Permision Ietter Under The Epidemic Diseases Act, 1897

BIMS Hospital, Punit Medical and surgical Nursing home, Pulse+ Multi specialist Hospital & Mahadev Hospital has subnited update proposal for increasing bed capacity as per annexure -2.

Mahadev Hospital, Sitaram (eneral Hospital and Samarpan Covid Center, Bhavnagar has no bed available with Ventilator, o hospitals are converted in to segment I facility with no ICU

facility. District administration, Bhavnayar and Bhavnagar Municipal Corporation has reviewed the

proposal, verified the healthcare facilities offered and has decided to permit the hospital to function as COVID-19 Hospital. The number of beds allowed can he increased in future with progress of the disease

By virtue of power vested under the provisions of The Fipidemic Diseases Adt, 1897 and Notification of Health and Family Welfare Depastrmernt, Ciorvemnent of ujarad, dated 13/3/2020, We hereby given permission to incrca* beds capacity in "COVID-19 Hospital" for the purpose of isolation and treatment of the caes for COVID-19.

The Hospítal shall be reviewed periodically for adherence to protocols and standards of care. The number of hospitals and beds required will be reviewed and increased or decreased in coming days depending on the case load and disease progresion.

This permission letter shall be effective from 13" of April, 2021 and remain in force till further notice by the undersigned

The Schedule of rates is preæribed in Annexure-1&3.

Further,following wmditios shall apply:

1. Horpitaks in Bhavnagar District limit, whether pemmíited or not, exoept if allowed otherwise, as COVID- 19 Hospital, shall not levy charges or rades higher than what has been pemitted in Annexure 2.

2. Doco, Nurwes & Paramedical Staff will corntinue to discharge their duties at the same hospital/work place as before, No chanye in hopital/work plae is allowed without prior pemission of District Authorities.

Medical Officer of Health, Bhavnagar Distria shall enecute an agreement with the hospital listed in

Anmexure-1 Any perwnm/ Iaituun/oryaniztion found violaing any provision of these regulations shall be deemed to

have commted puninshahle offience under wedion 188 of ndian Penal Code (45 of 1860).

SEGMENT I1- HEALTH CARE FACILITY WITH ICU

. Cost of Injection Tocilizumab and Remdesvir and Tablet Flavipavir and higher antibiotics

used for prophylaxis and treatment of secondary bacterial infection.

Special doctor visit, laboratory charges, dialysis ( rate per dialysis will be Rs.1500 and

Rs.3500- for bedside dialysis in ICU)

Note the visit fees of doctors under the category 'special doctor' will apply only to the visits of

specific super specialist like nephrologist, neurologist, cardiologist, intensivists etc. It would not

apply to in-house medical officer and physician.

Special Conditions: 1 The permitted Hospitals will have to maintain all protocols of Hospital Infection Control.

The Hospital shall adhere to all ules and regulation as laid by the Government from time to

3. The Hospital shall adhere to prescribed treatment protocols and patient management

guidelines.
