science and social science journals. Over this period, the program has attracted 40

grants, 19 from NHMRC/ARC, the total value being $32,796,730.94 million. These include a third nationally

collaborative NHMRC Program Grant in traumatic stress. In addition, over the period, the team has supervised

22 higher degree students and 5 ILP students.

The Program offers the full range of teaching, training and supervision across the LHD and related agencies.

The fifth-year medical student teaching program involving multiple units of the LHD always rates very highly.

In addition, academics and other personnel of the Program supervise registrars, ILP students, trainee

placements, higher degree students, overseas visiting students (two currently from Copenhagen and Amsterdam

Universities) and Fellows (including a past Fulbright).

The Psychiatry Research and Teaching Unit (PRTU)

The unit was established in 1991 under the Foundation Professor, Derrick Silove. The unit comprises three

academic staff: Professor Silove, psychiatrist (Google Scholar H-Index 55, citations 10423, i10 192), Senior

Lecturer Susan Rees, Social Scientist, previously ARC QE II Senior Research Fellow at the PRTU, now

appointed as a full-time academic under the competitive SPFO3 scheme, and newly appointed Lecturer, Jess

Baker, psychologist; two full-time Senior Research Scientists, Alvin Tay, psychologist and Mohsin Mohammed,

epidemiologist/statistician; and three support staff including a university funded admin/finance officer. Three

past deputy directors have been appointed to professorial positions at other institutions. The Director leads one

of the five key themes (Mental Health) of the Ingham Institute.

Members of the unit make direct contributions to inpatient and outpatient services (general adult psychiatry,

anxiety disorders clinic, refugee clinics in collaboration with STARTTS). Collaborators include STARTTS,

Carramar, Karetane, Bankstown Anxiety Clinic, national consortium (five universities) in traumatic stress,

Johns Hopkins, Karolinska, Harvard and Copenhagen Universities, amongst others.

Since its establishment, the PRTU has published >450 publications (most in peer-reviewed journals including

the Lancet Journals, JAMA/JAMA Psychiatry, BMJ, PLoS Medicine and the top ranking psychiatric and social

science journals). In the past five, years, publications include the highest cited paper in the refugee field for that

period (Steel et al, JAMA, 2009); a seminal study on gender based violence (Rees et al, 2011, nominated as one

of four best papers, BMJ), the largest and longest follow-up study in the post-conflict field (Silove et al, Lancet

GH, 2014); and the first multinational study of adult separation anxiety in the highest ranking psychiatric

journal, The American Journal of Psychiatry, Silove, in press). Since inception, the unit has attracted >$30

million in research and project grant funding (including CIB on three NHMRC Program Grants), supervised >15

higher degree students, and undertaken combined research/development projects in several countries in our

region including Timor-Leste, Vietnam, Cambodia, Indonesia, PNG, Bougainville and the Solomon Islands.

Members of the unit are regularly invited to present keynote and plenary addresses at national and international

conferences.

The unit is recognized as the national leader and one of the foremost centres internationally in research in the

post-conflict/refugee mental health field in which it is ranked first on conventional metrics (combined IFs,

number of publications, citations).

The unit also makes strong contributions to related fields of traumatology, transcultural psychiatry, gender and

mental health, human rights and mental health, global mental health and epidemiology, service development in

low-income countries, and anxiety.

The unit regularly provides consultation and advisor roles to local, national and international agencies including

WHO, UNHCR, World Federation for Mental Health, the World Psychiatric Association and the International

Society for Traumatic Stress Studies.

The research program of the unit extending over the next five years includes: 1. Impact of trauma and domestic

violence on refugee women in the perinatal period (Rees et al., NHMRC Grant, 2015-2017); 2. Developing the

evidence base for assessment and intervention for domestic violence (Multi-centre team including Rees, two

grants from ANROWS); 3. The transgenerational transmission of anger and violence in conflict-affected

countries including Timor-Leste (Silove et al., NHMRC 2015-2019); 3.The long-term mental health impact of

trauma; several national cohort studies (NHMRC 3rd

Program Grant, 2015-2019); 4. Separation anxiety,

oxytocin and attachment (with Professors Eapen and Dadds) (awaiting outcome of second ARC Linkage Grant);

5. Trials and evaluation of Mental Health Programs in Post-conflict countries (NIMH grant in preparation in

collaboration with Johns Hopkins University); 6. British Department of Foreign Aid (DFID) Grant with Johns

Hopkins University, Red Cross (Tanzania) and UNHCR): Interventions for gender-based violence in refugee

camps; 8. First trials for adult separation anxiety disorder with Black Dog Institute.

Honours include the Simpson Award (College of Psychiatrists) for outstanding contribution to services, a

Lifeline Column in the Lancet, an Honorary Professorship at the Karolinska Institute, the Dean’s Lifetime

Award, and the Inaugural Lady Fairfax Senior Researcher Award (Ingham Institute); “Top 10” Program Grant,

NHMRC 2010; an ARC Queen Elizabeth II Senior Research Fellowship (Rees) and appointment to an SPFO3

position (Rees)

Translational activities include key contributions to refugee mental health policy (national, international),

service development (establishment and major involvement in STARTTS, Carramar and state-wide; National

Mental Health Service, Timor-Leste, past Area Director of Mental Health, SWSAHS), development of

guidelines (national, WHO, UNHCR), training (Harvard Program in Refugee Trauma, Timor-Leste, multiple

countries in the region).

Academic Infant, Child and Adolescent Mental Health Academic Infant, Child and Adolescent Mental

Health

Professor Valsa Eapen leads a multidisciplinary team (psychiatrists, clinical psychologists, and nurses)

including conjoint appointments (A/Professor Stephen Matthey, Senior Lecturers Dr Amelia Walter, Dr Mona

Asghari-Fard, Dr My Trinh Ha; and several staff specialists. Research staff include Rachel Grove, Feroza Khan,

Elizabeth Fulton, Laura Nicholls, Nicole Lees, Olivia Wong, Janelle Cleary, Van Nguyen, Banosha Yacob,

Mary Ha.

Professor Eapen and her team are recognized internationally for the leading role they play in research into the

pathogenesis of Tourette Syndrome and other neurodevelopmental disorders. Key themes of ongoing research

include the prevalence and correlates of developmental disorders in pre-school children, genetic, neuroimaging

and clinical phenotypic characteristics of autism, biological underpinnings (including the role of oxytocin) in

maternal-infant attachment, child psychosocial development and separation anxiety (in close collaboration with

the PRTU), and metabolic syndrome in young mentally ill patients (working closely with the SRU).

A/Professor Matthey has a long track record of research into mental health issues related to pregnancy and the

postpartum, supported by a series of research grants. He has published over 97 publications largely devoted to

the postpartum mental health area. He has supervised >35 higher degree students.

The academic unit provides services to the Comorbidity Clinic, Carramar ICAMHS for young people with

complex mental health presentations including Autism Spectrum Disorder, Tourette Syndrome etc, and Hill Top

Special School, amongst other services. Research collaborators in the LHD include the Paediatric Disability

Unit at Fairfield, postnatal units at Bankstown and Liverpool hospital, ICAMHS teams at Park House and

Carramar and inpatient unit at Campbelltown. Developmental Surveillance Program:

Translational activities include developing policies and frameworks for early identification of developmental

disorders. The group’s “Watch Me Grow” study funded by NHMRC in partnership with NSW Department of

Health is tracking a birth cohort of 2000 children to study the determinants of developmental risk and to

ascertain barriers and enablers to service utilization, with a view to developing a policy and service delivery

framework for universal developmental surveillance in NSW.

Key themes of ongoing and future research include:

Autism Research: (funding from Cooperative Research Centre for Autism in collaboration with the Autism

Specific Early Learning and Care Centre, Liverpool, and centres in all the six states in Australia). Professor

Eapen leads the program which aims to identify subtypes of autism as a foundation for tailoring intervention and

offering accurate prognosis. The adaptation of the Early Start Denver Model to a group delivery format for

intervention represents a world first for the program.

Oxytocin research: Funded by an ARC Linkage grant in partnership with the PRTU, SWSLHD and Karitane,

involves longitudinal cohorts of mother-infant dyads (in collaboration with antenatal services at Liverpool)

identifying pathways involving separation anxiety, attachment patterns and maternal and infant

psychobiological characteristics associated with adverse infant outcomes. Findings from this world first study

(PLoS One, 2014) have provided significant insights into the biological underpinnings of separation anxiety and

the role of oxytocin in mediating social affiliative behaviours including bonding, mother-infant interactions and

the need for early intervention.

The Schizophrenia Research Unit (SRU)

The SRU established in 2000 is led by neuroscientist, A/Professor Philip Ward (H-index = 42, Citations = 5,029

[Google Scholar April 1, 2015]) in conjunction with the initiation of an Early Psychosis Intervention Program

(EPIP) at Liverpool. Prior to that, A/Prof Ward was the Scientific Director, Neuroscience Institute of

Schizophrenia and Allied Disorders (NISAD). The unit works in collaboration with local, national and

international teams, including Brain and Mind Research Institute, University of Sydney, School of Biomedical

Sciences, University of Padua, and the Institute of Psychiatry, London, amongst others, undertaking a wide

range of research into the cognitive and neurophysiological underpinnings of schizophrenia, bipolar disorder

and first-episode psychosis. Specific contemporary research themes are to identify neurobiological predictors of

transition to psychosis and long-term functional impairment in help-seeking youth, using clinical, neuroimaging

and neuropsychological measures; and to evaluate the effectiveness of cognitive and social-cognitive

interventions in recovery-focused care for people living with psychotic disorders; To develop implementation

strategies and evaluation frameworks for the roll-out of evidence-based lifestyle and life-skills interventions

aimed at improving physical health outcomes for people living with mental illness, to reduce the life-

expectancy gap experienced by this vulnerable population. The guidelines and protocols have been endorsed by

both the NSW and Federal Mental Health Commissions, ensuring that implementation of the targets to improve

physical health outcomes in young people with psychosis, as outlined in the International Healthy Active Lives

Declaration, is a key priority. The SWSLHD Mental Health Clinical and Quality Council agreed to develop a

funding proposal to ensure implementation of the evidence-based lifestyle and life-skills interventions for young

people commencing antipsychotics, with the goal of preventing the rapid weight gain and escalation in cardio-

metabolic risk factors that accompany initiation of treatment.

Awards include the Young Scientist Award for work for research into schizophrenia (twice awarded) and the

Organon Prize for Psychiatric Research, Australian Society for Psychiatric Research.

Plan for Use of Funds: If awarded, the program proposes to use the grant to part-fund three doctoral or post-

doctoral positions, an initiative that will greatly enhance the overall research program and productivity of the

program, building to the continuity and future academic success of the mental health research program of the

LHD and the Ingham Institute.

APPENDIX D:

RESEARCH STRATEGIC PLAN

Research and practice development Staff of the Mental Health Service are internationally recognised for the quality of research they undertake and for their contributions to improving clinical practice, particularly at a local level. Mental Health research has been identified as a key priority for SWSLHD, and the Ingham Institute for Applied Medical Research has identified mental health as a research stream to support its continued promotion and expansion. Numerous mental health research units are supported by the District. Of particular note are those provided in partnership with the University of NSW, including the Psychiatry Research and Teaching Unit, incorporating the Centre for Population Mental Health Research, the Schizophrenia Research Unit and the Infant, Child and Adolescent Mental Health Research Unit. The growth of the University of Western Sydney in south western Sydney has facilitated new partnerships through the funding of a Chair in Academic Psychiatry with further research expansion required to consolidate the partnership. The need to expand research in mental health is well recognised locally and internationally. The Fourth National Mental Health Plan 2009 - 2014 emphasises the need to focus on translational and multidisciplinary research practice. It includes recommendations to develop a national mental health research strategy to drive collaboration and inform the research agenda and refers to both quantitative and qualitative research, and research led by consumers and carers. The NSW Health Mental Health Research Framework (2010) provides a similar emphasis. The Research Strategy for South Western Sydney Local Health District 2012 - 2021 recognises the breadth and depth of research capability within the District and seeks to strengthen this across all clinical areas and staff disciplines. The strategy describes a vision in which the District’s researchers are recognised for undertaking high quality research which improves the health and health outcomes of local people and the broader community. There is a strong focus on building research capacity, capability, quality and outcomes. Current research interests and expertise within the Mental Health service relate to the mental health of refugees, social and cultural factors impacting on population mental health, gender and mental health, perinatal, infant and childhood mental health, developmental disorders, eating disorders, schizophrenia, trauma, anxiety and stress, and the relationships between physical health and mental health. Outside of the Mental Health Service, clinicians of the Liverpool Brain Injury Rehabilitation Unit, Cancer Services and Drug Health are engaged in mental health related research on suicide prevention, psycho-oncology and the presence of co-morbid conditions. Karitane and STARTTS also undertake significant research in collaboration with SWSLHD academic units in their state-wide roles. In undertaking this research, SWSLHD researchers collaborate with international and national partners including universities and research institutes, other government departments and the community managed sector. SWSLHD units continue to attract grants from the NHMRC and ARC and supervise a substantial number of higher degree students. Further opportunities exist to expand these relationships to better understand the needs of the local community, by undertaking peer led research and by collaborating with a broader range of organisations working with priority population groups.
