    The CLUB HUB

    AFTER SCHOOL ENRICHMENT PROGRAMS at MORIAH

    1st SessionOctober 2011 January 2012

    All clubs meet will meet for 10 sessions (with the exception of DubeZone which willmeet either 11 or 12 sessions) from 3:30 - 4:30 p.m. Transportation home is theresponsibility of each parent.

    Please submit all fees directly to the club of your choice information (checkpayable to and mailing address), will be on each individual registration form.

    Please be aware that in the event of inclement weather, it is the responsibilityof the parent to contact the school to confirm whether or not clubs will beheld that day - please do not expect a call from the school office. In the eventthat clubs are cancelled, your child will be sent home at regular dismissal time bytheir normal means of transportation.

    Registration forms are due by Wednesday, Oct. 5th. Listed below are the clubdates:

If the child must withdraw for a medically-documented reason, credit toward a future DubeZone class will beissued.     such minor to continue to participate in such activity, I will immediately discontinue the minor's future or further particip ation in such activity.

    2. I FULLY UNDERSTAND THAT: (a) such minor's participation in the aforesaid activities involve risk and danger of serious bodily injury, including permanent disability, paralysis, and death

    (risks); (b) these risks and dangers may be caused by said minors actions or inactions, the actions or inactions of others participating in the activity, the condition in which the activity takes

    place, or the negligence of DubeZone, LLC.; (c) there may be other risks and social economic losses either known or not known to me or not readily foreseeable at this time, and I f ully accept

    and assume all such risks and responsibilities for losses, costs and damages such minor may incur as a result of the minors participation in the activity.

    3. I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE DubeZone LLC, or, their respective administrators, directors, agents, officers, members, volunteers, and employees, otherparticipants, any sponsors, advertisers, and owner and lessor of premises on which t he activity takes place, (each considered one of the RELEASEES herein) from all liability, claims,

    demands, losses or damages on said minors account caused or alleged to be caused in whole or in part by the negligence of th e Releasees, or otherwise, including negligent rescue

    operations, and I further agree that if despite this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement, I or anyone on behalf of said minor makes a claim against

    the Releasees, I as parent or natural guardian, will indemnify, save and hold harmless ea ch of the Releasees, from any litigation expenses, attorneys fees, losses, liability, damage or costs of

    which they may incur as a result of such claim.

    4. I HEREBY GRANT PERMISSION FOR PHOTOGRAPHY AND VIDEOGRAPHY of my childs activities related to DubeZone or any DubeZone activities for public relations purposes. I agree that Iam to receive no compensation for my child s appearance and that this participation confers on me no ownership rights whatsoever.
