CITY AND COUNTY OF DENVER DEPARTMENT OF PUBLIC WORKS CONTRACT NO: 201735674 PROJECT NAME: Evergreen Lake House Bridge Replacement ADDENDUM NO. _1_ TO CONTRACT DOCUMENTS Bidders are hereby instructed that the drawings, specifications, and other contract documents are modified, corrected, supplemented and/or superseded for the above-mentioned project as hereinafter described in the following attachments: BID SUBMITTAL PACKET Use attached page BF-7 with added bid item 208-00301 Temporary Diversion, when submitting your bid packet at bid opening. TECHNICAL SPECIFICATION & DRAWINGS Section 250.7 should be deleted from the project specifications. Per the Hazardous materials report, there are no suspect asbestos containing materials associated with the bridge. Plan sheet 6, 7 and 13 - see attached revised plans and associated pay item QUESTIONS AND ANSWERS QI. Will we have to pay City and County of Denver sales tax on the materials delivered to the site? Al . No. If the vendor delivers the materials to the site, no Denver sales tax is due. Q2. Will we have to pay City and County of Denver use tax on our equipment? A2. No. As long as the equipment is used and stored outside of Denver, then no Denver use tax is due. Q3. Will we have to pay City and County of Denver Occupational Privilege tax on our employees? A3. No. The Denver Occupational Privilege tax is not due on employees working on site, since it is not located in Denver. Q4. Is stream flow data available for this stretch of Bear Creek? A4. Stream flow information (both current and historic) is available on the USGS website (waterdata.usgs.gov). The stream gauge "Bear Creek above Evergreen" (Station Number 06710385) is located approximately a quarter mile upstream of the Lakehouse Bridge, near the parking lot at the Evergreen Golf Course. QS. Contractors would like a previous NBIS inspection report to help determine demolition estimates. AS. See attached sketch from the 2014 NBIS inspection report. As-built plans are not available and contractors should be aware that sketches are limited to visible features. Q6. What type of training for soils containing asbestos is required for this project? A6. The minimum Regulated Asbestos Contaminated Soils (RACS) training will be provided by Steve Gonzales (Denver Department of Environmental Health) either in the field on site or at the City and County of Denver WEBB building at no cost. This training is required of all supervisors and field personnel of the apparent low bidder. Contractors can obtain additional asbestos awareness training provided by the Colorado Contractors Association (303-290-6611) at their Centennial, CO location for a nominal fee. This class is 4 hours and attendees are given a certificate of completion that covers the responsibilities of the contractor when significant contaminated debris is discovered. ADDENDUM NO. _1_ TO CONTRACT DOCUMENTS

Bidders are hereby instructed that the drawings, specifications, and other contract documents are modified, corrected, supplemented and/or superseded for the above-mentioned project as hereinafter described in the following attachments:

BID SUBMITTAL PACKET

• Use attached page BF-7 with added bid item 208-00301 Temporary Diversion, when submitting your bid packet at bid opening.

TECHNICAL SPECIFICATION & DRAWINGS

Section 250.7 should be deleted from the project specifications. Per the Hazardous materials report, there are no suspect asbestos containing materials associated with the bridge.

QUESTIONS AND ANSWERS

QI. Will we have to pay City and County of Denver sales tax on the materials delivered to the site? Al. No. If the vendor delivers the materials to the site, no Denver sales tax is due.

Q2. Will we have to pay City and County of Denver use tax on our equipment? A2. No. As long as the equipment is used and stored outside of Denver, then no Denver use tax is due.

Q7. What is the voltage on the overhead power line? A 7. 25,000 volts

Q8. If the plan set is followed directly for SWMP, will an engineer be required? AS. The permit requirement is explained in detail for the contractor on page 26 of the project special

provisions. Based on the estimated disturbance, a CDPS-SCP (permit through CDPHE) is not needed but a CASDP (CCD MS4 Permit) is required by submitting a SWMP plan. The SWMP plan (sheets 13 -17) is for reference only (AECOM will not be stamping the SWMP plan) and the contractor is responsible for developing a SWMP. The SWMP in the plan set may be used but the contractor takes full responsibility for it per the specs and have a CO P.E. endorse the SWMP plan regardless of "Major/Minor" modifications. The contractor will need to get a PE on board either from their own forces or contracted out and will need to assign a SWMP administrator per sheet 13 note 3.

Q9. The plans show 6 sign removals; are any to be reset? A9. Only the Park entrance sign is to be reset- all others are removal.

QlO. There is an existing mailbox on the NE comer of the bride, will this need to be reset? AlO. Yes. Location to be determined by field project manager. All costs associated shall be included in work.

Qll. Can alternate disposal sites be used in lieu of DADS? All. Reference spec 202.02

Q12. Can the Construction field trailer be in the parking lot by the restrooms or does it need to be in staging a area? What are the rules on where the trailer can and cannot be placed?

A12. The construction field trailer is to be only located in the staging area as per sheet 17.

Q13. How is water diversion (and payment) being handled during construction? Al3 See attached revised plan sheets 6, 7 and 13 .

This ADDENDUM shall be attached to, become a part of, and be returned with the Bid Proposal.

If the Manager mails a written Notice of Apparent Low Bidder, addressed to the Bidder's business address stated on this Bid Form, the Undersigned Bidder shall, in accordance with the Contract Documents, be ready to, and shall, within five (5) days after the date of the Notice: (i) execute the attached form of Contract in conformity with this bid; (ii) furnish the required proofs of insurance; and (iii) furnish the required bond in the sum of the full amount of this bid, executed by a surety company acceptable to the Manager.
