It is my pleasure to present the first annual report for Black Swan Health Limited (Black Swan Health), a company limited by guarantee. Emerging in 2014, following changes in the funding and service delivery model for primary health care, Black Swan Health, has been built around key personnel and expertise gained over 23 years as Panorama Health Network (formerly known as the Osborne Division of General Practice) which commenced operations in 1993. In January 2016, Black Swan Health also acquired One Healthy Community and its programs based in the Fremantle region. Operations and outcomes of direct service delivery programs were successfully maintained by the seamless transfer of expertise, policies, systems and processes to Black Swan Health.
Black Swan Health’s vision is to provide the best possible health and mental health outcomes for the community as a whole and for individuals who require care and/or support. The primary focus is the provision of high quality primary and mental health services that achieve positive health outcomes for the community.
In the organisation’s first year, the Western Australian market place has transformed rapidly and we have seen shifts not only in the not-for-profit health sector but in many other sectors. I commend the Board, Executive Management Team and staff for their ability to adapt quickly and thrive in this new environment.
In just twelve months, Black Swan Health has grown its service footprint to reach as far north as Two Rocks, east to Chidlow and south to Rockingham. Our grant funding grew to over a staggering $16 million by April 2016. In that time, Black Swan Health established 21 programs and employed 161 health professional staff as well as contracting 99 additional health professionals.
It has been a privilege to serve on the Black Swan Health Board as Chair. On behalf of the Board and Executive Management Team, I congratulate the Black Swan Health staff team who worked diligently to ensure that Black Swan Health delivered much needed services and programs to the community in 2015/16. We look forward to continuing to deliver these important primary and mental health services to the metropolitan community of Perth and beyond in the future.

Black Swan Health Limited commenced delivery of services on 1 July 2015 and has gone from strength to strength since its inception. Building on the knowledge and skills acquired from Panorama Health Network, we have continued to deliver exceptional primary and mental health services for the community.
The year’s highlight was successfully merging with One Healthy Community, a Fremantle-based not-for-profit service, in January 2016. Along with other operational changes, this increased our service footprint to cover the north, east and southern suburbs of the Perth metropolitan area. We are now delivering our primary and mental health care services from 12 locations and have grown to over 160 employees, with an additional 99 contracted health professionals. We have experienced multidisciplinary teams which includes clinical psychologists, occupational therapists, physiotherapists, pain specialists, registered nurses, dietitians, Aboriginal health workers and many others.
In the last 12 months, Black Swan Health has delivered over 70,000 occasions of service to clients throughout the Perth north and south metropolitan regions. We established 21 primary and mental health programs after winning over $16 million in grant funding.
In 2016 we launched our Healthy Lifestyle Program (HLP) which delivers much needed support for those with ongoing chronic conditions such as diabetes, asthma, cardiovascular disease and chronic renal disease. Our HLP team have expanded significantly and currently deliver their program from Osborne Park, Joondalup, Fremantle, Mirrabooka and our new Midland premises.
Finally, it is the people that make Black Swan Health unique and a special place to work. I take pride in recognising the passionate and committed staff at all our locations for their hard work throughout the year. I cannot thank them enough for their contribution to this rapidly growing organisation. I look forward to the year ahead and expanding our service reach to more people in the community.

ALISTAIR VICKERY - CHAIRMBBS, FRACGP, Graduate of the Australian Institute of Company Directors (AICD)Alistair brings a wealth of expertise in general practice, medical education, governance and research to Black Swan Health’s Board of Directors. He is an Associate Professor at the School of Primary, Aboriginal and Rural Health Care at the University of Western Australia and has conducted key research in primary healthcare. He has a special interest in the management of chronic disease and obesity. In 2014, Alistair was awarded the WA Health Excellence Award – Primary Care in research and the model of care for Familial Hypercholesterolemia.
STEVEN KAN - DEPUTY CHAIRMBBS (WA), Australian Institute of Company Directors courseSteven is an experienced general practitioner based at Jones Street Medical Centre in Balcatta. He lectures at the University of Western Australia and the University of Notre Dame.
DAVID CHEW - FINANCE DIRECTORMBBS, FRACGP, Australian Institute of Company Directors course David has been a general practitioner since 1992. His special interests include family medicine, oncology, cardiovascular, musculoskeletal and chronic medical conditions.
WARREN BAILLIEBachelor of Laws; Bachelor of Commerce; Graduate of the Australian Institute of Company Directors (AICD), Grad Dip Chartered Secretarial Practice; Fellow Chartered Institute of Secretaries and AdministratorsWarren is an experienced corporate and commercial lawyer, a qualified chartered secretary and company director. In addition to qualifications in Law and Commerce, he has extensive Board, committee and management expertise and a strong understanding of effective governance practices. For more than 10 years Warren has worked for Australia’s largest independent oil and gas company and is currently their Group Company Secretary where his work includes a focus on governance and management matters.

She has worked in clinical roles in hospital and community settings and held CEO positions in diverse health and medical organisations. She brings her knowledge of disability, rehabilitation and primary health care to the Board and is Chair of the Clinical Governance Advisory Committee. Her other roles include Board Member at the Disability Services Commission Board and Managing Director at Vaeda Consulting.
DAVID MANUELBachelor of Pharmacy, Australian Institute of Company Directors courseDavid is an experienced pharmacist, business owner and company director. He has 25 years of community pharmacy experience and specialises in aged care pharmacy services, opiate dependency treatments and primary health care. David is passionate about a sustainable and accessible health care system.

SARAH TADIER - OPERATIONS EXECUTIVE SOUTHSarah has an extensive background in the public health system and not-for-profit sector. Her areas of expertise include project planning and management, change management both cultural and process, development of organisational documentation and management of program and service delivery. She is passionate about providing quality health care services to the community and is responsible for overseeing health needs assessment, planning, service delivery, performance evaluation and partnership building at Black Swan Health.
KATE FLOYD - COMPANY SECRETARYBachelor of Science (Health Information Management)Kate is responsible for corporate governance processes including policy, risk management, quality management, internal audit functions and organisational compliance. She is passionate about corporate governance processes and has over 10 years of experience in this area.
ALAN WRIGHT - CHIEF FINANCE OFFICERBachelor of Commerce; Fellow of the Governance Institute Australia; Fellow of the Chartered Institute of Secretaries and Administrators; Fellow of the Institute of Public Accountants; Member of the Australian Institute of Company Directors; and Associate Fellow of the Australian Institute of Management.Alan is responsible for financial management, human resource management, and information management. JOANNE FLETCHER - OPERATIONS EXECUTIVE NORTHBachelor of Social Policy and Social WorkJoanne is passionate about delivering high quality, consumer focussed services to at risk groups in the community and has 15 years of experience in this area. Her professional achievements include developing and implementing a joint community response for at risk youth in the East Kimberley and the successful integration of a new community based early intervention youth mental health service in the Perth region. She is responsible for overseeing health needs assessment, planning, service delivery, performance evaluation and partnership building at Black Swan Health. Joanne is also the coordinator of the Joondalup Mental Health Professionals Network which provides networking and developmental opportunities for professionals working within the mental health field in the region.

Black Swan Health was established in 2014 as an independent not-for-profit company limited by guarantee. Our primary focus is the provision of high quality primary and mental health services that achieve positive health outcomes for the community.
Black Swan Health emerged from changes in the funding and service delivery model for primary health care and in 2015, acquired all direct service delivery programs previously provided by Panorama Health Network (formerly known as Perth North Metro Medicare Local, Osborne GP Network and Osborne Division of General Practice). In 2016 Black Swan Health also acquired One Healthy Community (formerly operating as Fremantle Medicare Local). Black
Swan Health and its related entities has operated primarily in the health, community and mental health sectors since 1993, developing a considerable body of knowledge in the delivery and evaluation of programs and services. As a registered provider of disability services, Black Swan Health has also embedded the principles of person centred practice into our day to day service delivery and we have well established outcome based reporting and measurement systems in place across all areas.
Black Swan Health delivers tailored health, mental health and wellbeing services that are high quality, evidence-based, safe and aim to achieve positive outcomes for our clients.

conduct, loyalty to the objectives of the organisation. This includes never denigrating other Board members or the operational arm of Black Swan Health outside the organisation

At the commencement of their term of office with Black Swan Health each Director signs the Directors Code of Conduct. Along with the Code of Conduct, the Board is bound by the Australian Charities and Not-for Profits Commission Act 2012 and the Corporations Act 2001, the Black Swan Health Board Charter, and Board Level policy and procedural documents.
To support the Board on key governance and risk areas three Board subcommittees are in place:
• Clinical Governance Advisory Committee (CGAC) – responsible for organisation-wide coordination of clinical governance activities. The CGAC is chaired by a Board Director with clinical background.
• Finance (Risk and Audit) Subcommittee – responsible for reviewing and advising the Board on all financial, audit and risk matters.
• Nominations and Remuneration Subcommittee – responsible for advising the Board on Director nominations and Board succession planning and remuneration.

QUALITY ACCREDITATIONBlack Swan Health is accredited against the Royal Australian College of General Practitioners (RACGP) Standards for General Practice; and the National Standards for Mental Health Services.
OCCUPATIONAL HEALTH AND SAFETYBlack Swan Health is committed to the safety, health and welfare of all employees, visitors and contractors and has a Board level policy that complies with Section 19 of the Occupational Safety and Health Act 1984.
In accordance with the policy, Black Swan Health:• Provides and maintains the workplace and systems of work such that those who come into
contact with it are not exposed to hazards• Provides information, instruction, training and supervision, as is necessary, to enable

CULTURAL AWARENESS TRAININGDuring the year all staff at Black Swan Health participated in Cultural Awareness Training facilitated by Ms Ingrid Cumming, a Whadjuk Nyoongar and Director and Chief of Engagement of local organisation Codeswitch.
Ms Cumming has worked with Black Swan Health and its related entities for many years and provides our team with practical and engaging strategies to improve their interactions and cultural competence across all programs.

Closing The Gap ABORIGINAL HEALTH TEAM NORTHThe Closing the Gap program is available to Aboriginal people living in or attending medical services in the Perth north metropolitan region which covers north to Two Rocks and east to Mirrabooka. Our program is made up of the following:
• Care Coordination for people with a chronic condition
• Supplementary Services for people receiving Care Coordination
• Medical Outreach for people with or at risk of a chronic condition
• Improving Indigenous Access to Mainstream Primary Care to inform health professionals and enable provision of culturally appropriate health care to Indigenous people.
The Closing the Gap Team actively supports around 75 unique clients and manages up to thirty referrals every month. The support they provide to Aboriginal people in the community makes a significant difference to the lives of our clients.
In the past year we have built and maintained collaborative relationships with key stakeholders such as the Aboriginal Interagency Network, Whadjuk Community Group, Aboriginal Workforce Development Centre, Public Health and Ambulatory Care, Department of Human
Services – Indigenous Community Forum, South West Community Drug Service at Uniting Care West, and STI and BBV Quarterly Forum by attending and contributing to meetings. We have also provided information and contributed resources such as “Lunch Right Menu Planners for Aboriginal Families” to other Aboriginal health programs.
Margaret is 33 years old, from a remote community and has struggled significantly with complex health needs. She has severe bronchiectasis and is dependent on an oxygen tank to keep breathing. She finds it very difficult to walk even short distances.
Her care is further complicated by issues around family, housing, communication (she speaks an Aboriginal language and her carer translates for her), drugs and alcohol, and the difficulties experienced by people from remote communities who struggle to adjust and survive in an unfamiliar community with little or no connection to family or country lands.
We work closely with Margaret and her carer and have provided extensive support over time. Most recently, we purchased a portable oxygen concentrator for her. This will improve her exercise tolerance, mental alertness, mood, stamina, ability to get a good night’s sleep and mobility.

Closing The Gap ABORIGINAL HEALTH TEAM SOUTHThe Aboriginal Health Program (AHP) South assists Aboriginal and Torres Strait Islander people in Fremantle, Cockburn and Melville areas to better manage their chronic conditions. This is achieved by helping clients access mainstream and Aboriginal health care services. The AHP team includes three Aboriginal staff with strong local connections who advocate the delivery of culturally appropriate and holistic care, and help implement individual GP Care Plans to improve quality of life. The team also contributes to closing the gap in life expectancy of Aboriginal clients.
The program helps around 30 clients each month from a clinic located at the Wesley Central building in Fremantle. Conveniently positioned opposite the Fremantle Post Office and adjacent to the Fremantle train station, this clinic enables easy access to support the local Aboriginal and Torres Strait Islander community.
Bill and Sandra’s story:
Bill and Sandra are Elders in the community. They came to us for ongoing support and timely access to appropriate care.
Bill and Sandra were frustrated because they weren’t sure of how to access services and get the help they needed. Once we explained the services and talked them through their care plan step-by-step, we could build their confidence in self-managing their chronic conditions. We followed up with continued outreach and engagement to help them stay on track.
As a result, Bill was able to interact more with family members and become involved in community social groups. He started fishing and going to local lakes. Sandra also joined a social activity group. Their emotional and social wellbeing increased and they both looked visibly healthier and happier.
Life now revolves less around their medical conditions and more around family and community networks. As a result of our outreach and support, we have seen their children and grandchildren start to attend GP appointments. Extended family members have approach the AHP team to help link them into local health services. The ripple effect has been a wonderful testament to the importance of culturally appropriate delivery of services.

ATAPS NORTH AND FREMANTLEATAPS (Access to Allied Psychological Services) provides low cost mental health and counselling services to hard-to-reach populations who cannot access other Medicare funded care or afford private mental health services. The program targets those with financial hardships that experience common mental health disorders of mild to moderate severity.
The ATAPS multidisciplinary team has a prompt and comprehensive triage process and reviews and responds to every referral to ensure that clients get the right services at the right time.
Collaboration with local universities has resulted in a number of Masters of Psychology student placements within the ATAPS program. This has enabled increased client services and provided invaluable learning opportunities for students.
The ATAPS program is provided in conjunction with a range of other Black Swan Health programs and clients are provided with the most appropriate service for their need

Melissa was referred to ATAPS Healthy Minds by her GP. She was suffering postnatal depression but did not consider counselling to be an affordable option. At ATAPS, Melissa was able to see a counsellor specialising in post-natal depression. Through a range of counselling techniques Melissa was able to move through her depression without the need for medication. By her sixth appointment Melissa was able to feel comfortable with the changes in her life.

Partners in Recovery NORTH AND SOUTHPartners in Recovery (PIR) delivers a recovery focussed, service coordination model for people who live with severe and persistent mental illness and have multiple unmet needs. The aim of the service is to connect consumers with services that will support their recovery and meet their needs.
Black Swan Health is the lead agency in a consortium of carer and consumer representatives and organisations including:
SOUTH AnglicareWA, Ruah Community Services, Fremantle Multicultural Centre, Community First International, Fremantle Hospital Mental Health Services, and St Patricks Community Support Centre.
NORTH The North Metropolitan Health Service, UnitingCare West, Women’s Health and Family Services, Mental Illness Fellowship of WA, Cyrenian House, Perth Home Care Services, Aftercare, AnglicareWA, Ruah Community Services, and Southern Cross Care.
These organisations are funded by Black Swan Health to employ Support Facilitators and undertake face-to-face work with PIR clients. We work closely with these community services to achieve strong support systems and services for our clients.
Over the past 12 months, the PIR North team developed their hoarding and squalor services and established the ’Buried in Treasures’ workshop series and ‘Morning Tea Conversations’. ‘Buried in Treasures’ is a 15-week program that offers support, guidance and encouragement to people with hoarding tendencies. ‘Morning Tea Conversations’ brings together consumers, carers and health professionals to develop and support services for people displaying hoarding behaviours. PIR North also facilitated laughter yoga sessions at local support groups and coordinated a five-day Residential Recovery Camp attended by 18 people.
Support Facilitators are located at a number of host agencies throughout the region to enable easy access to the program. Consortium partners are funded to implement projects that encourage social inclusion, life skills and mental health awareness. In the past year, the PIR Fremantle program exceeded participant target numbers by 36.

Healthy Lifestyle ProgramThe Healthy Lifestyle Program is designed around a successful, evidence-based early intervention model supporting informed lifestyle choices and health behaviour changes to reduce the burden of chronic disease. The program provides coordinated, client-centred, multidisciplinary care to clients who are:
• at risk of, or diagnosed with coronary heart disease
• at risk of, or diagnosed with type 2 diabetes
• diagnosed with chronic kidney disease (stable)
• diagnosed with chronic respiratory disease (newly diagnosed and mild-moderate COPD and asthma)
• diagnosed with chronic musculoskeletal conditions.
We have a multidisciplinary team of diabetes educators, dietitians, physiotherapists and respiratory nurses/asthma educators providing services. Clients are assessed at the start of the program then reviewed at three, six and 12 month intervals. Their GP is kept informed on their patient’s progress through the provision of regular progress reports.
Health and Wellness ProgramAll clients are offered the opportunity to join a free Health and Wellness Program. This is a six-week group education intervention that gives tailored advice and encouragement to support healthy eating and exercise behaviours.
The program is delivered from several locations in the region. Client appointments are held at Osborne Park, Joondalup, Mirrabooka and Midland. The Midland premises opened in April to expand Black Swan Health’s services to the eastern suburbs.
Performance and the futureOur clinicians have delivered over 1,500 occasions of service and have run eight client group programs since we started delivering the Healthy Lifestyle Program in January 2016.
George joined the Healthy Lifestyles Program following advice from his GP to improve his overall heath and better manage his diabetes. What started as a temporary commitment quickly developed into a long term health resolution for the 70 year old. Physiotherapist Simone Levy has worked with George from day one and couldn’t be more proud of his achievements.

CHRONIC PAINThe Chronic Pain program is a high quality, pain management program targeted at individuals aged over 18 who have been living with chronic pain for longer than 3 months. The program helps people with pain to improve function and return to a more normal life.
The early identification and treatment of pain at a primary care level is designed to facilitate reduced reliance on tertiary services and reduce waiting times for clients.
Client feedback in regards to the program showed that:
• 96% of clients found the group sessions useful
• 98% of clients found their individual sessions with the pain specialist beneficial
• 96% of clients found their individual sessions with the physiotherapist and psychologist beneficial
The Chronic Pain program allows a multidisciplinary diagnosis of chronic pain and individualised treatment plans. Clients treatment plans are developed through collaboration between specialists and care is delivered in a planned and coordinated manner. The program covers assessment, treatment, communication and education for the clients condition.
Chronic Pain uses an outcomes-based model which delivers multidisciplinary group education sessions and individual consultations to improve the client’s knowledge and skills so they can to manage their pain.
3 month follow up with clients showed that:
• there was a significant reduction in clients depression scores
• there was a significant reduction in clients strongest pain experienced
• there was a significant improvement in clients coping strategies
Care coordination to other programs is a key component of Chronic Pain for ongoing management and support. At the conclusion of the Chronic Pain program a final combined patient report is generated by the Chronic Pain specialists. The report is then forwarded to everyone involved in the client’s care. This includes information on the examination results, assessment, recommendations and a management plan.

Respiratory LinkageRespiratory Linkage provides education and support and empowers clients with a chronic respiratory condition to effectively manage their condition and improve their lifestyle.
The service is offered to people aged 18 and over, who live in the Perth north metropolitan area and are diagnosed with moderate to severe chronic obstructive pulmonary disease (COPD), or experience frequent exacerbations; interstitial lung disease (ILD) or bronchiectasis. The aim is to slow disease progression and reduce hospital admissions.
Black Swan Health is the only organisation providing a community based clinical respiratory service.
The demand for Respiratory Linkage services remains high, reflecting both the high prevalence of respiratory conditions and gaps in specialist community respiratory care in the Perth north metropolitan region.
Client feedback in response to the program showed that:
• 94% of clients reported an increased knowledge of lung disease
• 83% of clients reported that they felt confident to manage their lung condition effectively
• 100% of clients reported that our respiratory nurses were approachable and friendly
• 100% of clients said that they were treated courteously and with respect
• 94% of clients said they would recommend the service to others
A significant outcome of the program was that for this period a total of 23 hospital avoidance episodes were completed resulting in an estimated saving of 117.3 hospital bed days for clients.

Aged Care AccessThe Aged Care Access Program seeks to improve access to primary health care services for older people through the provision of allied health services to residential aged care residents and older people in the community. The service strengthens links between health and aged care and complements GP services.
Mary was experiencing extreme difficulty with swallowing and was reluctant to eat her meals which impacted on her general health. We noticed a quick decline in her
weight and wellbeing and immediately sent a referral request to Black Swan Health for a speech pathologist to attend to her at our aged care facility. The speech pathologist attended to Mary the following day and quickly assessed her swallowing difficulties and provided a management plan to treat the issue. This treatment rectified Mary’s swallowing difficulties and she began to eat her meals again. We saw an improvement in her weight and general wellbeing within a few days.

Living Longer, Living StrongerLiving Longer, Living Stronger (LLLS) is an evidence-based strength training program for people over the age of 50 years with a chronic condition.
The program is run by in-house physiotherapists and provides clients with chronic diseases health complications a higher level of supervision with a structured exercise program tailored to their needs.
University studies have repeatedly shown that strength training can decrease blood pressure, help with weight loss and improve overall balance which reduces the risk of falls and helps with daily movements such as sitting, standing and walking.

GP4YPGP4YP (General Practice for Young People) is a free, community-based early intervention service for young people aged between 12 and 25 years of age who live, work or study in the Fremantle region.
The service is provided by a ‘youth-friendly’ GP who sees young patients directly or provides pathways to counselling, support groups and advocacy. The GP4YP service is delivered from Wesley Central in Fremantle.
During the past 12 months, our GP4YP team attended Cockburn, Melville Fremantle Interagency Youth Forums to build and maintain partnerships within the sector, particularly in relation to youth mental health.
Our goal is to continue providing easy access to primary health services for young people, particularly those suffering from mental health issues.

Freo Street DoctorFreo Street Doctor is a mobile clinic that provides an easily accessible and non-judgemental primary health care service to less fortunate people in our community. Held at eight locations in and around Fremantle each week, the clinic caters to the marginalised, disadvantaged and homeless including those with mental health issues and Aboriginal and Torres Strait Islanders. There is an additional clinic in Rockingham that operates on a fortnightly basis.
Each Freo Street Doctor clinic is staffed by an Outreach Worker, Nurse and GP. The service has held a series of ‘Well Women’ clinics, using donations from Soroptimist International. In the last twelve months, 22 clinics were held in a second van, alongside the main Freo Street Doctor van.
In the past year, all services have performed well with attendance numbers increasing in seven out of nine clinics.
Now that the Fremantle Hospital ED is closed, the Freo Street Doctor is vital for clients who have no way of getting to the ED at Fiona Stanley Hospital.
Going forward we plan to deliver a series of Hepatitis treatment clinics. Freo Street Doctor currently sees 140 males and 70 females diagnosed with Hepatitis C who would greatly benefit from this service.
The Freo Street Doctor has countless stories of clients who have been helped by the service. Here are just two examples:
Client identified as a victim of domestic violence during their ‘Well Women’ check.
During her ‘Well Women’ check, a female patient was asked if she had ever suffered domestic violence. She replied that no one had ever asked her the question before and she was glad to be able to tell someone. She was consequently given details of organisations that could help her and offered counselling.
Sick and dehydrated homeless client may not have survived without Freo Street Doctor
A homeless client, who was very sick and dehydrated, attended the South Beach Clinic. His symptoms were serious and an ambulance was called. While waiting for the ambulance, the medical staff commenced treatment on him (setting up an IV drip). Had he not been seen by the Freo Street Doctor, it is very likely that his condition would have deteriorated quickly and he may not have survived.

headspace JOONDALUPheadspace Joondalup supports young people aged 12 to 25 years with mental health concerns. The service connects the young person, their families and carers with support services in the simplest and fastest way possible to ensure appropriate care is available when needed. Links to internal and external specialists and with local GPs ensures that care is provided for the young person during their time with the program and after.
Over the last year, headspace Joondalup has been proactive across a number of forums and networks in the northern suburbs. Engaging with other agencies allows the service to deliver targeted responses to youth affected by homelessness, mental health, vocational, drug, alcohol or any other issues. One of these networks includes a working group comprising of the Western Australian Police, Department for Child Protection and Family Support, North Metropolitan TAFE and Ngala which works together to target youth homelessness in inner city Joondalup and the surrounding areas.
headspace Joondalup consults directly with youth via a Youth Reference Group to ensure the program is providing the services they want and need. In 2015/16 this group grew from 8 to 13 members and successfully organised a number of events to raise awareness of youth mental health issues. This included a quiz night held during Mental Health Week and a youth art exhibition that was supported by the Department of Local Government
and Communities’ National Youth Week grants program.
With a ‘no wrong door’ ethos, headspace Joondalup ensures access to the service for any young person regardless of their geographical location.
headspace Joondalup aims to increase the number of young people who are able to access services from the far northern metropolitan suburbs such as Two Rocks and Yanchep.
Felecia, 19 year old female attended six sessions after three failed attempts at counselling provided by another service. Throughout counselling at headspace Joondalup, Felecia made significant progress in her stress, anxiety and self-criticism that had impacted her ability to function to the point of not being able to leave the house. By her final session, Felecia had enrolled in TAFE, found herself new independent accommodation, had a job and was feeling more capable, confident in herself and her abilities and hopeful for her future.

headspace OSBORNE PARKheadspace Osborne Park provides early intervention services to youth between the ages of 12 and 25, particularly those with an emerging or first-onset mental disorder. The program works to establish and maintain pathways which connect young people to the right support service in the simplest and fastest way. headspace Osborne Park provides treatment and coordinates access to internal and external specialists. The program also maintains or facilitates links with local GPs for continued care.
The centre hopes to further develop ‘Joes with my Bros’, a youth-led initiative aimed at reducing stigma surrounding mental health, in young men aged 18 to 25. The centre will continue to partner with local cities (Stirling/Subiaco) to meet the needs of hard-to-reach groups in the community.
headspace Osborne Park had a number of significant achievements in the past 12 months including partnering with Telethon Kids Institute to develop a tested and evaluated Bullying Education and Prevention Resource Kit. The aim was to assist headspace centres nationally achieve greater engagement and outcomes for teenagers impacted by bullying. headspace Osborne Park was instrumental in developing and delivering the following outcomes:
• Developed new materials in consultation with young people and other key stakeholders which aim to address bullying
• Piloted bullying prevention workshops in metropolitan and regional schools
• Provided materials to headspace National for distribution across Australia.
The centre has expanded its reach by co-locating at the Hainsworth Centre, Girrawheen one day a week with a focus on providing services to Aboriginal young people in the area.
Ryan, a 19 year old male with mild intellectual disability was referred by his mentor from Inclusion WA for support. It became apparent that Ryan had created a made up social network around him. Validating Ryan’s feelings and bringing focus back to real life people and discovering and working with Ryan’s strengths led to him reducing his made up world and engaging more in reality. We received a text from his mentor thanking us as she had seen a real positive change in him.

hYEPPheadspace Youth Early Psychosis Program (hYEPP) is a specialised service for 12 to 25 year olds experiencing, or at ultra-high risk of, a first episode of psychosis. hYEPP has a "no wrong door" policy - referrals from all sources are accepted with easy and timely access and a low threshold for assessment.
hYEPP is the only early psychosis service in the Perth north metropolitan area. The service commenced operations in January 2015, with Black Swan Health the lead agency for the hYEPP Joondalup hub and Osborne Park spoke.
The hYEPP service has three core elements:
1. Mobile Assessment and Treatment Team (MATT)
2. Continuing Care Teams (CCT)3. Functional Recovery Program (FRP)
These three components make up a comprehensive and flexible service model which covers a full continuum of care from acute assessment and intervention through to continued maintenance and functional recovery.
Our multidisciplinary team of experienced social workers, psychologists, occupational therapists and mental health nurses complete comprehensive biopsychosocial assessments and deliver holistic intervention with the young person and their family for between 2 to 5 years. A highly specialised medical component of the service is overseen by a team of Consultant Psychiatrists. Services are delivered in our youth friendly purpose built centre or wherever the client feels comfortable.
Client Outcomes of the program include:• Slowing progression of illness• Minimising development of disability• Social recovery• Functional recovery including
returning to/gaining employment and education
Client feedback from the program showed that:
• 90% of clients agreed that they felt that hYEPP staff listened to them
• 80.4% of clients agreed that they found it easy to get to their appointments
• 85.3% of clients agreed that they would recommend hYEPP to a friend who required similar help
• 82% of clients agreed that they had a say in their treatment
• 79.7% of clients agreed that their mental health had improved as a result of their contact with hYEPP
• 88.2% of clients agreed that hYEPP staff involved the clients family as much or as little as they wanted
Michael is a 21 year old university student who was referred to hYEPP following admission to a mental health unit for a second episode of psychosis precipitated by a long history of polysubstance use.
Previously, Michael had spent 2 years struggling with university studies and communicating with his family.
Since involvement in hYEPP, Michael has had a complete resolution of symptoms and no evidence of any return of psychotic, depressive or manic symptoms. He takes his medication regularly and has identified a plan to gradually reduce medication in conjunction with the medical staff in the future.
This has helped him in his decision to complete his university studies prior to a trial in reduction of medication. He has shown ability to cope with highly stressful psychosocial circumstances to seek help appropriately from the service.
Michael has re-engaged with previously enjoyed activities and friendships. He is now working in a professional field as well as completing his studies. Michael initially attended hYEPP on a weekly basis and has reduced this to fortnightly contact.
Michael and his family have commented on how supportive they have found the service. They have now regained confidence that Michael will complete his university studies and progress into the career he has been working towards for the past 6 years, which has resulted in a significant reduction in stress for the whole family.

DIRECTOR'S REPORT DIRECTOR'S REPORTPRINCIPAL ACTIVITIESBlack Swan Health, a company limited by guarantee, has been built around key personnel and expertise gained over 23 years as Panorama Health Network (previously known as Perth North Metro Medicare Local, Osborne GP Network and Osborne Division of General Practice), which commenced operations in 1993. In January 2016 Black Swan Health also acquired the programs of One Healthy Community (formerly Fremantle Medicare Local). Operations and outcomes of direct service delivery programs were successfully maintained by the seamless transfer of expertise, policies, systems and processes to Black Swan Health from Panorama Health Network and One Healthy Community.
Black Swan Health's purpose is to achieve the best possible health and mental health outcomes for the community as a whole and for individuals who require care and/or support. This is achieved by delivering tailored primary health and mental health services. The principal activity of the Company during the financial year was Administration of Grants to ensure the provision of quality and safe evidence-based frontline primary health care delivery and support.
OPERATING RESULTSThe surplus of the Company for the financial year amounted to $3,149,153. This includes one-off donations totalling $1,998,814.
THE OBJECTS OF THE COMPANYThe Company was established, and continued to strive to achieve the best possible health and mental health outcomes for the community as a whole and for individuals who required care and/or support by:
1. Supporting the implementation of initiatives that improve the prevention and management of disease;
2. Monitoring the impact of services and analysing the extent to which they have achieved the purpose intended;
3. Utilising available funding to provide services which target gaps in primary health care service provision;
4. Ensuring the patient journey is optimised through the development of integrated and coordinated services including the smooth transition of patients between the primary, secondary and tertiary health sectors and provision of better access to available and appropriate primary health care services;
5. Meeting the special (and localised) health needs of groups (such as Aboriginal and Torres Strait Islanders and those with non-English speaking backgrounds) or people with chronic conditions particularly where these needs are not adequately addressed by the current health system;
6. Engaging in such activities to promote improved health and mental health outcomes; and
7. Fostering community engagement and awareness of primary health care prevention and self-management to increase health literacy and promote health-seeking behaviour;

The Company implemented and managed innovative and effective primary health care initiatives, many of which were specifically developed to meet identified need and gaps in service. There was a dedicated focus on improving access, service delivery, quality and effectiveness of primary health care services within the region. This required effective, responsible and efficient governance and management.
The Company continued to pursue its Mission and address its Objects by focusing on a series of strategies, aligned with each of the Company Objects.
HOW THE COMPANY'S ACTIVITIES CONTRIBUTED TO ACHIEVING THOSE OBJECTSThe Company ensured the provision of quality frontline services to the community. The Company was committed to providing high quality and relevant information and health services within the region, and expanding the range of services available to the community.
The Company successfully ensured that the health programs and health services delivered were effective and accessible, and met the needs of the local community.
During 2016/2017, a wide range of programs and activities were delivered in relation to:
• clinical service provision via a multi-disciplinary workforce;
• improving health service delivery to the community through establishing referral pathways, partnering with health service organisations;
• health education and health promotion, with primary health care professionals and peer leaders facilitating information sessions;
• distribution of information to primary health care professionals and the community;
• liaison with other health care providers, consumers and community groups; and
• further developing well established links with the local area health services.
PERFORMANCE MEASUREMENTThe performance of the Company was measured by a wide range of key performance indicators. Formal evaluation was regarded as an important component of all programs. The Company established high standards of governance and program management, overseen by the Clinical Governance Advisory Committee.
The Company is proud of the high standard of professionalism and quality service provided.
Internally, performance against the Company's Annual Plan and Budget was reported and monitored monthly and year to date, and progress towards the entire Strategic Plan was reported and monitored annually. Inter- and intra-agency benchmarking to help deliver and maintain best practice was adopted across all programs and Board governance processes.
Externally, Black Swan Health's range of funders reviewed the performance of the Company against a variety of objectives.

MEMBERS' GUARANTEEIf the Company is wound up, the Constitution states that each member is required to contribute a maximum of $10 each towards any outstanding obligations of the Company.

REPORT ON THE FINANCIAL REPORTWe have audited the accompanying financial report of Black Swan Health Ltd ("the Company"), which comprises the Statement of Financial Position as at 30 June 2016, the Statement of Profit or Loss and Other Comprehensive Income, Statement of Changes in Equity and Statement of Cash Flows for the year then ended, Notes comprising a Summary of Significant Accounting Policies and other explanatory information, and the Directors' Declaration.
DIRECTORS' RESPONSIBILITY FOR THE FINANCIAL REPORTThe directors of the Company are responsible for the preparation of the financial report that gives a true and fair view in accordance with Australian Accounting Standards and the Australian Charities and Not-For-Profits Commission Act 2012, and for such internal control as the directors determine is necessary to enable the preparation of the financial report that gives a true and fair view and is free from material mis-statement, whether due to fraud or error.
AUDITOR'S RESPONSIBILITYOur responsibility is to express an opinion on the financial report based on our audit. We conducted our audit in accordance with Australian Auditing Standards. Those standards require that we comply with relevant ethical requirements relating to audit engagements and plan and perform the audit to obtain reasonable assurance about whether the financial report is free from material mis-statement.
An audit involves performing procedures to obtain audit evidence about the amounts and disclosures in the financial report. The procedures selected depend on the auditor's judgement, including the assessment of the risks of material mis-statement of the financial report, whether due to fraud or error. In making those risk assessments, the auditor considers internal control relevant to the entity's preparation of the financial report that gives a true and fair view in order to design audit procedures that are appropriate in the circumstances, but not for the purpose of expressing an opinion on the effectiveness of the entity's internal control.
