how much you impacted a person’s health.

It’s time to move your story from what you

did (outputs) to what you achieved for the

patients you served (outcomes).


Outputs are your program’s outreach efforts to the patients you serve. They include: Referrals, Screenings Exams Education Other events

If you operate more than one community program or a broad program with several parts, we recommend you select and measure outcomes for one. It could be your “star” program or the easiest one to measure.

The process of choosing metrics should be a joint effort between:

Program management Board members Hospital staff Community leaders Other parties beneficial to the process

This program saves lives (outcome) by screening young people for potentially serious heart problems that have not been diagnosed and determines the best way to address them (outputs).

Often outcomes are abstract. Indicators can serve as proxies for outcomes and help measure the program’s progress toward its intended outcomes.

If you can combine outcomes

with financial data, you can

calculate a monetary reflection of your program’s

impact. This allows those

financially supporting the

program to know what their

investment accomplished.

cost to generate an outcome.
