Fulbright, M.D.; Medical Director of Adolescent Young Adult (AYA) and Survive & Thrive Program Wendy McClellan, RN, BSN; Nurse Coordinator, Survive & Thrive and Long-term Follow-up Program Sripriya Raman, M.D.; Director, Endocrine Disorders in Cancer Survivors (EDICS) Clinic Objectives At the completion of this conference, participants should be able to: Describe the late effects of cancer treatments on various body systems and the need for ongoing assessment • Implement tools into your practice and clinic workflow to identify patients that have a history of childhood cancer • Implement quality improvement initiatives focusing on vaccine tracking and administration; Incorporate evidence based guidelines from the Children’s Oncology Group into managing adult survivors of childhood cancers • Communicate effectively with patients and families the risk factors for late effects of cancer treatments and the need for long term care • Develop a practice support network with subspecialists and other experts in the field of survivorship Target Audience Physicians, advanced practice nurses, physician assistants, nurses, case managers, patient navigators, social workers and other professionals involved in the care of adolescents and adult survivors of pediatric cancers Overview Significant successful improvements in the treatment of childhood cancer are leading to an increased number of childhood cancer survivors in the United States, thus introducing newfound struggles in the long-term care of these patients. Adolescent and adult survivors of childhood cancers are now at increased risk for numerous physical, emotional, psychological, social and financial late effects of cancer treatment. The need for long-term follow-up care has been well established and guidelines now exist to help providers monitor for late effects. National accreditation standards are pushing for higher standards including survivorship care. However, there continues to be a gap among health care providers in several areas related to monitoring and early diagnosis of late effects of cancer treatment. This conference is designed to provide primary care providers and other health care providers involved in the care of adolescent and adult survivors of childhood cancer with the most current information regarding late effects of treatment and use of the latest Children’s Oncology Group (COG) survivor guidelines. In addition, the workshop will provide a forum for networking between primary care providers and subspecialists. Sponsored By Keynote Speaker - Timothy E. He currently serves as the Clinical Director for The St. Jude LIFE & After Completion of Therapy(ACT) Clinics.
