Nurse Administrator should have

the following Health Forms – HF

security rights to create and

activate IHPs:

Create Forms – CF

WS\OF\HR\HF\CF

Maintenance – FM

WS\OF\HR\HF\FM

Selection – FS

WS\OF\HR\HF\FS

Step 1: Create IHP templates: In

Create Forms Add, Clone, Edit or

Import IHPs. This will give your nurses

IHP templates to choose from when

they use Create IHP button in Student

Profile.

Use the Tool Bar to format your template.

Select your Page layout, insert tables, and

insert Fields (merge fields that pull

information into IHP).

Insert text fields, check boxes, radial

indicators, and drop-down boxes.

STEP 2: Once your IHP

template is complete, go to

Maintenance – FM to select

the forms you want to make

available in Selection (Step

3).

Click Add. Name the form,

then Click Health Form to

select which IHP Custom

Form to add. Save.

STEP 3: Go to

Selection – FS

Check the IHP

templates you wish

to make available

to your nurses

when they use

Create IHP in

Student Profile.

Save.

Import/Export IHPs

(share the love)

In Create Forms, select

the IHP you wish to

share, and then click

Export Form.

Select Open with and

use Notepad (default)

and Save As to save a

copy of the document.

Attach the notes file to

an email and send it to

another Skyward user.

(Can also open/save as

Import Custom Form

Save As the Notes file they received.

Go to Create Forms – CF

1. Select Import Custom Form and

enter a Form name and select the

form Layout

Click Continue.

2. Click Browse, select your saved

Text file and Open. Upload and

Continue.

3. Upload

4. Continue. The IHP is now ready to

edit and use.

Create IHP

In Office or Student/Student

Profile IHP tab

WS\OF\HR\TB\IH

Select student and click Create

IHP. Select your school year,

select desired template and

click OK. Complete IHP fields

and Save.

View/print IHP using Print or Current.

Revise using Edit. Make sure to change

the school year if needed. You can re-

import data from last version and edit

as needed.
