ALISSimproving self-management of long term conditions
Context:• every CHP to have self management
framework (BHBC)• identify and map local support• Community engagement
what’s the problem?
• useful support is local, hard to capture, often informal with no web presence
• huge amount of information – disconnected, duplicated
• hard to find – no matter how useful
• hard to collate and publish
• no chance to contribute, can’t share information
• improve processes – link data, not another website
• empower - designed by people who are using it
• enable people to create content together
• distribute control, local not centrally managed
• sustainable
• create a platform for innovation
• transparent processes (reuse materials, open source software) – we can all join in

How did we do it?
• Be creative, use techniques developed by public service designers to generate ideas from us all
• Get out the silos - collaborate with people living with ltc, service designers (from art colleges), digital / social media experts, education and many sectors
• Mystery shopper – in community
• Adult literacy classes, poor literacy = poor health
• General Practice teams – linking with communities
• Social innovation workshops/hack days
