CAP FoundationFinancialsIn 2015, The College of American Pathologists, the CAP Foundation’s largest donor, continued to support the Foundation with a contribution of $500,000 consisting of a $300,000 cash gift plus an in-kind donation valued at $200,000 to provide additional staff resources. In this diverse urban setting, a large portion of the community lives without links to medical care, spurring a high prevalence of cancer detected in late stages. Here, our 2015 results demonstrate that our donors help make a di�erence for many.
A full 25% of Pap tests were abnormal, with several women connecting to immediate follow-up care. Among breast cancer screenings, 8.6% of the women required additional analysis and one malignancy was confirmed.

R E S U L T SLab tests performed included six Clinical BioChemistry labs (ie, Lipid, Metabolic, Thyroid); nine Clinical “special” labs based on prescreening responses and indicated risky behavior (ie, Hep B, Hep C, HIV, and Syphilis). Women who tested positive or had abnormal results were scheduled follow-up appointments/care.
For instance, a 60 year old woman who had never received a detailed women’s wellness exam, was diagnosed with severe hypertension and diabetes and was immediately connected to Medicaid care and follow-up.

A year and half after the first See, Test & Treat at Rhode Island Hospital, pathologist Latha R. Pisharodi, MD, FCAP, was still struck by the experience of one participant in particular: a 46-year-old mother of six children.
“A fairly recent émigré from a very poor country, she had never had a mammogram or Pap smear,”Dr. Pisharodi explained. “While her cervical screening showed no abnormalities, her mammogram led to a diagnosis of stage 2 breast carcinoma. She underwent surgery and treatment, and today she is cancer-free.”
“Had a family member not known about our See, Test & Treat day, this woman’s disease would probably have gone long undetected,” she said. “Cancer screening is simply something she knew nothing about.” Hers is not an isolated story.
More than one quarter of the women fitting the criteria for a mammogram at the Rhode Island See, Test & Treat had never been screened for breast cancer. Nearly one in six women had never had a Pap test.
“We know that screening saves lives,” Dr. Pisharodi explained. “But that message and the means to act on it are still not reaching the women at greatest risk of dying from cancer.”
In support of Dr. Pisharodi’s vision to produce an educational video explaining to patients the imperative story of cancer screening, the CAP Foundation recognized Dr. Pisharodi last year with the 2015 Gene and Jean Herbek Humanitarian Award.
“Being a part of See, Test & Treat is an inspirational experience,” Dr. Pisharodi said. “It is a day that removes barriers for women who may rarely seek medical attention and for pathologists who may rarely see the patients that they serve on a daily basis.”
By demystifying both the science and the practical aspects of breast and cervical cancer screening, the video, titled
“Screen for Life,” will reduce fears and correct misperceptions that participants might harbor as it shares stories of women who have benefitted from See, Test & Treat. The video will be shown at community organization meetings, health centers, and other educational venues, including See, Test & Treat programs, and will also be shared via social media and other hosting sites.
Dr. Pisharodi is an on-sta¦ pathologist and director of cytopathology at Rhode Island Hospital, The Miriam Hospital, and Newport Hospital in Rhode Island. She is an associate professor of pathology at The Warren Alpert Medical School of Brown University in Providence, and also serves as a CAP laboratory inspector.

Compassion in Action: The Herbek Humanitarian Award
The Herbek Humanitarian Award Named for CAP immediate past president and See, Test & Treat founder Gene N. Herbek, MD, FCAP and his wife Jean, the award grants up to $25,000 annually to a CAP Fellow who provides See, Test, & Treat program leadership. Latha R. Pisharodi, MD, FCAP, (Center) won the award.

Two Patients, One ProgramScreening the Spectrum of Women in Need
RenaRena was in transition when she came to a See, Test & Treat. Having moved to Texas from Chicago four years earlier, Rena had not had a routine cancer screening in years. In fact, she didn’t even have a doctor.
“A wellness exam, PAP smear, and mammogram are expensive,” she explained. “I am starting a new business, and I do not have extra funds. Although I did sign up for health insurance through the new health care marketplace, I just haven’t been able to a¦ord the $100 monthly premiums and the deductible is $3,000, which is also beyond my reach.”
Because of our program, Rena was able to see beyond the barriers to care.
“I don’t know and haven’t even visited a doctor since moving to Texas,” she said. “So I came here for screening, but everyone has been wonderful, positive, and willing to give me as much information as I need. Because of all the resources I found here, I’m not only grateful for the same-day results, which I love, but I’ll be much more comfortable coming back when I need a doctor.”
LeticiaBy contrast, Leticia came to the same See, Test & Treat with a pressing fear prompted by pain.
“I have been having pain in my right breast for some time now. I cannot a¦ord to see a doctor because of my lack of insurance and the financial position I am in,” Leticia said. “I found out about this program through a TV commercial. I called the phone number and made an appointment.”
Her same-day test results helped eliminate Leticia’s uncertainty. But there was more.
“This program doesn’t just give test results, it also prepares the woman and her family members on what to do next,” she said. “It helps women learn how to take care of themselves and start paying more attention to any discomforts they might have. Everything that has been presented to me today has been very helpful.”
Grass Roots Health Care Starts at the TopEva Wojcik, MD, FCAP, lead pathologist at Loyola University Medical Center (LUMC), in Maywood, Illinois, recognized a unique need right in her hospital’s backyard. Many Polish, African-American, Hispanic, and Caucasian women in the community don’t have access to health services.
So she went straight to the top.
“I started with Wendy, the LUMC president and her administrative team,” Dr. Wojcik said. “I told them about See, Test & Treat and how important it is. They listened. They decided this was exactly in line with Loyola’s mission: We serve the community.”
In fact, LUMC president Wendy S. Leutgens, RN, MSN, didn’t need much convincing. Providing vital health services through See, Test & Treat was a natural fit. So much so that during the screening day, President Leutgens stood at the door, greeting and welcoming patients, participants, attending families and friends.
“I hope that these women have a great experience and that none of them has a bad finding,” she said. “But if they do, I hope we catch it early, and they get connected to the care they need. Maybe some of them will connect with a doctor and realize how important preventative care is.”

Advancing Value Through Education and Training
Dr. Smith Goes to WashingtonAs you read the anecdotes from residents who were awarded the CAP Foundation Leadership Development Award who attend the annual CAP Policy Meeting, the Frank Capra film
“Mr. Smith Goes to Washington” comes to mind.
In that 1939 classic, the optimism of youth collides woefully with the world of politics. But in the end, the promise of youth prevails.
Scott & White Memorial Hospital/Texas A&M Health Science Center pathology resident Amelia B. Sybenga, DO, provides an account of the 2015 CAP Policy Meeting, sharing all the elements of a drama with scripted overtones of optimism:
“Developing a better understanding of the insidious process of attempts to usurp physician judgment, the way our health care system functions on an administrative/financial level, and how rapidly detrimental changes can happen if we stay uninvolved were the most important points I took home.”
Undaunted by the befuddling view of US health care in Washington, DC, Dr. Sybenga also reported she learned “that we also have the ability to prevent these changes, and the ability to change the system as a whole.”
“Every resident should attend this meeting at least during residency,” she said. “Learning these processes and skills during residency and networking opportunities in this small community will fortify one’s career and better prepare physicians to act as advocates for change.”

CAP FoundationInformatics AwardsAward benefits extend beyond the individuals. CAP Foundation Informatics awardee Valerie Lockhart, MD, summarily revamped her thinking after her CAP15 continuing education in informatics, which was made possible through our generous supporters.
“I’ve reframed my perceptions of electronic medical records,” she said. “It’s not just isolated charts with individuals’ medical information that are easy to access from any computer. It’s an entire database with detailed information about every single type of patient you can imagine.”
As the field of pathology rapidly evolves to deliver better patient care, pathology informatics continues to align technology and business strategy with the clinical needs of the future.

Advancing Gold Standards of Laboratory Medicine
The CAP Foundation’s 2015 John H. Rippey Grant for Laboratory Quality Assurance funded neuropathologist William C. McDonald, MD, FCAP, of Abbott Northwestern Hospital, Minneapolis, in a project aimed at improving the histologic evaluation and classification of pituitary tumors.
His research title alone speaks to the CAP and the CAP Foundation’s 50-year history at the forefront of defining quality in laboratory medicine: “Diagnostic Best Practices in the Absence of Gold Standards: An approach to IHC Panel Construction Using Tissue Microarray and Statistical Learning Techniques.”
“The proliferation of tests, reagents, and recommendations makes it di¯cult for pathologists to validate immunohistochemical (IHC) stains when gold standards are unavailable and to understand, in particular, the most e¯cient panel for complex disease characterization,” Dr. McDonald explained.
Supporters of the CAP Foundation made it possible for Dr. McDonald and his team to explore and expand the application of an improved process they had developed.
“In pursuit of the optimal combination of cost, accuracy, and ease of use, we have had marvelous success with the overall project,” he reported. “We published our groundwork in the Archives of Pathology & Laboratory Medicine’s early online release papers and are submitting abstracts to present at an upcoming conference.”
“This experience was an eye-opener. I witnessed the immense logistics and quality assurance process behind building new instruments and reagents that are sensitive, specific, accurate, and precise.”

On a cold morning, in pouring rain, I pulled up to our host site in Southwest Houston and saw a line of women wrapped around the building. I knew women in the community needed greater access to health services, but I had no idea so many underserved women would turn up seeking cervical and breast cancer screenings.
As a health educator who designs cancer education curriculums, I have volunteered and worked with See, Test, & Treat, connecting these women with materials not just on cancer but on cancer prevention, tobacco cessation, diabetes management, and other chronic illnesses as well.
I grew up in a disadvantaged household; my mom had a lot on her plate raising five of us kids. Then, when I was only eight years old, we were hit with an obstacle we had never imagined. I was diagnosed with Burkitt’s lymphoma and needed emergency surgery and chemotherapy. Though treatment was tough, the team at the MD Anderson Cancer Center helped me overcome the disease.
Now I am on a personal mission to improve access to health education, and See, Test & Treat has become a powerful channel to reach underserved populations both locally in Texas and nationally.
I have gone on to earn my master’s degree in health studies from Texas Woman’s University, and I’m working toward my PhD studying cancer control and health disparities research at Texas A&M University. But I’ve been involved in cancer education since my freshman year at the University of Houston when I delivered health programs to housing projects in the area.
I ask myself, the health care community, and policymakers: “Why are women still dying from preventable diseases?”
The CAP Foundation needs generous donors who understand the great need for early detection programs and culturally sensitive health education. Together we can prevent tragedy. And the prospect of more Texas-based See, Test & Treat programs fills me with hope for the fight against cancer.
