Notes, Short Comments, and Answers toCorrespondents.HEALTH OF NAPLES.

THE mean mortality of Naples is from 35 to 44 per 1000-a sanitary resultwhich can be due neither to her situation, which is salubrious, nor herclimate, which is delightful. Couched on the slope of green hills, whichsink gently into a spacious gulf; fanned by sea-breeze and mountain-airin incessant motion; sheltered from the keener and stronger winds by hersurrounding peaks and the remoter Apennines ; subject to the mildestbarometric, magnetic, and thermometric oscillations; subsisting enjoyablyon the farinaceous and fruit products which Nature affords in abund-ance,-Naples can have only herself to blame if she is not among themost enviable abodes on earth. "See Naples and die!" says the proverbin allusion to her lovely site and her genial sun. " Reside in Naples anddie!" says the matter-of-fact registrar. It is but fair, however, to admitthat the mortality above mentioned is found chiefly in her lower quarters, where a teeming population herd together in conditions which would beinadequately described by the epithet swinish. The slums of Naples, ifthe concurrent testimony of a host of witnesses be taken, are the worst inEurope, and their hideous inmates avenge themselves on the societywhich neglects them by periodically exploding in what may be termed a"morbific insurrection," and carrying disease and death into the abodesof their unworthy superiors. It is not only that the slow organic dete-rioration which takes the form of phthisis, for example, is a constantinteger in the sum of Neapolitan disease ; but the acute infections or con-tagions, typhus, small-pox, diphtheria, break out in sudden and virulentforce, and carry danger and destruction far beyond their originatingcentres and into the strongholds of the most favoured classes. Dr. E.Fazio, in some interesting letters on the health of Naples, points out thatin two important particulars the city is unhappily circumstanced : itsdwelling-houses for the poor and its water-supply. For a populationthree times that of Rome there is accommodation one-fifth less’ Again, the drinking-water runs contiguous to the drains, and, as these are oftenbelow the sea-level, they regurgitate and form a subsoil of filth, the oozeof which is a very seed-bed of typhoid. The sanitary rescue of Naples isa municipal rather than a medical undertaking; or, more strictly, it isone in which the municipality must work in concert with the mostenlightened medical opinion.

CASE OF NEURALGIA. OF THE LEFT SIDE OF THE TONGUE.

To the Editor of THE LANCET.SiR,-The following case, so rare comparatively, may apologise for its

publication.E. L-, aged seventy-two years, a gentleman of excellent health and

great activity, on the 27th April last drove from home, a distance of seven-teen miles, the day being bitterly cold, and without his usual muffer aroundhis throat. Nothing amiss was felt until, after the journey, he sat down toeat, and upon placing a bit of bread in his mouth there occurred most in-tense paiu in the tip, side, and under surface of the left half of the tongue, and shooting up the face into the left ear. With much difficulty he gotthrough a raw egg beaten up, and any further attempt at solids was attendedwith "dreadful agony," liquids being barely endurable. He came under treatment about the 2nd May, when he complained of

continual pain and uneasiness in the left side of the tongue, increased byany movement of the mouth, and with frequent dartings of pain into theleft ear. Palpation of the apex and left half of the tongue caused increasedsuffering, but no pain was felt in the right half. There was no other tenderpart or spot about the face or neck; neither did he perceive any loss oralteration of taste at any time. He distinctly referred the paiu to the leftside of the tongue. By way of medication he was ordered the iodide ofquina, grey powder, and Frederichshall water. The left ear was cleansed, and small fiying blisters were applied behind the ear and beneath the lobulefor three or four days. The continued pain and discomfort then ceased, whilst the mouth was at rest, but the darting pain into the ear continued. Then followed the hypophosphite of soda and quina in increasing doses. Aplate of" false" upper teeth were removed, and galvanism applied to thetongue and maxillary nerves by means of Mayer and Meltzer’s portablebattery. After the first application the patient was able to speak and drinkwith comfort, and the dartings diminished. Soft solids could be takenafter the second, all pain ceased, and after the third and final application hereported himself " quite well again." He was ordered to continue medica-tion a while longer. During the treatment his general health did not suffer. The pulse stood at 72, with the exception of one day, and normal; the urinewas likewise normal. He considers that he derived most and immediatebenefit from the galvanism. With respect to this, one sponge was appliedto. the left half of the tongue, and the other in turn to the maxillary andseventh nerves. At first the upper nerve seemed the more sensitive, thenthey became equally so, and the tip, side, and under surface much more sen-sitive than the upper surface, and reproduced the severe darting pains intothe left ear. In galvanising the tongue for tinnitus aurium I have noticedthe same severe pain run up into the ear. Does this further show thechorda tympani to be a sensory as well as a motor nerve to the lingualis, which is situate on the under surface of the tongue, and extending from thebase to the apex, precisely in the seat of increased pain ? Nothing is hereclaimed for the treatment of the case,.which lasted for about three weeks.

Yours truly,Newaastle-on-Tyne, July, 1876. Lucius HOLLAND, M.B., M.C., &c.
