1. PURPOSE
To provide details on the process for the management of stakeholder complaints pertaining to
operations at the Harewoods sand quarry.
To maintain positive relations with all stakeholders the community, regulatory and local authorities to
ensure compliance to the Extractive Licence.
2. SCOPE
This procedure applies to the Harewoods Sand Quarry operations pertaining to noise, dust, vibration,
road usage and any other non-amenity aspect that may give cause for a complaint from any
stakeholder.
3. LEADERSHIP, COMMITMENT AND RESPONSIBILITY
Leadership and commitment shall be demonstrated at all levels as described in the following table of
Responsibility and accountability. All personnel are responsible for:
§ Reporting of any non-compliances for dust, noise or vibration during operations.

Role Responsibilities

vibration controls.

4. PROCEDURE
4.1. Background
Harewoods is a sand pit operating for the sale of sand and limestone to customers. The operation
incorporates mining earthworks practices including excavation/dozing, loading and screening of sand;
crushing of limestone rubble. The sand and crushed limestone is made available for customers to cart
the product away.
The work is approved and carried out under the Extractive Industry Licence, all conditions of the
approval must be adhered to.
There is no blasting occurring at the operation, noise factors are as a result of the equipment operation
including the screening and crushing plant, loaders and conveyors loading transport trucks entering
the pit for carting the sand away.
Dust may be considered an issue in high/strong wind conditions and in particularly during the early
summer months, truck movement, loading screening and crushing operations.
Diesel Particulate Matter (DPM) is not considered an issue due to the surface mining operations
allowing for ventilation, the high frequency of maintenance on the equipment (greater than the OEM
standards) and the proximity to the residential housing with the buffer zone and windrow in place.
Vibration is as a result of heavy equipment operating on hard and rocky ground conditions. The
vibration is a risk to the operator, the equipment and through ground vibration.
4.2. Mitigation Practices
The Company is committed to their Environmental and Stakeholder Policy maintaining full responsibility
for ensuring sustainable mitigation processes are implemented and utilized with respect to the local
community and residents of Dalyellup and Gelorup. Mitigation practices include
Operations Times at the pit are between 7am and 5pm Monday to Friday; 7am to 12:00am on
Saturday with no operations on Sundays or Public Holidays. in accordance with the Extractive Industry
Licence.
Communications with the community are completed through mail drops, advertisements in local
papers and to and through the Capel Shire Council.
Dust
§ Weather conditions are monitored daily by the pit Supervisor.
§ Preventive controls used include use of water cart on roads and stockpiles to dampen all potential
dust zones.
§ Stockpile placement and management design reduces impact on the local community in medium
to low windy conditions.

§ When high to strong winds or greater are forecast, with the potential to impact the community,
crusher or screener operations cease until the wind drops to an acceptable wind speed that
minimises the impact on the community.
Noise
§ Control measure as stated in the Noise Impact Assessment – Harewoods Road Sand Quarry
Dalyellup and Environmental Assessment – Proposed Limestone Extraction Operations. These
included noise bunds and location of work operations.
§ Noise is monitored using an approved and NATA calibrated Noise Dosemeter completed by the
Senior HSE Advisor. All records from the noise survey are recorded into a database and any
reportable exceedances are processed immediately and investigated.
§ Machinery planned maintenance completed above the OEM requirements.
§ Vibration is monitored by the operators and the Supervisor through assessment of ground
conditions to ensure machine vibration is minimised resulting in reduced vibration through the
ground.

4.3. Complaints
The Company is respondent to complaints from the community verbally via telephone or face-to-face
contact, in writing via email or mail from both internal and external complainants.
Complaints are welcomed from both internal and external persons and shall be reviewed
independently, logged and recorded and where applicable investigated. The HSE department shall
track and monitor amenity type complaints (noise, dust, vibration) and the HR Department shall
monitor the non-amenity complaints.
All complaints shall be directed to the relevant department, pending the context of the complaint.
Once recorded the HR Manager, HSE Manager and Harewoods Project Manager shall liaise to
determine the most appropriate action required to acquit the issue for the complaint.
Each complaint is taken seriously and following a review, is recorded and appropriate actions are
determined and assigned. All actions relating to complaints are managed through the complaints
database system. Feedback and/or follow-up may be required to determine if an action was
effective and the stakeholder is satisfied with the outcome.
Anonymous complaints will not be deemed complaints. Complainants who wish to remain anonymous
will be advised that their personal details can remain confidential. If they still wish to remain
anonymous the matter will be logged as a concern and not responded to.
Appropriate behaviour regarding receiving, responding, and actioning complaints is described in the
Stakeholder and Complaints Management Procedure and all risks are described in the ENV03
Stakeholder and Complaints Management Critical Risk Standard.
4.3.1. Amenity Complaints
Amenity complaints include dust, noise, vibration from the operations at the sand quarry.
Dust complaints shall be monitored and reviewed from pre-shift meeting weather condition and
operational reports. Monitoring of dust over an agreed period shall be completed with outcomes
recorded in the database.
Noise complaints shall be monitored and where the noise level is found to be non-compliant with
permittable conditions the complainant will be advised of the recorded noise level. Noise monitoring
will continue over a period of time and once compliance has been achieved the complainant will be
advised and the complaint will be considered to be resolved.
Vibration complaints requires a written explanation to the complainant outlining the cause and how
this could be avoided in future. The complainant and the Capel Shire will be advised of this and the
complaint will be considered to be resolved. Regular monitoring through equipment maintenance
and ground conditions will be completed.

4.3.2. Non-amenity Complaints
These are complaints other than those relating to dust, noise or vibration, that are investigated by the
relevant department depending on the complaint issue. These will be investigated on an individual
basis, where they are deemed as having a significant impact on the stakeholders.
Typically, these complaints can be pertaining to but not limited to:
§ Traffic access and usage at the pit via Harewoods road
§ Piacentini personnel
§ Treatment of the stakeholder when making a complaint or asking questions
§ Failure to respond to a complaint – stakeholder not receiving feedback on their complaint.
All non-amenity complaints reaching Level 4 or 5 shall be investigated and processed accordingly.
4.3.3. Action on Complaints Trending in Nature
The immediacy of reporting enables the company to modify its activity to minimise disturbance.
Consistent complaints of a similar nature, the company shall complete an investigation into the reports
received to determine if there has been a regulatory breach or if the complaint has merit. The
outcomes of such investigations assist the company with future planning activity.
Complainants should be encouraged to report promptly following a disturbance to allow immediate
follow-up to occur or, where this is not possible, provide information for the planning of future mine-
related activity.
4.4. Permittable Limits
All exceedances in noise, dust and vibration shall be investigated to determine the root cause and to
provide determinations for mitigation of the causes which can then be utilised to provide
improvements in planning.
4.4.1. Dust
Dust is considered as having the greatest impact on the local community and nearby residential estate
of Dalyellup. Details on Dust Management is described in several procedures pertaining to dust
management in earthworks operations to minimise any dust impacting areas outside of the quarry.
The Company is committed to their Environmental and Stakeholder Policy as described in this excerpt
“…by:….Implementing and maintaining an Environmental Management System to identify, assess and
minimise environmental and community relations impacts, across all sectors of our operations;…….”
4.4.2. Noise
Noise complaints are followed up immediately if reasonable to do so (within 2 hours of the complaint
being received or at the same time the next day)
The Senior HSE Advisor is informed and a decision is made to measure the noise if practicable (e.g
noise monitoring may not be practicable if the wind speed is greater than 5.0m/sec).
Our aim is to be at or low to the level as stated in Noise Impact Assessment – Harewoods Road Sand
Quarry Dalyellup and Environmental Assessment – Proposed Limestone Extraction Operations and to
fully comply with Environmental Protection (Noise) Regulations 1997.
4.4.3. Vibration
Vibration complaints are checked and monitored through maintenance assessments of the equipment
to ascertain any vibration transfer and to mitigate the transfer to below acceptable rates.
4.5. Action and Resolution with the Complainant and Shire of Capel
The complaint is reviewed by the Project Manager, HSE Manager, and HR Manager to determine the:
§ Origin of the complaint (individual, company, local or state authority, etc)
§ Level of risk associated with the complaint context
§ Breach or non-compliance to extractive licence
§ Impact on the Company

All Complaints received by the company (email, postal, telephone or face-to-face) shall be directed
to either the Project Manager, HSE Manager, or HR Manager and reported into the relevant database.
All amenity complaints shall be recorded in MYOSH to ensure tracking and trending is enabled. Lower
level complaints (level 1,2,3) shall be managed through standard process and will not require
investigation unless requested. Level 4 and 5 complaints shall be investigated and thoroughly
reviewed.
4.5.1. All Complaints
Company protocol requires a prompt and timely response to the complainant within two hours of the
initial report where practicable. All complaints shall be completed as follows:
§ Immediate acknowledgement of receipt of complaint to the complainant
§ Conduct an assessment at the site to determine the cause
§ Take action to minimise or cease the cause which may include one or more of the following:
(i) Stopping the screening or crusher operations.
(ii) Initiate water cart to reduce dust.
(iii) A change in work tasks.
§ Respond with the outcome directly to the complainant
§ A full report of the complaint and all outcomes, actions and plans shall be submitted to the Capel
Shire within two hours
§ Record all actions in the database.
Determination of appropriate corrective action shall be completed by all responsible persons. Action
recorded against the complaint shall be included in the communications with the Capel Shire.
In all instances the complainant shall be notified and provided with feedback of the outcome of their
complaint including the determined actions to rectify the issue.
All complaints that are deemed unresolved by the stakeholder/complainant may result in legal action.
The company shall direct their legal representative to act on their behalf to resolve the matter aiming
for a positive outcome for all parties.
Once all actions have been addressed the outcomes will be communicated to the complainant. If
the complainant does not wish to escalate the complaint it will be considered to be resolved.
4.5.2. Level 4 and 5 Complaints
All major non-compliance (level 4 or 5) shall be investigated and may require approval to
recommence operations in the quarry. The Project Manager, HSE Manager, HR Manager and the CEO
in liaison with the Capel Shire representative shall determine when works may recommence and what
conditions, if any, have been imposed.
The HSE Manager or HR Manager shall respond to the complainant prior to the restart of any
operations following an investigation into a major non-compliance.

4.5.3. Complaints level based on Risk Matrix
The steps of complaints handling, after the complaint has been recorded into the database, is
managed in accordance to the associated risk and complexity of the complaint. The following table

4.6. Reporting
All complaints shall be reported through to management with major and catastrophic complaints
being reported through to the CEO.
Complaint trends are presented and reviewed by the site management team at quarterly
management reviews.
5. DOCUMENTED INFORMATION
The complaints process shall be reviewed and audited at least once every three years to ensure the
process is compliant and aligns with company objectives.

EXTRACTIVE INDUSTRY LICENCE CONDITIONS This licence is issued in accordance with the Shire of Capel Extractive Industry (EIL) Local Laws of 2016, subject to the following conditions: Clause 3.1 (3) (a) The Licence period expires on 24 January 2019
Clause 3.1 (4)(a) The payment of an annual licence fee in accordance with the Schedule of Fees and Charges adopted each year in the Shire of Capel annual budget prior to the commencement of sand extraction. Clause 3.1 (5) (a) The extractive industry area shall be limited to the area and levels as
detailed on the approved Development Plan dated 9 September 2016. (b) Access to the site shall be via Harewoods Road. A sealed crossover is to
be constructed and maintained by the licensee to the satisfaction of the Executive Manager of Engineering and Development Services. The internal haul road will be sealed for a minimum distance of 100 metres from the nearest point of Harewoods Road.
(c) Material may be stockpiled on site up to a maximum of 200,000m3 at any
one time and must be stabilised. (d) The hours of operation of the site shall be from 7.00a.m. to 5.00p.m.
Monday to Friday; and 7.00am to 12.00 noon on Saturday. No operations are permitted on Sundays or public holidays.
(e) Processing plant ie; crushing plant or screen plant, may only operate on
the site in accordance with the conditions of an approved Department of Environment Regulation Works Approval or Licence as per the hours stipulated in (d) above
(g) Depth of excavation shall be limited to 7 metres AHD. No over-
excavation below agreed levels is permitted.

(h) Minimum setbacks for the industry are as follows:
Forty (40) metres from the Harewoods Road and Minninup Road reserve boundaries
Forty (40) metres from the Eastern boundary Twenty (20) metres from the Southern boundary
No operations are permitted within the setback areas. Vegetation is to
be left undisturbed to screen the industry from the road and neighbouring properties.
(i) Safety practices for persons employed or visiting shall be in accordance
with Department of Minerals & Energy, Mines Safety and Inspection Act 1994 and Regulations 1995.
(j) Precautions against wind blown material shall be made in accordance with
the approved Dust Management Plan. (k) The existing vegetation buffer within the forty metre setback from
Harewoods Road shall be maintained for the period of the licence. (l) Prevention of dieback spread in accordance with DPaW guidelines shall be
made. (m) No discharge of stormwater other than pre-development runoff in defined
natural watercourses will be permitted. (n) Rehabilitation shall be in accordance with the rehabilitation plan contained
within the licence application document dated December 2006. (o) Minimum batters of 1:10 shall be applied to all rehabilitated slopes. (p) A detailed survey of the EIL site shall be submitted to the Shire prior to
16 November each year to allow monitoring of excavation works, annual licence payments and rehabilitation bond adjustments by 31 December of each year.
(q) The licensee to enter into an agreement with the local government by
which it agrees to pay any extraordinary expenses incurred by the local government in repairing damage caused to thoroughfares in the district by heavy or extraordinary traffic conducted by or on behalf of the licensee under the licence.
(s) Access to the extractive industry site shall be available to authorised
officers of the Shire of Capel at all times and without prior notice, for the purpose of inspection and monitoring of compliance with license conditions.

(t) No soil material to be imported into site without prior approval from the
Executive Manager of Engineering and Development Services. Clause 5 (1) Security for Restoration & Reinstatement The lodgement of a secured amount of $16,326/ha of disturbed land (or an amount as negotiated and adopted each year in Councils Annual Budget – Fees and Charges) is to be lodged prior to the commencement of extraction. This secured amount is to be reviewed annually in terms of Part 5 of the EIL Local Laws.
Clause 6.2
(a) Securely fence the excavation to the satisfaction of the Executive Manager
of Engineering and Development Services.
(b) Erect warning signs at the gate and at intervals of not less than 200 metres around the perimeter of the site, bearing the words, “DANGER EXCAVATIONS KEEP OUT” to the satisfaction of the Executive Manager of Engineering and Development Services.
Clause 7.1 A copy of a current public liability insurance policy for a sum of no less than $10,000,000 in respect of any one claim relating to any of the excavation operations is to be presented prior to the commencement of extraction. This policy is to remain current during the life of the EIL. Advice Note The licensee shall have due respect to the Aboriginal Heritage of the area of the works and all workers shall be briefed accordingly prior to commencement of any works on site. The works may be in proximity of areas that were part of the Aboriginal culture and so there is the possibility that artifacts or remains may be in the area. All workers are to be advised of this possibility. Should the presence of a site of possible significance be discovered or if any artifacts or remains are discovered then any works in the vicinity are to stop immediately and be reported to the Department of Indigenous Affairs. If bones are discovered then police are also to be advised immediately. The Licensee shall note that it is an offence under the WA Aboriginal Heritage Act to interfere with any Aboriginal site unless written permission is obtained from the Minister for Indigenous Affairs.
