Medical Record Request Phone CallsFrom Heather Hudson, Medical Records Coordinator

When a patient calls the clinic requesting medical records, please direct them to the TLC website and give the instructions below. Each patient will need to fill out a patient authorization form in order for records to be released. The patient can do this by:

1. Visiting TheLittleClinic.com.2. Clicking on “Contact Us.”3. Scrolling down to the “Medical Records” link. 4. Here they will need to fill out the medical release form.5. Email this form to [email protected] or faxing to me at 615-425-4344.

This will help us utilize the 5 | 5 | 95 strategy in preventing duplicate calls and cut down on the frustration of patients having to make multiple phone calls.

Bringing Family Meals Month Inside the ClinicFrom Allison Kuhn, Director of Retail Dietetics

The Little Clinic is celebrating National Family Meals Month™ in September. This is a nationwide event designed to underscore the benefits of family meals and the role we can play in helping our patients and their families share one more meal at home per week. Numerous studies show that home-cooked meals nourish the spirit, brain and health of all family members. In the larger Kroger store, sampling on September 9-11 and digital content will be supporting the campaign, but how will we bring this home to our own patients? In the month of September, we’re calling on our providers to participate in a “seatbelt campaign” with each of our patients.

Akin to a pediatrician asking, “Do you wear a seatbelt?” our providers will spearhead this campaign by asking our patients, “Do you eat family meals at home?” The provider will then present the patient with our “starter kit” booklet, which explains the campaign and provides coupons and recipes for family meals. This is a way to show patients how much TLC cares about them and their family’s health and well-being and to provide that “plus a little.” In markets with dietitians, the RDNs will also be offering free cooking and meal-planning classes that align with the recipes shared in the booklet, and will also be sharing the campaign during media segments.

Booklets will be delivered to each clinic in a one-time shipment for distribution to each patient from September 12-30. For markets with dietitians, additional advertising for dietitian services will be present on the booklet.

Meet a Dietitian in September and Win BigFrom Allison Kuhn, Director of Retail Dietetics

During the month of September, we’re reaching out to our associates in a big way. Each store-based associate (includes clinic, all store and pharmacy) that completes a free, private, HIPAA-compliant 15-minute nutrition assessment with a dietitian will be entered to win a fabulous prize from our vendor partners. Each dietitian will keep track of these meet-and-greets using our customer contact cards and, at the end of the month, there will be one winner drawn for each store. The dietitians will also be holding various outreach activities for associates during September. It’s the perfect time to take advantage of the 50% associate discount for dietitian services, address any biometric screening results that may need improvement and to head into the holiday season armed with nutritional know-how!

I wanted to send a note about my visit to TLC on Sunday, July 17. I was getting ready to leave for a week’s vacation on this date. I was the first patient on this day. The associate that checked me in was very helpful to make the process go very smoothly. The nurse practitioner, Emily Hanna, was very caring to my situation and medical condition. She provided excellent care and was able to get me on my way, prescribed the necessary medication. Also, she gave me her card to call while I was out if town if needed for continued care.

As a long time Kroger associate, I am so pleased that our company is affiliated with such an awesome medical care facility.
