PROJECT: Independence Mill Dam Repair Independence, Wisconsin BID DEADLINE: June 20, 2013 2:00 P.M., Local Time We (Name of Bidder) acknowledge that we have received the Contract Documents prepared by Aubian Engineering Inc. that are listed in the Project Manual Table of Contents. We hereby agree to provide all labor, materials, equipment, and services required to complete the work in strict accordance with the Contract Documents for the following stated amount(s). Strength Material. 1.04 Submittals A. Provide a Flowable Fill Plan containing the following information:

1. Submit the name(s) of flowable fill materials suppliers intended for use. Provide a Manufacturer's Certificate indicating that products meet or exceed specified requirements.

2. A schematic of pumps and piping used to place Flowable Fill. 3. Submit flowable fill mix design for specified strength. Submit separate mix designs

when admixtures are required for the following: a. Flowable fill work during hot and cold weather. b. Air entrained flowable fill work.

4. Identify design mix ingredients, proportions, properties, admixtures, and tests. 5. Submit test results to certify flowable fill mix design properties meet or exceed

specified requirements.

B. Submit delivery tickets indicating actual materials delivered to Project site.

C. Approval by Engineer of Contactor submittals shall not alleviate Contractor’s responsibilities for completing the work as specified. 1.05 Quality Requirements A. Flowable Fill Plan shall require approval of the Engineer before delivery and placement of flowable fill. B. Do not install flowable fill during inclement weather or when ambient temperature is less than 40 degrees F. C. Defective Flowable Fill: Fill delivered without proper documentation shall be rejected and replaced at the direction of Engineer.

1.06 Qualifications A. The proposed company specializing in manufacturing the products specified in this section shall have a minimum of three years documented experience in providing flowable fill mixes comparable to the specifications herein.

1.07 Definitions A. Flowable Fill: Lean cement concrete fill used where future excavation is not anticipated such as to fill lower portions of demolished structures and voids beneath concrete slabs. PART 2 - PRODUCTS 2.01 General A. Contractor shall be responsible for calculation of required volumes of flowable fill based on Drawings, consultations with Engineer, estimates of wastage during delivery, and placement, experience with similar materials, and other factors as identified by Contractor.

B. Contractor shall be responsible for all costs associated with delays or material quantity shortfalls due to volume miscalculations, or required rework resulting from not meeting material or placement specifications.

2.02 Cement A. ASTM C150, Type I or II, low-alkali containing less than 0.60 alkalies.

2.03 Pozzolan A. ASTM C618, Class F, fly ash having the following properties:

1. Loss on ignition limited to a maximum of 2.5 percent. 2. Contain less than 4 percent sulfur trioxide by weight.

B. Pozzolan shall be furnished from the same source and supplier for all flowable fill and that used in the submitted mix designs. Pozzolan from another source other than that used in approved mix designs shall be rejected.

2.04 Water A. Clean, potable and free from oil, salt, acid, alkali, organic matter, or other deleterious substances.

2.05 Fine Aggregate A. Hard, dense durable particles of either sand or crushed stone regularly graded from coarse to fine, free from loam, clay, lumps and foreign material. Maximum size aggregate for fine aggregate Flowable Fill shall pass a 3/8-inch sieve.

B. Conform to ASTM C33. Local aggregates not complying with ASTM C33 but which have shown by test and actual service to produce concrete of adequate strength and durability may be submitted for approval.

C. The fineness modulus of the fine aggregate shall not be less than 2.3 or more than 3.1, nor vary by more than 0.20 from the value assumed in selecting proportions of the flowable fill.

D. Aggregates for all flowable fill shall be from the same source quarry used to develop flowable fill mixes.

2.06 Admixtures A. Air Entrainment: Conform to ASTM C260 and be compatible with water reducing and any other admixture. If air-entraining cement is used, any additional air-entraining admixture shall be of the same type as that in the cement.

2.07 Mixes A. Provide Flowable Fill containing cement and water. At the Contractor’s option, Flowable Fill may also contain fly ash, fine aggregate or chemical admixtures in any proportions such that the final product meets the strength and flow consistency requirements include in this specification.

B. Mix and deliver flowable fill in accordance with ASTM C94/C94M, Option C.

C. Flowable Fill Design Mix:

D. Provide water content in design mix to produce self-leveling, flowable fill material at time of placement.

E. Design mix air entrainment and unit mass are for laboratory design mix and source quality control only.

2.04 Source Quality Control A. Prepare delivery tickets containing the following information:

1. Project Designation. 2. Date and time. 3. Class and Quantity of flowable fill. 4. Actual batch proportions.

PART 3 - EXECUTION

3.01 Examination A. Verify structure or excavation is dry and free of debris and ready to be filled.

3.02 Preparation A. If necessary, dry excavation and remove debris.

B. Block any openings that would allow flowable fill to extend into areas not to be backfilled.

3.03 Protection A. Protect reservoir, creek and wetlands from any and all materials used during fill process, including fill, admixtures, oil and grease, loose debris, and chemicals.

3.04 Field Quality Control A. Ensure that batch tickets match approved design mixture.

3.05 Placement A. Place flowable fill by chute, pumping or other methods as approved by Engineer.

B. If needed, place flowable fill in lifts to allow inspection of placement after each lift.

C. Place flowable fill to ensure filling of corners, areas around existing columns or other features.

D. Place flowable fill to elevations indicated on Drawings without vibration or other means of consolidation.

3.06 Cleaning A. Remove spilled and excess flowable fill from Project site.

B. Restore facilities and site areas damaged or contaminated by flowable fill installation to existing condition before installation.
