Introduction
The health and wellness of any community is essential for its well-being and prosperity.
One way to assess the level of wellness is by the use of a Community Health Assessment.
Certain measures such as resources, climate, population, and age are but a few that are examined
in a well-prepared assessment. Public health nurses rely on data from surveys to aid in the health
care delivery to a specific population. “Assessment for community health nurses means
collecting and evaluating information about a community’s health status to discover existing or
potential needs and assets as a basis for planning future action” (Allender, Rector, & Warner,
2014, pg. 492). The following paper will examine the statistics surrounding the community of
Hampstead, Maryland. A Community Needs Assessment was performed and the following data
compiled to exhibit the needs of the current population. Additionally, health goals for the
community are presented with specific objectives in order to attain them.
Community as a Place
Hampstead, Maryland is a small town in Carroll County, Maryland in the northeastern part
of the state. Settled in 1786, Hampstead covers 3.28 square miles and is located at 39 36’ 17.3
North Latitude and 76 50’ 59.9” West Longitude (see Appendix A.) The town is home to 6,354
people having seen a 25.6% increase in population since 2000 (city-data.com). The town consists
of rolling hills and farmland mixed in with a downtown that is currently undergoing a
revitalization. Hampstead prides itself in its family-centered community feel with friendly
neighborhoods, good schools, and small businesses. Driving through on Route 30, one notices a
rural community with newer construction sub-divisions with sidewalks, streetlights, and well-

maintained grounds. This is in contrast to the older, historic homes that line Main Street downtown.
Many of these homes are in disrepair or are vacant and on the market for sale. There are also
apartment complexes, condominiums, and townhomes. The estimated median house or
condominium value in 2012 was $235,369 and the median gross rent; $914 (city-data.com).
Environmentally, Hampstead is considered average when compared to the rest of the country.
In terms of waste management the town has trash and recyclables pick-up every Thursday or
Friday with bulk trash being accepted on specific dates throughout the year. Air quality statistics
reported in 2012 measured the Air Quality Index (AQI) at 88.0 which is in the average range (see
Appendix B). The water supply for the town’s residents come from the Wissahickon aquifer, which
lies 300 feet below the earth’s surface (mde.maryland.gov). This underground river is tapped by
drilling wells and in that process purification is aided. This source is naturally replenished by
rainfall. Currently, treatment facilities provide 150 million gallons of clean drinking water every
year (mde.maryland.gov, 2013).
People of the Community
Of the 6,354 people living in Hampstead in 2013, 49.6% were male (3,153) and 50.4%
were female (3,201). Of these, 91.9% were Caucasian (5,812); 3% Hispanic (188), and 1.9%
Black (123). The remaining 3.4% were Asian, American Indian, or Native Hawaiian (200). The
latest data reports the median resident age is 35.3 years while compared to the median resident
age of the state of Maryland of 34.4 years (city-data.com).
While compiling information pertaining to birth and death rates, no data was available for
the town of Hampstead specifically. For the purpose of this report, live birth and death statistics
were for Carroll County. In 2013 there were 1,551 live birth reported and 1,486 deaths. This

includes male and female of all races and ethnic groups residing in the county
(dhmh.Maryland.gov). More detailed information can be found in Appendix C. In addition,
according to the Community Benefit & Health Improvement Plan for Carroll County in fiscal
year 2014-2016, data indicate that the leading cause of death is cancer. The incidence of breast
cancer and melanoma are greater in Carroll County than the Maryland state averages. Other
conditions such as Obesity, Diabetes, and Cardiovascular disease are leading causes of morbidity
and mortality (healthycarroll.org, 2012)
Turning to education, 90% of the population of Hampstead are high school graduates or
better which is the 3rd most of other towns in the area. Included in that 90% figure are those that
have obtained a Bachelor’s degree or higher (29.9%) and those with some post-secondary
education (29.7%) (towncharts.com). The unemployment rate is 4.5% which is lower than the
national average although Hampstead has seen less recent job growth as compared
nationally.(See figure 1) The household income is $75,135 with the largest percentage of
families, 28.3%, earning between $100,000 and $150,000 annually.

There are various occupations represented in the town of Hampstead. Figure 2 details the
breakdown by percentage. Management and business along with Sales and Administrative

Support make up 46.38 of total positions held in the town. Although the town is traditionally
rural, the farming industry no longer plays a vital role in the occupation of Hampstead’s citizens.
Jos. A. Banks corporate center and Wal-Mart are the two largest employers in town.

Social System
Hampstead, Maryland has a vibrant public school system as well as several colleges and
universities within 25 miles of the town center. Hampstead Elementary and Spring Garden
Elementary serve the town’s youngest students in K-5, while Shiloh Middle School teaches those
in grades 6-8. North Carroll High School (See figure 3) is the only secondary school in
Hampstead and has a current enrollment of over 1800 students (city-data.com, 2013).

There are many public parks for recreation in the town. Hampstead Municipal Park is
centrally located and has 2 baseball diamonds, a multipurpose field, pavilion, picnic area,
restrooms, a walking trail, and volleyball courts (See figure 4). In addition, there are several
smaller parks including Lions Club Park, Melvin Miller Park, North Carroll Community Pond,
and Chief Sites Memorial Park (hampsteadmd.gov). The town hosts little league baseball, girls
softball, soccer, and lacrosse at the various parks. Reservations for private use are available a
several pavillions.

The North Carroll Senior Center is conveniently located in the main shopping and
restaurant area of the town. The Center is open 6 days a week and its mission is to have keep the
seniors happy, healthy, and well-informed. Nearby the center is the North Carroll Branch of the
Carroll County Public Library. A health club, The Four Seasons Sports Complex houses several
indoor soccer fields, weight rooms, dance floors, and aerobic rooms. In the summer there are two
pools for members and a swim team that the whole community can join. Lastly, Oakmont Green,
an 18- hole PGA golf course located on Golf View Lane and is open to the public. The course
holds classes for adults and children and is open daily from sunrise to sunset.

Childcare and daycare services are available both in private homes and in centers.
Currently there are 9 centers located within the town limits including Celebree Learning Center
(See figure 5) and Shiloh Stepping Stones. See
Appendix D for additional information.

The nearest airport and Amtrak station are located at Baltimore-Washington Thurgood
Marshall International Airport (BWI) which is 24 miles from the city center. There are no public
means of transportation in Hampstead and residents rely on their own vehicles for commuting to
and from work. Butler Mobility is a private company that provides reduced-fare transportation
for seniors and other town residents. Carroll Area Transit provides free transportation for special
needs residents special needs residents. (See Figure 6 and 7). Barwood Taxi Service is available
for hire 24 hours a day.

Hampstead saw just 4 violent crimes and 73 property crimes in 2012. This calculates to a
1 in 83 chance that a resident would be the victim of a crime which is far better than the majority
of other U.S. cities its size (movoto.com, 2014). The good news for the town is that the total
number of all crimes has slightly fallen within the past 5 years. See Appendix E for more
information. Hampstead
has its own city Police
Department and its motto is
Integrity, Community, and
Service. As far as fire
protection, The Hampstead
Volunteer Fire Company has been in existence since 1900 providing emergency support services
to the town and surrounding areas. There is a single station which is located on Main Street,

housing 2 Engines, 1 Truck, 1 Brush Unit, and 2 Paramedic equipped ambulances, and 2 Utility
vehicles (hampsteadvfd.org).
A search for healthcare providers revealed numerous private practices and an urgent care
center within 15 miles of the city center. While Hampstead does not have a community hospital,
18 miles away is Carroll Hospital Center. Carroll Hospital Center is a 193-bed private, non-profit
hospital with nearly 400 physicians that see over 432,000 patients per year (healthycarroll.org).
The Wal-Mart in Hampstead has a Vision Center which has an optometrist on staff who provides
basic eye care. Additionally, there are 4 full-service physical therapy facilities including North
Carroll Physical Therapy pictured below. (See Figure 8). A search revealed no inpatient
addictions treatment centers but North Carroll Counseling Center offers outpatient treatment for
dual diagnosis.

A wonderful gem in the community is the Residences at Hampstead School, a beautiful,
fully restored historic building that was constructed in 1919 as the town’s premier school. It is
now serving as low-income senior housing. The facility has many amenities including an on-site
fitness and wellness center. See figure 9.

Hampstead Community Comprehensive Plan
In 2010, the Hampstead Community Comprehensive Plan was published to map out
strategic goals for the town. Community involvement was encouraged and with the help of the
county commissioners, several key elements were put forth as the vision for planning area goals.
Several key proposals were: 1) To limit the ultimate size of the town to maintain the small-town
atmosphere, 2) To ensure the character of new development, 3) To encourage the development of
sidewalks and pedestrian paths, 4) To encourage alternate means of transportation, 5) To
enhance personal and public safety, 6) To ensure the parks are used efficiently by all user groups,
7) To support the Hampstead Volunteer Fire Department and the North Carroll Police Auxiliary,
8) To promote water conservation, 9) To focus economic development efforts on businesses that

provide high-quality jobs, 10) To support local farmers and to establish a local farmer’s market,
11) To work with property owners to help restore historic structures, 12) To promote Town-wide
“Wi-Fi”, 13) To support community recreational events, 14) To create pedestrian links with
other communities, and 15) To support private sector housing opportunities to meet the needs of
the young and elderly (www.mdp.state.md.us). The plan goes on to identify how the goals can be
accomplished and will be re-evaluated with an updated analysis in 2016.
Current Community Health Needs
A windshield survey was conducted on March 27, 2015. Driving through town one can
see a flurry of activity and the many opportunities for recreation and shopping. On this
particular day, traffic in the outlying shopping area was heavy and the Wal-Mart parking lot was
¾ full. Observing patrons one can see a high level of obesity. Many of the restaurants were small
family-owned business, however, several were observed advertising “happy hour prices” and
other alcohol specials. Two community parks were visited and there were no observations of
anyone utilizing the facilities on this bright, sunny day. There were only a few people walking on
the various sidewalks and one pedestrian was observed walking a dog on the paved walking trail.
Outside of the senior center, a van was picking up clients to transport. Several teens were
observed outside of a convenience store near the high school and all were smoking cigarettes.
When visiting the historic downtown area, it was noted that the sidewalks in many areas are
crumbling and the 2 local police officers were engaged in a traffic stop and appeared to be
searching the inside of a vehicle while the driver sat hand-cuffed on the curb.
In the residential areas, where the many newer homes exist, one sees well-maintained
yards with various degrees of improvement; some elaborate. The residences are situated outside
of the business district but have easy access to schools and restaurants. The busy main road,

Route 30, is a hazard for pedestrians and it appears that it is not conducive to walking or cycling.
No bike lanes were observed and no pedestrians were observed in the cross-walks. Observations
on this day expose some health concerns for the community. These concerns include obesity,
which leads to cardiovascular disease and diabetes, tobacco use, which causes cancer, and access
to health care, which can lead to non-compliance with a health regimen. These concerns are
inter-related when forming diagnoses as they all contribute to poor health and need to be
addressed.
Community Health Diagnosis
The Windshield Survey and Community Assessment serve as the basis for diagnosing the
above-mentioned health needs of the town of Hampstead, Maryland. When formulating
recommendations one must look to the national health promotion and disease prevention agenda
entitled Healthy People 2020. In the publication, objectives and targets for the health of the
nation are discussed and “By encouraging collaboration across communities, empowering
individuals to make better choices, and measuring progress toward set bench-marks, Healthy
People 2020 provides a road map for achieving longer and healthier lives for all Americans”
(Allender, Rector & Warner, 2014, pg. 463.) Using county data, it is estimated that 68.9% of the
citizens are obese. (healthycarroll.org). This is a concern because obesity leads to diabetes,
cardiovascular disease, limited mobility, and eventually early death. Secondly, with cancer as the
leading cause of death in the county in which Hampstead is a part, one must address tobacco use
and the need for community outreach in assisting its members to quit, and lastly, poor access to
health care such as long travel distances form rural areas are a hindrance to many. The following
recommendations can be made when addressing these objectives:

1. Reduce the level of obesity of the citizens of the county by initiating wellness programs at
area grocery stores, community centers, and fitness centers. Promoting healthy eating habits
in schools and libraries, as well as making the town more walker-friendly. As stated in
Healthy People 2020, individuals need to be empowered to make better choices as nutrition,
physical activity, and obesity are public health priorities. Changing eating and exercise
habits will go a long way in decreasing the prevalence of obesity.
Diagnosis: Imbalanced Nutrition: more than body requirements
Recommendations:
Identify causes and contributing factors
Establish weight-reduction programs
Promote well and physical activity
2. Reduce the number of individuals who use tobacco product by initiating community
education through Carroll Hospital Center and public service announcement with a possible
campaign to get local pharmacies to stop selling tobacco products. Teaching about the
effects of cigarette smoke and the harm is does to one’s health will help in cessation.
Diagnosis: Impaired Gas Exchange
Recommendations:
Identify what leads someone to start smoking
Encourage, through to attend smoking cessation classes
Financial assistance for medication for smoking cessation
3. Increase ease of access to health services especially to those individuals who have mobility
difficulties or live far distances from medical facilities. Compliance with medication use and
keeping appointments with physicians should be encouraged. Many patients have trouble

understanding their health care needs and instructions. More user-friendly discharge
instructions from health care providers can assist patients with compliance thus leading to
better outcomes. The leading indicator mentioned first in Healthy People 2020 is access to
health care.
Diagnosis: Readiness for Advanced Knowledge
Recommendations:
Develop a plan for learning
Encourage and enhance optimum wellness
Provide mobile access to health facilities for those with greatest hardship
Conclusion
Hampstead, Maryland is a historic community that is currently undergoing a
revitalization of its downtown area. The residents enjoy a beautiful rural setting with the
amenities of a modern city. There is a lot of open space that promotes recreation and a sense of
community. There are, however several areas that can be improved upon. These mainly
encompass healthy living and the need for education as to how to attain optimum wellness. The
incidence of obesity, the prevalence of cigarette smoking, and the difficult access to health care
make up the top three issues recommended by this assessment for improvement. The strategic
plan for the community is in line with attaining these goals.
