Avtex personnel have identified certain areas within the Avtex facilityshowing contamination by polychlorinated biphenyls (PCBs) resulting primarilyfrom sudden and unplanned failure of an electrical transformer. PCBs werepopular from the 1930's through the 1960'• for insulating use in electriccapacitors and transformances. They are both chemically and biologicallystable and are very poor conductors of electricity. In high concentrations,PCBs form an oily liquid heavier than water and are not readily dissolved inwater. They also have an affinity for organic matter that helps explain theirbehavior in the environment, particularly in water. According to the VirginiaDepartment of Health, PCBs are everywhere in the environment. Surveys ofambient air, soil, food, surface water and sediments, as well as human fat,blood, and milk have demonstrated measurable levels of PCBs.
Presence of PCBs on the Avtex site has been confirmed by the Virginia StateWater Control Board (SWCB) by its analysis of samples collected onAugust 28, 1989 and September 1, 1989. On September 20, 1989 the VirginiaDepartment of Waste Management (VDWM) requested an evaluation of the Avtexsite and possible assistance from USEPA to address the possible contaminationat the site. In response to this request, USEPA determined that thecompressor room roof and the polyester plant area at the site may pose athreat to public health, welfare or the environment.
In addition to acknowledgement of the presence of measurable levels of PCBs atthe Avtex facility, the USEPA has stated that a number of solvent-containingdrums may pose a potential threat for fire or explosion. The USEPA alsoidentified several process areas where certain hazardous substances may bepresent and could pose a potential threat of release.
Therefore, based on (1) a potential threat to worker health from exposure toPCBs on the site; (2) the potential for offsite migration of PCBs; (3) a fireor explosion potential from solvent"containing drums; and (4) potentialunidentified hazards from other facility process operation, the USEPA has
determined that a removal action is appropriate to abate, minimise, stabilize,mitigate, or eliminate the releases or threat of releases.

SECTION 1STATEMENT L OF UNDERSTANDING
The Regional Administrator, Region III, United States Environmental ProtectionAgency (USEPA) executed an Administrative Order concerning sampling and cleanup actions at the Avtex Fibers Front Royal, Inc site located at 1169 KendrickLane, Front Royal, Virginia. This order was issued on October 31, 1989 andbecame effective on November 1, 1989. The order was issued pursuant to theauthority vested in the President of the United States by Sections 106(a) and104(e) of the Comprehensive Environmental Response, Compensation, andLiability Act of 1980 (42 U.S.C. §§9606(a) and 9604(e)> as amended by theSuperfund Amendments and Reauthorization Act of 1986, Pub. L. No. 99-499, 100Stat. 1613 (1986). Regional Administrators of EPA have been delegated thisauthority. This order was issued to Avtex Fibers, Inc., and Avtex FibersFront Royal Inc. Notice of issuance -of this Order has been given to theCommonwealth of Virginia, pursuant to Section 106(a) of CERCLA, 42 U.S.C.§9606(a). ;

Avtex has owned and operated a rayon producing facility in Front Royal,Virginia from August 1976 until the present. The Avtex plant occupiesapproximately 440 acres of which 60 are under roof and includes 23 basinswhere process and other wastes are disposed. Manufacturing operations beganat the plant in 1940 under ownership \6f the American Viscose Corporation; thegeneral operation has remained consistent. The facility is located on theeast bank of the South Fork of the Shenandoah River. A site map is includedas Figure 1-1*

2.1 WORK PLAN CONTENTS
The purpose of this work plan document is to describe immediate responsemeasures to be implemented at the Avtex site necessary to eliminate the threatto public health, welfare, or the environment that may exist due to potentialworker exposure to PCBs, the potential for offsite migration of PCBs into theShenandoah River, and the potential for fire, explosion, or spillage of wastesfrom leaking drums. This work plan is^consistent with the provisions andrequirements of the National Oil and Hazardous Substances Contingency Plan,40 CFR 300 and CERCLA.

• Act of 1980, we have undertaken the following work.1) The polyester chip area was Isolated from our sewer system early 1n
September, 1989.2) All roofing and accompanying debris was removed from our Compressor room
roof by mid October and clean up of all concrete 1s well underway.3) The present piping from the roof drains of the Compressor room Is being
removed and replaced with new piping and will be complete by next week.4) The complete storm sewer system has been cleaned and all sediment removed.
Most manhole bottoms have been encapsulated.5) The process sewer on the north side of the plant Is presently being cleaned
and tested. This Is the process sewer to which the polyester dock areawas connected*
6) The stored transformer area and the 2A substation area has been clearedand all transformers removed. This area has been cleaned and we arepresently awaiting results of sampling.
7) Maintenance work has been accomplished at our Waste Treatment plant asfollows: AR I 001*68

a) the south final clarlfler center section was replaced andthe remainder of the mechanism repaired and the basin completelycleaned and returned to service.
b) The north final clarlfler was repaired and returned to servicefollowing the work on the south unit.
c) The sludge thickener was drained and the mechanical partsrepaired. The unit was thoroughly cleaned.
d) The Aerobic digester 1s being drained and cleaned.e) The thickener sludge pump has been overhauled.
Implementation of these cleanup efforts, and especially the cleaning of theprocess sewer on the north side of the plant, could result 1n the dislodgingof PCB-contamlnated sediments to the Waste Treatment plant Influent. We aretaking all precautions possible to minimize the danger of any substantialreleases.ThU notice Is being provided as requested In The Administrative Order DocketNo. 111-90-01-DC, paragraph 8.6.

3.2 REPORTING REQUIREMENTS FOR AVTEX FIBERS
Within five (5) business days of effective date of this Order, Avtex willperform the following activities.
Avtex will retain all appropriate contractors.
Avtex will notify the USEPA in writing regarding the identity andqualifications of persons primarily responsible for carrying out the terms ofthis Order. This will be accomplished by this work plan.
All supervisory personnel employed by hazardous waste contractors will meetOSUA 29 CFR 1910.120 and applicable state law.
If contractor personnel are disapproved, Avtex will select replacements withinfive (S) days.
Avtex will submit work plan and schedule to USEPA*
The work plan will be consistent with 40 CFR 300.
The work plan will consist of the following components:
Health and Safety Plan.
Site Security Plan*
Fire Protection Plan for drum storage and disposal areas.
Plan to identify, segregate, and dispose of—
a* substances in drums
b. PCBs in the following areas

If deficiencies in the submitted work plan are noted by the USEPA, Avtex willcorrect those deficiencies within 5;business days of disapproval*
Within 5 business days of approval of the work plan, Avtex will implement thework plan. It should be noted that implementation of all major provisionsother than formal reporting has commenced.
Within 24 hours, Avtex will give notice to EPA if Avtex believes at any timePCBs have the potential to reach the influent of the waste treatment plant.
Seven days after approval of the work plan and until USEPA notifies Avtex thatwork is completed, Avtex will provide a progress report for each 7 day period.This progress report will include the following information:
1. description of actions taken to complete order.
2. description of all data anticipated and activities scheduled for thenext 7 days.
, 3. description of problems encountered or anticipated.
4. Actions taken to prevent or mitigate problems.
S. A schedule for when such actions will be completed.
6. Copies of all analytical data received during reporting period (7days).
7. All modifications to the work plan.
8. Report of any discharge of PCBs into the Shenandoah River.
All correspondence regarding activities pursuant to this Order will be sentvia certified or express mail to USEPA.
All reports, plans, specifications when approved, become part of this Order.If disapproved, corrections shall amend and submit revised submission thatresponds to specified deficiencies*' .
Upon request of USEPA, Avtex will provide any and all information resultingfrom this actions including the following information:
1. Analytical data. '

4. Operational logs and copies of hazardous waste manifests includingsigned receipts from disposal facilities.

Within 30 days of completion Avtex will submit a written report to USEPAdetailing actions taken and notifying USEPA of completion.
If USEPA notifies Avtex of deficiencies (no deadline). Avtex will correctdeficiencies.
If it becomes necessary to change project coordinators during the course ofwork required under this. Order, Avtex will notify USEPA three (3) days priorto the change.
Avtex will consult with USEPA prior to all sampling and analysis. Avtex willuse a laboratory which complies with EPA QAMS-005/80.
Fines for failure to comply are payable in 5 days of demand. A copy of thesubmittal letter will be sent to USEPA coordinator; letter and check will besent to EPA Region III Hearing Clerk (3RCOO), 841 Chestnut Street,Philadelphia, PA 19107.
Avtex Project Coordinator will notify EPA of any delay or anticipated delaybecause of FORCE MAJEURE, in achieving this order. Notification will beverbally as soon as possible, at least within 2 days, and in writing within 7days. The notification will include the following information:
1. Fully describe nature of delay.
2. Fully describe reasons delay is beyond control of Avtex.
3. Actions taken to mitigate/prevent/minimize delay*
4. Failure to provide timely notice will constitute a waiver of Avtex'sright to a delay.
Avtex understands that minor modifications to this work plan and schedule maybe made by the mutual consent of Project Coordinators. Such modificationsmust be memorialized in writing by Project Coordinators.
Any notice, report, certification, data presentation or other documentsubmitted shall be certified by an Avtex corporate officer.

SITE HEALTH AND SAFETY PLAN
4.1 PURPOSE
The purpose of this plan is to describe the measures to be implemented at theAvtex site necessary to eliminate the threat to public health, welfare, or theenvironment due to potential exposure to hazardous substances, the potentialfor off site migration of hazardous substances, and the potential for fire,explosion, or spillage of waste from drums.
Any entry onto a site potentially containing hazardous materials must be madewith caution. A safety plan for work performed in response to this Order(Docket No. 111-90-01-DC) has been developed to ensure that all the hazards ofthe site are evaluated prior to entry. The Occupational Safety and HealthAdministration requires that a written safety plan be present on site and thatall personnel working on the site have read and understood the plan.
Because of the fact that the Avtex site includes an operating manufacturingfacility and because of the significant number of individual projectscontractors involved in removal activities, the focus of this safetymanagement plan is to define clearly those areas of the Avtex propertyinvolved in removal or assessment activities and to ensure that procedures arein place for segregating those areas and defining where possible, anypotential hazards in those areas. Individual contractors have been,maderesponsible for their own safety plans, and these are included in Appendix Gof this work plan.
4.2 GENERAL INFORMATION

The Avtex Site is located on the south side of Kendrick lane in Front Royal, 'Virginia. The site is bordered by Kendrick Lane on the north, a wooded areaon the east, West Main Street and Luray Avenue on the south, and the southfork of the Shenandoah river on the vest. The site contains approximately 400acres.
4.2.4 Directions to Site
Front Royal is serviced by interstate route 66, and by state route 55, and340. Interstate route 66 has exits onto routes 55 east of Front Royal andonto route 340 north of the town. Route 55 west combines with route 340 tobecome Royal Avenue, the major north/south road in Front Royal. Turning westonto 6th Street will lead directly into Kendrick Lane and to the Avtex Site.A map is included in Appendix E of this work plan.

4.3 CHEMICAL HAZARDS
PCB's are defined as a series of technical mixtures consisting of many isomenand compounds that vary from mobile oily liquids to white crystalline solidsand hard non-crystalline resins. "They are...components of dielectric fluidsin transformers and capacitors." (from Monsanto MSDS, appended)
Any additional materials discovered as a result of sampling activities will bereported to the USEPA and their hazards documented. MSDS's will be included.
4.4 PROJECT SUMMARIES
4.4.1 Objectives
Avtex's objectives for this project are to manage, control, and ensurecompletion of all required assessments and removal activities, provide allreasonable precautions to protect the public, workers, contractors,subcontractors, and the environment from exposure to hazardous substances, todefine clearly those areas of the Avtex property involved in removal orassessment activities, and ensure that procedures are in place for segregatingthose areas and defining where possible, any potential hazards in those areas.

4.4.5 Access Route*. Evacuation Routes, Problem Areas
Access to the compressor room roof is through the turbine room downthe stairs located in the northwest and southeast corners of theroom to the ground level and to the exterior of the building.
The Substation 1A, and polyester dock areas are already outside ofthe plant buildings.
Access to each manhole is through the manhole access cover opening.In these cases, climb the ladder to ground level.
The two drum storage areas are immediately adjacent to large openloading docks, and to normal marked exits. Leave these sitesthrough the opening furthest away from the hazard. All exits leadto the exterior of the plant.
In all cases, once outside of all plant buildings, proceed to themain gate via the perimeter road in the direction furthest from thehazard •
Egress from storm sewer manholes is limited, therefore everyprecaution must be taken to strictly limit the number of workers ineach manhole.
Safety plans for individual contractors are included as Appendix Gof this work plan.

5.9.2 The gate guard will assure that only fire-personnel and police withproper identification are allowed on plant property. If necessary,request the assistance of the Front Royal Police to assure that thepublic is protected from possible exposure to hazardous materials.
5.9.3 Community Relations and Communications
A copy of this procedure will be mailed to:
• Chairman of the local Hazardous Chemical Response Team
Avtex Fibers will meet with local officials to communicatethe nature and the extent of the chemical clean-upproject, and to communicate the possible hazard due tofire.

5.8.3 PCBs can be absorbed through the skin which may lead to drying andcracking of the skin.
5.8.4 PCB liquid and vapors may be mildly irritating to eye tissues.
5.8. 5 Prolonged exposure to PCBs may cause chloracne.
5.8.6 All solids and liquids handled during the clean-up may contain PCBs.
5*8.7 All enclosed spaces may contain C$2 or H2S gas or may be deficientin oxygen. Plant "Tank entry and entry of enclosed spaces"procedures should be followed at all times.
5.8.8 MSDS for PCBs is attached.
5.8.9 PCBs are fire resistant. They may decompose to CO, C02, HC1,phenolics, aldehydes and other potentially toxic products if exposedto heat or flame.
5.8.10 PCBs in electrical fires may form chlorinated dioxins (PCDDs) andfurans (PCDFs).
5.8.11 Standard self contained breathing equipment should be worn whenfighting fires.
5.8.12 PCBs are listed in the International Agency for Research on Cancer(IARC) monographs (1982)-Group 2B and in the National ToxicologyProgram (NTP) Annual Report on Carcinogens (third).
5.9 FIRE REPORTING
5.9.1 Reporting Fires
All plant phones are equipped with a label that gives the number tocall (300) to report a fire as well as the fire assembly station foreach area.
To report a fire, dial 300 give the guard the following information:

* The drum storage and the drum disposal areas are contained inconcrete floor steel frame masonry buildings and are barricaded andposted with yellow caution tape.
In the event that outside fire fighting help is requested, securethe gate and permit only fire, emergency, police, and fullyidentified volunteers onto plant property.
5.7.3 Site Fire Protection
Sites are equipped with sprinklers.

The two areas are separated by approximately 1/2 mile of plantbuilding so it is highly unlikely that a fire at one site.wouldspread to the other.
Both storage areas are in one story buildings which are used for noother purpose, and are not normal passage ways.
* No smoking is allowed in either storage area or outside of the areasor adjacent to the areas* '••
Both buildings are of steel frame-masonry construction.
Easy access is possible both by road and by walkway, into bothareas.
Both areas are at the 1st floor level.
5.8 HAZARDOBS* COMMUNICATIONS ^
5*8*1 Fire in the drum storage area or in the drum disposal area mayinvolve used lubricating oil and therefore may be very hot andvolatile.
5.8*2 Drums in these areas have been sampled but are awaiting analysisbefore disposal. All fires therefore may involve the combustion ofunknown and potentially hazardous materials.

of these areas are on fire, call the Front Royal Fire Department at 911immediately, then call the normal plant call list.
5.5 PROJECT SUMMARY
To eliminate the threat to public health, welfare, or the environment, due topotential exposure to hazardous substances, the potential for off sitemigration of hazardous substances, and the potential for fire, explosion, orspillage of waste from drums, the material and containers must be assembled,identified and properly removed from the site. Methods and procedures aregiven in individual project work plans.
5.6 SAFETY MANAGEMENT
5.6.1 The Site Safety and Health Plan vill be followed.
5.6*2 Fire fighting by Avtex personnel will be limited to flamesuppression and preservation of adjacent structures. Actual firefighting at these sites will be by trained members of the FrontRoyal Volunteer Fire Department.
5.7 SITE SECURITY
5.7.1 Responsibilities of Avtex Employee*
Stay away from all drum storage and drum disposal sites. It shallbe cause for immediate discipline (up to and including discharge)for any AVTEX employee to enter a drum storage or drum disposal areaexcept to place a drum or drums in the area, monitor the conditionof the drums, transfer the contents of one drum to another toprevent a leak, place a drum in a drum contained to contain a leak,or count drums for an inspection or inventory.
Report all fires in a drum storage or drum disposal area by dialing300 and report the problem to the guard giving the fire assemblystation number posted on each phone, the help needed, and your name.
Report all real or potential fire hazards to your supervisor.
Report all deficiencies in fire fighting equipment to yoursupervisor*

5.2.4 Direction* to Site
Front Royal is serviced by interstate route 66, and by state routes 55, and340. Interstate route 66 has exits onto routes 55 east of Front Royal andonto route 340 north of the town. Route 55 west combines with rout 340 tobecome Royal Avenue; the major north/south road in Front Royal. Turning westonto 6th Street will lead directly into Kendrick Lane and to the Avtex Site.
5.2.5 Site History
Manufacturing of rayon fiber began at the Front Royal site in 1940 under theAmerican Viscose Corporation. Ownership was transferred to the FMCCorporation in 1963 (as the American Viscose Division of FMC Corporation andsubsequently the Fibers Division of FMC Corp.) In August 1976 Avtex Fiberswas formed from the fibers plants in that division. Approximately 300 workersare employed at the site. The principal product is rayon yarn for thecarbonizing industry*
5.2.6 Site Description
The Avtex Site consists of an approximately 400 acre fenced plant facilitysouth of Kendrick lane in Front Royal, Virginia. The Site is located on thesouth side of Kendrick lane in Front Royal, Virginia. The site is bordered byKendrick Lane on the north, a wooded area on the east, West Main Street andLuray Avenue on the south, and the south fork of the Shenandoah river on thewest.

, FIRE PROTECTION PLAN FOR DRUM .• STORAGE AND DISPOSAL AREA
5.1 PURPOSE
Because the Avtex Site contains a manufacturing operation which has beenreduced in scope by approximately 85%, many areas of the plant are nowabandoned. Lubrication supplies, cleaning supplies, and manufacturingsupplies in these areas are now waste. To eliminate the threat to publichealth, welfare, or the environment, due to potential exposure to hazardoussubstances, the potential for off site migration of hazardous substances, andthe potential for fire, explosion, or spillage of waste from drums, thematerial and containers must be assembled, identified and properly removedfrom the site.
Any entry onto a site potentially containing hazardous materials must be madewith caution. A safety plan for work performed in response to this order(Docket No. 111-90-01-DC) has been developed to ensure that all the hazards ofthe site are evaluated prior to entry. The Occupational Safety and HealthAdministration requires that a written safety plan be present on site and thatall personnel working on the site have read and understood the plan.
Because of the fact that the Avtex site includes an operating manufacturingfacility and because of the significant number of individual contractorsinvolved in removal activities, the focus of this safety management plan is todefine clearly those areas of the Avtex property involved in removal orassessment activities and to ensure that procedures are in place forsegregating those areas and defining where possible, any potential hazards inthose areas* Individual contractors have been made responsible for their ownsafety plans, and these are included in Appendix G to this plan.
Because the majority of the materials found are fuels, it is imperative thatevery reasonable precaution be taken to avoid the possibility of fire. If afire does occur, to avoid possible'threat to the general public, employees,and contractors of exposure to potentially hazardous materials, the provisionsof this plan must be followed. "-•''••'

SITE SECURITY PLAN
6.1 PURPOSE
Avtex will prevent unknowing entry and minimize the possibility forunauthorized entry onto the portions of the Avtex facility site subjectto the conditions of this Order. In addition, to achieve maximumprotection, security will be extended to all areas within the facilityboundary .
According to the Administrative Order, EPA and/or their authorizedrepresentative shall have the authority to enter and freely move aboutthe site at all reasonable times for the purpose of, inter alia:inspecting records, operating logs, and contracts related to the site;reviewing the progress of the Respondents in carrying out the terms ofthis Order; conducting such tests as EPA deems necessary; using a camera,sound recording or other documentary type equipment; and verifying thedata submitted to EPA by the Respondents. The Respondents shall permitsuch persons to inspect and copy alt records, files, photographs,documents, and other writings, including all sampling and monitoringdata, in any way pertaining to work undertaken pursuant to this Order.Nothing herein shall be interpreted as limiting the inspection authorityof EPA under federal law, or Virginia under state law.
Although these provisions of the Order allow for unrestricted access tothe Site by USEPA personnel, Avtex cannot sacrifice worker safety.Therefore, Avtex will require that all visitors to the site underauthority of this Order, as well as Other personnel conducting removaloperations, adhere strictly to the regulations described in this generalSecurity plan.

All drum storage areas are inside of plant buildings. All drum storage areasand all PCB sites except the storm sewer'line and manholes between the fenceand the river, are within the plant perimeter fence.
6.3*1 Access to Avtex Site
Access to the site is controlled by a 24 hour guard post at the plant's onlygate and is restricted to employees of Avtex Fibers, contractors,subcontractors, railroad and trucking delivery and shipping workers, andfederal and state regulatory officers* Access to PCB sites and Drum StorageAreas is limited to workers and agency personnel required to be in thoseareas.
6.3.2 Barrier* to access
The unfenced portion of the plant property is bordered on the north byadjacent fenced industrial property, on the east by the plant fence, on thesouth by large industrial waste lagoons and a wooded field, and on the west bythe Shenandoah river.
6.3.3 Posted Sign*
There are no road ways into the unfenced portion of the plant property otherthan those which originate on Avtex property. The land along West Hain Streetand Luray Avenue have been posted with "No Trespassing" signs.
6.3.4 Telephone System
The plant working spaces are equipped with a standard telephone system whichallows point to point contact anywhere within the plant and through theswitchboard to an outside line. Many units are "9" enabled so that directoutside dialing is possible. The guard can be reached at extension 141, andin the event of fire or emergency at extension 300 which will be givenpriority.
6.3.5 Fire Signal
All plant spaces are equipped with a fire signal. This consists of a siren inworking spaces and a loud bell in office spaces* Workers are then directed bya light code system to go to a fire assembly station for fire duty. Everyphone is marked with the fire assembly station of that location.
6.4 PLANT ACCESS CONTROL
Access to the site is controlled by a 24 hour guard post at the plant'sonly gate and is restricted to employees of Avtex Fibers, contract/ ,] 005 I I

6.4*2 Plant Security Procedures
6*4.2.1 Main Gate at Kendrick Lane: A guard will remain on duty at alltimes, 7 days a week, 24 hours a day.
6.4.2*2 Rear Gate on West Main Street: Shall remain locked.6*4*2.3 A roving guard will patrol all plant areas, the plant perimeter
fence, and PCB sites not under constant work. All problems andtrespass will be reported immediately to Mr* Kenneth Wagner.
6*4.2.4 Persons other than Avtex Employees and Contractor Employeeswhen in contractor supplied buses, are stopped at the gate andasked for identification. Each visitor is issued a plantvisitor's gate pass. Each visitor is asked for the name of theperson he is visiting and that person is call to escort thevisitor. Governmental agency personnel are held at the gateuntil an authorized person is available to provide escort.
6.4.2.5 Governmental agency personnel and government contact personnelon full time assignment at the plant, and who are aware of
. plant safety and health rules and restrictions may be grantedunescorted access to the facility.
6.4.2.6 It is the responsibilities of Avtex employees to stay out ofand away from all PCB sites. It shall be cause for immediatediscipline (up to and including discharge) for any Avtexemployee to cross marked PCB clean-up boundary withoutspecific authorization from the Avtex PCB Project Coordinator:C. 6.4.4 PCB Site Security
6.4.4*1 Contractors will barricade off the entire work area at each PCBsite.
6.4.4.2 The perimeter of each site will clearly be posted with signsdirecting all non-authorized persons to stay out.

6.4.4.4 Contractors will challenge unauthorized percons at the job siteand will direct them to leave their work area.

1.0 INTRODUCTION
The Avtex Fibers Site, located in Front Royal, Virginia, containsapproximately one thousand drums containing various chemicalsubstances including waste lubricating oil, acids and unknowns.This sampling and analysis plan details the methods chemical WasteManagement will use for collection and analysis of the drummedsubstances. These substances will ba characterized forcompatibility, consolidated and classified for off-site disposal.2.0 SMtPLINO PLAN
2.1 ORGANIZATION
Sample collection will be performed by CWM and/or itssubcontractors. One drum sampling team will be required for thisproject. This team will consist of a sampling supervisor and twosampling technicians. The sampling personnel will report throughthe sampling supervisor to the CWM Project Manager. Technicalguidance and support will be provided via a dotted line report tothe ENRAC Technical Manager. Tha sampling team will be responsiblefor tha collection, fingerprint characterization and documentationof all drummed waste samples. Their responsibilities will alsoinclude the shipment of samples for off-site analysis.2.2 SAMPLE COLLECTION
CWM will sample each drum at the Avtex Fibers site for the purposesof characterizing and consolidating the drummed substances for off-site treatment/disposal. This section describes the protocols CWMintends to use for sampling at the Avtex Fibers site. Specific CWMprotocols for tha sampling of drummed materials can be found inAttachment A.
A discrete sample of each staged drum will ba collected andcharacterized via fingerprint analysis (Sea section 4.0). Basedon tha results of this analysis and previous knowledge of the drumcontents, compatible drums will be composited for furthertreatment/disposal analysis. For those drums which are clearlymarked and for which the contents are previously known, onecomposite sampla par group of 20 drums will ba prepared. Forunmarked drums* one composite sample per group of 10 drums will bemade.
Site specific sampling protocols for unanticipated environmentalor wasta sampling will be submitted as the need arises.

2.3 SAMPLE CONTAINERS. PRESERVATION AND HOLDINO TIMES
Figure 2-1 is a listing of required sample containers, preservationtechniques and holding times for all analytical parameters whichmay be associated with the Avtex Fibers site. Also included in thetable are the required volumes and number of containers for eachparameter and matrix. Sample containers, preservatives, volumes,and holding times will vary with organic drum samples due toobvious incompatabilities. Typically organic drum samples will becollected in 1/2 pint or pint wide mouth glass jars with teflonlined lids. These samples may or may not be cooled until aftereach composite is made. All sampling containers will be obtainedfrom a commercially available source of EPA-approved samplecontainers.
2.4 8AMPLINQ EQUIPMENT DECONTAMINATION
Due to the nature of tha drummed substances expected at the AvtexFibers site (i.e. high concentrations of organic material) it isanticipated that disposable sampling equipment will be used.However, in the event that sampling equipment is to be reused, thedecontamination methods described in this section will be utilizedto minimize cross-contamination between sampling events.BULK LOADS/DRUMS OP 8QLID3 OR WASTE PILES REQUIRING METALS.CYANIDE. 5ULPIDB AND/OR GENERAL SCREENING ANALYZES.
PROCEDURE

2.5 QUALITY CONTROL PROGRAM
Chemical Waste Management will collect and analyze Quality Control(QC) samples as part of the overall field sampling program at theAvtex Fibers sita. These QC samples will be analyzed by CWM and/orits subcontractors as an internal quality control check. Thefollowing will be considered individual matrices for determiningthe proper number of QC samples:
ODrum Samples
Tha following types of samples will be collected as part of thefield QC program:
O Duplicates - Duplicate samples at a frequency of ona duplicatefor avary 20 samples will ba collected at tha same time andsampling location, using tha same sampling methodologyemployed to collect tha original sample.
In addition to tha field QC program the CWM Technical CanterLaboratory will analyze QC samples as per their StandardOperating Procedures.
2.6 HEALTH ANP SAFETY
Specific requirements for personnel protective equipment/airmonitoring during sampling at the Avtex Fibers Site, are outlinedin tha Sita Specific Health and Safety Plan, Attachi&RtJ Q.Q 5 3 2
3.0 SAMPLE CUSTODY AND DOCUMENTATION
CWM will institute sample custody procedures at the Avtex FibersSite to ensure the integrity of the sample from time of collectionto analysis. The components of the sample c4. 3.3 CHAIN OF CUSTODY RECORD
To establish the documentation necessary to trace sample possessionfrom the time of collection, a chain of custody record (Figure 3-2) will be completed and will accompany every sample. Whentransferring the possession of samples, the transferee will signand record the date of transfer on the chain of custody, tacord.

Tha parson taking custody of the sample will raconcila theinformation on tha chain of custody record with that on tha samplelabel before signing and dating tha chain of custody. Tha originalchain of custody records will accompany tha sample and a copy willbe placed in the project file.
In addition, any samples sent to off-site laboratories by CWM willbe accompanied by a Sample Analysis Request Form (Figure 3-3).The usa of this form ensures that tha receiving laboratory has beennotified of pending sample shipment and provides additionalinformation as to the source of tha samples.3.4 SAMPLE PACKAGING AND SHIPPING
All samples requiring shipment to off-sita laboratories, will beshipped in accordance with appropriate DOT regulations.
Samples collected at tha Avtex Fibers Sita will, for the purposesof packaging and shipping, be considered hazardous substancesamples and will be transported according to the followingrequirements:a If tha hazardous substance in the sample is known or can ba
identified, tha sample will be packaged, labeled and shippedaccording to the specific instructions for tha material (ifit is listed) in the DOT Hazardous Materials Table (49 CFR172.101).
O If tha hazardous substance in the sample is unknown, 49 CFR172.402 (h) allows for the designation of hazard class basedon the shippers knowledge of the material and the selectionof the appropriate hazard class from 49 CFR 173.2 (as listedin Figure 3-4). CWM will, generally, ship hazardous samplesof unknown content as "Flammable Liquid" or "Flammable Solid"if tha hazard classification listed above these on Figure 3-4 (i.e. radioactive material, Poison A, flammable/non-flammable gas) can be eliminated. Sample categories listedbalow flammable liquid/solid on Figure 3-4 will generally notbe considered sinca classifying substances into thesecategories requires flashpoint testing (which is not requiredof most samples from this sita)*
Thus, unless tha sample is known to consist of material listedbalow flammable liquid/solid on Figure 3-4, it is considereda flammable liquid/solid and shipped as such.
SAMPLE PACKING AND SHIPPING PROCEDURESt

and solids, as described in 49 CFR 173.118 and 173.153. Limitedquantities of flammable solids are defined as one pound net weightper inner container and not greater the 25 pounds net weight in the ,outer container. The procedure is as follows:
a) Secure sample jar lids with strapping tape or evidence tape.
b) Place sample jar in Ziploc polyethylene bag (one sample perbag) . Seal bag,
c) Place sealed bag inside metal can and cushion it with enoughnon-combustible , absorbent material (i.e. -vermiculite) toprevent breakage and absorb leakage .
d) Pressure close the can and secure the lid with tape.e) Place the following information, either handprinted or in
label form, on the metal can:
O Laboratory name and address.
O "Flammable Liquid, N.O.S., UN1993" or"Flammable Solid, N.O.S., UN1325".Not Otherwise Specified (N.O.S.) is not used if theflammable liquid (or solid) is identified. Chemical Waste Management will provide personnel for all samplingactivities required at the Avtex Fibers Sita.
In order to ansura tha compatency of the personnel who performsampling at tha Avtex Fibers site, Chemical Waste Management willinstitute a Sampler Training Program, whereby any and allindividuals involved in sampling at tha sita will be required tocomplete training in tha site specific sampling procedures asdescribed elsewhere in this plan. flR | 00536

4.4 Scoop - A davica that is of a size suitable for the quantityand siza of the particles to ba sampled. It is used fortaking small, equal portions at random spots near thasurface of tha waste material.
4.5 Concentric Tuba Thief (Grain Thiaf) - This device is similarto a split tuba thief. The grain thief has an inner movabletube. Both tha inner and outer tubes hava several openingsto allow free flowing solids to pass into tha inner tube.Tha thief is removed from the material and tha inner tubeemptied into a sample container (cee Figure A, Appendix I).
4.6 Soil Saapla Auger - This device is similar to a large drillbit with a handle. It is screwed or turned into thamaterial then pulled straight out. Tha saapla incraaant istakan from this extracted portion (sae Figure B,Appendix I).
4.7 corar (Split Tube Thiaf) - This device is a natal orplastic tuba (about 3/4-inch in diameter) from which about1/3 to 1/2 of the side of tha tube has been removed to forma slot along its antira length. Tha thiaf can be up to 4faet long and should have a sharp/ angled point at its lowerand (sea Figure C, Appendix I). A saapla will normally beraaovad fr.-m tha thiaf with a spatula or similar instrument(reamer) and placed in the cample container.

7.1 Mechanically- mixed tank samplingTanks with mechanical mixers are used primarily for blendingand ctorage of materials of different compositions that alsohava slightly soluble dr suspended matter pracant that willracolubiliza or rasuspend upon mixing. Under normalconditions, tha mixing process can homogenize a tank so thata cingla point saaplc from a campling port on tha eide oftha tank can ba takan with a high degree of certainty thattha cample is raprasantative of the material in the tank.7.1.1 -Allow the tank to adequately mix prior to sampling.
Tha mixer on the tank should be on for a minimum ofthirty (30) minutes before a saapla is taken.
7.1.2 Placa a bucket tinder tha saapla port. Open tha valveand allow at least three times tha saapla port volumeto flow through the saapla port.
7.1.3 Place a suitable saapla container under tha sampleport.

7.2 Multi-point saapling
Multi-point sampling involves taking samples from severalpoints of tha tank using sample ports installed verticallyalong tha height of tha tank. These samples can beanalyzed individually to certify homogeneity or can becoapositad to obtain a full tank assay (Sea Section 8.0).7.2.1 Select sample parts on tha tank that will giva you
tha bast vertical section of tha tank. Taka a sampletakan from each of tha top, aiddla and bottoa of theliquid in tha tank at a ainiaua. Ba sura samplestakan from these locations ara labeled as such.
7.2.2 To -take tha saapla, follow steps 7.1.2 through7.1.8.
7.3 Weighted bottle sampling
Tha weighted bottla is used to obtain liquid and free-flowing slurries froa tha top of large stationary tanks. Asampla must ba takan from at least four (4) points; a topsaapla no aora than six (6) inches froa tha surface, anuppar saapla, a aiddla saapla, and a lowar saapla.7.3.1 Clean tha bottla, bottla assembly, and inspect line

7.4. Sampling tanks with recirculation loopsTanks with recirculation loops can provide accurateraprccentativc camples by obtaining aatarial from a sampleport on the recirculation lina. This method applies to bulkliquids and pumpabla clurriac only.7.4,1 Allow tha tank to mix adequately. At least half the
volume of tha tank should be circulated prior tocampling.
Note: It ic recommended that the pump suction point ba neartha bottoa of the recirculation tank. This minimizespump damage due to cavitation. Tha pump dischargeshould ba through an aduction cyctaa elevatedslightly above the bottoa. Eduction cpeedc themixing process, and a ring of aductorc gives moreavan mixing to all parts of tha tank. Tha volumecirculated ic determined by dividing tha volume inthe tank by tha pump rate. Adequacy of the mixingsystem may ba determined by taking multipleanalytical samples over a period of time. Therasultc are then plotted to chow at what time the•implec become consistent. ASTM method E 300 is a»afsrence for cample port design.
